Objectives: The purpose of this study was to analyze the change of area and average velocity of the center of pressure (COP) according to gait improvement in post-stroke hemiplegic patients.Methods: We measured the area and the average velocity of COP displacement of ten post-stroke hemiplegic patients while they stood quietly on a force plate at the time of dependent and independent gait . We also measured the area and the average velocity of COP displacement of normal persons to verify differences between them and post-stroke hemiplegic patients.Results: The area of COP displacement decreased significantly, and the average velocity of COP displacement tended to decrease, but not significantly. There were significant differences in the area and the average velocity of COP displacement between post-stroke hemiplegic patients and normal persons.Conclusions: The results of this study show that postural control ability improves as gait improves.
일개 한의원에 내원한 소화불량 환자의 사상인 분포에 관한 연구

1. Objectives: The purpose of this study was to find out the distributional characteristics of dyspepsia according to Sasang Constitution. 2. Methods: The medical records of 108 subjects who have received prescriptions and diagnosis of Sasang constitution from June 2007 to April 2009 at a Korean Medical clinic in Seoul were reviewed. General factors and the frequency of the prescriptions according to Sasang constitution are compared and analyzed. 3. Results: Among the 108 subjects, 57(52.78%) persons were Soyangin, 27(25.00%) Taeumin, and 24(22.22%) Soeumin. The frequencies of the prescriptions of interior and exterior disease were compared. Exterior disease(89.47%) was higher than interior disease in Soyangin and interior disease was higher than exterior disease in Taeumin(88.89%) and Soeumin(91.67%). (p-value<0.001) The most common disease was Mangeum-syndrome(73.68%) of exterior disease in Soyangin, Dry-febrile symptom(88.89%) of interior disease in Taeumin and Taeum-syndrome(66.66%) of interior disease in Soeumin. 4. Conclusion: There were significant differences on the distribution of dyspepsia in interior or exterior disease according to Sasang Constitution.
아급성 기저핵 뇌졸중 후 편마비 환자의 보행양상의 변화 : 후향적 연구

Objectives: This study observed changes in gait pattern according to the motor grade of the paretic lower limb in patients with basal ganglia stroke who are in the subacute phase. Methods: We used the Manual Muscle Test (MMT) to evaluate the motor grade of the paretic lower limb of 21 patients with subacute basal ganglia stroke and then divided them into two groups based on the MMT results. Stroke patients with a motor grade above Gr. III were put in group I (15 people) and those with a grade less than Gr. III in group II (6 people). We also estimated spatiotemporal factors using treadmill gait analysis equipment. The values were gait velocity, step length, step time, double support phase, and cadence. The first measure was conducted during the early period of admission and the second was between four and five weeks after admission. Results: In Group I, the gait velocity and step length of both legs significantly increased. In Group II, the step length and step time of the paretic side and the gait velocity tended to decrease, but not significantly. The step length of the paretic side in Group II was significantly longer than that in Group I at the first measure. The step time of the paretic side in Group I was significantly shorter than that in Group II and gait velocity and cadence in Group I were significantly higher than in Group II at the second measure. Conclusions: The gait parameters of all stroke patients improved in terms of time. In addition, the changes in gait pattern were different depending on the motor grade of the paretic lower limb.
소양인(少陽人) 신열두통망음증(身熱頭痛亡陰證)과 신한복통망음증(身寒腹痛亡陰證)의 소증(素證) 특성에 관한 단면연구

1. Objectives: The purpose of this study was to survey the Original symptoms(素證) of Soyangin's and to compare the tendency of their symptoms between Soyangin Heat-related diarrhea accompanied by headache(身熱頭痛亡陰證) and Soyangin Cold-related diarrhea accompanied by abdominal pain(身寒腹痛亡陰證). 2. Methods: We recruited 56 Soyangin subjects in the Korean Medicine clinic and researched their original symptoms. Subjects were diagnosed by the Sasang Constitutional specialists and Sasang Constitutional Medication. Subjects were two groups, confirmed to Soyangin Heat-related diarrhea accompanied by headache and Soyangin Cold-related diarrhea accompanied by abdominal pain. All subjects answered the questionnaire about their Original symptoms. We found the differences between 2 groups using the Independent-Samples T-test. Analysis was conducted using SPSS 12.0 for Korean. 3. Results and Conclusions: In Original symptoms, Tendency of overeating(p=0.01), Condition of digestion(p=0.04), Perspiration after exercise (p=0.02), Usual perspiration(p=0.00), Tiredness after perspiration(p=0.05) Cold intolerance on extremities(p=0.04), General edema(p=0.01) etc. are different between 2 groups. We found the differences of heat and cold in symptoms between two Soyangin syndromes.
중량조끼를 착용한 뇌졸중으로 인한 보행장애 환자의 즉각적인 보행 개선 효과 1례

Objective: To confirm the immediate gait improvement in a post-stroke gait disturbance patient equipped with a weighted vest. Methods: We selected a patient who was able to walk without another’s help or with tools. The selected patient had an unstable gait because she had only started an independent gait within the past week, so we thought that a weighted vest could be very helpful for her. We first collected gait parameters using a treadmill gait analysis system while the patient walked on the treadmill without the weighted vest. After a 10-minute break, gait parameters were collected again while the patient walked on the treadmill while wearing the weighted vest. The gait parameters we collected included step length (cm), stance phase (%), swing phase (%), SW/ST, and gait line length (mm). For objective evaluation of gait improvement, we calculated the ratio of gait parameters of the right and left limbs. Results: The gait of the post-stroke patient was more symmetrical when wearing the weighted vest than without the weighted vest. Without the weighted vest, her step length ratio was 0.78, stance phase ratio was 0.88, swing phase ratio was 1.50, SW/ST ratio was 1.70, and gait line length ratio was 0.91. With the weighted vest, her step length ratio was 0.88, stance phase ratio was 0.90, swing phase ratio was 1.38, SW/ST ratio was 1.54, and gait line length ratio was 0.98. No side effects were observed due to the weighted vest.

Objectives : The aim of this study was to analyze relationship between post-stroke depression (PSD) and cold, heat, deficiency and excess patterns. Methods : Twenty-eight PSD patients were recruited from STROKE center and measured with questionnaires for cold, heat, deficiency and excess patternvalues and saliva for cortisol awakening response (CAR). Saliva samples were collected immediately, 15, 30 and 45 min after awakening. In addition, Beck Depression Inventory (BDI) and Hamilton Depression Rating Scale (HDRS) were conducted for PSD severity. We conducted correlation analysis to find the relationship between cold, heat, deficiency and excess patterns and CAR or BDI and HDRS. Results : Deficiency and excess patterns werepositively correlated with area under the curve with respect to the increase (AUCi), but not with area under the curve with respect to the global (AUCg), in CAR. Furthermore, it was negatively correlated with BDI and HDRS, while cold and heat patterns were not correlated with CAR, BDI and HDRS. Conclusions : In terms of deficiency and excess patterns, the higher the PSD severity, the higher the deficiency and the lower the PSD severity, the higher the excess. However, there was not a significant relationship between PSD and cold and heat patterns.

Objectives: This study investigated the change of foot rotation angle of gait analysis parameters according to gait improvement in post-stroke hemiplegic patients. Methods: We measured the foot rotation angle of eight post-stroke hemiplegic patients at the time of dependent and independent gait. Results: The foot rotation angle of the paralyzed side reduced closer to normal average according to gait improvement, but the non-paralyzed side not significantly. Conclusions: Improvement of foot rotation is an important thing for independent gait of post-stroke hemiplegic patients so this treatment seems worthy of being considered in clinical trials.

Objectives: The aim of this study was to analyze the change of lateral shift of Center of Pressure (CoP) according to the gait improvement in post-stroke hemiplegic patients and to investigate relationship between the change of motor grade and lateral shift of CoP. Methods: We measured the lateral shift of CoP and motor grade of eight post-stroke hemiplegic patients at the beginning of dependent gait and independent gait. Results: We found that CoP tended to be shifted to the non-affected side when patients started to walk independently. Furthermore, there was no relationship between the change of motor grade and lateral shift of CoP. Conclusions: This result may suggest it is more important to control the non-affected side than the affected side, at least until the beginning independent gait.
