건강보험심사평가원 자료를 이용한 5년간(2003-2007년) 치매 환자치료 추세동향

In Korea, the elderly population and the prevalence of dementia has increased rapidly. The aim of this study is to describe and analyze the status in treatment of dementia for recent 5 yr in Korea. Methods: The Health Insurance Review & Assessment Service (HIRA)’s data which had various variables during 5 yr from 2003 to 2007 was used. Dementia was analyzed by the visit days, benefit amounts, and real patients in Out Patient Department (OPD) and admission. In addition, the distributions of the clinical department for dementia were presented. Results: The days of OPD visit and admission for dementia increased from 328,967 (2003,OPD) and 1,821,825 (2003, admission) to 765,599 (2007, OPD) and 7,433,913 (2007, admission). The benefit amounts of OPD and admission increased from 11,884,657,000 won (2003,OPD) and 64,742,232,000 won (2003, admission) to 40,849,297,000 won (2007, OPD) and 366,694,852,000 won (2007, admission). The real patients of OPD and admission increased from 59,861 (2003, OPD) and 18,515 (2003, admission) to 132,869 (2007, OPD) and 63,141(2007, admission). Among the clinical departments, in 2007, department of neurology had the greatest percentage in visit days (OPD). Conclusions: This study clarifies the magnitude of the dementia treatment in the viewpoint of visit days, benefit amounts, real patients, and the distribution of the clinical department treated at. Department of Neurology had a major role in dementia care, which had been increasing during 5 yr.
방풍산(防風散)이 실험동물(實驗動物)의 심혈관계(心血管系)에 미치는 영향(影響)

The present experiments were designed to investigate the effects of BangPoongSan on the cardiovascular system in the experimental Animals. And thus the change of blood pressure, auricular blood flow, artery contraction, death rate, platelet aggregation repression, plasma coagulation factor activity, plasma antithrombin activity, whole blood viscosity and plasma viscosity were studied. The result were summarized as the followings: 1. BangPoongSan dropped the blood pressure in the spontaneous hypertensive rat. 2. The drug increased the auricular blood flow in rabbit. 3. The drug relaxed the artery contraction by pretreated norepinephrine in white rat. 4. The drug inhibited the death rate of mouse which was led to thromboembolism by serotonin and collagen. 5. The drug inhibited the platelet aggregation in rat. 6. The drug prolonged the prothrombin time and activated partial thromboplastin time on the test of plasma coagulation factor activity in rat, but was not valuable. 7. The drug presented the antithrombin activity in rat. 8. The drug reduced the whole blood viscosity and plasma viscosity in rat, but the latter was not valuable. According to the results, Bangpoongsan increased the blood flow and dropped the blood pressure by dilatation of blood vessel smooth muscle. And the drug presented the antithrombin acivity, inhibited the platelet aggregation and reduced blood viscosity. Therefore these effects are assumed to improve the cardiovascular circulation disorder and prevent thrombosis.

Purpose: Virtual colonoscopy is the most recently developed tool for detecting colorectal cancers and polyps, but its effectiveness is limited. In our study, we compared the result of preoperative virtual colonoscopy to result of preoperative and postoperative colonoscopy. We evaluated also the accuracy of preoperative virtual colonoscopy in patients who had obstructive colorectal cancer that did not allow passage of a colonoscope. Methods: A total of 164 patients who had undergone preoperative virtual colonoscopy and curative surgery after the diagnosis of a colorectal adenocarcinoma between November 2008 and August 2013 were pooled. We compared the result of conventional colonoscopy with that of virtual colonoscopy in the nonobstructive group and the results of preoperative virtual colonoscopy with that of postoperative colonoscopy performed at 6 months after surgery in the obstructive group. Results: Of the 164 patients, 108 were male and 56 were female patients. The mean age was 62.7 years. The average sensitivity, specificity, and accuracy of virtual colonoscopy for all patients were 31.0%, 67.2%, and 43.8%, respectively. In the nonobstructive group, the average sensitivity, specificity, and accuracy were 36.6%, 66.2%, and 48.0%, respectively, whereas in the obstructive group, they were 2%, 72.4%, and 25.4%. Synchronous cancer was detected via virtual colonoscopy in 4 of the 164 patients. Conclusion: Virtual colonoscopy may not be an effective method for the detection of proximal colon polyps, but it can be helpful in determining the therapeutic plan when its results are correlated with the results of other studies.

Background: The effects of omega-3 polyunsaturated fatty acids (PUFAs) on cerebral vessels have not been clarified until now. Thus we investigated the efficacy of omega-3 PUFAs supplementation on cerebral blood flow velocity and vascular resistance via transcranial doppler (TCD). Methods: Consecutive twenty patients (13 male and 7 female) with at least 1 cerebrovascular risk factor or a known cerebrovascular disease were enrolled. Patients were treated with omega-3 PUFAs (1 g, two times per day) for 12 weeks. Cerebral blood flow velocity, resistance index, and pulsatile index were checked before and after 12 weeks of treatment using TCD. Results: The change of resistance index in right MCA (from $0.58{\pm}0.07$ to $0.55{\pm}0.07$, p = 0.042) and left PCA (from $0.56{\pm}0.07$ to $0.53{\pm}0.06$, p = 0.037) showed significant improvement after 12 weeks of omega-3 PUFAs treatment. The changes in other vessels, however, failed to show any significant changes compared to the baseline. Conclusions: Omega-3 PUFAs treatment showed feasible efficacies for cerebral vascular resistances in this open label trial. To confirm these results, larger samples of patients and longer period of follow-up is warranted.

Eosinophilic fasciitis (EF), also known as Shulman syndrome, is an inflammatory disorder of unknown etiology. It usually presents with pain, swelling, and tenderness of the proximal aspect of the limbs, chest, or neck, with subsequent induration of the skin and subcutaneous tissues, in association with peripheral eosinophilia. EF is differentiated from scleroderma by the pattern of skin involvement and non-involvement of muscle. We report a case of progressive EF presented with tightness and stiffness in all limbs.
