해면상 혈관종(cavernous hemangioma)은 인체 내 다양한 장기나 기관에서 발생할 수 있는 혈관기형으로, 순수 경막외 해면상 혈관종(pure epidural cavernous hemangioma)은 해면상 혈관종의 드문 경우에 속한다. 해면상 혈관종이 원인이 되어 발생한 증상에 한의학 치료를 적용한 연구가드물게 보고되었으나, 순수 해면상 혈관종의 출혈로 인한 증상에 대한 것은 없었기에 관련 임상증례를 보고하고자 한다. 척추의 순수 해면상 혈관종으로 진단받은 51세 남성이 2022년도에 해면상 혈관종 출혈로 인해 발생한 보행장애에 대해 치료를 받기 위해 한방병원을 방문하였다. 척추의 순수해면상 혈관종 출혈로 인해 발생한 보행장애 환자는 침 치료, 전기 침 치료, 온침 치료, 한약, 추나, 뜸, 부항 치료를 시행 받았다. 평가지표는 25 foot walk test (25FW), timed up and go test (TUG), comfortable gait speed tests (CGS), 요통과 하지 통증에 대한 numeral rating scale (NRS), Berg balance score (BBS), Tinneti-score, manual muscle test (MMT), EuroQol-five dimension (EQ-5D), EQ-5D visual analog scale (EQ-5D VAS)이었으며, 25FW, TUG, CGS, NRS, BBS, EQ-5D (VAS)에서는 치료 후 호전을 보인 반면, Tinetti Score, MMT, EQ-5D의 지표는 치료 후에도 동일하게 유지되었다. 한의학은 척추 해면상 혈관종의 출혈로 인해발생한 보행장애나 하지 강직감에 대한 치료의 한 방안으로 고려될 수 있다. CA안경테 가공기 주측시스템의 설계 안정성 분석

Purpose: In this study, the stability of high-speed spindle system of CA(Cellulose Acetate) frame equipment for eyewear with 40,000 rpm was examined at the stage of structure design. Methods: Natural frequency characteristics, maximum deformation, and thermal deflection were estimated by analyzing the static/dynamic/thermal characteristics through bearing lubrication and pre-load structure of spindle system. Results: Oil-air type was applied to spindle's lubrication system, which is the minimal micro-lubrication method. As the result of Static and Dynamic analysis, stability was guaranteed but the maximum 0.135 mm of thermal deflection was found. There were some problems with spindle balancing and performance. Conclusions: High-speed spindle system of CA frame equipment for eyewear with 40,000 rpm, lubrication and pre-load structure were selected from the design stage. Through static and dynamic analysis, excellent stability of structure was proved but thermal rise caused by rotation and thermal deformation were reported. Therefore, this study shows that the related measures may be needed. 목 적: 본 연구에서는 CA(Cellulose Acetate; CA)안경테 전용 가공장비의 40,000 rpm급 고속 주축시스템에 대하여 구조설계 단계에서 안정성을 검토하였다. 방 법: 안경테 전용 주축시스템의 베어링 윤활과 예압구조 선정 및 주축계의 정/동/열적 특성 분석을 통하여 고유진동수, 최대변형, 열변위를 예측하였다. 결 과: 안경테 가공장비 주축의 윤활시스템은 최소미량급유방식인 오일-에어 방식을 채택하였으며 정동적분석 결과 안정성이 확보되었으나, 열변위량은 최대 0.135 mm로 고속 회전 시 주축 발란싱과 성능에 문제가 발생 할 것으로 예측되었다. 결 론: CA안경테 전용 가공장비의 40,000 rpm급 고속 주축시스템을 설계 단계에서부터 윤활 및 예압구조를 선정하였으며, 정동적분석을 통하여 구조안정성이 우수함을 예측하였다. 그러나 회전에 따른 열상승 및 열적변형에 취약하여 이에 대한 대책이 필요할 것으로 나타났다.

소아 복막투석환자에서 혈관내피성장인자 유전자 다형성이 복막의 용질이동성에 미치는 영향

목 적 : 복막투석환자에서 복막의 용질이동성에 영향을 미치는 요인으로 유전적 요인과 임상적 요인이 있다. 복막평형검사는 복막투석환자의 복막 용질이동속도를 평가하는 데에 유용한 검사로, 본 연구에서는 소아 복막투석 환자에서 복막평형검사 결과를 이용하여 혈관내피성장인자(VEGF)의 유전자 다형성과 복막 용질이동성의 상관관계를 연구하였다. 방 법 : 서울대학교 어린이병원과 삼성서울병원의 소아 복막투석환자 중에서 복막염의 병력이 없고 복막투석 시작한 지 1년 이내에 복막평형검사가 시행된 환자를 대상으로 하였다. VEGF 유전자 -2578 C/A, -14978T/C, -1154G/A, -634G/C, +936 C/T의 단일 유전자다형성분석을 시행하였고 VEGF 유전자 다형성의 분포와 복막평형검사 결과간의 상관관계를 분석하였다. 결 과 : 복막평형검사를 분석한 결과, 4시간 크레아티닌의 투석액/혈장비(D/P creatinine)는 $0.56{\pm}0.13$였고 4시간 투석액/0시간투석액포도당비(D/$D_0$ glucose)는 $0.43{\pm}0.11$였다. 배수체 CTGGC의 이형접합체 또는 동형접합체인 경우가 배수체 CTGGC를 가지지 않은 경우에 비해 높은 4시간 D/P creatinine ($0.67{\pm}0.12$ vs $0.50{\pm}0.09$, P=0.007)과 은 4시간 D/$D_0$ glucose ($0.35{\pm}0.12$ vs $0.47{\pm}0.08$, P=0.037)를 보였다. 결 론 : VEGF 유전자 다형성은 복막 용질이동성에 영향을 미칠 수 있다. Purpose : Genetic and clinical factors can influence the permeability of the peritoneal membrane. The peritoneal equilibration test (PET) is helpful in measuring peritoneal permeability in peritoneal dialysis (PD). We investigated the influence of genetic polymorphism of vascular endothelial growth factor (VEGF) on the PET parameters. Methods : Pediatric patients who underwent PET within 12 months of initiating PD at Seoul National University Children's Hospital and Samsung Medical Center were selected. The patients with positive history of peritonitis before PET were excluded. The VEGF -2578C/A, -14978T/C, -1154G/A, -634G/C, and +936C/T single-nucleotide polymorphisms were genotyped. Results : The mean 4-hour dialysate-to-plasma ratio for creatinine (D/P creatinine) and the mean 4-hour dialysate glucose to baseline dialysate glucose ratio (D/$D_0$ glucose) were $0.56{\pm}0.13$ and $0.43{\pm}0.11$, respectively. The patients with haplotype CTGGC showed higher 4-hour D/P creatinine ($0.67{\pm}0.12$ vs $0.50{\pm}0.09$, P=0.007) and lower 4-hour D/$D_0$ glucose ($0.35{\pm}0.12$ vs $0.47{\pm}0.08$, P=0.037) than those without haplotype CTGGC. Conclusion : The VEGF genetic polymorphism may influence the peritoneal solute transport.

교원 인적자원 배분의 관점에서 본 초빙교사제 시행 학교의 특성 분석

초빙교사제가 2010년에 확대 실시되면서 특정 지역으로의 교사 쏠림과 이로 인한 지역 간 교육 격차 등이 문제로 대두되고 있다. 이에 교원 인적자원 배분의 관점에서 초빙교사제의 실태에 대해 연구할 필요가 있다. 이 연구에서는 2009년과 2010년의 초빙교사제 시행 학교의 특성을 비교 분석함으로써, 초빙교사제의 확대 실시가 교원인적자원 배분의 형평성에 주는 시사점을 살펴보고자 하였다. 이를 위해 2009년과 2010년의 초빙교사제 시행 학교의 특성을 학교 규모, 학생의 SES, 학업성취도, 거주지 변수를 중심으로 하여 로지스틱 회귀분석을 실시 하였다. 분석 결과, 2009학년도에는 무료급식 학생 비율이 높은 학교일수록 초빙교사제를 시 행한 반면, 2010년도에는 영어, 수학 보통학력이상 학생들이 많고 대규모 학교일수록 초빙교 사제를 시행하였고, 강남교육지원청 관내 학교에 비해 타교육지원청 관내 학교가 초빙교사제 를 실시할 승산이 점점 줄어드는 양상을 보였다. 또 영어와 수학의 보통이상학력 학생 수 비 율이 높을수록, 대규모학교일수록 2010년도에 새로이 초빙교사제를 실시하거나 초빙교사 수 를 늘릴 승산이 많았다. 교원인적자원 배분의 관점에서 볼 때, 현재의 초빙교사제는 우수한 교사를 좋은 여건의 학교로 모이게끔 하는 인적자원 배분의 불평등을 양산하는 기제로 작용 할 가능성이 크므로 이에 대한 정책적, 제도적 보완책 마련이 이루어져야 한다.

저자들은 당뇨병성 신병증의 자연경과에 맞지 않는 요검사상 이상 소견을 보인 제 1형 당뇨병을 가진 소아에서 신생검을 시행하여 막증식성 사구체 신염을 진단하였기에 보고하는 바이다. Renal diseases other than diabetic nephropathy can occur In diabetic patients. Urinary abnormalities or clinical courses inconsistent with the natural progression of diabetic nephropathy are suggestive of non-diabetic renal diseases and should lead to more extensive investigations. Presence of non-diabetic renal diseases in diabetic patients can alter the treatment plan and the prognosis. We report a 9-year-old girl who had type 1 membranoproliferatiye glomerulonephritis as well as type 1 diabetes mellitus.

Time-lapse imaging of sentinel lymph node using indocyanine green with near-infrared fluorescence imaging in early endometrial cancer

Objective: Indocyanine green with near-infrared fluorescence imaging (NIR-ICG) is a newtracer modality in the limelight used for lymphatic mapping. The advantage of this methodis to provide real-time image during surgery. To use ICG for image guided lymph nodedissection, a surgeon needs to know initial appearing time and duration. Methods: A 52-year-old woman undertook surgery diagnosed with endometrial cancer. Shehad no past medical history and her body mass index was 25.3 kg/m2. Preoperative magneticresonance imaging examination revealed 2.7 cm sized cancerous mass in the endometrialcavity with superficial myometrial invasion without lymph node enlargement. Four mL(1.25 mg/mL) of ICG solution was prepared for injection. For each site, 1 mL of solution wasinjected superficially, 2–3 mm into the cervical submucosa and another 1 mL was injecteddeep, 1–2 cm into the stroma of the cervix [1,2]. We recorded video with 30° 10 mm scopeequipped with a specific lens and light source emitting both visible and NIR light (KARLSTORZ GmbH & Co. KG, Tuttlingen, Germany). Results: Pelvic lymph node was visualized from around 5 minutes. ICG was dispersed intoorgans after hysterectomy (53 minutes after ICG injection), yet we could clearly identifysentinel lymph node (SLN). Pathology revealed endometriod adenocarcinoma grade I,myometrial invasion with less than half of myometrium and no lymph node metastasis. Conclusion: Cervical injection of ICG provides good visualization of SLN from 5 minutes toover an hour. Our film gives an idea about time management to make a plan for surgery andnot to miss SNLs.
