        복부 대동맥류의 임상적 고찰

        Purpose: In patients with small or large aneurysms, the decision for surgical treatment is not so simple. The mortality of ruptured abdominal aortic aneurysm (AAA) is high. This study was designed to retrospectively analyse the clinical characteristics of patients with AAA. Methods: Ninety-one cases of AAA were surgically treated between January 1991 and January 2003 at the Department of Surgery, Chonnam National University Hospital. Patients were divided into 49 elective cases and 42 emergency cases, and retrospective analysed on the basis of age, sex, chief complaints, physical examination, associated diseases, size of aneurysm, diagnostic modalities, operative mortality and causes of death. Results: The initial presentations were mainly palpable masses in the elective cases. On the other hand, in the emergency cases which were ruptured, many patients complained of abdominal or back pain. There was a positive relationship between the size of AAA and the incidence of the rupture in our study, especially in the case of transverse diameters above 10 cm (P<0.001). There was no death in the elective cases, but there were 22 surgical mortalities in the 42 emergency cases (52.3%, P<0.001). Overall surgical mortality was 24.1%. The causes of death were intraoperative and postoperative bleeding (11), myocardial infarction (5), acute renal failure (4), and sepsis (2). Conclusion: Surgical mortality in ruptured AAA was high. Consequently, surgical intervention is recommended and the operation must be performed. In that way we can reduce the operative mortality and improve the treatment outcome.         3세 소아에서 토혈을 주소로 내원한 위식도 탈출증 1례

        저자들은 반복적인 구토와 복통이 발생한 후 토혈을 주소로 내원한 3세 된 남아에서 상부소화관 내시경으로 진단된 위식도 탈출증 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Prolapse gastropathy is not uncommon in adult, but is not reported yet in previously healthy children. A 3-year-old child came to our emergency room after a 1-day history of emesis episodes with coffee-ground hematemesis. During the endoscopic procedure, and the process of retching and vomiting was observed and a tense knuckle of gastric mucosa was seen to be forcefully and repeated prolapsed into the distal esophageal mucosa, and mucosal hematoma was found in the gastric fundus. Upper gastrointestinal study revealed no abnormality and 24 hour pH monitoring revealed no pathologic gastroesophageal reflux. Retching is thought to cause the forceful prolapse and induce subsequent trauma of gastric mucosa. This case illustrates that the episodes of vigorous retching and resultant gastric mucosa are now considered to be the cause of the hematemesis and epigastric pain in children.

        NAAL: 뉴로모픽 아키텍처 추상화 기반 이기종 IoT 기기 제어용 소프트웨어

        Neuromorphic computing generally shows significantly better power, area, and speed performance than neural network computation using CPU and GPU. These characteristics are suitable for resource-constrained IoT environments where energy consumption is important. However, there is a problem in that it is necessary to modify the source code for environment setting and application operation according to heterogeneous IoT devices that support neuromorphic computing. To solve these problems, NAAL was proposed and implemented in this paper. NAAL provides functions necessary for IoT device control and neuromorphic architecture abstraction and inference model operation in various heterogeneous IoT device environments based on common APIs of NAAL. NAAL has the advantage of enabling additional support for new heterogeneous IoT devices and neuromorphic architectures and computing devices in the future. 뉴로모픽 컴퓨팅은 일반적으로 CPU와 GPU를 이용하여 신경망 연산을 하는 것보다 전력, 면적, 속도 측면에서 매우 뛰어난 성능을 보여준다. 이러한 특성은 에너지 사용량이 중요시되는 자원 제약적인 IoT 환경에 매우 적합하다. 하지만 뉴로모픽 컴퓨팅을 지원하는 이기종 IoT 기기에 따라 환경설정 및 응용 프로그램 동작을 위한 소스코드의 수정이 필요하다는 문제점을 가지고 있다. 이러한 문제점을 해결하고자 본 논문에서는 NAAL을 제안하고 구현하였다. NAAL은 공통의 API를 기반으로 다양한 이기종 IoT 기기 환경에서 IoT 기기의 제어와 뉴로모픽 아키텍처의 추상화 및 추론 모델 동작에 필요한 기능을 제공한다. NAAL은 향후 새로운 이기종 IoT 기기 및 뉴로모픽 아키텍처와 컴퓨팅 장치의 추가적인 지원이 가능하다는 장점을 가진다.

        연속적인 서비스를 위한 휴대 인터넷과 cdma2000 이동통신망의 연동 방안

        현재 cdma2000 In 및 1xEV-DO 이동통신망을 통해 패킷 데이터 서비스가 제공되고 있으며, 보다 높은 대역폭의 서비스에 대한 사용자 욕구를 만족시키기 위해 휴대 인터넷 서비스에 대한 표준화가 추진되고 있다. 그런데, 휴대 인터넷은 고속의 데이터 서비스는 가능하나, 서비스 커버리지가 상대적으로 작다. 따라서, cdma2000 이동통신망과 효율적으로 연동하여 서비스를 제공한다면, 사용자는 서비스 지역에 따라 최적의 서비스를 받을 수 있음은 물론, 이동시 연속적이고 끊김 없는 데이터 서비스를 이용할 수 있고, 사업자는 망 구축비용 및 운용비용 절감을 얻을 수 있다. 이를 위해, 본 논문에서는 기존의 cdma2000 이동통신망과 휴대 인터넷의 효율적인 연동망 구조, 각 노드의 동작, 노드간 프로토콜 구조 및 인터페이스 등 세부적인 연동 방안을 제시한다. 본 논문에서 제안된 연동 방안은 밀결합 연동 구조로 인해 통합된 사용자 인증 및 과금을 수행하기 용이하며, 끊김없는 연속적인 서비스를 제공할 수 있다. 또한, 기존 cdma2000 이동통신망의 수정 또는 추가 구현 없이 개발 가능하며, cdma2000 및 휴대 인터넷 서비스를 동시에 제공하는 이중 모드 단말의 구현시에도 복잡도가 최소화된다. cdma2000 이동통신망과 휴대 인터넷간의 핸드오프 성능 모델에 기반한 실험을 수행한 결과 본 논문에서 제안한 방안은 패킷 손실 측면에서 매우 우수함이 검증된다. Nowadays, wireless packet data services are provided over cdma2000 1x/1xEV-DO mobile communication network and Portable Internet are being standardized for users demanding higher data rate services. Portable Internet can provide high data rate services, but its service coverage is relatively small. If Portable Internet may be integrated with cdma2000 mobile networks, users are able to choose the best service according to service areas and get seamless services while they are moving around. At the same time, it is cost-effective for operators to construct and maintain the integrated network. For the purpose of effectively integrating Portable Internet into cdma2000 networks, we propose an integration scheme including network architecture, protocol architecture, functions in network elements, interfaces between them, and call-flow procedures. The integration scheme proposed in this paper adopts a tightly-coupled architecture for unified authentication/accounting and seamless services. In addition, the scheme can be implemented without modifying the existing cdma2000 mobile communication networks. It is also simple to develop the dual-mode mobile station. Through the simulation results based on the performance model for handoffs between cdma2000 and Portable Internet, it has teen validated that the proposed scheme diminishes packet losses compared with the loosely-coupled architecture.

        갑상선 결절의 비수술적 치료

        Thyroid nodules are a common clinical problem. Routine use of neck ultrasonography increase in the frequency of nodular thyroid disease, however, only a small percentage of the nodules currently being detected will prove to be malignant. The clinical importance of thyroid nodules rests with the need to exclude thyroid cancer, which occurs in 5∼10% of cases. Most thyroid nodules are cytologically benign and can be managed nonsurgically. Fine-needle aspiration cytology has a central role in identification of malignant nodules, which are generally treated with surgery. Benign nodules that are completely asymptomatic require follow-up without treatment. Cosmetic problems with or without compressive symptoms may be indications for surgery. When surgery is contraindicated or refused, several nonsurgical approaches are available. These include TSH suppressive therapy, radioactive iodine treatment, ethanol ablations, laser ablation, and radiofrequency ablation. TSH suppressive therapy is the most widely used, however, its clinical efficacy and safety are controversial. When surgery is not available, radioactive iodine therapy is the therapy of choice for treatment of toxic nodules or symptomatic nodular goiters. Ethanol ablation can be used as the first-line therapy for treatment of symptomatic cystic nodules. Radiofrequency ablation is another safe and effective treatment for benign nodules and may also have an effective complementary role in management of recurrent thyroid cancers. Laser photocoagulation should be reserved for selected patients undergoing treatment in experienced centers. Through higher understanding of thyroid and neck anatomy, surgeons should actively participate in various non-surgical managements.

        응급의료전화상담원 교육 과정에 대한 현황조사 및 교육 프로그램 개발을 위한 전문가 조사

        Purpose: We investigated current dispatcher education requirements and proposed how to further improve and develop the EMD education curriculum in Korea. Methods: The contents of EMD training programs from the US were used in the development of the questionnaire used to investigate the status of EMD education in Korea. The Delphi method was used to obtain expert consensus on which specific training objectives to include, how many hours would be required, who was qualified to receive the training, and how to confirm their successful qualification. Results: A consensus of the experts was made for core educational objectives, the appropriate number of education hours, qualifications for trainers, passing marks for the exams, and the format of re-education as follows: The didactic education time requirement was 18 hours and the experiential education time requirement was 10 hours. The target qualification requirement for trainees was EMT level 1, and they were required to have an experience of firstaid as a prerequisite. The minimum annual re-education time was 8 hours (61.1%). The passing mark for examinations was 70.0 (median, IQR: 65.0 - 70.0) for the didactic test and 70.0 (median, Interguatile range [IQR]: 60.0 - 80.0) for the experiential test. The format of the required re-education included both comprehensive and individual forms. Conclusion: We propose the core contents of EMD education should be based on the results of the expert consensus.

        현사시나무(Populus alba × P. glandulosa)에서 분리한 non-specific Lipid Transfer Protein (ns-LTP) 프로모터의 특성 분석

        In order to study genetic engineering in trees, the characterization of genes and promoters from trees is necessary. We isolated the promoter region (867 bp) of Pagns-LTP from poplar (P. alba × P. glandulosa) and characterized its activity in transgenic poplar plants using a β-glucuronidase (GUS) reporter gene. High-level expression of the Pagns-LTP transcript was found in poplar roots, while comparatively low-level expression was found in the young leaves. Pagns-LTP mRNA was not detected in other poplar tissues. Additionally, transgenic poplar plants that contained a Pagns-LTP promoter fused to a GUS reporter gene, displayed tissue-specific GUS enzyme activity localized in root tissue. In silico analysis of the Pagns-LTP promoter sequence reveals the presence of several cis-regulatory elements responsive to phytohormones, biotic and abiotic stresses, as well as those regulating tissue-specific expression. These results demonstrate that the Pagns-LTP promoter has tissue-specific expression activity in poplar roots and leaves that may be involved in organ development and plant resistance to various stresses. Therefore, we anticipate that the Pagns-LTP promoter would be a useful tool to genetically optimize woody plants for functional genomics.
