        Pathology Reporting of Thyroid Core Needle Biopsy: A Proposal of the Korean Endocrine Pathology Thyroid Core Needle Biopsy Study Group

        In recent years throughout Korea, the use of ultrasound-guided core needle biopsy (CNB) has become common for the preoperative diagnosis of thyroid nodules. However, there is no consensus on the pathology reporting system for thyroid CNB. The Korean Endocrine Pathology Thyroid Core Needle Biopsy Study Group held a conference on thyroid CNB pathology and developed guidelines through contributions from the participants. This article discusses the outcome of the discussions that led to a consensus on the pathology reporting of thyroid CNB.

        With the recent development of the ultrahigh-speed optical time division multiplexed system, high-repetition rate optical-pulse stream generation is necessary. This is different from conventional approaches, which use fiber or integrated waveguide delay line circuits. The high-repetition-rate optical-pulse multiplication phenomenon occurs when the optical pulse's spectral width is greater than the transfer bandwidth of the coupler used. From the analysis, the output repetition rate can be controlled by using fiber couplers with different equivalent transfer bandwidths. The pulse seperation spacing is controlled by number of cascaded coupler in optical loop mirror coupler scheme.

        위암에서 예후인자로서 점액소 표현형과 CDX2 단백 발현의 역할

        Background : Mucin phenotypic markers and CDX2 are widely expressed in gastric carcinomas, however, recent studies have produced conflicting results regarding whether the expression patterns of these markers have clinicopathologic significance. Methods : We examined samples from 217 gastric carcinoma patients immunohistochemically to determine if the expression of mucin phenotypic markers and CDX2 was correlated with postoperative survival and other clinicopathologic factors. Results : All tumors were phenotypically classified as gastric (type G, 81 cases), gastric and intestinal mixed (type GI, 55 cases), intestinal (type I, 43 cases), or unclassified (type U, 38 cases). The occurrence of type G and GI tumors was positively correlated with tumor progression whereas that of type U tumors was negatively correlated with tumor progression. CDX2 expression was correlated with type I tumors. Tumors that expressed MUC5AC or MUC6 had a better prognosis than those that did not. When the relationship between phenotype and prognosis was considered, type GI had the best prognosis, followed by type G, then type U. Conclusions : The mucin phenotypic markers may be useful for predicting tumor progression and survival in patients with gastric carcinomas. Additionally, CDX2 may play an important role in gastric carcinogenesis of type I tumors.

        DWDM을 위한 광필터의 구조

        최근 광통신 용량은 VoIP, AUdio/Video 스트리밍의 멀티미디어 서비스 수요로 인한 정보통신 용량의 증가에 비례하여 증가하고 있다. 초고속 광통신망에서 추가적인 광섬유망과 고속장비 없이 이를 해결하기 위한 방법으로 DWDM(Dense Wavelength Division Multiplexing) 기법이 대두되고 있다. 따라서, 본 논문에서는 이러한 기능을 갖는 광필터를 설계하기 위하여 광섬유 한쪽의 클래드를 연마하여 다층 슬래브 도파로에 결합시킨 형태의 광섬유/다층 슬래브 결합구조의 광필터를 제안하였다. 제안된 광필터는 $1.5{\mu}m$ 통신창에서 분리간격이 $4.15{\mu}m$일 때 65nm의 편광 독립성, TM 모드와 TE 모드에 대해 중심파장이 각각 ${\lambda}_0=1.54946\;{\mu}m$와 ${\lambda}_0=1.6144\;{\mu}m$일 때 0.1 nm의 FWHM(Full Width at Half Maximun) 특성을 가진다. The recent, a large capacity of communication is required in order to it is in proportion to capacity of information communication increase and to satisfy a demand because of the demand about Internet, a multimedia service of Video of internet protocol(VoIP), Audio/Video steaming. DWDM(Dense Wavelength Division Multiplexing) technique has been emerged as the method of solving it without additional optical fiber network building and hish-speed equipment. Therefore this thesis proposed the optical filter of fiber/multilayer slab coupled structure combining it to multilayer slab waveguide by polishing the cladding of one side of fiber to design the optical filter having these functions. The optical filter proposed as the simulation result satisfies DWDM filter characteristic; 1) when the separation distance between fiber and slab is $4.15{\mu}m$ at the communication window of $1.5{\mu}m$, the polarization independence is 65nm, 2) when the center wavelengths about TM mode and TE mode are each ${\lambda}_0=1.54946\;{\mu}m$ and ${\lambda}_0=1.6144\;{\mu}m$ and, FWHM(Full at Half Maximun) is 0.1nm.

      • 대통령의 국가위기관리 리더십에 관한 연구 - 김영삼, 김대중, 노무현을 중심으로 -

        When The National crisis happen to, The President’s role, especially crisis management leadership is very important. Because State subsistence and reduce demage or not depend on the chief of state’s decision-making. Nevertheless, there were few research our country. So I try to analysis about three pre- President’s National Crisis Management leadership. Accordingly I established a model to analyze how much President display his crisis management leadership in national crisis. The model consist of four elements; president’s character, state system operating style, decision-making style, and power control method. I compared three pre-President with each other in three example. As a result of case stud, I derive from national crisis management leadership style as follows. First, Kim, Young-Sam’s crisis management leadership is activepositive, radial State system operation, centralized power control, and authoritative decision-making style. Second, Kim, Dae-Jung’s crisis management leadership is active-negative, cone type State system operation, centralized power control, and authoritative decision-making style. His leadership pattern is similar to Kim young-sam in power control type and decision-making style. Lastly, Rho, Moo-Hyun’s crisis management leadership is active-positive, parallel state system operation, decentralized power control, and democratic decision-making style. As we know, his crisis leadership is unique but undesirable and improper style for confront South with North Korea. Conclusionally I would like to propose the effective method of the National crisis Management leadership. First, crisis management organization must be realignment in oder to display the integrated function. Second, to bring up a professional person for crisis management. Third, future political leaders must be trying to learn advanced crisis intelligence & information, tactics, and strategy etc. Last, various crisis management ability should be procured in political, military, diplomatic field because of to prepare for war time operational control transition in April, 2012. 국가위기사태 발생시 대통령의 위기관리 리더십의 발휘여부는 국가존망과 직결 되어 있기 때문에 매우 중요하다. 그러므로 전문적인 지식과 자질을 갖춘 대통령이 존재한다면 더없이 바람직하겠지만, 결코 쉬운 과제가 아닐 것이다. 본고에서는 사례분석을 통해 3명의 전임 대통령들의 국가위기관리 리더십을 살펴보았다. 먼저, 김영삼 대통령은 과업 지향적이고 성취욕이 강해 국정운영에 적극적이었으나 즉흥적이고 권위적인 행태로 인해 체계적인 국가위기관리를 했다고 보기엔 미흡한 부분이 있다. 둘째, 김대중 대통령은 야심이 많고 공격적이며 권력지향적의로 매사를 치밀하고 꼼꼼하며 집행하는 성격에도 불구하고 국가위기발생시 정치적 이유로 전면에 나서 위기관리를 하기보다 국방장관 등에게 위임하고 최종의사 결정만 하는 간접적인 국가위기관리 리더십을 발휘하였다. 한편 노무현 대통령은 이전 대통령과 달리 분권적 권력통제와 수평적 국가기관운영 그리고 민주적 의사 결정을 통한 위기관리 리더십을 발휘하려고 했으나, 위기발생시 컨트롤 타워 (control tower) 부재로 신속한 상황판단과 의사결정 그리고 적극적인 대응 측면에서 다소 아쉬운 국가위기관리 리더십을 발휘했다고 분석되었다. 그러나 이들 대통령들은 국가안보와 국민의 안전한 삶을 담보하기 위해 처해진 상황에서 지도자로서 헌신적으로 노력하였음을 간과해서는 않될 것이다.

        갑상선 세침흡인세포검사에서 소포병변 및 소포종양 진단의 딜레마

        Fine needle aspiration cytology (FNAC) of thyroid nodules is the best screening test for the selection of patients that may require surgical management. However, the diagnosis of follicular neoplasm on FNAC remains a gray area and the main differential diagnosis of follicular neoplasm includes benign (nodular hyperplasia and follicular adenoma) and malignant (follicular carcinoma and follicular variant of papillary carcinoma) lesions. The cytologic diagnosis of follicular neoplasm is based on the high cellularity, microfollicular or trabecular pattern, overlapping follicular cells, and scanty or absent colloid. Histologically, the diagnosis of follicular carcinoma requires the presence of tumor capsular invasion or vascular invasion. In the follicular variant of papillary carcinoma, nuclear features of papillary carcinoma may be subtle and seen in only a small number of cells, and thus may not be readily appreciated. Analyses of BRAF and RAS point mutations and RET/PTC and PAX8/PPARγ rearrangements have been reported to be a useful ancillary tool for diagnosing thyroid cancer in cytology specimens. The presence of BRAF or RET/PTC mutation is a strong indicator of papillary carcinoma. PAX8/PPARγ rearrangement is exclusively found in follicular carcinoma. RAS mutations are found in follicular adenoma/carcinoma and follicular variant of papillary carcinoma. Therefore, molecular testing of FNAC samples can improve the accuracy of cytologic diagnosis.
