Rental circulatory and tubular function impairments are frequent in advanced liver cirrhosis. Renin catalyses the production of angiotensin which promote the secretion of aldosterone. Plasma renin activity is often increased in patients with cirrhosis, particularly those with ascites, but interrelationships between those have not yet been precisely definded. For the purpose of evaluating renin-angiotensin aldosterone axis, 24 hour urine sodium, creatinine clearance, plasma renin activity, and plasma aldosterone by radioimmunoassay kit were measured in patients with liver cirrhosis with ascites and normal control group who were admitted to Chosun University hostpital. The results are summarized as following : 1. The laboratory findings in cirrhotic patients with mild ascites revealed significant reduction of 24 hour urine sodium (81=25mEq)(P<0.01) and increase of PRA, plasma aldosterone levels (3.82±1.75 ng/ml/hour, 157.83±13.27 pg/ml)(P<0.01, P<0.01) as compared to those of normal controls. 2. The levels of 24 hour urine sodium, PRA and plasma aldosterone were 31±15, 6.92±2.32, and 250.61±35.32 respectively in cirrhotic patients with severe ascites. There were significant decrease in 24 hour urine sodium (P<0.005), increase in PRA and plasma aldostorone (P<0.005, P<0.01) as compared to normal controls and cirrhotic patients with mild ascites. 3. PRA was related to plasma aldosterone (r=0.87, P<0.001) but inversely related to creatinine clearance (r=-0.73, P<0.005)
백서 폐 Submitochondrial Particles의 Superoxide 생성에 대한 Beomycin 효과

Bleomycin is used clinically against various tumors, but diffuse interstitial pulmonary fibrosis has been recognized as a severe complication. Ferrous iron and bleomycin in the presence of reducing agents stimulate DNA degradation. This process causes the reduction of oxygen and the production of toxic oxygen metabolites, which may play a role in the ability of this drug to break down DNA. We studied the effect of bleomycin on superoxide production, as well as enzyme activities related to toxic oxygen metabolites in lung submitochondrial particles and the mitochondria of rats, respectively. The results were as follows; 1) In the saline-pretreated group, the amount of superoxide produced and the ratio of superoxide production to NADH consumption increased by 12.5% and 76.6% compared with basal levels at 2㎍/㎖ and 5㎍/㎖ of the added bleomycin concentration, respectively. 2) In the bleomycin-pretreated group, the amount of superoxide produced and the ratio of superoxide produaction to NAHD consumption increased by 18.2% and 238.7% compared with basal levels at 2㎍/㎖ and 10㎍/㎖ of the added bleomycin concentration, respectively. 3) Superoxide production in the bleomycin-pretreated group was elevated by 37.5% at basal level, and by 44. 4% at 2㎍/㎖ of the added bleomycin concentrations compared with the saline-pretreated group. 4) The cytosolic enzyme activities of superoxide dismutase, catalase, and peroxidase in the lung of bleomycin-pretreated rats increased by 125%(p<0.001), 54%(p<0.01) and 78%(p<0.01) compared with the saline-pretreated groups, respectively. Of the mitochondrial enzyme activities, superoxide dismutase increased by 203%(p<0.001), catalase decreased by 17%(p<0.01) and peroxidase increased by 7% compared with the saline-pretreated groups, respectively. The above findings suggest that superoxide production is stimulated by bleomycin administration in the mitochondria of rat lungs.

Malignant histiocytosis, originally described in 1939 as histiocytic medullary reticulosis by Scott and Robb-Smith, is a rare histiocytic proliferative disorder that often shows an acute onset and used to progress to death within a few months. This disorder characterized clinically by fever, generalized weakness, lymphadenopathy and hepatosplenomegaly, and shows a variable range of pancytopenia. Extranodal involvement is common, with an incidence ranging from 50% to as high as 90%, skin involves8ment was noted in 10% to 15%. Typical skin lesions were mainly founded in extremity. i.e. erythematous papule and nodule occasionally become to necrosis and ulceration. We experienced one case of malignant histiocytosis in a 46-years-old female. The major clinical findings are fever, malaise, hepatosplenomegaly and erythematous skin lesion. In the laboratory study, pancytopenia is noted on the peripheral blood. And also aggregation of many atypical histiocytes were shown on skin and bone marrow biopsy. So we reported one related case with malignant histiocytosis as well as reviewing literature .
