        Subarachnoid hematoma after spinal anesthesia - A case report -

        Spinal subarachnoid hematoma is a very rare complication of spinal anesthesia. This complication can, and is, often overlooked and dismissed as a fatal neurological consequence of (what amounts to) delayed diagnosis. In this case, a 59-year-old female patient with no specific medical history underwent right knee arthroscopy under spinal anesthesia. The arthroscopic surgery concluded without complications but, on the first postoperative day, the patient complained of lower back pain, headache, nausea, vomiting. On the fifth postoperative day, magnetic resonance imaging was taken and it revealed evidence of a subarachnoid hematoma involving the L3 and L4 vertebral levels. Hematoma evacuation was performed, and the patient recovered without sequelae. Here, we report this case that lumbar spinal subarachnoid hematoma was found five days after spinal anesthesia which was done in a patient without coagulopathy

        No-Touch Radiofrequency Ablation: A Comparison of Switching Bipolar and Switching Monopolar Ablation in Ex Vivo Bovine Liver

        Objective: To evaluate the feasibility, efficiency, and safety of no-touch switching bipolar (SB) and switching monopolar (SM) radiofrequency ablation (RFA) using ex vivo bovine livers. Materials and Methods: A pork loin cube was inserted as a tumor mimicker in the bovine liver block; RFA was performed using the no-touch technique in the SM (group A1; 10 minutes, n = 10, group A2; 15 minutes, n = 10) and SB (group B; 10 minutes, n = 10) modes. The groups were compared based on the creation of confluent necrosis with sufficient safety margins, the dimensions, and distance between the electrode and ablation zone margin (DEM). To evaluate safety, small bowel loops were placed above the liver surface and 30 additional ablations were performed in the same groups. Results: Confluent necroses with sufficient safety margins were created in all specimens. SM RFA created significantly larger volumes of ablation compared to SB RFA (all p < 0.001). The DEM of group B was significantly lower than those of groups A1 and A2 (all p < 0.001). Although thermal injury to the small bowel was noted in 90%, 100%, and 30% of the cases in groups A1, A2, and B, respectively, full depth injury was noted only in 60% of group A2 cases. Conclusion: The no-touch RFA technique is feasible in both the SB and SM modes; however, SB RFA appears to be more advantageous compared to SM RFA in the creation of an ablation zone while avoiding the unnecessary creation of an adjacent parenchymal ablation zone or adjacent small bowel injuries.

        천칠봉 풍경화의 양식적 특성 연구: 비원파 활동을 중심으로

        Biwon-pa is the name of a painting group comprising Cheon Chilbong, Byun Siji, and Son Eungsung, who painted landscape paintings of Korean traditional palaces during the 1960s. Cheon persistently pursued landscape paintings through his artistic life, and his stylistic property lies on the painter’s voluntariness of perception. His use of exaggerated bright colors can be also thought to stem from the locality discourse which began during the Japanese colonial period. The significance of Cheon’s landscape paintings has parallels with eighteenth-century landscape painting in England. We might call his paintings “Korean picturesque” in that the artist sought to express the value of Korea in the process of modernization. 천칠봉이 활동했던 ‘비원파’는 1960년대에 전통 궁궐의 풍경 중에서도 특히 창덕궁의 후원인 비원을 작품의 주요 소재로 삼아 사생을 기초로 한 풍경화를 집중적으로 그렸던 변시지와 손응성을 함께 일컫는 명칭이다. 이들 중에서 천칠봉의 풍경화가 갖는 조형적, 양식적 특성은 사생을 통한 풍경임에도 외부 세계의 지각에 따라 자의적이고 주관적인 특성이 강하게 나타날 수밖에 없다는 점과 그러한 자발성이 구도의 문제로 양식화되어 드러냈다는 점에서 의의를 찾을 수 있다. 그리고 지나치게 화려하거나 밝은 그의 색채는 우리 문화의 시각성을 청명함과 광휘에서 찾고자 했던 향토색 논쟁과의 연관성 속에서 규명해보고자 했다. 한국 근현대 미술의 풍경화 전개 과정에서 비원파와 천칠봉이 갖는 의의는 한국적 ‘픽처레스크(Picturesque)’라고 불릴만한 것이다. 픽처레스크 풍경이 18세기 영국에서 본격화되어 산업혁명 시기를 거치면서 형성된 소박한 자연주의적 특성을 반영했듯이, 천칠봉의 비원파 시기 풍경화는 대한민국의 현대화 과정에서 자연적 요소를 간직한 궁궐의 정원에서 풍경의 소재로 삼아 전통의 가치를 지키며 정신적 위로를 찾고자 했던 데에 있는 것은 아니었을까 짐작해본다.

        A portable high-intensity focused ultrasound system for the pancreas with 3D electronic steering: a preclinical study in a swine model

        Purpose: The aim of this animal study was to evaluate the safety and feasibility of a portable,ultrasonography-guided, high-intensity focused ultrasound (USg-HIFU) system to treat thepancreas. Methods: Eight swine were included. Using a portable HIFU device (ALPIUS 900, AlpinionMedical Systems), ablations were performed on the pancreas in vivo. Different acousticintensities were applied (1.7 kW/cm2or 1.5 kW/cm2, n=2 [group A for a pilot study]; 1.5 kW/cm2, n=3 [group B]; and 1.2 kW/cm2, n=3 [group C]). Magnetic resonance imaging (MRI) wasperformed immediately (group A) or 7 days (groups B and C) after HIFU treatment. In groups Band C, serum amylase and lipase levels were measured on days 0 and 7, and performance statuswas observed every day. Necropsy was performed on days 0 (group A) or 7 (groups B and C) toassess the presence of unintended injuries and to obtain pancreatic and peripancreatic tissue forhistological analysis. Results: Ablation was noted in the pancreas in all swine on MRI, and all pathologic specimensshowed coagulation necrosis in the treated area. The mean ablation areas on MRI were85.3±38.1 mm2, 90.7±21.2 mm2, and 54.4±30.6 mm2in groups A, B, and C, respectively(P>0.05). No animals showed evidence of complications, except for one case of a pseudocyst ingroup B. Conclusion: This study showed that pancreas ablation using a portable USg-HIFU system may besafe and feasible, and that coagulation necrosis of the pancreas was successfully achieved with arange of acoustic intensities.

        한국인 여성 노인의 단백질 섭취 수준과 근력의 상관성 연구: 국민건강영양조사 제 7기(2016-2018년) 자료를 이용하여

        Objectives: Decreasing muscle strength in old age has become a significant health problem because it increases the risk of falls or fractures and transfers to other diseases. The precise role of dietary protein intake in preventing or reducing muscle weakness is unclear. This study examined the relationship between handgrip strength and protein intake in Korean female elderly. Methods: This was a cross-sectional study that used data from the Seventh Korean National Health and Nutrition Examination Surveys (KNHANES) on female subjects aged 65 years and older. Low handgrip strength (LHGS) was defined as a handgrip strength below than 18kg. Dietary intake data were obtained using the 1-day 24-hour recall method. Multiple regression was performed to test whether there is an independent relationship between the grip strength and protein intake, and the association between protein intake and LHGS was confirmed through multiple logistic regression. Results: The mean age of the 2,083 elderly females was 73.3 ± 0.1 years, and the prevalence of LHGS was 35% (n=734). Elderly women with an LHGS consumed less energy, total protein, and animal-based protein than those in the normal group. A multiple regression analysis after adjusting for covariate revealed a significant positive association between the handgrip strength and energy, protein, and animal-based protein intake. Multiple logistic regression analysis showed that the odds ratio (OR) of LHGS in female elderly with the highest quartiles of consumption of energy [OR, 0.65; 95% confidence interval (CI), 0.430.82; P for trend=0.004], and animal-based protein [OR, 0.59; CI, 0.40-0.87; P for trend= 0.037] were significantly lower than those in the lowest quartiles. Conclusions: The energy intake and animal-based protein intake were negatively associated with the LHGS. These results suggest that adequate energy intake and protein intake, particularly those from animal-based sources, for elderly women in Korea are beneficial in lowering the risk of LHGS.
