DIY 3 (DIY 3) : Itch treatments: new perspectives and insights from recent trials

tch represents one of the most bothersome symptoms in allergic disorders and numerous dermatological and systemic diseases. In recent years, pruritus has gained more and more attention and is appreciated as an important symptom that can dramatically reduce quality of life in patients. This new focus on itch as a specific target has led to the development of novel drugs that are currently tested in clinical trials or which are currently under development. Among them are for example histamine H4 receptor antagonists and κ-opioid agonists. Furthermore, some drugs have been reported in case reports or small case series to be highly effective in suppressing pruritus. For example, the neurokinin 1-receptor antagonist was shown to be able to control almost completely the severe and treatment refractory pruritus in patients with Sezary syndrome and prurigo nodularis. In addition, IL-31 inhibitors were shown to have a pivotal role as the potential treatment candidate of severe itch and chronic dermatitis. However, more randomized controlled trials have to be performed to test the potential of these substances to be used as novel antipruritic drugs. This presentation will summarize novel key players in the pathophysiology of itch with subsequent introduction of promising, novel and experimental treatment strategies.
Vitamin or Its Derivatives and Skin Barriers

The skin barrier plays key roles in immune surveillance and homeostasis, and in preventing penetration of microbial products and allergens. Common inflammatory skin disorders such as atopic dermatitis and psoriasis exhibit decreased barrier function, and recent studies suggest that the complex response of epidermal cells to barrier disruption may aggravate, maintain, or even initiate such conditions. Either aiding barrier reestablishment or dampening the epidermal stress response may improve the treatment of these disorders. Nutrition is one of many factors needed for the maintenance of skin homeostasis. Poor nutritional status alters the structural deformation and skin barrier integrity, which in turn leads to an impaired biological function of the skin. The importance of micronutrients (such as certain vitamins and minerals) to skin health has been emphasized in vitro and in vivo. These micronutrients are not only active compounds of treatment to treat certain skin diseases such as atopic dermatitis and psoriasis but are also used as ingredients for cosmetics. The goal of this review is to discuss the potential roles and current knowledge of selected micronutrients (vitamin and its derivatives) in skin health and cutaneous barrier function.

The clinical course of alopecia areata (AA) is unpredictable. Up to 50% of patients will recover within a year without treatment. However, the majority of AA patients will show an irregular relapsing-remitting course, with multiple episodes of hair loss. The major prognostic factors with clinical relevance include the extent and duration of disease. Many therapeutic options exist for AA and the recommended treatment strategies are based on the age of the patient and the extent of disease. Therapeutic modalities include topical minoxidil, topical and intralesional corticosteroids for mild AA, and diphenylcyclopropenone (DPCP) for severe AA. Unfortunately, to date, there is no approved cure that provides complete hair regrowth, and all treatments are considered “off-label”. For many reasons, including a high rate of spontaneous remission and paucity of randomized-controlled studies investigating treatment efficacy, it is difficult to assess therapeutic effects for AA. Moreover, long-term follow-up is not reported in many of the publications. Another important drawback is the great variation in treatment outcome evaluation. To minimize this factor, the Severity of Alopecia Tool (SALT) score has been used as a method for measuring the extent of scalp AA. Recently, we introduced the Alopecia Areata Progression Index (AAPI) which seems to represent a system capable of quantifying overall hair loss activity in AA patients with different severity. In this review, we discuss the clinical course, prognosis, evaluation of treatment, and measuring tools of AA. (Korean J Dermatol 2018;56(1):10∼16)

Self-regression of Kaposi`s sarcoma is rare with the exception of the iatrogenic immunosuppressive type. No cases of classic type Kaposi`s sarcoma showing spontaneous regression have been reported in the Korean dermatologic literature. We report a case of classic type of Kaposi`s sarcoma in a 69-year-old man which followed a clinical course of spontaneous regression. He presented with multiple asymptomatic beefy red papules and nodules scattered on both cheeks and upper extremities during a state of increase in the number of skin lesions at the initial visit. He had no history of immunosuppressive treatment and the laboratory findings were unremarkable. An anti-HIV antibody test was negative. A histopathologic examination revealed typical findings of Kaposi`s sarcoma and positive staining with CD31 and D2-40. HHV-8 DNA was detected in a biopsy specimen. Without any specific treatment, the skin lesions healed without leaving any complications after 15 months, and did not show any evidence of recurrence during the ensuing 5 years.

본 논문에서는 귀의 형태와 색을 통해서 질병을 자가진단 후 귀의 특정 부위를 자극하는 이침요법을 위한 시술 보조 시스템을 제안한다. 제안 시스템은 피시술자의 귀의 이미지 정보와 질병에 대한 정보를 처리하여 이침을 위한 혈자리를 귀 이미지에 표시해 주는 시스템을 구현하였다. 특히 귀를 인식하는 부분에 있어서, Haar-like feature와 Adaboost알고리즘을 사용하는 OpenCV내의 함수를 사용하였고 인식된 귀영역을 그리드 영역으로 나누고 질병에 대한 사전 정보에 따라서 그리드 영역내의 이침혈 자리 시스템을 표시하는 시스템으로 구성하였다.
