      • 섬유근육통 환자들에서 우울 증상과 심장 박동수 변이

        배경: 섬유근육통은 우울과 흔하게 관련되어 있다. 우울은 관상동맥 심장질환을 가진 환자에게 위험 인자이다. 낮은 심장 박동수 변이는 과도한 교감 신경 또 는 부적절한 부교감 신경 긴장도를 반영한다. 증가한 교감 신경 또는 감소한 부교감 신경계 활성도는 환자들을 관상동맥 심장질환에 취약하게 만든다. 섬유근육통에서 우울 증상이 관상동맥 심장 질환을 증가시키는 위험 인자인 심장 박동수 변이 감소와 연관이 있는가에 대해서는 거의 알려진 바가 없다. 목적: 섬유 근육통 환자에게서 우울과 심장 박동수 변이의 관련성을 조사하기 위함이다. 방법: 24-73세 사이 섬유근육통 환자 130명의 5분간 EC 를 power spectral analysis로 분석하였다. 모든 대상자는 한국식 섬유근육통 진단 설문지(Fibromyalgia Impact Questionnaire; FIQ) 및 Beck 우울 척도(Beck Depression Inventory; BDI)를 이용하여 평가되었다. Beck 우울 척도(BDI)는 3개의 그룹: low (1-16, N=30), moderate (17-30, N=44), 그리고 severe (31 이상, N= 56)로 분류하였다. Very low frequency (VLF) power (0.0033-0.04 Hz/ms2)는 교감 신경과 부교감 신경 자극에 영향을 받으며 low frequency (LF) (0.04-0.15 Hz/ms2)는 교감 신경 및 부교감 신경의 긴장도 모두의 영향을 받고 혈압 조절과도 강하게 연관되어 있다 high frequency (HF) power (0.15-0.40 Hz/ms2)는 호흡으로 조절되고, 주로 미주신경 긴장도를 반영한다. 결과: one way ANOVA의 결과는 BDI 군들 사이에 로그(log) 전환된 high Frequency (HF) 수준에 차이가 있다는 것을 나타낸다[F(2, 127)=3.89, p=.002]. Turkey B test를 이용한 post-hoc 비교는 severe BDI 점수 군(3.9+/-1.2)은 다른 군들 에 비해 HF를 현저히 낮춘다는 것이 드러났다. Low BDI 점수 군(4.7+/-1.3)은 moderate BDI 점수 군(4.5+/-1.3)과 크게 차이가 나지 않았다. 섬유근육통 환자들에게서 Very low frequency (VLF)와 Low frequency (LF)는 현저한 차이가 없었다. BDI 점수는 InHF (r=-0.23, p<0.01)에서는 음의 상관관계를 보 이며 나이 보정된 BDI 점수의 부분 상관 계수는 -0.21이었다(p=0.01). 결론: 섬유근육통 환자들에게서 BDI 점수는 InHF와 음의 상 관계가 있고 severe BDI 점수 군은 다른 BDI 점수 군보다 현저히 HF를 낮추는 것으로 보인다. 감소한 미주 신경 긴장도는 섬유근육통 환자들의 우울 증 과 연관이 있다.

        Response of Vancomycin according to Steroid Dosage in Pediatric Patients with Culture-Proven Bacterial Meningitis

        Background: This study aimed to evaluate the efficacy of combined vancomycin and steroid therapy for the treatment of culture-proven bacterial meningitis in pediatric patients. Materials and Methods: We identified a total of 86 pediatric patients with culture-positive cerebrospinal fluid who were treated at our facility between 2005 and 2015. Ten of these patients (5 boys and 5 girls) received first-line treatment with vancomycin as the initial form of therapy. All cultured bacteria were sensitive to vancomycin. We retrospectively analyzed these cases to examine the relationship between concomitant steroid dosage and antibiotic treatment effectiveness. Results: Nine of the 10 patients included in our analysis received steroid treatment. Of these, 3 received high-dose steroid therapy and 6 received low-dose steroid therapy. Five patients did not respond to vancomycin, including all 3 patients in the high-dose steroid group and 2 patients in the low-dose steroid group. Our analysis confirmed that the response rate to vancomycin treatment was significantly reduced in accordance with steroid dosage (P = 0.035). Patients who did not to respond to vancomycin with concomitant high-dose steroid administration improved clinically after the substitution of vancomycin with teicoplanin. Conclusion: The use of steroids, especially in high doses, may impair the effectiveness of vancomycin for treating bacterial meningitis in pediatric patients. Physicians should be cautious when administering concomitant steroid therapy and should carefully monitor the steroid dosage.

        Clinical and Pathological Findings of Renal Biopsy in Children: Outcomes from a Single Center Over 27 Years

        Purpose: To classify the results of renal biopsy in pediatric patients and to compare pathological findings with clinical features. Methods: This study included data of 318 children who underwent renal biopsy at our hospital between December 1987 and November 2014. Biopsy specimens were examined histopathologically using light, immunofluorescence, and electron microscopy. Results: Asymptomatic urinary abnormalities was the most common clinical diagnosis (35.9%), followed by nephrotic syndrome (29.3%), and acute glomerulonephritis (18.0%). Glomerular disease was identified in 98.1% of the renal biopsy specimens. The most common primary cause of glomerulonephritis was IgA nephropathy, with gross hematuria in 61.9% of the patients, hypertension in 14.2%, proteinuria >1.0 gm/24-hr in 33.3%, and impaired renal function in 3.6% patients. Conclusion: The most common clinical diagnosis was asymptomatic urinary abnormalities, with primary glomerular disease being the most common renal biopsy finding, and IgA nephropathy the most common histopathological lesion. This study provides a 27-year overview of pediatric renal disease at our center and underlines the importance of renal biopsy for accurate diagnosis and proper management.

      • TGF-β1에 의해 유도되는 세포사멸 신호전달 과정에서 Rac1 GTPase의 역할 규명

          Transforming growth factor (TGF) β1 is a potent inducer of apoptosis in the liver. Recently, c-jun NH₂-terminal kinase (JNK) was shown to be involved in signal transduction in response to TGF-β1. However, the detailed mechanism of TGF-β1-mediated signaling of apoptosis remains largely unknown. In the present study, we investigated the role of Rac1, a member of Rho family GTPases, in the TGF-β1-induced apoptosis of FaO hepatoma cells. Our results showed that TGF-β1-induced apoptosis detected by propidium iodide staining and caspase activation, was significantly blocked in FaO-Rac1N17 cells expressing a dominant negative Rac1 mutant, suggesting a crucial role for Rac1 in TGF-β1 signaling to apoptosis. In addition, we found that TGF-β 1 induced the activation of JNK in a Rac-dependent manner. It thus appears that Rac1 acts upstream of JNK, and that a Rac1-JNK -dependent cascade mediates the TGF-β1-induced apoptosis of FaO cells.

        The Role of High Mobility Group Box 1 in Innate Immunity

        With growing accounts of inflammatory diseases such as sepsis, greater understandingthe immune system and the mechanisms of cellular immunity have become primaryobjectives in immunology studies. High mobility group box 1 (HMGB1) is a ubiquitous nuclear protein that is implicated in various aspects of the innate immune system as a damage-associated molecular pattern molecule and a late mediator of inflammation, as well as in principal cellular processes, such as autophagy and apoptosis. HMGB1 functions in the nucleus as a DNA chaperone; however, it exhibits cytokine-like activity when secreted by injurious or infectious stimuli. Extracellular HMGB1 acts through specific receptors to promote activation of the NF-κB signalingpathway, leading to production of cytokines and chemokines. These findings furtherimplicate HMGB1 in lethal inflammatory diseases as a crucial regulator of inflammatory,injurious, and infectious responses. In this paper, we summarize the role of HMGB1 in inflammatory and non-inflammatory states and assess potential therapeuticapproaches targeting HMGB1 in inflammatory diseases.

        Complete genome sequence of Variovorax sp. PMC12, a plant growth-promoting bacterium conferring multiple stress resistance in plants

        본 연구에서 생물 및 비생물학적 스트레스에 내성을 유도하는 식물 생육 촉진 세균인 Variovarx sp. PMC12 균주의 유전체 염기서열을 분석하였다. PMC12 균주의 유전체는 5,873,297 bp와 1,141,940 bp 크기의 원형 염색체 2개로 구성되었다. 총 6,436개 단백질 유전자, rRNA 9개, tRNA 64개, ncRNA 3개와 유사유전자 80개가 확인되었다. 유전체상에서 발견된 1-aminocyclopropane-1-carboxylate (ACC) deaminase, 항산화 활성, 인산 가용화, 프롤린 생합성, 시드로포어 생합성과 관련된 유전자들은 PMC12 균주가 염, 온도, 병원균에 대한 스트레스에 대한 식물의 내성 유도와 관련되어 있을 것으로 판단된다. Variovorax sp. PMC12 is a rhizobacterium isolated from tomato rhizosphere and enhanced the plant resistance to abiotic and biotic stresses. Here we present the complete genome sequence of strain PMC12. The genome is comprised of two circular chromosomes harboring 5,873,297 bp and 1,141,940 bp, respectively. A total of 6,436 protein-coding genes, 9 rRNAs, 64 tRNAs, 3 ncRNAs, and 80 pseudogenes were identified. We found genes involved in 1-aminocyclopropane-1-carboxylate (ACC) deaminase, antioxidant activity, phosphate solubilization, and biosynthesis of proline and siderophore. Those genes may be related to capability of improving plant resistance to various stresses including salinity, cold temperature, and phytopathogen.
