        각막 간질내 공기 주입과 평형염액 주입이 간질세포 자멸사에 미치는 영향 비교

        Purpose: To evaluate the effects of intrastromal air injection and intrastromal balanced salt solution (BSS) injection on corneal keratocyte apoptosis. Methods: Twelve right eyes of New Zealand White rabbits were divided into an air-injected group (n=6) and a hydro-injected group (n=6). Contralateral eyes served as a control. Air or Balanced salt solution (BSS?, Alcon, USA) was injected into the deep corneal stroma at the paracentral area to propagate into nearly the entire cornea. To reduce the intraocular pressure, anterior chamber paracentesis was performed. The animals were sacrificed 4 hours (n=6) and 24 hours (n=6) after surgery. Central cornea buttons were retrieved to stain with Hematoxylin & Eosin and TUNEL (Apoptag?, Chemicon). The mean number of apoptotic keratocytes was counted in 24.67±4.04 consecutive high power field (HPF). Results: The mean number of TUNEL-positive cells at 4 hours was 12.85±7.25/HPF and 0.25±0.44/HPF in air-injected and hydro-injected eyes, respectively. It was reduced to 6.25±4.02/HPF and 0.15±0.37/HPF in air-injected and hydro-injected eyes after 24 hours. The air-injected group showed significantly more TUNEL-positive cells compared with the hydro-injected or control group until 24 hours (p=0.001, p=0.001, Mann-Whitney U test). Conclusions: Intrastromal air injection induces significant apoptosis of keratocytes suggesting some damages in the peripheral cornea when used in deep lamellar keratoplasty.

        대전천 수계의 장기 모니터링 (2004$\sim$2007)에 의한 물리적, 화학적, 생물학적 평가 변수 분석

        본 연구에서는 대전광역시를 관통하는 도심하천인 대전천의 4개 지점을 선정하여, 2004년 8월에서 2007년 10월까지 총 8차례에 걸쳐 생물학적 건강도 평가 및 물리적 서식지 평가를 실시하였고, 환경부 수질측정 망 지점자료를 이용하여 화학적 수질특성을 분석하였다. 물리적 서식지 평가 지수(QUEI)는 평균 78.2(범위: 47$\sim$124)로 "보통상태"(Fair)로 나타났다. QUEI 값은 최상류지점(S1)에서 최대치(112, "양호상태")를 보인 반면, S3에서 SS(보통$\sim$악화상태)로 가장 불량한 서식지 상태를 보였는데, 이는 심각한 하상구조의 교란, 하상도로 및 인위적활동에 의한 것으로 사료되었다. 화학적 수질변수들은 하류로 갈수록 점차 악화되는 것으로 나타나 하천주변 도심으로부터 유입되는 유기물 오염에 의한 것으로 사료되었다. 또한 TP 및 전기전도도(Conductivity)는 집중강우로 인한 희석효과가 뚜렷이 나타난 계절적 변이가 큰 것으로 나타났다. 본 연구에서는 총 5과 24종 6909개체의 어류가 채집되었고, 피라미(Zacco platypus)가 81.6%로 우점하는 것으로 나타났다. 다변수 어류 평가모델인 생물통합지수(IBI)는 평균 17(범위: 12$\sim$30)"보통상태"(C)로 나타났으며, 하류지점으로 갈수록 생태건강성이 악화되는 양상(S4: 14, "불량상태")을 보였다. 따라서 대전광역시 도심의 상가밀집지역 및 주거지역을 관통하면서 생활하수 유입 및 각종 오염원에 의한 심각한 수질저하 현상에 의한 것으로 사료되었다. We evaluated stream health, based on integrative approaches of qualitative habitat evaluation index (QHEI), chemical water quality and the index of biological integrity (IBI) using fish assemblages in Daejeon Stream during August 2004 - October 2007. The QHEI values in this study averaged 78.2 (n=28, range: 47$\sim$124), indicating "fair" condition based on the criteria of US EPA (1993). The highest value was in the headwater site (S1) that was 112 (good), whereas the value in S3, mid and downstream was SS (fair-poor) that was the lowest owing to a disturbance on the bed and an artificial bank. Concentrations of chemical parameters such as BOD, TP and conductivity increased from the upstream to the downstream due to higher inputs of organic matters and nutrients from the urban area. Conductivity and TP showed a dilution of stream water by rainwater during summer monsoon. Analysis of fish compositions showed that total number of species was 24 and Zacco platypus was predominated (82%). The mean IBI was 17 (range: 12$\sim$30) during four years and the IBI values also decreased from S1 to S4. These results indicated that ecological health was degradated distinctly as the stream goes down through the city-center. We concluded that Daejeon Stream running through the city are influenced by chemical impact such as an influx of domestic sewage as well as the quality of habitat.

        Therapeutic effect of chitosan modification on salmon-calcitonin-loaded PLGA nanoparticles

        Although salmon-calcitonin (s_CT) has been known as a potent drug for the treatment of osteoporosis, its oral dosage form products have not been commercially available primarily due to a poor oral bioavailability. Chitosan has been extensively examined as a potential oral absorption enhancer, whereas encapsulation with PLGA has been widely studied to address the delivery problems of typical peptide drug molecules. We investigated the mechanism and therapeutic effect of chitosan modification on the s_CT-loaded PLGA nanoparticles. Chitosan was added onto these particles in two ways: by incorporation during W/O/W emulsification and by solid dipping. Particles were characterized by particle size analyzer, Zeta potential analyzer, scanning electron microscopy, and so forth. Nano-sized particles of 430-590 nm in fairly spherical shape with a narrow size distribution were produced. The PLGA encapsulation efficiency greater than 50% of added s_CT was achieved regardless of chitosan modification. It turned out that s-CT-loaded PLGA particles with chitosan modified during W/O/W emulsification (referred to as s_CT-PLGA-CHT1) showed better therapeutic behavior in terms of sustained release effects as well as short-period hypocalcemic effects than the others. It was concluded that the beneficial effect was greatly associated with the formation of embedded structure of chitosan molecules when particles were modified with chitosan.

        누락된 공변량을 가진 원인별 비례위험모형의 분석

        In the analysis of competing risks data, some of covariates may not be fully observed for some subjects. In such cases, excluding subjects with missing covariate values from the analysis may result in biased estimates and loss of efficiency. In this paper, we studied multiple imputation and the augmented inverse probability weighting method for regression parameter estimation in the cause-specific proportional hazards model with missing covariates. The performance of estimators obtained from multiple imputation and the augmented inverse probability weighting method is evaluated by simulation studies, which show that those methods perform well. Multiple imputation and the augmented inverse probability weighting method were applied to investigate significant risk factors for the risk of death from breast cancer and from other causes for breast cancer data with missing values for tumor size obtained from the Prostate, Lung, Colorectal, and Ovarian Cancer Screen Trial Study. Under the cause-specific proportional hazards model, the methods show that race, marital status, stage, grade, and tumor size are significant risk factors for breast cancer mortality, and stage has the greatest effect on increasing the risk of breast cancer death. Age at diagnosis and tumor size have significant effects on increasing the risk of other-cause death.

        Emergency numbers, such as 112 and 119, are used in many countries to connect people in need with emergency services such as police, fire, and medical assistance. We describe development directions of intelligent response technology for emergency calls. The development of this technology refers to enhancing the efficiency and effectiveness of response systems by using advanced methods such as artificial intelligence, machine learning, and big data analytics. We focus on a system that assists the receptionist of an emergency call. In the future, the recognition rate and decision-making accuracy of intelligent response technologies should be improved considering characteristics of public safety and emergency domain data. Although the current technology remains at the level of assisting a receptionist, a fully autonomous response technology is expected to emerge in the future.

        과민성 방광을 동반한 쇼그렌 증후군 소양인 환자 치험 1례

        Introduction This study aims to report a significant improvement of Sjögren's syndrome patient with irritable bladder, diagnosed as 'Soyangin chest-binding symptomatology' based on Sasang medicine. Methods The patient had treatment with 'Dojeokganggi-tang' and western medications. We evaluated the treatment outcome of Sjögren's syndrome symptoms by using a numerical score of 0 to 10, based on the patient's subjective discomfort of dry eye and dry mouth, and irritable bladder symptoms by recording the number of night urination every month. Results Sjögren's syndrome symptoms were significantly improved, and the number of night urination was decreased. After 41 weeks of treatment, the patient didn't have dry mouth any more and had only a little dry eye. The number of night urination was decreased from 3-4 times to once a night. Discussion The patient diagnosed with Sjögren's syndrome, treated with Dojeokganggi-tang and western medications for 41 weeks, showed a significant improvement in dry eye, dry mouth, and night urination compared to the first visit.

        만성비염 환아의 수면의 질과 성장에 관한 임상적 연구

        Objectives : The study was designed to find out the relationships between rhinitis, quality of sleep, growth among children with chronic rhinitis. Methods : This research was performed in 50 children from November 2007 to May 2008. We measured height, weight, body mass index of those children and asked to answer to the questionnaires that consist of nasal symptoms, period of time with rhinitis, sleep symptoms, parent's height, present illness, and medical history. We also analyzed correlations with this factors obtained from questionnaires. Results : 1. In 50 subjects, there were significant correlations between nasal obstruction, the quality of sleep, and the between quality of sleep and growth. But, there were not significant correlations between nasal symptoms and growth. 2. In 24 preschool children, frequency of waking up during sleeping and the number of snorers were higher and quality of sleeping was worse than those in the children who were attending school. And there were significant correlations between quality of sleep and nasal obstruction. An average of RH-MPH(Recent Height-Mid-Parental Height) is $-7.74{\pm}36.51(%)$ which means they aren't as tall as MPH(Mid-Parental Height). 3. In 26 school children, there were significant correlations between symptoms and period of rhinitis, quality of sleep and growth. Conclusions : Children with chronic rhinitis have low quality of sleep and low growth. But more studies are needed.

        성조숙증(性早熟證)에 관한 연구의 최근 동향;최근 중의(中醫) 잡지를 중심으로

        Objectives The aim of this study was to investigate the treatment of precocious puberty in current traditional Chinese medical journals. Methods To figure out diagnosis methods, treatment methods, treatment effects of oriental medicine about precious puberty, Chinese medical journals from China National Knowledge Infrastructure (CNKI) which was published from 1999 to 2007 were analyzed and precocious puberty was also been searched. Results 82 articles were selected. 1. Yin and purging Fire(滋陰瀉火) from traditional Chinese Medicine was usually used. 2. The herbs were almost used such as Rehmannia glutinosa(地黃), Anemarrhena asphodeloides (知母), Phellodendron amurense(黃柏), Thesium chinense(夏枯草), Paeonia suffruticosa Andrews(牧丹皮), Alisma canaliculatum(澤瀉), Testudinis Plastrum(龜板), Paeonia japonica(白芍藥), Bupleurum falcatum(柴胡), Genianae Scabrae Radix(龍膽草). 3. The total improvement rate was 72.50 to 100% Conclusions The oriental medical treatments for precocious puberty were presumed to be effective, but more studies are still needed.
