간경변증 환자의 복수치료 전후에 있어서 혈장 Endothelin-1 및 혈관활동성호르몬의 변화에 대한 연구

Background/Aims: The purpose of the present study was to clarify the possible correlation of endothelin-1 with other vasoactive neurohormonal activities and its effects on fluid-electrolytes balance and renal function in ascitic patients with liver cirrhosis after control of ascites. Methods: The plasma concentration of endothelin-1(PET-1) was measured by specific radioimmunoassay with simultaneous measurement of serutn sodium concentration, serum albumin and other blood chemistries, renal functions with creatinine clearance, urinary sodium excretion, plasma renin activity, plasma aldosterone and plasma norepinephrine in 35 patients with liver cirrhosis with(n=19) and without(n=16) ascites. The same measurements were repeated on the 8th day of admission after control of ascites. Results: The PET-1 in ascitic group was significantly increased from 16.4±10.6 pg/mL to 26.6±16.8 pg/mL after conventional therapy of ascites for 7 days, but not in the non-ascitic group. After control of ascites, plasma renin activity(PRA)(from 5.4±3.3 to 8.6±5.0 ng/mL/hr), plasma aldosterone(frorn 238±197 to 313±239 pg/mL) and norepinephrine (from 714±162 to 814±226 pg/mL) concentrations were significantly increased in ascitic group(p=0.003, p=0.014, p=0.001). The PET-1 showed a significant negative correlation with serum sodium(R= -0.75, P=0.0001), creatinine clearance(R=-0.52, P=0.0012) and 24-hour urinary sodium excretion (R=-0.63, p=0.0000) after therapy for ascites. The PET-1 was significantly correlated with PRA (R=0.69, p=0.000), plasma aldosterone(R=0.67, p=0.000) and norepinephrine (R=0.62, p=0.000) concentrations. Conclusions: These results suggest that conventional therapy for ascites with diuretics or paracentesis in decompensated liver cirrhosis induces the decrease of effective circulatory volurne and increase of PET-1 which is well correlated with other vasoactive hormones inducing renal dysfunction and sodium-free water retention.
정상 한국인에서 연령증가에 따른 혈장 Dehydroepiandrosteronesulfate ( DHEA-S ) 농도 및 DHEA - S / cortisol 비의 변화

Backgroud: DHEA-S is the most abundant steroid hormone in circulation, and primarily secreted from the adrenal cortex, but its physiological role is little known. One of the characteristic features of DHEA-S is progressive decrement of plasma DHEA-S level with advancing age, in contrast, plasma levels of other adrenal hormones are not chaging or littie decreasing. To grasp the trends of plasma DHEA-S level and DHEA-S/cortisol ratio by age in healthy Korean, we measured the plasma DHEA-S levels and DHEA-S/cortisol ratios in healthy Korean. Methods: Healthy Korean(men: 99, women: 102, age range: 15-97 year old)were studied. Subjects were not taking drugs(such as glucocorticoid or androgenic medication) or cigarettes known to modify the plasma level of DHEA-S and cortisol, and had no evidence of hepatic, renal disease or hyperlipidemia as determined by serum lipid, bilirubin, SGOT, SGPT, BUN, creatinine. Data were analyzed by 10-year age group for men and women: i.e, 10-19, 20-29, 30-39, 40- 49, 50-59, 60-69, 70-79, 80-89 and 90 year or more. Plasma DHEA-S levels were measured by using a commercially available RIA kit with 125I labeled-DHEA-SO4(Coat-A Count DHEA-SO4), and for the measurement of plasma cortisol levels, commercial Gamma Coat TM[125I] Cortisol Radioimmunassay Kit was used. Results:. 1) In both men and women, plasma DHEA-S level showed high interindividual variation within the same age group. 2) There were individual sex differences in plasma levels of DHEA-S, in all age groups, plasma DHEA-S levels were significantly higher values for men than for women. 3) Maximum plasma DHEA-S levels(men; 237+-3.35 ug/dL, women; 108+-17.5 ug/dL) were at third decade in both men and women. 4) Both men and women showed the continuous decline in plasma DHEA-S level with age. These age-related decline was more prominent in men than in women(men; y= -3.152 * +292.6, r2= 0.8459, P$lt;0.05, women; y= -1.417 * +143.3, r2 = 0.7278, P$lt; 0.05). 5) As an index of aging, there was no stastical difference between DHEA-S and DHEA- S/cortisol ratio. Conclusion: In healthy Korean, there were high interindividual variation of plasrna DHEA-S levels. In both men and women plasma DHEA-S level was peak at third decade, and from when it declined progressively with age. These results suggest that although the reliability of single plasma DHEA-S measurement are limited, the decline of DHEA-S with advancing age might be a specific marker of endocrinologic hormonal milieu(aging index). Also, concerning to individual adrenal secreting capacity, we measured DHEA-S/cortisol ratio. But we did not found that plasma DHEA-S/cortisol ratio is superior to the plasma DHEA-S level as an aging index. (J Kor Soc Endocrinol 12:245-254, 1997)

Consumer concerns and federal regulations make pesticide residue analysis an important component of ensuring food safety. Also with growing global trade as well as varying international regulations, the need for multi-analyte screening procedures to efficiently detect violating residues is ever increasing. It is desirable to rapidly and reliably screen samples for a large number of pesticides with sensitive and confirmatory methods as possible. Limitations may exist due to the complexity of sample matrices and differing physicochemical characteristics of contaminating compounds, thus requiring replication of work, to ensure all residues of interest can be targeted. This work reports the development of a screening method for the determination of multi-class and multi-residue pesticides from agricultural processes in complex foodstuffs. All solvent standards, matrix matched standards and sample extracts were analyzed using UPLC coupled with a high sensitivity tandem quadrupole mass spectrometer. Further enhancements in analytical efficiencies were achieved utilizing the functionality of the ESI source versus UniSpray, operated at atmospheric pressure. Exploiting the key functionality of the high velocity droplet stream, ionization efficiency is increased while extending the scope of multiclass analytes screened to excelled levels of detection in a single workflow. Also a novel confirmatory strategy (Product Ion Confirmation Scanning, PICS) was assessed to ensure the method that was acquired using multi reaction monitoring (MRM) on a tandem quadrupole mass spectrometer.
