        Laparoscopic Primary Closure: A Better Method of Treatment in Perforated Peptic Ulcer Disease than Open Repair

        Purpose: Perforation is a dreadful complication of peptic ulcer disease requiring immediate management. This study examined the feasibility of laparoscopic primary closure in perforated peptic ulcer disease to allow an earlier return to normal life. Methods: This study reviewed our experience retrospectively with 72 consecutive patients treated with the simple closure of a perforated peptic ulcer disease in our hospital from December 2002 to January 2011. Thirty five patients were treated laparoscopically and the rest underwent open surgery. The operative time, nasogastric tube utilization, abdominal drain usage, time to oral feeding, hospital stay, postoperative complications and recurrence in both groups were compared. A student’s t-test was used to make the comparisons. A p value<0.05 was considered significant. Results: The operative time, use of nasogastric tubes, and abdominal drainage were similar in both groups. After laparoscopic surgery, the patients showed an earlier return to normal oral feeding and discharge than the open surgery group (4.17±0.62 vs. 5.03±2.34 days, p=0.040, 8.63±1.96 vs. 10.24±3.59 days, p=0.021, respectively). The decreased handling of tissue in laparoscopic surgery led to less wound infection (0 in laparoscopic surgery vs. 3 in open) and postoperative ileus (0 vs. 2). Conclusion: Laparoscopic repair of a perforated peptic ulcer is a safe and feasible treatment that offers early oral feeding and a shorter postoperative hospital stay.

        Impact of Patient’s Pain and Fatigue on Decision of Discharge After Laparoscopic Surgery for Colorectal Cancer

        Purpose: Hospital stays after laparoscopic surgery for colorectal cancer tend to be much shorter than those after conventional open surgery. Many factors, including surgical outcomes and complications, are associated with patient discharge planning. However, few studies have analyzed the impact of patient subjective discomfort (including pain and fatigue) on the decision to discharge after surgery. Therefore, the purpose of this study was to determine how patient pain and fatigue play a role in the decision to discharge after laparoscopic surgery for colorectal cancer. Methods: Between March 2014 and February 2015, we conducted a questionnaire survey of 91 patients who underwent laparoscopic surgery for colorectal cancer to estimate the expectation criteria for discharge and patient subjective discomfort at that time. Patients were divided into the following 2 groups: group A, those who complied with the medical professional’s decision to discharge; and group B, those who refused discharge despite the medical professional’s decision. The participants’ subjective factors were analyzed. Results: Preoperatively, 78 of 91 patients (85.7%) identified activity level, amount of food (tolerance), and bowel movements as important factors that should be considered in the decision to discharge a patient postoperatively. Postoperatively, 17 patients (18.7%) refused discharge despite a discharge recommendation. Subjective pain and fatigue were significantly different in linear-by-linear association between the group of patients who agreed to be discharge and those who disagreed. Despite this difference, there was no significant difference in mean length of hospital stay between the 2 groups. Conclusion: A patient’s subjective feelings of pain and fatigue can impact their decision regarding hospital discharge.

      • Vortex를 적용한 3D 컨테이너 크레인 시뮬레이터에서의 로프 표현

        3D 컨테이너 크레인 시뮬레이터는 직접적으로 컨테이너 크레인에 올라가서 교육을 할 수 없는 환경에서 크레인 운전자를 교육하기 위해 개발한 것이다. 이 시뮬레이터는 가능한 한 현장 상황과 거의 유사한 상황을 만들어 내는 것이 제일 중요한 요소가 될 것이다. 본 연구에서는 컨테이너 크레인의 시뮬레이터에서 가장 중요한 요소로 인식되는 로프의 움직임을 사실과 유사하게 만들기 위해 Vortex 라이브러리를 사용하였다. Vortex 라이브러리는 다양한 물리 운동에 대한 내용들을 포함하고 있는데 이중에서 로프의 움직임을 중력, 로프의 무게, 외부의 힘 등을 활용하여 표현하는 안정된 라이브러리로 이를 활용하는 방법에 대해 소개할 것이다.

      • 스마트폰 애플리케이션 개발 환경 비교와 GUI 프로그램 제작 툴 설계

        스마트폰의 등장으로 시작된 스마트폰용 애플리케이션들의 개발은 새로운 플랫폼에 대한 접근이라고 볼 수 있다. 다양한 아이디어를 갖고 있는 개발자들이 스마트폰 애플리케이션개발에 참여하여 수많은 공개 애플리케이션들을 제공하고 있다. 거기에 SDK의 공개와 온라인 마켓의 등장으로 인해 소프트웨어 업체에서 애플리케이션을 공급 받기만 하던 일반사용자들 또한 개발에 참여 할 수 있게 되었다. 하지만 스마트폰을 위한 통일된 플랫폼이 제공되지 못하므로 각 회사별로 제공되는 개발 환경, 툴, 그리고 프로그래밍 언어를 습득해야 한다. 본 논문에서는 보다 쉬운 앱들의 개발을 위해 IOS와 안드로이드의 개발 환경을 비교해보고 일반 사용자들도 어렵지 않으면서 빠르게 개발을 할 수 있게 도와주는 GUI 환경의 프로그램 제작 툴을 통해 애플리케이션 개발을 할 수 있는 방안을 제공한다.

      • 생활체육 수영 참여정도가 회복탄력성에 미치는 영향

        The purpose of the study was to identify the effects participation degree in sport swimming on resilience. The participants were 400 adults who had attended for sport swimming in Chungbuk province and were asked to fill out questionaries; all except 60 questionaries were regarded for having low reliability, 340 questionaries were analyzed for the study. SPSS 19.0 program was used for the data analyses and the significant level was α=.05. In order to analyze the data, an analysis of frequency, Cronbach’s α, a t-test, a post hoc test, Pearson’s correlation, and multiple regression were carried out. The results were as follows: Firstly, in terms of the resilience level, the female groups, the groups over 50 years old, the everyday-practising groups, and the over-2000m swimming groups showed higher scores than the male groups, other age groups, the intermittent groups, and the below-2000m swimming groups respectively. Secondly, from the results of the relationship between the swimming participation degrees and the sub-elements of resilience, the swimming participation degrees (participation periods, participation days and excercise distance) showed positively significant influence on all of the sub-elements of resilience. Influences such as self-regulation, interpersonal skills, and positive characteristics were shown. Lastly, the effect of the swimming participation period on the sub-elements of resilience showed the highest score in terms of emotion regulation: the swimming participation days self-expansion, and the swimming exercise distance emotion regulation respectively. From the results above, it can be said that participation for adults' sport swimming may have positive effects on their resilience and it also could help them enjoy their lives with a healthy body as well as a healthy mind.

        교통사고 심각도 예측 모형의 활용방안에 관한 연구 (서해안 고속도로를 중심으로)

        It is important to study on the traffic accident severity reduction because traffic accident is an issue that is directly related to human life. Therefore, this research developed countermeasure to reduce traffic accident severity considering various factors that affect the accident severity. This research developed the Accident Severity Prediction Model using the collected accident data from Seohaean Expressway in 2004~2006. Through this model, we can find the influence factors and methodology to reduce accident severity. The results show that speed limit violation, vehicle defects, vehicle to vehicle accident, vehicle to person accident, traffic volume, curve radius CV(Coefficient of variation) and vertical slope CV were selected to compose the accident severity model. These are certain causes of the severe accident. The accidents by these certain causes present specific sections of Seohaean Expressway. The results indicate that we can prevent severe accidents by providing selected traffic information and facilities to drivers at specific sections of the Expressway. 교통사고는 인간의 생명과 직결되는 문제이므로, 교통사고 예방 및 심각도 감소를 위한 연구는 매우 중요하다고 할 수 있다. 따라서 본 연구에서는 교통사고심각도에 영향을 줄 수 있는 다양한 영향요소들을 고려하여 교통사고로 인한 피해를 최소화할 수 있는 대응책을 수립하고자 하였다. 본 연구에서는 2004~2006년 동안 서해안 고속도로에서 발생한 174개의 구간별 사고자료를 이용하여 사고 심각도 예측모형을 만들었다. 이렇게 분석된 모형을 이용하여 사고심각도에 영향을 미치는 주요요인을 찾고 이를 이용하여 교통사고로 인한 피해를 줄일 수 있는 다양한 방법들을 고려해 보았다. 분석결과 과속으로 인한 사고, 차량결함, 차대차 사고, 차대사람 사고, 교통량, 곡선반경 및 종단경사 변동계수에 의해 사고심각도가 정의되는 것을 알 수 있다. 이와 같은 사고심각도와 밀접한 관련이 있는 독립변수들을 대상으로 구간에 따른 각 영향의 정도를 그래프를 나타내 보았으며, 그 결과 심각한 사고를 유발하는 특정원인이 몇 가지 존재하며 이러한 특정원인 및 사고유형은 서해안 고속도로의 특정구간에서 주로 발생한다는 것을 알 수 있다. 이러한 결과들은 서해안 고속도로의 특정 구간에 속도제한 단속 카메라, 차내/외 경고정보제공등과 같은 선택적 교통정보 및 교통시설을 제공함으로써 사고를 예방하는 방법에 활용될 수 있을 것이다.

        이용자 중심의 자전거도로 서비스수준 모형 개발

        South Korea is seeking for a solution to the problems of traffic congestion and environment: the increase of bicycle use. However many people feel that using a bicycle is inconvenient. Therefore, we developed bicycle level of service model from the user's perspective so that the existing bicycle roads can be evaluated and improved. The purpose of this paper is to develop a bicycle level of service (LOS) model by considering user's satisfaction and multiple factors that affect bicycle LOS. Bicycle LOS criteria is estimated by applying an ordered probit model, which is suitable for research relating to choice. In addition, we determine the bicycle LOS using three-level(A-C) LOS structure from the user position considering the satisfaction level that people can distinguish clearly. The results show that the bicycle LOS is largely determined by the bicycle road width. Other factors are involved as well, including bicycle road type, the number of access and egress point on the bicycle road corridor, pedestrian volume, and frequency of meetings. 현재 우리나라에서는 심각한 교통 혼잡문제와 도시 환경문제를 해결하고자 자전거 이용 활성화 정책을 추진하고 있다. 그러나 정부의 이러한 정책에도 불구하고, 자전거 이용자들은 여전히 자전거를 이용하는데 불편을 느끼고 있다. 따라서 본 연구에서는 이용자 중심의 자전거도로 서비스수준 모형을 개발하여 기존의 가전거도로 및 시설을 이용자 입장에서 평가하고 개선하는데 활용하고자 하였다. 본 연구에서는 실제 자전거 이용자들이 자전거 도로를 이용하면서 느끼는 만족도로 자전거도로의 서비스수준을 정의하였다. 분석모형은 순차적 의미를 주는 선택, 선호와 관련된 분석에 적합한 순서형 프로빗 모형을 이용하였다. 또한, 자전거도로의 서비스수준은 실제 이용자들이 구별할 수 있는 수준인 3개(A∼C) 범위로 정의하였다. 분석 결과, 이용자 중심의 자전거도로 서비스수준은 자전거도로 폭, 자전거도로축의 진⋅출입로 수, 자전거도로 유형, 보행자교통량, 대면횟수 순으로 정의되었다.
