        장의 급성 이식편대숙주병에서 대변미생물무리이식을 시행한 2예

        급성 이식편대숙주병은 동종 조혈모세포이식의 비-재발(non-relapse) 사망률과 연관되어 있는 중요한 합병증 중 하나이다. 대변미생물무리이식을 통해 장의 미생물무리를 복원함으로써 장의 이식편대숙주병을 치료하려는 노력이 시도되고 있다. 저자들은 장의 스테로이드 불응 이식편대숙주병이 있는 환자 2명에게 대변미생물무리이식을 시행한 경험을 문헌고찰과 함께 보고하고자 한다. 첫 번째 증례는 43세 남자로 형제로부터 동종 조혈모세포이식을 받았고, 두 번째는 70세 여자로 아들로부터 사람백혈구항원 반일치 조혈모세포이식을 받았다. 두 환자는 각각 이식 후 7주 및 4주째에 급성 이식편대숙주병이 장에 발생하였다. 두 환자 모두에서 조혈모세포이식의 공여자로부터 대변을 제공받아 대변미생물무리이식을 시행하였다. 이후, 두 환자 모두에서 임상적인 증상의 호전을 보였으며, 첫 번째 환자는 완전 반응을, 두 번째 환자에서는 부분 반응을 보였다. RNA 염기서열 분석을 통해 대변미생물무리이식 전과 후의 대변을 분석하였을 때 대변 미생물무리이식 후 미생물무리의 다양성이 회복되는 것이 관찰되었다. 이로써 동종 조혈모세포이식 후에 발생하는 장의 스테로이드 불응-급성 이식편대숙주병의 치료 방법으로 대변미생물무리이식을 적극적으로 고려해야 하겠다. Restoring the microbiota via fecal microbiota transplantation (FMT) can be an effective treatment for steroid-refractory acute graft-versus-host disease (GVHD) of the gut. Here, we report two adult patients who underwent FMT to treat steroid-refractory acute GVHD of the gut. The first patient was a 43-year-old man who underwent allogeneic hematopoietic stem cell transplantation (HSCT) with cells from a matched sibling donor. The second patient was a 70-year-old woman who underwent haplo-identical HSCT with cells from her son. Gut GVHD developed at 7 and 4 weeks after HSCT, respectively. After undergoing FMT, the clinical symptoms improved; the first patient had a complete response and the second patient had a partial response. Microbial analyses using RNA gene sequencing showed that a diverse fecal microbiome was recovered by 4 weeks after FMT. FMT should be considered an effective therapeutic option for managing steroid-refractory acute GVHD of the gut. (Korean J Med 2021;96:358-362)

        CSG 재료의 압축강도 특성 및 동결융해 저항성

        The cemented sand and gravel (CSG) method is a construction technique that adds cement and water to rock-like materials, such as rivered gravel or excavation muck which that can be obtained easily at areas adjacent to dam sites. This study was performed to evaluate the unconfined compressive strength properties and freezing and thawing resistance of CSG materials with unit cement content. The three types of CSG-80, CSG-100 and CSG-120 with cement content were designed to evaluate the optimum water content, dry density, strength, stress-strain, micro structure and durability factor. As the results, the optimum water content ratio with cement content showed almost similar tendency, and the unconfined compressive strength and dry density increased as cement content increases. The strength ratio of 7 days for 28 days were in the range of 55~61 % and the strain ratio in stress-strain curve were in the range of 0.8~1.6 % nearby maximum strength in 28 days. It is expected that this study will contribute to increasing application of CSG method as well as to increasing the utilizing of CSG materials as a environmentally friendly CSG method.

      • GFRP 쉬트로 보강된 폴리머 콘크리트 보의 휨 특성

        폴리머 콘크리트는 압축, 인장 및 휨강도 등이 우수할 뿐만 아니라 동결융해 및 화학적 저항성과 같은 내 구성이 뛰어나 건설산업 분야에서 신소재로 많이 활용되고 있다. 최근 선진국에서는 폴리머 콘크리트를 교량, 항만시설 및 건축물에 활용하기 위한 연구 및 실용화가 점차 확대되고 있는 추세이며, 이에 따른 폴리머콘크리트의 구조 설계 및 해석, 보강에 관한 연구가 활발히 진행되고 있다. 한편, 구조물의 보강을 위한 섬유보강 폴리머(Fiber reinforced polymer, FRP) 복합재료의 활용이 확대되고 있으며 특히, 탄소섬유보강쉬트(Carbon fiber reinforced polymer sheet, CFRP Sheet) 및 유리섬유보강쉬트(Glass fiber reinforced polymer sheet, GFRP Sheet) 등에 대한 연구가 활발히 진행되고 있다. 따라서 본 연구에서는 GFRP 쉬트의 보강 두께에 따른 폴리머 콘크리트 보의 하중-처짐 관계, 하중에 따른 중립축의 변화, 연성지수, 극한 모멘트 등을 구명하여 GFRP 쉬트로 보강된 폴리머 콘크리트 보의 휨 특성 및 보강 효과 등을 제시히고자 한다. 폴리머 콘크리트 보 제작을 위한 결합재는 불포화 폴리에스터(Unsaturated polyester) 수지를 사용하였으며, 불포화 폴리에스터 수지의 작업성을 개선하기 위하여 MMA를 결합재 중량에 대한 30 %의 비율로 혼합하여 사용하였다. 또한, 수축 저감을 위하여 수축저감제를 사용하였으며, 중질탄산칼슘을 충전재로 사용하였다. 굵은골재 및 잔골재는 함수율이 0.1% 이하가 되도록 건조된 규사를 사용하였다. 폴리머 콘크리트 보의 인장측 보강을 위한 GFRP 쉬트는 Woven roving cloth를 사용하였고 Hand lay up 공법으로 유리섬유를 적층 성형하여 제조하였다. 폴리머 콘크리트 보의 모재는 단면 10(폭)×15(높이) cm와 길이 100 cm의 크기로 제작하였으며, GFRP 쉬트의 두께는 0(무보강), 1.2(2ply), 3(5ply), 4.8(8ply), 6(10ply), 7.8(13ply), 9(15ply), 12(20ply)mm로 총 8개의 시험체를 제작하였다. 인장측을 GFRP 쉬트로 보강한 폴리머 콘크리트보에 대한 하중 재하시험을 ASTM C78-84의 시험방법에 준하여 수행하였으며, GFRP 쉬트의 보강 두께에 따른 보의 하중-처짐, 중립축 변화, 변형율 등을 분석하기 위하여 보의 중립축, 압축측, 인장측 및 GFRP 쉬트에 총 8개의 스트레인게이지 및 지간의 중앙에 LVDT를 설치하였다. 하중 재하는 Instron사의 만능시험기를 사용하였으며, 하중제어 방식으로 1.54 kN/min의 속도로 하중을 가력하였다. 실험결과 GFRP 쉬트의 보강 두께가 증가함에 따라 항복 및 극한 하중이 크게 증가하는 것으로 나타났으며, 하중에 따른 처짐 또한 GFRP 섬유를 보강하지 않은 배합에 비하여 크게 증가하는 것으로 나타났다. 특히 GFRP 쉬트를 보강하지 않은 무보강 폴리머 콘크리트 보는 균열의 발생과 동시에 파괴가 이루어지는 반면에 GFRP 쉬트로 보강된 폴리머 콘크리트 보는 쉬트의 두께가 증가함에 따라 균열이 발생된 이후에도 파괴에 도달하는데 오랜 시간이 소요되었다. GFRP 쉬트로 보강된 보의 파괴 형태에서 초기 균열은 양 지점부근의 쉬트와 모재의 부착면에서 발생되었으며, 하중이 증가함에 따라 부착면의 경계를 따라 지간의 중앙으로 균열이 진전되는 휨부착 파괴 형태를 나타내었다. 폴리머 콘크리트 보의 연성지수는 GFRP 쉬트의 보강 두께가 증가함에 따라 크게 증가하는 것으로 나타나 쉬트의 보강에 의해 보의 연성 파괴를 유도할 수 있을 것으로 판단되었다.

      • 사무소의 하계 슬랫각도 최적제어를 위한 ANN 모델 개발 및 부하저감 효과

        Windows are the only part of a building that can directly penetrate the solar radiation into the space and thus the shading devices are needed to control the solar penetration. A variety of research have been conducted to develop the optimized slat angle control in the existing literature, but the research incorporating artificial intelligence technique with slat angle control to consider dynamic operating conditions is limited thus far. Therefore, in this study, the ANN (Artificial Neural Network) model was applied to minimize the combined total load consisting of lighting and cooling loads through automatic slat angle control of venetian blinds. A three-story rectangular office building was simulated using EnergyPlus, and dimming control was applied to control the lighting. The interlocked simulation between Matlab and EnergyPlus was conducted through BCVTB. As a result of comparing automatic blind control via the ANN to fixed blind slat angle, the automatic blind control via the ANN showed 10.8% lower total load than the blind angle fixed at 40°. It was confirmed that the cooling and lighting load could be reduced by real-time automatic control via the ANN under various operating conditions, rather than fixing the blinds at one angle.

        외부법의 종양치료활용에 대한 고찰

        Objectives: The purpose of this study was to explore some new therapies to control clinical symptoms of patients with terminal cancer by using external applications. Methods: We investigated some literatures on the external applications for cancer patients and made diagrams. Results: The results are summarized as follows. External applications are one of the traditional oriental medical methods and these are effective for pain control, ascites & pleural effusion and palpitable mass. It has some characteristics which are simple, safe and popular, but we must pay attention carefully to allergic reaction and toxicity in using external applications. The therapeutic portion of external applications are decided by discipline of syndrome and disease differentiation, and the prescriptions are composed of antitumor herb medicines. Conclusions: From the above results, it is expected that external applications are useful to improve clinical symptoms and quality of life(QOL) for patients with terminal cancer who cannot intake foods or medicines.

        각종 암환자 69례에 대한 항암단의 항전이 및 재발억제효과

        Purpose : Among numerous biological symptoms of cancer, matrix metalloproteinases (MMPs) are essential for tumor invasion and metastasis. HAD is used as an inhibitor of MMP gene. This study was designed to evaluate the effects of HAD on anti metastasis and preventing recurrence in cancer patients. Materials and Methods : We retrospectively analyzed the medical records of 69 cancer patients who had been administered with HAD for over 12 months continuously in East-West Cancer Center of Oriental Hospital of Daejeon University, from January 1993 to May 2002. Results : We analyzed gender, portion, stage and anti-metastasis & recurrence rates of cancer patients. Analysis of sex cases showed that the percentage of male is 62.3%, female is 37.7%. Analysis of cancer portion showed that the percentage of stomach is 31.9%, colorectum is 26.1%, lung is 21.7%, liver is 8.7%, breast is 8.7% Analysis of stage showed that the rate of III is 78.3%, IV is 13.0% and II is 8.7%. Analysis of anti-metastasis and recurrence rates showed that colorectal cancer is 77.8%, stomach cancer is 63.6%, lung cancer is 33.4% and breast cancer is 33.3% (mean : 53.6%). Conclusions : HAD has significant effects on anti-metastasis and preventing recurrence of tumor on cancer patients. So it helps to prolong the survival rates of cancer patients.

        항암단(抗癌丹)을 투여(投與)한 대장암(大腸癌) 환자(患者) 83례(例)에 대한 임상보고(臨床報告)

        Clinical studies were carried out 83 cases of patients with colorectal cancer treated by Hangamdan(抗癌丹) from January 1th 1998 to September 30th 2000. The results were summarized as follows; 1. Distribution of those attached by colorectal cancer, by sex, showed that Male is more then Female, by age, showed that the number of fifties is majority. 2. Distribution of diagnostic stage, in descending order; stage III(53%, top), stage IV(45.8%). 3. The effects of maintenance and improvement in the symptoms with traditional oriental therapy(83.3%) and combined treatment of western and oriental therapy(92.1%) were observed. The effects of the symptoms were as follows: diarrhea(37.3%), abdominal pain (25.3%), general body weakness(22.9%), nausea(20.5%) and etc. in orders. 4. Analysis of hematology attached by colorectal cancer, maintenance and increasing of WBC(89.9%), RBC(74.7%), Hgb(81.1%), Platelet(92.4%) were observed. After taken Hangamdan, the safety of the liver and kidney were as follows; maintenance and decreasing of AST(85.9%), ALT(94.8%), GTP(87.5%), Creatinine(90.9%) were observed. 5. of IL-12 and $IFN-\gammer$ attached by colorectal cancer, increasing of IL-12(53.3%), IFN-{\gammer}(80%)$) were observed. 6. Analysis of QOL attached by colorectal cancer, maintenance and improvement of combined treatment of western and oriental therapy(89.6%), traditional oriental therapy(83.3%) were observed. 7. Analysis of survival in patients with IV stage of colorectal cancer, above 7 months(18.4%), 12 months(65.8%). 8. Analysis of antitumor effects, maintenance of traditional oriental therapy(83.3%) and maintenance and improvement of combined treatment of western and oriental therapy(80.5%) were observed. Analysis of tumor marker attached by colorectal cancer, maintenance and decreasing of CEA(78.8%) were observed. 9. Analysis of curative valuation, maintenance and improvement of traditional oriental therapy(83.3%), combined treatment of western and oriental therapy(72.7%) were observed. From the above results, it is suggested that Hangamdan has significant effects of antitumor and immune activity, also could be usefully applied for colorectal cancer patients by combination with western therapy or alone.

        서구(西區) 대체의학(代替醫學)의 암(癌) 치료(治療)에 관(關)한 연구(硏究)

        At here, we investigated the historical background, current stage, and concrete therapies of alternatives in cancer therapy. We obtained the conclusion below. 1. Alternative medicine(AM) is a unothodoxial medical physiology, medical practice or intervension that overcome the side effects, faults, limits of coventional medicine. 2. Recentely, as emerging New-age movement, collapse of Decarte's mecanical cosmology, and emphasis on patient rights, the interest of AM is increased, and foreign & domestic use rates of AM are risen. 3. After coventional medicine in cancer therapies that depends on operations, radiations, and chemotherapy is bounded, Alternatives in cancer therapy are rapidly developed, and the majority of users are often considered satisfactory. 4. Alternatives in cancer therapy are classified six boundaries; Diet and Nutrition, Mind-body Technique, Integrated System, Pharmacologic and Biologic treatments, Immuno Enhanced Therapy, Natural and herbal medicine. 5. Looking at contents of Alternatives in cancer therapy, we get to know most of them are similar to the treatment of oriental medicine in holistic method, therapy, philosophy. If we synthesize the alternative medicine's treatment mentioned above and investigate it with oriental medical viewpoint, that may help us approach the conquest of cancer and improve the rate of cure. 6. It is certain that foreign AM will be imported reversely, and we must prepare for rebuilding of Korean traditional AM systemically, investing for activity positively, making national policy for medical system.
