Crohn's disease is one of the chronic inflammatory disorders of unknown cause involving the gastrointestinal tract. The inflammation may occur in the colon, either alone or with small intestine. Crohn's disease has been associated with increased risk for coloretal carcinoma in patients with long-standing colitis, strictures and fistulae. Recently, we experienced a case of Crohn's disease complicating carcinoma in a 85 year old male. Ten years ago, he compained severe abdominal pain and diarrhea. and was detected colon obstruction and enteric fistula. But, he spontaneously improved. Recently, the patient was admitted complaining abdominal pain. He complaind abdominal pain and indigestion chronically and aggravated recently. He was examined with colonofibroscopy, and computered tomography, and carcinoma was detected at the site of Crohn's disease. (Korean J Med 59:80-84, 2000)
Methyl Methacrylate(Radiopaque Bone Cement)를 이용한 두개골 성형술의 임상적 연구

Cranioplasty has long been performed by many physicians with different materials from the prehistoric era. Among the materials, methyl methacrylate has been known to be the superior to any other substitutes since 1940s in biologic, chemical and physical characteristics and its availability. So the authors review 72 patients who underwent cranioplasty with methly methacrylate at out hospital from 1985 to 1989. The clinical analysis were as flollows : 1) There was male prefonderence and epidural hematomas, compound comminuted depressed fractures by traffic accidents or fall down injuries were the main cause of craniectomy. 2) The location of defects was mainly frontal and/or parietal area and average size of defects was 17㎠ in plain skull film measurements. 3) The cosmetic results were satisfactory. 4) Complication rate was 6% and there was no mortality.

Mesenteric venous thrombosis is an rare type of intestinal ischemia associated with significant mortality and morbidity because of its delay in diagnosis. Mesenteric vasculitis associated with allergic vasculitis can also produce an acute intestinal ischemia. We recently experienced a case of mesenteric venous thrombosis associated with allergic vasculitis, successfully treated with segmental bowel resection and anticoagulant therapy, we report this case with a brief review of the literatures. A 30-year-old man was admitted to our hospital because of severe abdominal pain and hematochezia. Abdominal CT scan showed superior mesenteric venous thrombosis extending into main portal vein. There were large amounts of ascites and the wall of the jejunum was markedly thickened. On the 5th hospital day, his abdomen was tense and diffusely tender: bowel sounds were absent. Exploratory laparotomy was performed under the impression of intestinal infarction. At laparotomy thrombosis of the mesenteric vein and gangrene of the jejunum were found. A small bowel resection with an end-to-end anastomosis was done. Histopathological examination confirmed venous bowel gangrene and showed leukocytoclastic vasculitis, the hallmark of allergic vasculitis. The patient recovered fully and was discharged on anticoagulant therapy. At follow-up after 6 months, the patient was well with no gastrointestinal symptoms.

Parasagittal meningioma often presents as peritumoral brain edema (PTBE). The risk of edema increases when the tumor occludes the superior sagittal sinus (SSS). Although PTBE may be expected based on the patient’s symptoms or radiologic findings, extensive brain swelling and extracranial herniation during elective surgery are rare. Herniation during surgery could lead to irreversible neurological damage and even brain rupture. We report a case of a failed routine craniotomy for a parasagittal meningioma with complete occlusion of the posterior third of the SSS in a 30-year-old male patient. The patient developed extensive brain swelling and extracranial herniation during surgery.
Switchable Multiwavelength Filter Based on a Hybrid Sagnac Interferometer

A switchable multichannel filter based on a hybrid Sagnac interferometer was proposed and experimentally demonstrated. The hybrid Sagnac interferometer was realized by inserting a sampled fiber Bragg grating (SFBG) into a fiber loop mirror by using a polarization-insensitive 3-dB coupler. By using a beam scanning technique, we fabricated a sampled fiber Bragg grating by exposing a photosensitive fiber to a frequency-doubled 244-nm Ar+ laser. The fabricated SFBG had multiple reflection peaks corresponding to the chirp ratio and the spacing of grating. The multiple peaks in the hybrid Sagnac interferometer could be controlled by changing the relative phase difference between two counter-propagating signals. The transmission characteristics of the proposed switchable multichannel filter were analyzed by using a Jones matrix method. Consequently, a switchable multichannel filter based on a sampled fiber Bragg grating incorporating a Sagnac interferometer could be achieved.
