Poncet`s disease is an aseptic polyarthritis developing in the presence of active Tuberculosis occurring elsewhere, and is not due to direct involvement of joints but to an immunological reaction to tuberculoprotein. We experienced a case of Poncet`s disease accompanying erythema nodosum in a 55-year-old female patient with pulmonary tuberculosis. She had multiple tender erythematous nodules on both lower limbs for 3 months and a cough and sputum from one month ago. She felt severe pain in both knees and ankles with swelling one week before admission. Her chest X-ray, computed tomography (CT) scan and positive sputum AFB stain results revealed that she had active pulmonary tuberculosis accompanying erythema nodosum and aseptic polyarthritis. Her arthritis and erythema nodosum were dramatically improved within four weeks after anti-tuberculosis therapy. We report a case of Poncet`s disease in pulmonary tuberculosis accompanying erythema nodosum.

Background/Aims: Melanosis coli is browinsh or black pigmentation of the colonic mucosa associated with long-term ingestion of anthraquinone compounds as laxatives. The discoloration of the colonic mucosa is due to accumulation of macrophages containing brown lipofuscin pigment in the lamina propria. The purpose of this study was to investigate the clinical characteristics of melanosis coli. Methods: We reviewed the clinical records of 50 cases of melanosis coli, which were confirmed by colonoscopy and tissue biopsy in Wallace Memorial Baptist Hospital from December 1992 to Jannuary 1997, with regard to age and sex of patients, sites of involvement, ingredients of laxatives used, clinical symptoms, and concurrent colorectal diseases. Results: The average age of the patients included in this study was 46.0 years, the lowest was 20, and the highest 74. The ratio of male to female was 1:2.6, suggesting that melanosis coli occurs more frequently in the female than in the male. Considering the combination of the involved segments, melanosis coli was most frequently distributed from the cecum to the recturn. The most frequently used agents were antraquinone derivative laxatives including aloe(74.0%) and the duration of ingestion was variable from 2 months to 20 years. Abdominal pain(82.0%) and constipation (64.0%) were the most frequent synptoms at examination. Concurrent colorectal diseases were he- morrhoid, polyps, and cancer in the order. Conclusions: Melanosis coli itself may not have any clinical significance and may not be associated with any colonic functional impairrnent. However, it may provide a clue to laxative abuse, Considering that anthraquinone laxatives such as aloe are widely used in Korea and and the use of laxatives by the elderly is increasing melanosis coli will be detected more frequently.

Background/Aims: The reports on herbal medicine-induced hepatitis are increasing in Korea, China, America, Japan and other countries. To find the differences between other types of hepatitis and herbal medicine-induced hepatitis, we studied patients clinical manifestation, biochemical liver function test and liver biopsy finding. In addition, we tried to find thc definitive causative agents. Methods: We studied 17 patients who had herbal medicine-induced hepatitis, and were admitted to Wallace Memorial Baptist Hospital, Pusan, Korea from January 1995 to January 1997. Their symptoms at admission were jaundice, general weakness and indigestion. The patients who had viral hepatitis, alcoholic hepatitis, liver cirrhosis and other causcs of hepatitis were excluded in this study. We studied the patient's medication history, clinical manifestation, biochemical liver function test and liver biopsy finding. Results: The ratio of male to female was 1:1.8. Age distribution was from 26 to 56 years and mean age was 41 years. Clinical manifestations at admission were jaundice, general weakness, indigestion in order of frequency, On the liver biopsy findings, heavy infiltration of inflammatory eells around the portal tracts, degeneration of hepatocytes and diffuse hepatic necrosis were observed, but there were no significant difference between herbal medicine-induced hepatitis and that of other causes of hepatitis. Conclusions: There was no significant difference in clinical manifestations, biochemical liver function tests and liver biopsy findings between herbal medicineinduced hepatitis and other acute hepatitis. Thus, we conclude that cowork with oriental medicine partner would be needed for evaluating the eauses of herbal medicine-induced hapatitis.
