        새로 고안된 금속스텐트 : 개혈관에서의 실험적 연구

        Three-types of expandable metallic stents were constructed to evaluate the differences between the stents:one, formed in a cylindrical zigzag with stainless steel wire (Gianturco stent). another, made by coating the Gianturco stent with silicone rubber (Silicone stent), the third, made by coating the Gianturco stent with 24 carat gold (Gold stent) for tissuse acceptance. A total of 69 stents (each 23 stents of Gianturco, Silicone, and Gold stents) of 8-12mm in diameter and 20mm long were placed into normal abdominal aorta, inferior vena cava and iliac artery of nine adult dogs for 2 weeks to 11 months. It was more difficult to intrcuce Silicone stent into an introducing sheath than Gianturco or Gold stent due to the thickness of silicone rubber. Four Gianturco stents and three Silicone stents showed migration but Gold stent did not. Luminal narrowing or occlusion was noted in 3 Silicone stents, in 2 Gianturco stents, and in 1 Gold stent. Neointimal proliferation over the stent wires was more rapid and even in Gold stent than Silicone or Gianturco stent. Although further study is needed, Gold stent seems to be better than Guanturco or Silicone stent as an endovascular graft material.

        팽창성 금속스텐트: Tracheobronchial Tree에서의 실험적 연구 및 예비적 임상경험

        To maintain or reestablish an adequate airway in a patient with tracheobronchial narrowing coming from various causes, we constructed self-expanding metallic stents the same way Gianturco did, using them in 2 patients after an experimental study with rabbits. Twenty stents(10mm in diameter fully expanded and 20mm in length) were introduced into the trachea or bronchis of 10 Newzealand rabbits(weight, 2.5-3.0Kg)through a 8.5 French Teflon sheath. No difficulties were encountered in the placement of the stents. At follow-up(4-12 weeks), no stent showed migration. Three rabbits died of pneumonia or bronchial perforation. Histologically, mucosal inflammation was noted at the sites of stent placement, and stent wires were covered by proliferated epithelium with intact cilia. During the last 4 months, 2 stents were used in 2 patients, one in a patient with endobronchial tudobronchial tuberculosis(3.0cm in length and 1.0cm in diameter fully expanded)and the other(3.0cm in lenght and 1.5cm in diameter) in a patient with a subglottic mass. In both patients the stents were successfully placed. Just after the placement of the stents, dyspnea subsided in both patients, and there was no mortality or morbidity. These stents seem to be effective in the treatment of tracheobronchial stenosis, tracheomalacia, and airway collapse following tracheal reconsturction.

        바쁜 의사를 위한 영어발표 요령

        When you present a medical scientific paper or give a lecture in English, It is important to visit your presentation room in advance to gain familiarity with the controls and the computer. You can check how to use the mouse or laser pointer and determine where the microphones are. You should meet the Chairman and introduce yourself. You may ask the Chairman any questions you have. I am confident this strategy will work for beginners because they are usually nervous before their presentations. Most Chairmen prepare one or two questions for the presenters to stimulate discussion. Speaking with the Chairman also allows you to determine the type of English that he or she speaks (i.e., French-English, Spanish-English, and British-English). You need to be comfortable with the Chairman's English before the presentation because the Chairman will repeat the questions for you when you do not understand a question from the audience. Although I have given a number of lectures and moderated many scientific sessions and symposiums in English over the past 20 years, it is still difficult and stressful for me to do these in English because English is not my first language. To overcome the problems, I have written down the questions and useful English expressions that I have received during or after my presentation because I found them helpful for my next presentation. I hope the examples will be of great help to you.

        ETBE 함량 및 점화시기 변화에 따른 성능 및 미세입자 배출특성에 관한 실험적 연구

        It has been known that oxygenates increase octane number of gasolines. There are MTBE and bio-ethanol known as typical oxygenates and a lot of increasing interests have been given to the researches on ETBE to alternate these oxygenates. In this study, the performance and emissions of fine particle number with changing ETBE contents and spark ignition timing in single cylinder engines have been studied. There were trends that oxygen contents and RON increased with increasing ETBE contents of gasolines. And the ignition delay became longer and the burning velocity was reduced with increasing oxygen contents. Net caloric value and RON of gasoline has affected BMEP and BSFC for various ignition timings. Knocking intensity sharply increased with advancing ignition timing, but the difference in the knocking intensity of high RON fuels was relatively low. And fine particle number emissions rapidly increased with advancing ignition timing.

        풍선내 압력변동에 의한 식도파열의 진단방법에 관한 실험적 연구

        To make an accurate diagnosis of esophageal rupture during balloon dilatation without the help of esophagograph, an infusion pump, a pressure transducer and a radiopaque esophageal balloon were connected through a three-way connector. The pressure transducer was connected consecutively to an amplifier, a differentiator and a speaker to detect a pressure drop in the esophageal ballon. Under fluoroscopic monitoring, a radiopaque balloon catheter was inserted in the mid-thoracic esophagus of 30 rabbits and inflated with air until the esophagus was ruptured. A pressure drop in the balloon at the time of esophageal rupture was not only recorded graphically. but also was identified through a speaker. To examine esophageal rupture grossly, the rabbits were sacrificed after esophagography. We could detect the time of esophageal rupture during balloon dilatation in all rabbits accurately by observing the pressure drop in the pressure recorder and by hearing the sound made on a speaker. In 8 patients with esophageal stricture, a deflated radiopaque balloon catheter was inserted to the position inside the narrowing point and inflated by infecting air until the 'hourglass' deformity created by the stricture disappeared from the balloon contour while the pressure change in the balloon was monitored with a pressure recorder. The balloon pressures at the time of disappearance of the "hourglass" deformity from the balloon contour ranged from 200mmHg to 2000mmHg in 8 patients with esophageal strictures. Esophageal ruptuere did not occur in these patients. In conclusion, our results indicate that this new method is not only safe but promising for patients in the future with esophageal strictures. First, it would reduce the chance of mediastinitis in patients of esophageal rupture. Second, esophageal balloon dilatation can be performed more effectively and safely. Third, it is cost-effective. Forth, radiation esposure to the patient can be reduced.e reduced.

        컴포넌트 추출을 위한 UML 기반의 역할 모델링에 관한 연구

        컴포넌트 소프트웨어 개발에서 양질의 컴포넌트를 추출하기 위해서는 사용자의 요구사항을 정확하게 표현할 수 있는 체계적인 모델링 방법이 필요하다. 피러나 컴포넌트는 일반적으로 객체 모델링을 기반으로 설계되고 개발되기 때문에 상호작용과 협력의 표현, 상속으로 인한 재사용 문제 등을 해결하는데는 한계가 있으며 복잡한 구조를 가진 대규모 시스템에서 컴포넌트를 추출하기에는 용이하지 않다. 따라서 객체 모델링을 보완하기 위하여 객체 중심이 아닌 객체의 역할을 중심으로 하는 역할 모델링 기법을 활용할 필요가 있다. 역할 모델링은 대체들의 패턴을 추상화하고 복잡한 대규모 시스템을 관계의 분리를 통하여 간단한 모델로 생성할 수 있도록 객체의 역할을 중심으로 모델링하는 기법이다. 본 논문은 UWL 기반에서 역할 모델링을 통하여 컴포넌트를 추출하는 방법을 연구한다. UML은 다양한 관점으로 모델을 표현하고 있으므로 다양한 관점의 역할 모델을 표현하여 컴포넌트를 추출하는데 활용할 수 있도록 한다. Systematic modeling methods. which can correctly represent user requirements, are absolutely required to extract components when developing components. But, components are designed and developed In general based on the concept of object modeling so that they lacks in supporting representing cooperation and interaction as well as reuse. It means extracting components is not easy to be applied to complicated and sophisticated large-scale systems. This paper utilizes a role modeling method based on the role of objects, not on objects themselves, to complement object modeling. The Role modeling can break down a complicated system into simple models through abstracting the pattern of objects based on roles of the objects. In this study, an UML-based method extracting components through role modeling is embodied. UML can represent models from various standpoints so that role models arc also viewed from several points of view.
