        굴절교정수술로 유발된 부등시와 양안시기능의 상관관계

        Purpose: To evaluate the effects of induced anisometropia by refractive surgery on binocular vision and to determine the usefulness of screening tools. Case summary: Anisometropia was induced in 7 patients by photorefractive keratectomy (PRK). Binocular vision was determined with the Titmus stereo test, Worth 4-dot test, 4 prism diopter base-out test and Bagolini striated lens test. Each test was performed before surgery, after PRK of one eye and after PRK of both eyes. After PRK of one eye, mean stereopsis by the Titmus stereo test was 1877.14 seconds of arc and after surgery of both eyes, 40 seconds of arc. After surgery of one eye, 6 patients (86.7%) showed suppression with the Worth 4 dot test at distance fixation and one patient (14.3%) at near fixation. After surgery of both eyes, all patients showed fusion with the Worth 4 dot test. After surgery of one eye, 3 patients (42.9%) showed suppression with the 4 prism diopter base-out test and after surgery of both eyes, 2 of the patients showed fusion. All patients showed fusion with the Bagolini striated lens test after surgery of one or both eyes. Conclusions: Most cases showed decreased binocular vision by the Titmus stereo test at near fixation and the Worth 4 dot test at distance fixation. The changes of binocular vision determined by each tool were variable. Therefore, improved results in evaluating binocular vision will be obtained with a combination of various tools including the Bagolini striated lens test and 4 prism base-out test.

        New Scoring System for Predicting Mortality in Patients with COVID-19

        Purpose: We aimed to develop a novel mortality scoring system for inpatients with COVID-19 based on simple demographic factorsand laboratory findings. Materials and Methods: We reviewed and analyzed data from patients who were admitted and diagnosed with COVID-19 at 10hospitals in Daegu, South Korea, between January and July 2020. We randomized and assigned patients to the development andvalidation groups at a 70% to 30% ratio. Each point scored for selected risk factors helped build a new mortality scoring system usingCox regression analysis. We evaluated the accuracy of the new scoring system in the development and validation groups usingthe area under the curve. Results: The development group included 1232 patients, whereas the validation group included 528 patients. In the developmentgroup, predictors for the new scoring system as selected by Cox proportional hazards model were age ≥70 years, diabetes, chronickidney disease, dementia, C-reactive protein levels >4 mg/dL, infiltration on chest X-rays at the initial diagnosis, and the need foroxygen support on admission. The areas under the curve for the development and validation groups were 0.914 [95% confidenceinterval (CI) 0.891–0.937] and 0.898 (95% CI 0.854–0.941), respectively. According to our scoring system, COVID-19 mortality was0.4% for the low-risk group (score 0–3) and 53.7% for the very high-risk group (score ≥11). Conclusion: We developed a new scoring system for quickly and easily predicting COVID-19 mortality using simple predictors. This scoring system can help physicians provide the proper therapy and strategy for each patient.

        빅데이터를 활용한 웰니스 관광에 대한 인식과 태도 분석: 의미연결망 분석과 감성분석을 중심으로

        This study used big data to conduct text mining and sentiment analysis on individuals' perceptions and attitudes toward wellness tourism. From October 8th, 2018 to October 7th, 2021, web documents containing keywords such as 'wellness tourism' and 'wellness travel' were collected from Naver, Daum, and Google. A total of 23,770 documents were used to analyze keywords frequency, TF-IDF, N-gram, degree centrality, CONCOR, and sentiment analysis after duplicated posts and meaningless words were removed. The findings revealed that the words most frequently appearing and having high TF-IDF values were mentioned in the context of wellness tourism destination selection and development, purposes and types of wellness travel, and information on domestic wellness tourist spots. Words associated with wellness, tourism, tourism destinations, selections, and so on played critical roles in the network, according to degree centrality measures. Six clusters emerged from CONCOR analysis, including core attributes, projects, wellness tourism development, and so on. Finally, sentiment analysis revealed that people had a generally positive impression of wellness tourism. This study contributes to a better understanding of wellness tourism perceptions and aids in the development of marketing strategies to revitalize wellness tourism. 이 연구는 웰니스 관광에 대한 개인의 인식과 그에 대한 전반적인 감성을 알아보고자 빅데이터를 이용해 의미연결망 분석을 하였다. 국내 주요 포털사이트인 네이버, 다음, 구글에서 ‘웰니스 관광’과 ‘웰니스 여행’이라는 키워드로 2018년 10월 8일부터 2021년 10월 7일까지 3년 동안 게시된 웹 데이터를 수집하였다. 중복 게시글, 의미 없는 단어들을 제거하는 전처리 과정을 거친 후 총 23,770건의 분석 데이터를 바탕으로 단어 빈도, TF-IDF, N-gram, 중심성, 군집분석(CONCOR), 감성분석을 실시하였다. 웰니스 관광 연관어로 국내 웰니스 관광지의 선정과 개발, 웰니스 관광의 목적과 유형, 웰니스 관광지 정보 등에 관한 단어들이 상위권에 노출되었다. 중심성 분석 결과에서 웰니스, 관광, 관광지, 선정 등의 단어들이 전체 네트워크에서 주요한 역할을 하고 있었으며, CONCOR 분석 결과 웰니스 관광 핵심 속성, 웰니스 관광 사업, 웰니스 관광상품 개발 등과 관련된 6개의 군집이 형성되었다. 마지막으로 웰니스 관광의 감성분석에서는 긍정어가 압도적으로 높게 나타났다. 이 연구는 코로나19 이후 더 많은 관심을 받게 된 웰니스 여행에 대한 텍스트 마이닝을 통해 숨겨진 인사이트를 찾아내고 국내 여행 활성화를 위한 의미 있는 정보를 제공하였다는 점에서 의의가 있다.

        수술시기에 따른 영아내사시의 임상양상 및 수술결과

        Purpose: To evaluate the clinical features, surgical outcome and factors influencing the surgical outcome of infantile esotropia according to the age at surgery. Methods: Seventy patients with infantile esotropia followed for at least 6 months after surgery, were enrolled in a retrospective study. Patients were divided into 2 groups. Group 1 included 25 patients who underwent surgery before the age of 24 months and group 2 included 45 patients who underwent surgery after 24 months of age. The clinical characteristics including age at the initial visit, refractive error, angle of deviation, associated strabismus, success rate and reoperation rate were analyzed according to the age at surgery. Results: The mean angle of esodeviation before surgery was 41.71PD and refractive error was +1.26D. Patients underwent bimedial rectus recessions at the mean age of 67.33 months. The age at the initial visit was significantly older in group 2. Surgical success is defined as the ocular alignment within ±10PD in primary position at the last visit. Fourteen patients (56%) from group 1 and 29 (64.4%) from group 2 obtained surgical success. Reoperation was performed in 8 patients (32%) from group 1 and 8 (17.8%) from group 2, which was not statistically significantly different. Stereopsis better than 3,000 seconds of arc was obtained in 3 patients (50%) from group 1 and 16 (76.2%) from group 2. Conclusions: In group 2, the late-surgery group, the success rate was 64.4% and stereopsis was obtained in 76.2%. Surgical results were not different between the two groups.

        Cytomegalovirus Retinitis after Intravitreal Bevacizumab Injection in an Immunocompetent Patient

        We report a case of cytomegalovirus (CMV) retinitis after intravitreal bevacizumab injection. A 61-year-old woman with diabetic macular edema developed dense vitritis and necrotizing retinitis 3 weeks after intravitreal bevacizumab injection. A diagnostic vitrectomy was performed. The undiluted vitreous sample acquired by vitrectomy was analyzed by polymerase chain reaction and culture. Polymerase chain reaction of the vitreous was positive for CMV DNA. Other laboratory results did not show evidence of other infectious retinitis and systemic immune dysfunction. Human immunodeficiency virus antibodies were also negative. After systemic administration of ganciclovir, retinitis has resolved and there has been no recurrence of retinitis during the follow-up period of 12 months. Ophthalmologists should be aware of potential risk for CMV retinitis after intravitreal bevacizumab injection.

        Playground Equipment Related Injuries in Preschool-Aged Children: Emergency Department-based Injury In-depth Surveillance

        In this study, we investigated playground equipment related injuries in preschool-aged children. This was a retrospective observational study using Emergency Department based Injury In-depth Surveillance, (2011–2014). We included the preschool-aged children with playground equipment related injuries. We surveyed the mechanism and incidence of injuries, and estimated the odds ratio (OR) of traumatic brain injury (TBI) and upper/lower extremities fracture. There were 6,110 patients, mean age was 4.14 ± 1.95 years old. Slide and swing related injuries were 2,475 (40.5%) and 1,102 (18.0%). Fall down (48.5%) was the most common mechanism. The OR of TBI in children 0–2 years old was 1.88 times higher than children 3–7 years old, and in swing was 4.72 (OR, 4.72; 95% confidence interval [CI], 2.37–9.40) times higher than seesaw. The OR of upper extremity fracture in children 3–7 years old was 3.07 times higher than children 0–2 years old, and in climbing was 2.03 (OR, 2.03; 95% CI, 1.63–2.52) times higher than swing. The OR of lower extremity fractures in horizontal bars, tightropes, and trampolines was 2.95 (OR, 2.95; 95% CI, 1.55–5.61) times higher than swing. The most common mechanism and playground equipment were fall down and slide. TBI was associated to younger children (0–2 years old) and swing. Fracture of upper extremities was associated to older children (3–7 years old) and climbing. Fracture of lower extremities was associated to others such as horizontal bars, tightropes, and trampolines.
