A case of pulmonary atresia with intact ventricular septum was presented in a 10-month-old cyanotic female patient, which was congenitally rare. Infant with pulmonary atresia and intact ventricular septum usually require urgent surgical intervention. Angiogram showed the pulmonary atresia at the level of the pulmonary valve, the hypoplasia of tricuspid valve and atrial septal defect without patent ductus arteriosus. We performed the pericardial patch graft on the right ventricular outflow tract and pulmonary artery after ventriculotomy using pacemaker wire as electrical saw and main pulmonary arteriotomy and then modified Waterston shunt from the ascending aorta to patch on the right ventricular outflow tract without extracorporeal circulation. Patient was postoperatively good condition.

The study consisted of all patients undergoing surgical repair of isolated secundum type atrial septal defect in patients ages 35 years or older for the period from October 1986 to October 1991. ASD was closed with direct suture in all patients. Response to surgery was excellent. Two patients who had atrial fibrillation was taken low-dose warfarin therapy to prevent stroke. All patients survived operation, and improved by at least one New York Heart Association functional classification. An old age was not a contraindication to surgery.

Two patients with uncorrectable cyanotic cardiac anomalies underwent total cavopulmonary shunt[modified Fontan operation]. Case I was a 14 years old male with dyspnea and cyanosis after birth. Aortogram showed TGA combined with overriding of aorta, pulmonary stenosis, complete atrioventricular septal defect, interruption of inferior vena cava, and situs inversus totalis. We had performed total cavopulmonary shunt using with 16 mm Gortex Graft in single atrium to bypass the hepatic vein to pulmonary artery. Postoperatively, patient sustained low PaCO2 and low cardiac output and then expired at 19th postoperative day. The cause of death of the patient would be low cardiac output. Case II was a 6 years old female with dyspnea and cyanosis after birth. Aortogram showed tricuspid atresia[Type IIb], transposition of great arteries, atrial septal defect, ventricular septal defect and pulmonary stenosis, We had performed total cavo-pulmonary shunt using intraatrial baffle[tunnel] with Goretex patch. The postoperative course of this patient was good without event.
수치모사를 통한 가스-스팀 발사체계의 열유동과 탄의 운동성능 예측

이상유동 모델과 동적격자계를 활용하여 탄의 사출관 내부의 열유동과 탄의 운동성능을 해석하는 수치모사를 진행하였다. 고온의 공기와 냉각제간의 상호작용 및 유동장을 해석하기 위해, Realizable k-ε 난류 모델과 VOF (Volume Of Fluid) 모델을 선정하고 냉각제 유량변이에 따른 수치 해석을 진행하였다. 해석결과, 사출관의 압력은 냉각제의 유뮤에 따라 큰 차이를 보였고, 냉각제량에 따라서도 각각의 차이를 보였다. 탄의 속도와 가속도의 변이는 압력에 종속하여 나타났다. Numerical simulations were carried out to analyze thermo-fluid dynamic and missile-motion performance by using two-phase flow model and dynamic grid system. To analyze the interaction among the hot gas, coolant, and mixture flow, Realizable k-ε turbulence and VOF(Volume Of Fluid) model were chosen and a parametric study was performed with the change of coolant flow rate. As a result of the analysis, pressure of the canister showed a large difference depending on the presence or absence of the coolant, and also showed a dependancy on the amount of coolant. Velocity and acceleration were dependent on the canister pressure.
