        경비강 식도경의 임상적 활용

        The development of office-based, unsedated transnasal esophagoscopy (TNE) has proven to be a major technological advance and with time and experience the application of this technology is becoming more widespread. TNE has allowed otolaryngologists to perform a variety of diagnostic and therapeutic procedures in the office setting. Studies consistently demonstrate that the image quality and diagnostic capability of TNE is equivalent to conventional esophagoscopy. The modern TNE endoscopes offer high quality optics, air-insufflation, and irrigation capability through a 2-mm working channel, and the ability to perform biopsies and select procedures. In general, the role of TNE in the head and neck patient is three-fold : to screen for synchronous and metachronous esophageal squamous cell carcinoma (ESCC) ; to differentiate post-treatment changes/symptoms from malignancy ; and to perform certain office-based procedures. TNE offers many specific advantages to the head and neck patient that are not afforded by conventional esophagoscopy. Because of surgical and postirradiation changes, many HNSCC patients have trismus or neck stiffness preventing completion of conventional transoral esophagoscopy. Perhaps most importantly, TNE provides enhanced patient safety, increased tolerability, better practice efficiency, and cost savings. For these reasons, TNE has become a particularly useful tool in the otolaryngologist’s armamentarium.

        이하선에 발생한 다형선종유래 타액관암종 1예

        Carcinoma ex pleomorphic adenoma of the salivary gland is a relatively uncommon tumor that accounts for roughly 4% of all malignancies at this location. The lesion occurs when a malignant tumor arises in the epithelial component of a pleomorphic adenoma. It usually arises in the parotid gland. Typically, it is a high grade carcinoma, frequently leading to metastasis and disease-related death. We experienced a case of a salivary duct carcinoma that arose in the epithelial component of a pleomorphic adenoma in the parotid gland with neck metastasis. After total parotidectomy and modified radical neck dissection, he was given 10,980 cGy of radiation postoperatively for 7 weeks. The clinicopathologic feature of this tumor are presented with a review of literatures.

        흡입화상 환자에서의 기도처치

        Background and Objectives:Respiratory failure remains one of the leading causes of death in burn patients with inhalationinjury. Obtaining and maintaining a secure airway are both essential and challenging. Although different airway managements areperformed in burn patients with inhalation injury, its exact indications and timing remain controversial. The purpose of this studyis to define the principle of airway management in burn patients with inhalation injury. Subjects and Method:A retrospectivestudy was performed on 177 burn patients with inhalation injury who were admitted to Hangang Sacred Heart Hospital at HallymUniversity Medical Center from July 2002 to June 2005. Results:Severty-seven patients underwent mask O2 supply for initialairway management. A total of 77 patients survived. One hundred patients underwent endotracheal intubation for initial airwaymanagement. Of these, 42 patients underwent tracheotomy after endotracheal intubation. A total of 42 patients reported abnormalchest X-ray findings. Of these, 10 patients survived and had significant improvement in PaO2/FiO2 ratios within 3 days followingtracheotomy. Conclusion:In most cases, laryngotracheal edema subsides within 72 hours, permitting short periods ofairway management. Deterioration of respiratory function permits prolonged intubation and ventilator support. Although tracheotomydoes not improve general condition, it offers some advantages in terms of pulmonary toilet, patient comfort and airwaysecurity. If patients show deterioration of respiratory function, tracheotomy should be performed earlier. (Korean J Otolaryngol2006;49:918-21)

        노인성 후두의 조직병리학적, 생리학적 특성

        Age-related changes in larynx can have a direct impact on voice quality and general comfort level. Observations of vocal aging have spanned perceptual, acoustic, aerodynamic, physical, electromyographic (EMG) and histological levels. Evidence of differential vocal aging in relation to gender and physical condition has been reported. Perceptual, acoustic, aerodynamic, kinematic, EMG and histological data document age-related changes in laryngeal structure and function with advancing age. These changes contribute to a functional age-related impact of vocal hypofunction or compensatory hyperfunction. This review will focus on the current understanding of the clinical and cellular changes in the larynx that lead to presbyphonia.

        반도체 확산공정에서의 컨베이어 적정속도와 길이를 구하는 시뮬레이션

        Semiconductor wafer fabrication is a business of high capital investment and fast changing nature. To be competitive, the production in a fab needs to be effectively planned and scheduled starting from the ramping up phase, so that the business goals such as on-time delivery, high output volume and effective use of capital intensive equipment can be achieved. Project executed that use conveyor in bay semiconductor A line. But conveyor capability is lacking and rundown happened in equipment. Do design without normal simulation and conveyor system failed. The comparison is peformed through simulation using .AutoMod a window 98 based discrete system simulation software, as a tool for comparing performance of proposed layouts. In this research estimate optimum conveyor capability, there is the purpose.

        두경부 암 영역에서의 Epigenetics

        While genetic alterations in head and neck cancer have long been documented, the appreciation of epigenetic changes is more recent. Epigenetics of head and neck cancer becomes an area of emerging research direction due to a growing understanding of specific epigenetic pathways and rapid development of detection technologies. Epigenetic changes alter expression of tumor suppressor genes without changes in DNA sequence. Epigenetic mechanisms such as DNA methylation, histone modification and alteration in genomic imprinting have been shown to silence key genes involved in cell proliferation, differentiation and genome integrity, and clearly have a central role in head and neck cancer. The pattern of hypermethylation in any individual tumor can now be accurately determined, which may find application in molecular staging. The study of epigenetic changes should provide a global view of gene profile in head and neck cancer, and epigenetic markers could be used for early detection, prognosis, and therapy of cancer patients.

        전산해석을 통한 열증기압축기 설계와 실험적 검증

        A thermal vapor compressor in which the subsonic/supersonic flow appears simultaneously, has been accurately designed through the CFD analysis for the various shape parameters such as the primary nozzle shape, converging duct shape, mixing tube diameter, and so on. The performance of the developed thermal vapor compressor has been experimentally verified to be installed in a Multi Effect Desalination(MED) plant as an important element. In this paper, the effects of each parameter are discussed on the basis of CFD results and the experimental results for various boundary conditions(motive pressure, suction pressure, and discharge pressure) are presented in compared with CFD results. The two results show a good agreement with each other within 2 % accuracy with regard to the entrainment ratio.

        두경부암에서 인간유두종바이러스 : 역학

        HPV와 연관된 두경부암은 원발부위에 따라 다양한 빈도를 보이며 HPV와 두경부암에 대한 지금까지의 많은 연구들은 대상군이 적으며, HPV의 감염을 확인하기 위하여 다양한 검출방법을 사용하여 방법에 따라 결과의 많은 차이를 보였다. 향후, HPV와 연관된 두경부암의 보다 표준화되고 정확한 역학 조사는 HPV 감염에서 두경부암으로 발전해 나가는 빈도는 물론, 고위험 지역에서 HPV관련 두경부암의 선별검사 및 예방적 백신의 유용성과 새로운 치료적 접근전략에 도움이 될 것으로 생각한다.

        수평원관상의 LiBr-H$_{2}$O 액막흡수현상 해석

        A numerical study for vapor absorption into LiBr-H$_{2}$O solution film flowing over horizontal circular tubes has been carried out. The momentum, energy and diffusion equations, which are parabolized by the boundary- layer approximation, are solved simultaneously for various mass-flow rates and inlet conditions. The results for the velocity, temperature and concentration fields, as well as the heat and mass flux at the free surface are presented. The effects of inlet conditions, i.e., flow rate, temperature and concentration, on the absorption process are also examined and discussed.
