        The Short-Term Effects of Risperidone-Induced Hyperprolactinemia on Lipid Metabolism in Drug-Naïve Children and Adolescents

        ObjectiveaaThe present cross-sectional study was designed to assess the risk of elevated prolactin levels and other hormonal or metabolic changes in children and adolescents taking risperidone. MethodsaaTwenty-five children and adolescents [aged 7–18 years, 12.1±3.3 years (mean±SD); 19 boys and 6 girls] who had been taking risperidone for at least 3 months were enrolled. The following blood parameters were measured: serum levels of prolactin, thyroid hormones, alanine transaminase (ALT), sex hormones, lipids. ResultsaaThe median risperidone dosage was 1.55 mg/day (SD 1.14 mg/day, range 0.25–4.00 mg/day). The prolactin level (33.65± 16.71 ng/mL, range 5.8–68.3 ng/mL) was higher than normal, and was elevated (≥15 ng/mL in male, ≥23.3 ng/mL in female) in about 84% of the patients. The dosage of risperidone was positively correlated with serum prolactin level (r=0.767, p<0.001). The TG/HDL ratio was higher in the group with higher prolactin levels (i.e., ≥30 ng/mL), and hence might be a useful marker of insulin resistance. ConclusionaaIn young patients taking risperidone, a high serum prolactin level may influence lipid metabolism, even when cholesterol levels are within the normal range. Further investigation is needed around this issue. Serum prolactin assessment is recommended for children and adolescents taking risperidone.

      • Monolayer of Sugar- and Adenosine-terminated Dendrimers at the Air/Water Interface

        Sugar and adenosine terminated dendrimers, [1,2-ο-isopropylideneribosyl-(G<sub>1</sub>-12acid), -(G<sub>2</sub>-36acid)] and [Adenosyl-(G<sub>1</sub>-12acid), - (G<sub>2</sub>-36acid)], were sysnthesized using Newkome’s dendrimer synthetic method. Langmuir - and Langmuir Blodgett monolayers of these dendrimers have been studied at the air-water interface and on solid substrates by measuring surface pressure - area (Π-A) isotherms, atomic force microscopy (AFM), ex-situ ellipsometry. The results showed that these dendrimers formed a stable and homogeneous monolayer without aggregation at these interface, and indicated the nucleoside(adenosine)-terminated dendrimers in the monolayer had spherical structures, while sugar-terminated dendrimers had ellipsoidal structures. Formation of the molecular sphere of the adenosine terminated dendrimers may result of strong intra-molecular hydrogen bonding and relatively weak hydrophilicity of these molecule. On thymine or uracil containing subphase, the estimated area was significantly increased, due to interaction between denarimers and those bases in the subphase.

        대상결정(대법원 2017. 6. 1. 자 2017스515 결정)은 민법 제959조의20이 규정하는 법정후견 보충성의 원칙에 대하여 최초로 판시하였고, 법정후견 개시 심판 중에 임의후견이 등기된 경우에도 동조항이 적용된다는 점을 밝혔다. 그리고 법정후견이 예외적으로 임의후견에 우선하게 되는 ‘본인의 이익을 위하여 특별히 필요한 때’에 관하여 구체적인 판단 기준을 판시하였다. 그러나 임의후견의 의의, 임의후견 우선의 원칙의 배경 등에 비추어 볼 때 대상결정은 본인의 의사를 고려하지 않은 점, 대상결정이 제시한 판단 기준에 의하더라도 구체적인 판단이 이루어졌다고 보기 힘든 점 등 아쉬움이 있다. This decision of the Supreme Court of Korea(Supreme court 2017. 6. 1. 2017Seu515 decision) decides on the principle of complementation of legal guardianship in respect of Korean Civil Law 959-20 for the first time. This decision mentions that the principle of complementation of legal guardianship is applied even when contractual guardianship is concluded and registered while the procedure of legal guardianship is proceeded. When legal guardianship is necessary for one’s particular interest, legal guardianship can be prepared, and the supreme court suggests the detailed standard for necessity for one’s particular interest. However, the court didn’t consider one’s opinion which was to make a guardianship contract as a standard and didn’t consider sufficiently even by its standard. There remains a room to be improved regarding the right to self-determination.

        Shrink-Wrapping 알고리즘을 이용한 단층영상으로부터의 표면 재구성

        영상으로부터 임의의 등밀도 표면을 재구성하기 위한 새로운 방법을 제안하였다. 제안된 방법은 이 분야의 가장 대표적인 알고리즘인 마칭큐브에서와는 달리 단층영상에서 직접 등밀도 표면을 생성하지 않고 등밀도점을 먼저 추출하며, 셀경계 알고리즘을 이용하여 구한 초기메쉬의 각 정점들을 이들 등밀도점 방향으로 표면축소포장(Shrink-wrapping)하는 과정을 반복적으로 적용하여 등밀도 표면을 추출한다. 이렇게 함으로써 마칭큐브에서 나타나는 크랙이나 표면결정의 모호성이 발생하지 않고 안정적인 표면을 재구성 할 수 있다. 또한, 마칭큐브에서 O(1)-인접한 등밀도점만을 사용할 수 있는데 비해, 제안된 방법은 O(2) 및 O(3)-인접한 등밀도점을 표면재구성에 활용할 수 있게 함으로써 이론적으로 마칭큐브 알고리즘보다 더 정밀한 표면을 추출할 수 있도록 하였다. 실험을 통해 제안된 표면 재구성 알고리즘이 단층영상에서부터 등밀도면을 재구성하는데 매우 안정적이고 정확하며 효과적임을 확인할 수 있었다. paper addresses a new surface reconstruction scheme for approximating the isosurface from a set of tomographic cross sectional images. Differently from the novel Marching cube algorithm, our method does not extract iso-density surface(isosurface) directly from the voxels but calculates the iso-density point(isopoint) first. After building the relatively coarse initial mesh by the Cell-boundary algorithm approximating the isosurface, it produces the final isosurface by iteratively shrinking and smoothing the initial mesh. Comparing with the Marching Cube algorithm, our method is robust and does not make any crack in resulting surface model. Furthermore, the proposed method surmounts the O(1)-adjacency limitation of MC in defining the isopoints by permitting the O(2) and O(3)-adjacent isopoints in surface reconstruction, and can produce more accurate isosurface. According to experiments, it is proved to be very robust and efficient for isosurface reconstruction from cross sectional images.

        급성 심근경색 환자의 증상클러스터

        Purpose: Acute myocardial infarction (AMI) leads to death if the patient does not receive emergency treatment. Thus it is very importantto recognize the symptoms in the early stage. The purpose of this study was to identify clusters of symptoms that representAMI in Koreans. Methods: The study used a retrospective, descriptive design with secondary data analysis. Data were abstractedfrom 725 medical records of AMI patients admitted from June 1, 2006 to August 15, 2014 at a university hospital. Results: Analysis ofthe AMI symptoms revealed five symptom clusters; Cluster 1 (n=140): middle chest pain (100%), shortness of breath, and coldsweating, Cluster 2 (n=256): substernal pain (100%), cold sweating, and shortness of breath, Cluster 3 (n=47): substernal pain(95.7%), left arm pain, shortness of breath, cold sweating, left shoulder pain, right arm pain, and the lower neck pain, Cluster 4(n=212): shortness of breath (28.3%), left chest pain, and upper abdominal pain, and Cluster 5 (n=70): cold sweating (100%), leftchest pain, shortness of breath, left shoulder pain, and upper abdominal pain. Length of hospital stay and mortality rate were significantlydifferent according to symptom clusters (F=2.52, p=.040; F=3.62, p=.006, respectively). Conclusion: Symptom clusters ofAMI from this study can be used for AMI patients in order to recognize their symptoms at an early stage. The study findings shouldbe considered when developing educational prevention programs for Koreans with AMI.
