기계환기폐렴의 원인균 진단에서 인공기도 흡인액을 이용한 Multiplex PCR과 세균배양 결과의 비교

연구배경: 기계환기폐렴의 원인균을 조기에 진단하면 이 질환의 예후를 개선할 수 있다. 본 연구에서는 기관내튜브 흡인액을 이용하여 기계환기폐렴의 주된 원인균인 녹농균, 폐렴막대균 그리고 메치실린내성 포도알균에 대한 multiplex PCR법을 시행하여 기계환기폐렴의 원인균을 조기에 진단할 수 있는지 살펴보았다. 방법: 기계환기폐렴으로 진단된 환자에서 24시간 이내에 기관내튜브 흡인액을 채취하여 -20°C 냉동실에 보관하였고, 추후에 multiplex PCR을 시행하였다. Forward & reverse primer는 각각의 원인균에 특이한 부위에 맞춰서 제작하였다(녹농균의 oprL gene, 폐렴막대균의 16S rRNA, 메치실린내성 포도알균의 mec gene). 기관내튜브흡인액의 배양검사를 포함한 기계환기폐렴 환자의 임상적 및 검사실 소견을 함께 분석하였다. 결과: 총 24명(남자 18명, 여자 6명)의 기계환기폐렴 환자가 연구에 포함되었다. 나이(평균±표준편차)는 70±11세였다. 모든 환자는 기저질환을 가지고 있었다. 기관내튜브 흡인액의 배양에서 11명에서 원인균이 검출되었다(녹농균 2예, 폐렴막대균 1예, 메치실린내성 포도알 균 2예, 기타 그람음성간균 3예, 폐렴알균 2예, 복합감염 1예). 녹농균에 대한 multiplex PCR은 3예에서 양성이 나왔으며, 2예가 배양결과와 일치하였다. 폐렴막대균에 대해서는 4예에서 양성이었으며, 1예가 배양결과와 일치하였다. 메치실린내성 포도알균은 2예에서 양성이었으나, 모두 배양결과와 일치하지 않았다. 기계환기폐렴의 예후는 적절한 항생제의 사용 여부와 폐렴 발생 당시의 APACHE III score에 영향을 받았다. 결론: 기관내튜브 흡인액을 이용한 multiplex PCR법은 기계환기폐렴에서 그람음성균의 진단에는 가능성을 보였으나, 향후 실제 유용성에 대해서는 추가 연구가 필요 할 것으로 사료된다. Background: Early identification of pathogens can improve the prognosis of patients with ventilator associated pneumonia (VAP). In the present study, we evaluated the feasibility of performing multiplex PCR for endotracheal aspirates to detect three important pathogens (P. aeruginosa, K. pneumoniae and MRSA) in patients with VAP. Methods: The endotracheal aspirates of 24 patients were collected within 24 hours of the diagnosis of VAP for performing multiplex PCR. Forward and reverse primers were designed to target the specific site of each pathogen (the oprL gene for P. aeruginosa, 16S rRNA for K. pneumoniae and the mec gene for MRSA). We analyzed the clinical data of the VAP patients, including the culture reports for the endotracheal aspirates. Results: Twenty-four patients (M:F=18:6, mean age=70±11) with VAP were enrolled. Pathogens were isolated from 11 patients (P. aeruginosa in 2, K. pneumoniae in 1, MRSA in 2, other enteric Gram negative bacilli in 3, S. pneumoniae in 2 and mixed infection in 1). Multiplex PCR detected three cases of P.aeruginosa (2 cases coincided with the culture reports) and four cases of K. pneumoniae (1 matched with the culture report). PCR detected two MRSA cases, which did not coincide with the culture reports. Conclusion: Multiplex PCR of the endotracheal aspirate showed some ability to detect Gram negative bacilli, although caution is required when interpreting the results. Traditionally, androgens and ageing have been considered confirmative main factors of prostate enlargement. McNeal noted that the proliferation closely resembled developmental differentiation and posited that epithelial hyperplasia may be the result of reactivation of embryonic pathways. He postulated that the combined stromal and ductal hyperplasia in aging men resulted from a re-awakening of these mesenchymal-epithelial inductive interactions. Latest years, acute and chronic inflammation of the prostate and decreasing testosterone levels may important in the aging related re-awakening. So aim of this article is to provide a review of the role of chronic inflammation in benign prostate hyperplasia.

Therapeutic proteins can prevent or treat wide ranges of diseases from cancer and viral or bacterial infections. Production of the therapeutic proteins has been well established mainly in mammalian and bacterial cells including transgenic organisms by recombinant DNA techniques. Recent plant and insect biotechnology and advanced molecular immunology have established molecular biofarming system as an alternative way to produce recombinant pharmaceutical proteins such as immunotherapeutic monoclonal antibodies (mAb). The plants and insects as an expression system have several advantages, which include the lack of animal pathogenic contaminants, low cost of production, and ease of agricultural scale-up compared to other currently available systems. Thus, the paradigm of plant is being shifted as a food source to so-called plant bioreactor for production of therapeutic proteins. Currently, we have successfully developed a plant and insect cell expression system for production of anti-cancer and anti-virus monoclonal antibodies. The effective heterologous production system for recombinant therapeutics requires the appropriate expression machinery with optimal combination of transgene expression regulatory conditions such as control of transcriptional and post transcriptional events. In this paper, the possibility of targeting proteins to the ER and possibly storing them to the protein storage vacuoles with glycosylation modification are discussed in plant and insect cells.

The metabolic syndrome includes insulin resistance, hypertension and obesity. It is well known as risk factors for cardiovascular diseases. Urologic diseases are also linked to metabolic syndrome. Recent researches note that the metabolic syndrome associated with prostate tumors, hypogonadism, nephrolithiasis, overactive bladder, and erectile dysfunction. Urologists need to be aware of the relationships between metabolic syndrome and urologic diseases. But evidence based data are not enough to understand its associations. The relationships between metabolic syndrome and urologic diseases are very extensive issues. So, in this article we will focus principally on the influence of established aspects of the metabolic syndrome and benign prostate hyperplasia/lower urinary tract symptoms.
천연소재 MS-10의 에스트로겐 수용체 조절을 통한 여성건강 증진

엉겅퀴와 타임의 복합추출물인 MS-10이 여성호르몬 수용체를 가역적으로 활성화해 여성갱년기에 감소하는 에스트로겐이 효율적으로 사용될 수 있도록 작용한다는 것이 확인되었다. 12주간의 인체적용시험에서 MS-10은 안면홍조 및 야한증, 감각마비, 수면장애, 신경과민, 우울, 현기증, 피로, 관절 및 근육통, 두통, 가슴 두근거림(심계항진), 그리고 질건조 등의 여성갱년기 증상이 개선되었음이 확인되었다. 이러한 MS-10의 여성갱년기 증상 개선은 MS-10에 의한 insulin-like growth factor-1의 개선에 기인한 것으로 판단된다. MS-10은 여성갱년기 증상을 개선하는 천연소재 건강기능식품으로 사용될 수 있다. In this study, the expression level of estrogen receptor in an ovariectomized rat model was effectively enhanced by MS-10, Cirsium japonicum and Thymus vulgaris extract complex, in a reversible manner. MS-10 plays a positive role in enhancing estrogen activity at low concentrations, leading to improved women"s health. In order to determine whether or not MS-10 improves menopausal symptoms clinically, a randomized, double-blinded, and placebo-controlled clinical study was carried out on 62 middle-aged women treated with 500 mg of MS-10 or placebo daily for 12 weeks. Clinical menopausal symptoms were evaluated by Kupperman"s index (KI) detecting various menopausal symptoms, including hot flushes, parenthesis, insomnia, nervousness, melancholia, dizziness, fatigue, rheumatic pain, palpitations, formication, and headaches. Total KI score decreased significantly by about 18% upon ingestion of MS-10. Colpoxerosis, a main symptom of menopause, was significantly reduced by about 21% upon ingestion of MS-10 in contrast to placebo. In addition, reduction of insulin-like growth factor-1 with age was improved by over 10% upon ingestion of MS-10, whereas there were no significant difference with placebo. No side effects appeared after treatment with MS-10. Thus, MS-10 can be suggested as a plausible natural substance for improving women"s health.
진행된 방광이행세포암 환자의 생존율에 대한 예후인자 분석

The objective of this study is to determine the survival rate associate with the prognostic factor such as age, tumor stage. tumor grade, treatment modality, tumor size, and tumor multiplicity. We retrospectively reviewed the cases of 32 patients diagnosed advanced transitional cell carcinoma (TCC) of the bladder after radical cystectomy from 1985 to 1997. Probable prognostic factors for survival rate were investigated by statistic analysis (Kaplan-Meier Method, Log-rank test). The tumor stage (stage C; 7 cases, 36.22% vs stage D1; 18 cases, 24.00% vs stage D2; 7 cases, 18.85%) and the tumor grade (grade I; 3 cases, 50.66% vs grade Ⅱ: 15 cases, 27.47% vs grade Ill; 14 cases, 27.71%) have significant prognostic value in survival rate. The treatment modality (radical cystectomy only group; 15 cases, 32.73% vs adjuvant M-VAC group; 17 cases, 27.12%) were not correlated with survival rate. Also the other prognostic factors were not correlated with survival rate. So We concluded that the tumor stage and tumor grade seem to be correlated with the survival rate. Adjuvant M-VAC chemotherapy may be not the valuable treatment modality in order to prolong the survival time for the patients with advanced TCC of the bladder.
