Huge hepatic cysts are rarely encountered in autosomal dominant polycystic kidney disease. Huge hepatic cyst bothers the patient either by compression or complications such as intracystic infections or hemorrhages. We report, here, alcohol instillation to treat a huge hepatic cyst causing compression symptom with improvement in size & symptom. A 49-year-old female patient presented with a 4-week history of indigestion and epigastric discomfort. Physical examination was remarkable for tender hepatomegaly. Computerized tomography revealed huge hepatic and kidney cysts. Ultrasonographic guided percutaneous drainage and four times alcohol sclerotherapy of the huge heptic cyst was successful. Relief of the symptoms was maintained after 18 days. This case shows that alcohol sclerotherapy can be a effective nonsurgical method of treating symptomatic huge hepatic cysts.

This 45-year-old woman had undergone renal transplantation on November 25, 1998. In January 1999 she developed dry cough and mild fever. Chest radiographs revealed multifocal patchy and conglomerated consolidation on both lung field, especially lower lung field. Open lung biopsy was performed and aspergillosis and CMV infection were demonstrated in the biopsy specimen. The patient was treated with conventional amphotercin and ganciclovir, but she was progressively aggravated due to fatal combined cytomegalovirus and aspergillosis pneumonia.

Leptospirosis is a worldwide spirochetal zoonosis that spans a clinical spectrum from a mild febrile illness to a serve icteria disease with renal failure (Weil's syndrome) and characterized by extensive vasculitis. The illness is characteristically biphasic pattern with an acute influenza-like symptoms and an immune meningitis during the later phase of illness. The diagnosis is confirmed by serology, culture of blood or spinal fluid during the first pahse of illness, or by culture of urine during the second phase. Doxycycline and amoxacillin administration are effective within the first several days of illness. This 54-year-old male was admitted with mental confusion, myoclonic jerk on face, and generalized myalgia for 5 days. He presented with petechia on soft palate and associated with meningitis, jaundice and renal failure. He has strong positive leptospirosis by serologic test. After the antimicrobial(amoxicillin+clavulanate) & conservative treatment, patient's condition was improved.
