Coronal approach 시행 후 발생한 측두부 함몰의 재건

관상접근법 후 발생한 측두부 함몰의 기전은 아직 명확하지는 않으나 중측두동맥의 손상에 의한 측두지방대의 소실과 변위, 측두근의 부피감소, 봉합시의 잘못된 위치에 의해 발생된다. 이에 저자등은 관상접근법 후 발생한 측두부 함몰을 빠른 혈관화와 골조직의 성장, 낮은 합병증을 보이는 $Medpor^{(R)}$를 이용하여 심미적인 개선을 얻을 수 있었고 이후 현재까지 양호한 경과를 보이기에 문헌고찰과 함께 보고하는 바 이다. The coronal approach has been used for over a century by neurosurgeons to access to the anterior cranium. Indications for the coronal approach expanded from use in the correction of congenital skeletal anomalies to applications in acute maxillofacial trauma and secondary deformity correction, oncologic surgery and reconstruction, and esthetic surgery. Complications were such as injury to frontal branch of the facial nerve, motor nerve paralysis, hematoma under flap, trismus, ptosis, epiphora, infection and anterior temporal depression. $Medpor^{(R)}$ is made up of dense polyethylene connected in porous structures. It is easily shapable without collapsing the pores due to it's hardness and tissue growth takes place at the porosities. Based on these advantages, $Medpor^{(R)}$ has been used in augmentation and restoration in craniofacial defect. A temporal depression after the coronal approach for treatment of Le Fort III fracture was successfully reconstruction with $Medpor^{(R)}$ and we report this case with review of literature.

Sinus lifting was introduced for prosthetic reconstruction in 1960 decades by Boyne and is specifically used for implant surgery in maxillary posterior region with insufficient bony height and has been indicated for reconstruction of oro- antral and palato-alveolar cleft. We report that subtotal maxillectomy using sinus lifting and skin graft in mild maxillary cancer cases results in good prevention of oro-antral and oro-nasal fistula by preserving intact maxillary sinus mucosa and improves better functional outcome after maxillectomy.
국내 유통 중인 석유제품 내 석유계 총 탄화수소화합물(TPH) 분석

국내 토양오염의 60~70% 이상이 석유제품에 의한 오염이며, 석유제품에 의해 토양오염이 발생될 경우, 토양환경보전 법 상 B·T·E·X와 total petroleum hydrocarbon (TPH)를 분석하도록 명시하고 있다. 본 연구에서는 국내 유통 중인 석유제품에 대한 구간별 TPH 패턴을 분석하였다. 또한 C<sub>8</sub>~C<sub>40</sub> 구간만을 분석하는 현행 토양오염공정시험기준의 문제 점을 보완하여 석유제품 내 TPH를 정량분석 하였다. 분석결과, 토양오염공정시험기준 분석조건으로 분석한 결과값과 보완된 분석방법을 이용했을시, 휘발유와 용제 1호 같은 저비점 유류의 경우, 최대 85%의 차이가 발생하는 것을 확인 하였다. Over 60~70% of the domestic soil contamination have occurred by petroleum products. B·T·E·X including benzene, toluene, ethylbenzene, xylene and total petroleum hydrocarbon (TPH) have to be inspected for the contaminated soil by petroleum products. An accurate contamination analysis is necessary to estimate the are of contaminated soil and also establish an appropriate purification scheme. In this study, we analyzed a sectional TPH pattern for current domestic distributed petroleum products. Also, the TPH content was analyzed by compensating the defect of current Korea standard analytic methods for soil where the analytic range is just for C<sub>8</sub>~C<sub>40</sub>. The light distillate petroleum products such as gasoline and solvent 1 showed the maximum of 85% difference in the TPH content between the standard analytic and improved methods.
