Intra-familial Infection due to Trichophyton tonsurans in Korean General Population

Trichophyton (T.) tonsurans is a common anthropophilic species, which causes tinea capitis and tineacorporis particularily in Europe and America, but has not been prevalent in Korea. One case withmycoses infection of the scalp and the other with tinea corporis due to T. tonsurans were observed inKorean general population. We confirmed T. tonsurans infection on the basis of the direct microscopicexamination and culture from scales and infected hairs of skin lesions. Systemic itraconazole andterbinafine therapy with topical ketoconazole cream led to clinical and mycological recovery in ourcases. Infection of T. tonsurans appears to have spread gradually among the general population inKorea. Therefore, athletic and medical associations should make constant observation and take closeconsideration about T. tonsurans infection.

국제해사기구(IMO)의 해상인명안전조약(SOLAS)은 모든 선박이 필수적으로 공인 종이해도, 혹은 전자해도(ENC)를 탑재해야만 한다고 명시하고 있으며, 특히 안전성이 검증된 전자해도 디스플레이 시스템인 ECDIS의 사용을 장려하고 있다. 국제수로기구 내에서는 이러한 ECDIS에 사용되는 공인 전자해도 자료가 지역전자해도 관리센터(RENC)를 통해 보급되어야만 한다는 주장이 커지고 있다. 전 세계적으로 ECDIS의 사용이 일반화되면서, 유럽에 현존하는 두 RENC만으로는 부족하다는 문제점이 제기되었다. 특히, 급격한 경제성장과 더불어 증가하는 아시아 지역의 통항 량은 동아시아 RENC의 필요성을 증명하였다. 본 연구는 2012년에 발효되는 IMO의 ECDIS 강제탑재조건과 현재 RENC의 운영 상황을 바탕으로 RENC 역할의 정의함으로써, 동아시아 수로 위원회의 전략을 참고한 동아시아 지역 독자적 모델(Phase-In)을 통한 RENC 설립의 당위성 및 전략을 연구하였다. Recently, the argument that all Electronic Charts(ENC) displayed in ECDIS should be distributed through Regional ENC Coordinating Center is being raised within the IHO. The use of ECDIS was generalized, but the existing two RENCs in Europe are thought to be not enough. Especially, East Asian region, due to its rapid growth in economy and marine traffic, RENC is found necessary. This research establishes the legitimacy and strategy of East Asian RENC by defining the roles of RENC based on the IMO ECDIS Mandatory Carriage Requirement and RENC operation status through suggesting a "Phase-In" RENC model, which was built upon the strategy of East Asian Hydrographic Commission(EAHC).

ASP 용의 데이터베이스와 고객 데이터베이스 연동에 관한 연구

In the ASP(Application Service Provider) business, applications using database sometimes require some data from clients’ databases. These days such data are extracted from client database using manual database operations as an EXCEL file and the ASP, once receiving this file, transfers it into the application’s database using manual database operations. This paper describes how to deal with data transmitting between the client database and ASP database on the web without using database manual operations for data extraction and insertion. We propose a framework which enables to transmit client data in a systematical way, to match different attribute names of each database for sharing same attribute values, and to avoid exposing information about the network path of client database to the ASP. This approach consists of two steps of data processing. The first is extracting data from client database as XML format by using a downloaded client program from ASP site, the second is uploading and storing the XML file into the ASP database. The implemented prototype system shows the suggested data integration paradigm is valid and ASP business needing integration of client database can be activated using it. ASP(Application Service Provider) 사업에서 응용프로그램이 데이터베이스를 사용해야 하고 이 데이터베이스가 ASP 고객의 데이터베이스로부터 일련의 데이터를 제공받아야 하는 경우가 점점 더 증가하고 있다. 이를 위한 방법으로 엑셀(EXCEL)과 같은 파일 형태가 이용되는데 고객의 데이터베이스로부터 수작업으로 추출되어진 데이터가 엑셀 파일로 ASP 사업자에게 전달된다. 이는 다시 애플리케이션이 이용하는 데이터베이스의 테이블내로 수작업 처리되어 저장된다. 본 연구는 두 데이터베이스 연동 시에 이러한 엑셀 파일 등을 이용하여 발생하는 데이터 추출과 저장시의 데이터베이스 수작업 조작의 비 효율성을 극복하는데 있다. 이 때 두 데이터베이스 상의 속성명은 상이한 경우를 일반적인 경우로 전제하였으며 또한 웹 상에서 두 데이터베이스의 연동 시에 피할 수 없는 고객 데이터베이스의 경로 정보를 ASP 사업자에게 노출을 피하도록 한다. 제안된 방법은 두 단계로 걸쳐 데이터 프로세싱이 이루어지는데 먼저, ASP 사이트로부터 다운로드 받은 프로그램을 이용하여, 고객 데이터베이스를 접속한 후 프로그램의 유저 인터페이스를 이용하여 용이하게 속성 매칭하고 속성치를 XML 파일 형태로 자동 추출하는 단계와 추출된 XML 파일을 웹 상에서 ASP 서버에 접속하여 전송하고 ASP 데이터베이스에 자동 저장하는 후처리 프로세싱 단계로 구성된다. 본 연구에서는 이러한 방법에 맞는 프로토타입 시스템을 구현하여 데이터베이스 연동의 타당성을 검증하였으며, 제안된 방법은 ASP 고객의 데이터베이스를 이용해야 하는 ASP 사업의 활성화에 기여할 수 있다.

Purpose: To report a case of visual field defect and optic atrophy in a patient with periventricular leukomalacia. Case summary: A 22-year-old woman was referred to our hospital for further glaucoma evaluation. She was diagnosed with normal tension glaucoma by her local ophthalmologist. Best corrected visual acuity and intraocular pressure were 1.0 and 16 mm Hg in both eyes, respectively. Fundus examination showed a cup disc ratio of 0.36 in the right eye and 0.34 in the left eye. However, the optic disc had temporal pallor. Red-free photograph and optical coherence tomography showed supratemporal retinal nerve fiber layer atrophy. Visual field examinations revealed inferior visual field defect in both eyes. The patient was born at 34 weeks of gestation with a birth weight of 1600 g. Brain magnetic resonance imaging (MRI) showed periventricular leukomalacia. Conclusions: The author presents a case of periventricular leukomalacia with inferior visual field defect and optic atrophy in a patient who was previously diagnosed with glaucoma. If non-specific clinical features that differ from typical glaucomatous clinical features are observed in a patient with a history of prematurity, a brain MRI can aid the diagnosis.

Purpose: To investigate the biologic effects of preservative-free artificial tear drops on cultured human corneal epithelial cells in vitro. Methods: The effects of preservative-free artificial tear drops (sodium hyaluronate 0.1% (Kynex<SUP>Ⓡ, Alcon, Seoul, Korea), sodium hyaluronate 0.18% (Kynex2<SUP>Ⓡ, Alcon, Seoul, Korea), and sodium hyaluronate 0.3% (Hyaluni eye drops 0.3%<SUP>Ⓡ, Taejoon, Seoul, Korea)) on human corneal epithelial cells were evaluated. Cellular proliferation was determined using MTT (3- (4,5-dimethylthiazol- 2yl)-2,5-diphenyl-2H-tetrazolium bromide) and Ki-67 assays. Cellular migration was determined using CD44 and scratch wound assays. Cell damage was determined using the lactate dehydrogenase (LDH) assay and cellular morphologies using electronic microscopy and inverted microscopy. Results: Proliferation of corneal epithelial cells, as determined by the MTT and Ki-67 assays, was not significantly different between the different eye drops (<EM>p</EM> > 0.05). The measured value of cellular migration after exposure of the sodium hyaluronate 0.3%, as determined by mean CD44 percentage and scratch wound assay, was higher than that of the sodium hyaluronate 0.1% and sodium hyaluronate 0.18%, but the CD44 value was not significantly different (<EM>p</EM> > 0.05). The LDH titer tended to increase as the concentration of sodium hyaluronate increased (<EM>p</EM> > 0.05), but influence on cytoplasm, as determined by electronic microscopy, was not different. Conclusions: Among 3 preservative-free artificial tear drops, sodium hyaluronate 0.3% demonstrated increased migration of corneal epithelial cells. As the concentration of sodium hyaluronate in eye drops increased, the corneal cytotoxicity of corneal epithelial cells also increased.
