둔상에 의한 흉부대동맥 손상에서 TEVAR에 관한 임상연구

Purpose: Thoracic aortic injury is a life-threatening injury that has been traditionally treated by using surgical management. Recently, thoracic endovascular aortic repair (TEVAR) has been conducted pervasively as a better alternative treatment method. Therefore, this study will focus on analyzing the outcome of TEVAR in patients suffering from a blunt thoracic aortic injury. Methods: Of the blunt thoracic aortic injury patients admitted to Eulji University Hospital, this research focused on the 11 patients who had received TEVAR during the period from January 2008 to April 2014. Results: Seven of the 11 patients were male. At the time of admission, the mean systolic pressure was 105.64±24.60 mm Hg, and the mean heart rate was 103.64±20.02 per minute. The median interval from arrival to repair was 7 (4, 47) hours. The mean stay in the ICU was 21.82±16.37 hours. In three patients, a chimney graft technique was also performed to save the left subclavian artery. In one patient, a debranching of the aortic arch vessels was performed. In two patients, the left subclavian artery was totally covered. In one patient whose proximal aortic neck length was insufficient, the landing zone was extended by using a prophylactic left subclavian artery to left common carotid artery bypass before TEVAR. There were no operative mortalities, but a patient who was covered of left subclavian artery died from ischemic brain injury. Complications such as migration, endovascular leakage, collapse, infection and thrombus did not occur. Conclusion: Our short-term outcomes of TEVAR for blunt thoracic aorta injury was feasible. Left subclavian artery may be sacrificed if the proximal landing zone is short, but several methods to continue the perfusion should be considered. [ J Trauma Inj 2015; 28: 232-240 ]
킥모터 FM 규격 연소관에 대한 강성저하 평가 및 파열압력 측정

필라멘트 와인딩으로 제작된 고체 모터 연소관의 구조강성저하 평가 및 파열특성을 확인하기 위한 수압시험을 수행하였다. 본 연소관의 파손 요구조건으로서 운용 중 최대 예상압력(MEOP)의 1.5배 이상의 압력에서 실린더 파손이 일어나야함을 제시하였다. 해석 결과 연소관의 내부압력이 2088psig 일 때 실린더층의 섬유가 끊어지는 것으로 나타났으며 수압시험을 수행하여 2200psig 수준에서 실린더 부위가 파손됨을 검증하였다. 또한 제작 후 1년 정도 경과 후에도 강성저하가 없음을 알 수 있었다. A hydraulic test on a filament wound case of Kick-Motor was conducted to evaluate the structural stiffness degradation and to confirm the burst performance. Failure criteria have been defined with bursting above 150% of MEOP(Maximum Expected Operation Pressure) and failure in the cylinder. The analysis result showed that filament fiber in the cylinder should be broken at about 2088psig. From a hydraulic test it has been verified that composite case meets the failure requirements, and that the stiffness does not decrease even after a year since the manufacturing.
자궁내막증 환자의 복강액의 Interleukin-8에 관한 연구

Objective : This study was investigated to know the relationship between the level of interleukin-8(IL-8) in the peritoneal fluid of women with and without endometriosis Design : Prospective and case-control study. Setting : University hospital. Patients : Twenty six patients who had been confirmed endometriosis by operative and pathological findings were studied. The another twenty six women with benign gynecologic disease except endometriosis was studied by control groups. Main outcome and Measures : IL-8 levels in peritoneal were determined using commercial ELISA set. IL-8 concentration were compared between women with and without endometriosis, and also compared according to severity of endometriosis by which was graded the revised American Fertility Society classification. Results : IL-8 concentration was higher in the peritoneal fluid of women with endometriosis than in matched controls. The concentrations in the proliferative phase were higher than those in the secretory phase. IL-8 concentration in endometriosis stage III, IV were significant higher than stage I, II. Conclusions : Increased level of IL-8 in peritoneal fluid of patients with endometriosis may be related to the pathogenesis of endometriosis that partially contribute to the disturbed immune response.
