There were 6 deaths in the last 4 years in our dental clinic. The patients died form squamous cell carcinoma, odontogenic infection, adult respiratory distress syndrome(ARDS), and general anesthesia. Each case is analysed and presented, for this subject is rarely dealt in the literature and have relation with a complex medicolegal problem.

We experienced one patient who died of the adult respiratory distress syndrome(ARDS) following odontogenic infection. This syndrome is a common life-threatening process with myriad causes and occurs from a variety of diffuse pulmonary injuries which are either direct or indirect attacks on the lung parenchyma. Once lung damage occurs, occurs, exudation of fluid and loss of surfactant activity lead to impaired gas exchange and reduced pulmonary compliance. The syndrome presents clinically nonspecific. Principles of management include adequate support of oxygen transport, ventilation and circulation employing volume respirators with positive end-expiratory pressure(PEEP). For successful management, high index of suspicion and early examination of arterial blood gases are essential.

The purpose of this study was to investigate the distribution of stress within the regenerated bone surrounding the implant using three dimensional finite element stress analysis method. Using ANSYS software revision 6.0 (IronCAD LLC, USA), a program was written to generate a model simulating a cylindrical block section of the mandible 20 mm in height and 10 mm in diameter. The 5.0 × 11.5-mm screw implant (3i, USA) was used for this study, and was assumed to be 100% osseointegrated. And it was restored with gold crown with resin filling at the central fossa area. The implant was surrounded by the regenerated type IV bone, with 4 mm in width and 7 mm apical to the platform of implant in length. And the regenerated bone was surrounded by type I, type II, and type III bone, respectively. The present study used a fine grid model incorporating elements between 250,820 and 352,494 and nodal points between 47,978 and 67,471. A load of 200N was applied at the 3 points on occlusal surfaces of the restoration, the central fossa, outside point of the central fossa with resin filling into screw hole, and the functional cusp, at a 0 degree angle to the vertical axis of the implant, respectively. The results were as follows: 1. The stress distribution in the regenerated bone-implant interface was highly dependent on both the density of the native bone surrounding the regenerated bone and the loading point. 2. A load of 200N at the buccal cusp produced 5-fold increase in the stress concentration at the neck of the implant and apex of regenerated bone irrespective of surrounding bone density compared to a load of 200N at the central fossa. 3. It was found that stress was more homogeneously distributed along the side of implant when the implant was surrounded by both regenerated bone and native type III bone. In summary, these data indicate that concentration of stress on the implant - regenerated bone interface depends on both the native bone quality surrounding the regenerated bone adjacent to implant and the load direction applied on the prosthesis.

Twelve patients with advanced oral cancers were treated with a combination chemotherapy consisting of cisplatin, vincristine, bleomycin before definitive therapy. There were 7(58%) partial and 2(17%) complete responders, with an overall response rate of 75%. This regimen was well tolerated in spite of its toxicities as nausea, vomiting, alopecia, fever and nephrotoxicity. The toxicities were managed symptomatically with antiemetic drugs, hydrocortisone, reduction of doses.
미성견에서의 편측성 하악과두돌기 탈구성골절의 외과적 처치 및 보존적 처치에 대한 비교실험적 연구

This study evaluated the results of surgical and conservative treatment of managing simulated unilateral dislocated fractured mandibular condyles. Group A left the fractured condyle in its dislocated position outside the glenoid fossa. Group B consisted of surgical treatment by completely detaching the condyle, then replacing it as free graft and its subsequent fixation with interosseous wiring. Immediate postoperative mandibular function was performed in both groups. In both groups, we performed simulated retrodiscal injury and discal displacement by incision. Clinical, morphologic, functional, radiologic, and histologic examinations were conducted on all animals. We got the following results. 1. There were no functional and morphological deformities in both groups. 2. The radiographic examination showed the good union of fractured line and condylar remodeling at 3 months in both groups. 3. In all experimental period, no inflammatory reaction was observed. In both groups, good condylar remodeling change was observed. The chondroid tissues apperared between the hypertrophic zone and subchondral bone area. None of the condyles were lost as result of necrosis in group B.

이 연구는 임플란트를 식립하기를 원하는 전신건강상태가 양호하며 구강위생상태가 좋은 14명 환자(남자:8명, 여자:6명, 평균나이 : 44세)의 20개의 발치와 내에 흡수성 차폐막(BioMesh. Sam Yang Corporation, Korea)과 함께 탈회냉동 건조동종 골(dem-ineralized freezedried bone allografts, $250-500{\mu}m$. Pacific Coast Tissue Bank, U.S.A.)과 이종골(Bovine-Bone, Bio-Oss 0.25-1.0 mm, Geistlich, Biomaterials and Osteohealth, Switzerland)을 1:1(부피)로 혼합하여 이식한 후 그 치유양상을 관찰하고자 조직학적 및 면역조직화학적으로 평가하였다. 이직재가 탈락되는 것을 방지하기 위하여 발치한 후 1개윌이 경과된 후에 이식재와 차폐막을 위치시켰다. 표본제작을 위하여 이식술을 시행한 지 약 6개윌 후에 임플란트를 식립하기 직전 식립부위에서 trephine bur로 골을 채취하였는데, 20증례 중 7증례에서 임플란트를 식립하기 전에 차폐막이 노출되었다. 차폐막이 노출되지 않은 것을 대조군으로, 노출된 것을 실험군으로 설정하였다. 조직학적인 관찰을 위하여 통상적인 방법에 따라 탈회 표본을 제작하였고, alkaline phosphotase(ALP)틀 이용하여 면역조직화학적 염색을 시행한 후 골 형성 상태를 평가하여 다음과 같은 결과를 얻었다. 본 연구에서는 발치와내에서 골유도재생술 후 나타나는 치유 형태를 5가지 형태로 분류할 수 있었다. Type I, II와 III는 새로운 골 형성을 나타내지 않았고, 면역조직화학적 검사 시 ALP 음성 소견을 나타내었다. Type V는 새로운 골 형성과 ALP 양성 소견을 나타내었으나 염증, 괴사, 결합조직의 증식 등은 없었다. Type IV와 Type V의 차이는 결합조직의 증식여부로 구분되었다. 막이 노출되지않은 증례들 중 7 증례에서는 Type V의 치유 형태를, 2증례에서는 Type IV의 치유 형태를 나타내었다. 막이 노출되었던 증례에서는 Type I, II, III의 다양한 치유 형태를 나타내었다. 본 연구결과, 발치와 내에 골유도재생술을 시행한 후 차폐막의 노출 여부가 신생골 형성에 중요한 영향을 미칠 것으로 사료되며, 본 연구에서 분류한 치유 형태가 향후 골유도재생술 후의 결과 분석에 활용될 수 있을 것으로 사료된다.

Objectives : Currently, it is being reported that new health technology (HT) is introduced and spread throughout Korea at a very fast rate. However, the current status of new HT related to dentistry has not been known or studied. Therefore, this study aims to understand the present state and awareness on dentistry-related domestic new HT via surveys and research, and thus apply this as a reference to construct an effective new HT-related system in the field of dentistry. Methods : The assessment status of New Health Technology Assessment (nHTA) committee was searched on the website for any updates on new HT5). The cases were searched in the range between Jan 1 2012 and Jul 31 2016 by submission date and were analyzed by year, division of technology, progress and major operating practitioner. Among those, technologies utilized by dentists were additionally analyzed. The survey result was obtained by combining responses of 25 participants from the workshop for new HT application and 11 members of Korean Association of Oral and Maxillofacial Surgeons (KAOMS). The workshop was co-hosted by KAOMS and Clinical Translational Research Center for Dental Science (CTRC) under Seoul National University Dental Hospital (SNUDH). Results : Most of the respondents to the survey have heard of new health technology but were unaware of the application procedure. ‘Safety and effectiveness’ was selected the most for things to consider when introducing new HT, followed by ‘scientific evidence of the technology.’ For new dental HT status, submissions between 2012 and Jul 31 2016 were analyzed by major operating practitioner, including cases where there are multiple practitioners. Yet, only 19 cases (17 %) of dentists were reported. Among 385 cases approved by nHTA, only 2 cases included dentists. Conclusions : Active publicity by the government on new HT and its application procedure, and continued interest and research in the dental and medical community is likely to be required for the development of dentistry.

Tetanus is a disorder of neurotransmission associated with infection by Clostridium tetani, and is characterized by convulsive spasm. The organisms produce the exotoxin tetanospasmin which involves the neuromuscular organs. Most of tetanus are caused by wounds, but in some of cases, portal of entry may not be obvious. History and clinical findings suggest the diagnosis. The most therapeutic maneuver is to insure airway patency, other therapeutic principles include antimicrobial drugs, active and passive immunization and supportive care. A 51-year-old woman with moth opening limitation and dysphagia visited our hospital. We had diagnosed her as tetanus and treated successfully.
