Objective To predict the probability of lymphedema development in breast cancer patients in the early post-operation stage, we investigated the ability of quantitative lymphoscintigraphic assessment.Methods This retrospective study included 201 patients without lymphedema after unilateral breast cancer surgery. Lymphoscintigraphy was performed between 4 and 8 weeks after surgery to evaluate the lymphatic system in the early postoperative stage. Quantitative lymphoscintigraphy was performed using four methods: ratio of radiopharmaceutical clearance rate of the affected to normal hand; ratio of radioactivity of the affected to normal hand; ratio of radiopharmaceutical uptake rate of the affected to normal axilla (RUA); and ratio of radioactivity of the affected to normal axilla (RRA). During a 1-year follow-up, patients with a circumferential interlimb difference of 2 cm at any measurement location and a 200-mL interlimb volume difference were diagnosed with lymphedema. We investigated the difference in quantitative lymphoscintigraphic assessment between the non-lymphedema and lymphedema groups.Results Quantitative lymphoscintigraphic assessment revealed that the RUA and RRA were significantly lower in the lymphedema group than in the non-lymphedema group. After adjusting the model for all significant variables (body mass index, N-stage, T-stage, type of surgery, and type of lymph node surgery), RRA was associated with lymphedema (odds ratio=0.14; 95% confidence interval, 0.04–0.46; p=0.001).Conclusion In patients in the early postoperative stage after unilateral breast cancer surgery, quantitative lymphoscintigraphic assessment can be used to predict the probability of developing lymphedema.

In this paper, we propose the method that extracts the speech feature using the hearing model through signal precessing techniques. The proposed method includes following procedure ; normalization of the short-time speech block by its maximum value, multi-resolution analysis using the discrete wavelet transformation and re-synthesize using thediscrete inverse wavelet transformation, differentiation after analysis and synthesis, full wave rectification and integration. In order to verify the performance of the proposed speech feature in the speech recognition task, korean digita recognition experiments were carried out using both the dTW and the VQ-HMM. The results showed that, in case of using dTW, the recognition rates were 99.79% and 90.33% for speaker-dependent and speaker-independent task respectively and, in case of using VQ-HMM, the rate were 96.5% and 81.5% respectively. And it indicates that the proposed speech feature has the potentials to use as a simple and efficient feature for recognition task.
이종 분산 플랫폼 자동 구축을 위한 호스트 자동 설치 방법

네트워크 기반 운영체제 설치 기술은 PXE를 기반으로 대상 호스트에 자동으로 운영체제를 설치하는 기술이다. 하지만 기존의 기술은 서로 다른 운영체제와 소프트웨어로 구성된 이종 분산 시스템을 구축 하는 경우 같은 시스템 별로 각각 설치해야 되는 문제가 존재한다. 따라서 본 연구에서는이종 시스템의 분산 시스템을 동시에 설치하기 위한 자동화 시스템을 제시한다. 본 논문에서 제시하는 시스템은 사용자가 입력한 정보를 시스템 구성 설정 자동화 기능과 호스트 모니터링 기능을 통해자동으로 반영할 수 있게 하였다. 이종 시스템 설치 시간 단축 확인을 위해 단일 호스트 및 소프트웨어로 구성된 시스템 구축 시간과 이질적인 호스트 및 소프트웨어로 구성된 시스템을 구축하는데 걸리는 시간을 측정하였다. 호스트의 수가 증가할수록 평균 설치시간이 증가하지만 호스트의 하드웨어 성능이 다른 두 호스트가 동시에 설치하는 경우 설치 과정에 서로 영향을 적게 주는 것을 발견하였다. Network-based OS installation technologies which utilize the PXE are the technologies that install the OS automatically. However, the original technology has several problems such as building heterogeneous distribution system that is consisted of different OS and software’s, when we install each system. In this paper, we propose the automatic installation system for heterogeneous distributed platform at the same time. This system automatically reflects the information that the user gave by using the function of system configuration of administrator information and host monitoring data. In order to reduce the time of installing the heterogeneous system, we measure the time it takes construct a same or heterogeneous environment platforms. Average installation time increases proportionally by number of hosts, but installing two different hosts that have different hardware functions at the same time barely affects the installation steps.

Polymer-based amorphous solid dispersion (PASD) technology has attracted attention as one of the most feasible approaches for improving the solubility, dissolution rate, and bioavailability of insoluble drugs. Tegoprazan (TPZ) is a promising new drug used to treat gastroesophageal reflux disease with poor water solubility (∼0.03 mg/mL). This study developed novel PASD materials containing TPZ. Three polymers were used for this study: PVP, HPMCAS, and carbomer. The PASD powders were prepared via solvent evaporation at 50% drug loading. The physico-chemical properties of PASD solids were characterized using PXRD, MDSC, TGA, FT-IR, 1H SS-NMR, and stability testing. PASD powders fabricated with the neutral polymer PVP showed poor stability against drug crystallization. In contrast, those prepared using HPMCAS and carbomer showed no signs of crystallization even after three months of storage at 40oC/75% RH. A correlation between intermolecular interaction and physical stability was inferred for the TPZ PASD formulations. Amorphization of the crystalline TPZ with HPMCAS and carbomer resulted in a greatly increased in vitro dissolution rate. These two polymers showed similar performance, eliciting appreciable improvement in the in vivo absorption tests in rats. In summary, PASD formulations using acidic polymers (HPMCAS and carbomer) are novel formulations for improving the therapeutic effects of TPZ.

We report on a 44-year-old man with multiple peripheral neuropathies who developed bilateral lower extremity weakness. After heavy alcohol abuse, he experienced pain in both lower legs and severe erythematous swelling. The initial laboratory test suggested non-traumatic rhabdomyolysis caused by a heavy alcohol binge. Magnetic resonance images showed edema in the bilateral gluteus minimus and medius, pyriformis, and anterior calf muscles. The electrodiagnostic study showed bilateral sural, tibial, and peroneal neuropathies. In conclusion, in patients with acute and chronic alcohol abuse state, multiple neuropathies can be caused by integration of immobilization, electrolyte imbalance, compression, and direct myotoxicity. The patient began gait training with bilateral UD-flex orthosis with crutches. He was trained to use the crutches safely and an electrical stimulation test (EST) was applied. He was able to walk outdoors. We suggest consideration of early diagnosis and appropriate rehabilitative management to prevent severe secondary complications such as neuropathy.
