        골밀도 평가방법과 임플란트 안정성 변화의 관찰

        Purpose: The aim of this study was to examine the correlation of the subjective and the objective evaluation of edentulous ridge bone quality, and to evaluate the change of the dental implant stability in each bone density group for early healing period after implant installation. Methods: Sixty-seven implants(Osstem implant$^{(R)}$, Seoul, Korea) were included in this study. We evaluated the bone density by 2 methods. The one was the subjective method which was determined by practitioner s tactile sense, the other was the objective bone type was based on Hounsfield units. The implant stability in each bone type group was assessed by resonance frequency analyzer(Osstell mentor$^{(R)}$). Data were analyzed for the change of the implant stability, and they were compared to verify the difference of groups at the time of installation, 2, 6, 10, 14 weeks postoperatively. Spearman's correlation was used to demonstrate the correlation between the subjective and the objective evaluation of the bone density, and analysis of variance(ANOVA) was used to analyze the differences of implant stability at each time point. Results: There was no close relation between the subjective and the objective evaluation of the bone density(r=0.57). In the subjective groups, there was statistically significant difference between the type 1 and 3 at 10 weeks and between the type 2 and 3 at 14 weeks. In the objective groups, there was no statistically significant difference between the D 1, 2, 3, 4, and 5 group with regard to RFA from baseline to 14 weeks(P>0.1). Conclusions: The implant stability increased over time during the study, and it was improved with bone density proportionally after 2weeks postoperatively. It is recommended that the decision of bone density is base on Hounsfield unit for implant loading time.

        The 'classic' triad of medial medullary infarction (MMI) consists of contralateral hemiplegia, loss of posterior column sensation on the contralateral side and involvement of the tongue on the ipsilateral side. Facial muscle is usually spared in MMI. A 51-year-old patient with MMI came to our department. His major clinical manifestations were contralateral hemiparesis, and contralateral central type facial palsy. He had neither sensory change nor tongue deviation. This report support the hypothesis that part of the corticobulbar fibers supplying the lower facial muscles descend ipsilaterally in the ventromedial part of the medulla and then, after decussation, ascend rostrally to the facial nucleus in the dorsolateral part of the medulla.

        Implant 보철물 access hole의 깊이에 관한 연구

        A total of 605 implant fixture prosthesis delivered by 3 clinics and 2 laboratories were examined in this study, The object of this study was to determine the proper length of screw head. The depth of access hole were measured and compared to the type of fixture, abutment, gold screw and prosthesis. The results were as follows : 1 The average number of fixtures per patient were 2.97. 2. The number of fixture installed in the upper posterior area are 327(55.56 %), the upper posterior area 171 (28.25%). 3. The depth of access hole is 4.23 mm in shallow area, and 5.46 mm in deep area and the differences were 1.23 mm. 4. The average depth of the aceess hole of the UCLA abutment were 5.02 mm. 5. The number of 4-5 mm access hole depth were 60(22.39%) in abutment screw level and the number of 4-5 mm depth in fixture level were 101 (29.19%). 6. In the shape of screw head, hexed type were 576(95.21%), slotted type were 29(4.79%).

        고령자 삼킴기능저하의 평가와 중재

        Korea is on the verge of becoming a super-aged society, and the structure of the disease is changing accordingly. One of the measures to lower the medical and social burden for the elderly is early detection and prevention of elderly frailty. Oral frailty in elderly is reported to be manifested prior to whole body frailty. So, if oral frailty is diagnosed and treated early, independency in mobility and healthy aging of elderly can be effectively promoted. The Korean Academy of Geriatric Dentistry launched establishing a clinical guideline of oral frailty for Korean elderly with diagnostic criteria and treatment modality. If two out of the six criteria, chewing function, occlusal force, tongue force, salivary gland function, swallowing function, and oral hygiene, are checked as not ‘normal’ but ‘at risk’, the patient is diagnosed as having oral frailty. In this paper, evaluation methods and exercise therapies for swallowing function (out of the six criteria for oral frailty) is described in detail.

        기능획득 돌연변이 인삼 모상근의 대량생산

        본 연구는 아그로박테리움 공동배양법을 이용한 기능획득 인삼 모상근의 대량생산을 위한 조건 확립에 대한 것이다. 일반적으로, 인삼과 같이 형질전환을 통한종자의 확보가 어려운 식물에서는 loss-of-function을 이용한 기능유전체 연구에 한계가 있다. 한편, 유전자의 기능을 활성화시키는 방법 (gain-of-function)인 activation tagging 기술은 이러한 문제점을 극복할 수 있는 대안이 될 수 있으며, Agrobacterium rhizogenes를 이용한 모상근 생산 시스템은 대량의 돌연변이체를 안정적으로 용이하게 얻을 수 있다는 점에서 최적의 시스템이라고 할 수 있다. 본 연구에서는 activation-tagging된 효율적인 형질전환 모상근 생산에 있어서의 최적의 아그로박테리움 균주 및 인삼조직, 배지조성 등에 대한 조건을 확립하였으며, 다양한 배지에서의 형질전환 모상근의 생장률 및 분지율, 표현형 등을 조사하였다. 엽병 절편을 activation-tagging vector pKH01을 가지고 있는 A. rhizogenes R1000와 공동배양하였을 때 배양 4주후 85.9%의 빈도로 모상근이 생산되었다. 모상근의 최대 생장과 분지도를 나타내는 배양조건을 조사한 바 엽병절편을 1/2 SH 배지에서 4주 배양하였을 때 왕성하게 생장하였으며 2.6의 분지도를 보여주었다. 최종적으로 1,989개체의 독립적인 형질전환 모상근 line을 생산하였으며, 이들 모상근 line은 인삼 진세노사이드 생합성 관련 유전자의 발굴 및 기능해석에 유용하게 쓰일 것이다. This study describes conditions for the mass production of activation-tagged mutant hairy root lines of ginseng by cocultivation with Agrobacterium rhizogenes. Because it is not currently possible to produce progeny from transgenic ginseng, a loss-of-function approach for functional genomics cannot be appliable to this species. A gain-of-function approach is alternatively the choice and hairy root production by cocultivation of A. rhizogenes would be most practical to obtain a large number of mutants. Various sources of explants were subjected to genetic transformation with various strains of A. rhizogenes harboring the activation-tagging vector pKH01 to determine optimum conditions for the highest frequency of hairy root formation on explants. Petiole explants cocultivated with A. rhizogenes R1000 produced hairy roots at a frequency of 85.9% after 4 weeks of culture. Conditions for maximum growth or branching rate of hairy roots were also investigated by using various culture media. Petiole explants cultured on half strength Schenk and Hildebrandt medium produced vigorously growing branched roots at a rate of 2.6 after 4 weeks of culture. A total of 1,989 lines of hairy root mutants were established in this study. These hairy root lines will be useful to determine functions of genes for biosynthesis of ginsenosides.

        Opcode 빈도수 기반 악성코드 이미지를 활용한 CNN 기반 악성코드 탐지 기법

        As the Internet develops and the utilization rate of computers increases, the threats posed by malware keep increasing. This leads to the demand for a system to automatically analyzes a large amount of malware. In this paper, an automatic malware analysis technique using a deep learning algorithm is introduced. Our proposed method uses CNN (Convolutional Neural Network) to analyze the malicious features represented as images. To reflect semantic information of malware for detection, our method uses the opcode frequency data of binary for image generation, rather than using bytes of binary. As a result of the experiments using the datasets consisting of 20,000 samples, it was found that the proposed method can detect malicious codes with 91% accuracy. 인터넷이 발달하고 컴퓨터 이용률이 높아짐에 따라 악성코드로 인한 위협 또한 함께 증가하고 있다. 매년 발견되는 악성코드의 수는 급격히 증가하여 자동으로 대량의 악성코드를 분석하기 위한 시스템이 필요한 상황이다. 본 논문에서는 딥러닝 알고리즘을 활용한 악성코드 자동 분석 기법을 소개한다. CNN(Convolutional Neural Network)라는 이미지 분류에 활용도가 높은 알고리즘을 이용하여 악성코드의 특징을 이미지화한 데이터를 분석한다. 제안하는 방법은 악성코드의 Semantic한 정보를 탐지에 활용하기 위하여 단순 바이너리 바이트를 기반으로 생성한 이미지가 아닌, 바이너리의 명령어 빈도수를 기반으로 생성한 이미지를 CNN으로 분석한다. 악성코드 10,000개 정상코드 10,000개로 구성된 대량의 데이터 셋을 활용하여 탐지 성능을 확인한 결과, 제안하는 방법은 91%의 정확도로 악성코드를 탐지할 수 있음이 확인되었다.

        Various immunotherapeutic modalities have been used based on the autoimmune pathogenic mechanisms of myasthenia gravis (MG). Cell-mediated immunity as well as auto-antibodies may play a role in the remission and relapse of MG. We recently experienced two patients with MG who showed spontaneous remission fter inadvertent severe leukopenia. These findings suggest that the cell-mediated immune process is important in the treatment of MG, and selective suppression of leukocyte may induce remission in the patients with intractable MG.

        Two cases of bilateral ulnar neuropathies caused by telephone overuse are described in people engaged in the job of longtime telephone use. They had worked using telephone all through the working hours. Although they were right handed, they usually used the telephone in their left hand so as to leave their right hand free for writing, and would lean his elbow on the desk. Telesales or teleconsultation is a rapidly expanding field of business. These cases highlights the need of proper training and supplies appropriate to do their job.
