        Unmet Clinical Needs in the Treatment of Patients with Thyroid Cancer

        The increased incidence of thyroid cancer is a worldwide phenomenon; however, the issue of overdiagnosis has been most prominent in South Korea. The age-standardized mortality rate of thyroid cancer in Korea steeply increased from 1985 to 2004 (from 0.17per 100,000 to 0.85 per 100,000), and then decreased until 2015 to 0.42 per 100,000, suggesting that early detection reduced mortality. However, early detection of thyroid cancer may be cost-ineffective, considering its very high prevalence and indolent course. Therefore, risk stratification and tailored management are vitally important, but many prognostic markers can only be evaluatedpostoperatively. Discovery of preoperative marker(s), especially for small cancers, is the most important unmet clinical need for thyroid cancer. Herein, we discuss some such factors that we recently discovered. Another unmet clinical need is better treatment of radioiodine-refractory (RAIR) differentiated thyroid cancer (DTC) and undifferentiated cancers. Although sorafenib and lenvatinib areavailable, better drugs are needed. We found that phosphoglycerate dehydrogenase, a critical enzyme for serine biosynthesis, couldbe a novel therapeutic target, and that the lymphocyte-to-monocyte ratio is a prognostic marker of survival in patients with anaplasticthyroid carcinoma or RAIR DTC. Deeper insights are needed into tumor-host interactions in thyroid cancer to improve treatment.

        액와에 발생한 아급성 괴사성 림프선염 1례

        Since 1972, a unique lymphadenitis called as subacute necrotizing lymphadenitis has been described in many Japanese literature. Originally described in Japan, it now appears worldwide. It mainly affects young women and usually manifests as fever and lymphdenopathy. The pathology is characterized by necrosis with loss of nodal architecture, infiltration with many histiocytes, and an absence of granulocytes. We report a case of subacute necrotizing lymphadenitis in axillary area. 4 10-year-old male child was admitted with a one-month history of fever, swelling in the axillary area. He treated with intravenous antibiotics. Despite treatment he remained febrile for the next ten days. 4 dissection of the axillary swelling was then performed, as was a diagnostic lymph node biopsy. The patient's condition resolved over several days and he was discharged.

        활성 용가재를 이용한 세라믹 및 스테인레스강의 접합

        The direct brazing technology which could be used for the simplification of brazing process and the improvement of brazed joint quality was studied with $Al_2O_3$ and stainless steels. The brazing of $Al_2O_3$ to STS304 or STS430 was performed under different brazing conditions such as brazing filler metal, temperature, heating rate and brazing time. Microstructural observation and chemical analysis be SEM/EPAM were carried out to verify the quality of brazed joints. 4-point bending strength of brazed joints was also measured to find the optimal brazing conditions. The results showed that, in brazing of $Al_2O_3$, the mixed oxide layer resulted from the reaction between Ti in filler metal and oxide layer on the material surface to be brazed was found to be bery important for the joint quality. The width of oxide layer varied with the brazing conditions such as brazing time, heating rate and chemical composition of filler metals. The strength of brazed joints was more affected by the type of materials and their thermal properties than by brazing heat cycle.

        원소상 황에 의한 함티탄자철광의 황화반응

        원소상 황의 새로운 황화제로서의 사용가능성을 조사하고자 탄소존재하에서 원소상 황과 함티탄자철광의 반응을 시도하였다. 반응변수로서는 황분압, 반응온도, 함티탄자철광에 대한 탄소첨가량의 무게비, 반응시간, 함티탄자철광의 입도등이 선정되었으며 검토결과 본 황화반응의 적정 조건은 황분압 0.2atm., 반응온도 800℃, 함티탄자철광에 대한 첨가탄소량의 무게비 0.7, 반응시간 90분, 함티탄자철광의 입도 0-150mesh였다. 위 조건에서 얻엉진 황화반응생성물을 1M HCl로 침출한 후 얻은 잔사의 TiO₂ 함량은 78.48%였다. The reactions in the carbon-elemental sulfur-titaniferous magnetite system were investigated in order to find the possibility of elemental sulfur being an alternative sulfidization agent. The study of this reaction included reaction variables such as partial ptessure of sulfur, reaction temperature, weight ratio of added carbon to titaniferous magnetite, reaction time, and particle size of titaniferous magnetite. The proper conditions for this sulfidization were that partial pressure of sulfur 0.2 atm., reaction temperature 800℃, weight ratio of added carbon to titaniferous magnetite 0.7, reaction time 90min., and particle size of titaniferous magnetite under 150 mesh. The content of TiO₂ of residue obtained by leaching the sulfidized product with 1M HCl was 78.48%.

      • Slide法에 依한 免疫學的 姙娠診斷에 關하여

        From clinical studies on immunological diagnostic pregnanoy test with slide method for diagnostic an follow up evaluations in normal pregnacy, hydatidiformmole, choriocarcinoma and others, the following results were obtained. 1. The positive HCG test was able to read within 2 minutes using 1 drop (0.07ml) of the using to be tested and all urines from normal nonpregnant women, normal helathy men an saline revealed negative results. 2. The rate of positivity was 96.6% in 89 cases of normal pregnency and 3 cases of negativity were noted among first and second month of the pregnancy. 3. The highest quantity of HCG was estimated at the third month of the pregnancy. i.e 16 fold dilution (91,424 I.U.) of urine exhibited positive result. 4. The positivity of HCG test was increased by Zondek's alcoholic concentration method with pregnant urine. Two of them were resulted in positive conversion in 2 fold dilution by this method from negative HCG test in indiluted urine an another one was increased the positivity to 6 fold dilution from 4 fold, and two were 25 fold dilution from 16 fold. 5. The negative conversion of HCG test was initiated approximately from the fifth day of postpartum. 6. The minimally positive limit of the reaction on this slide test was 0.4 I.U. per drop of the urine to be tested by using standard HCG preparation (500 I.U.) 7. No specific influence to the storage of the urine to be tested in the refrigerator (4℃) was observed during 3weeks. 8. On the relations between temperature and HCG slide test, the reaction time was shortened as the temperature became high and as the temperature became low, the reaction was delayed. But it was most reliable to read the reaction at the room temperature. 9. No influence to the pH of urine (5.5∼7.0) on HCG test was boserved. 10. In the influence of the protein in the urine from non pregnant which was negative HCG test, the protein (0.2% or more) induced false positive results. 11. The rate of positivity of choriocarcinoma was 100% and was 98.2% in hydatidiform mole. 12. There was remarkable change in progress of choriocarcinoma with methotrexate therapy and in most of hydatidiform mole the negative conversion of HCG test noted after evacuation.

        소청룡탕의 임상연구 분석

        Objective : This study's purpose was to review the clinical studies of Socheongryong-Tang. Method : We searched papers about Socheongryong-Tang using KISS, KTKP, PUBMED, Embase, Science Direct, and the key words "Socheongryong-Tang", "Shoseiryuto", "Xiao Qing Long Tang", and "Minor Green-Blue Dragon Decoction" were used. Papers not matched with inclusion criteria were excluded. Until today, there have been 143 studies on the effects of asthma, allergic rhinitis, common cold, eczema and other various effects of Socheongryong-Tang. Of these, 15 were classified as clinical research papers. Results : There were 4 cases of asthma, 2 cases of allergic rhinitis, 2 cases of common cold, 1 case of pneumonia in patients with dementia, 1 case of eczema, 1 case of drug interaction study, 1 case of ephedrine drug removal, 3 cases of side effects causing pneumonia. Conclusion : It can be seen that Socheongryong-Tang has established the basis for application for the purpose of asthma, allergic rhinitis, wind-cold type cold, preventing pneumonia in patients with dementia, dyshidrotic eczema. On the other hand, considering three cases that cause side effects of pneumonia, it is necessary to confirm whether or not the symptoms of pneumonia are certainly progressing when observing the progress of the patient.

      • 腦下垂體 腺腫의 各種 호르몬 生成에 關한 免疫組織化學的 硏究

        Before introduction of immunoperoxidase technic, routine histologic examination of the pituitary tumor without information on clinical and laboratory findings was rarely possible to predict functional activity of the tumor. The author had performed an immunoenzyme histochemistry on 18 cases of formalin fixed and paraffin embedded surgical specimens of pituitary adenomas using antihuman antibodies of growth hormone(GH), prolactin(PRL), and adrenocorticotrophic hormone(ACTH). And the findings obtained from microscopic examination were compared with the results of hormonal assay of the serum in each case. The results obtained are as follows; 1. By radioimmunoassay, 18 cases of pituitary adenoma corresponded with PRL producing adenomas in 7 cases, GH producing adenomas in 3 cases, ACTH producing adenoma in 1 case, GH and PRL producing adenomas in 4 cases, GH and ACTH producing adenoma in 1 case and noiisecretory 2 cases. 2. Immunoperoxidase staining of 18 cases of pituitary adenomas revealed GH producing adenomas in 3 cases, PRL producing adenomas in 3 cases, ACTH producing adenoma in 1 case, GH and PRL producing adenomas in 6 cases, PRL and ACTH producing adenoma in 1 case, GH, PRL, and ACTH producing adenoma in 1 case, and nonsecretory (as far as GH, PRL, and ACTH are concerned) adenoma in 3 cases. 3. correiation between findings of tissue and serum in individual case revealed complete matching in 7 cases (38.9%), partial matching in 9 cases(50.0%), and non-matching in 2 casrs(11.1%). Considering above results, the author proposes to classify pituitary adenomas in combination of tissue and serum findings.
