Castleman' s disease is a rare, reactive lymphoid proliferation of unknown cause and pathogenesis. Depending on histololgic findings, it has been divided into two distinct histologic subtypes that include hyaline-vascular type(90%) and plasmacell type(10%). This disease occurs most commonly in mediastinal lymph nodes and cervical lymph nodes, inguinal lymph nodes respectively. But occurs rarely in retroperitoneum. We report a case of hyaline-vascular type asymptomatic Castleman' s disease of the retroperitoneum in a 47-year-old man.

연구배경: 감염질환은 혈관질환과 함께 당뇨병의 중요한 사망원인의 하나이며, 혈당조절이 불량할수록 감염 빈도가 증가됨은 잘 알려져 있다. 간농양의 발생이 있어서도 당뇨병은 중요한 기저질환 중의 하나이다. 일반적으로 간농양의 가장 흔한 원인균은 Escherichi coli라고 알려져 있으나, 최근 당뇨병을 가진 간농양 환자에서 Klesiella pneumoniae의 검출이 증가되고 있다. 또 최근 연구에 의하면 K.pneumonie가 간농양의 가장 흔한 원인균이며, K. pneumoniae에 의한 간농양은 K.pneumoniae 이외의 균주에 의한 간농양보다 당뇨병과 밀접히 관련되어 있다고 보고된 바 있다. 이에 저자들은 간농양 환자를 대상으로 당뇨병환자의 빈도를 알아보고, 그 원인균 및 특징을 비당뇨병 환자들의 경우와 비교해 보고자 하였다. 방법: 1992년 1월부터 1999년 6월까지 방사선학적 검사 혹은 수술에 의해 간농양이 확인되거나, 경피적 천자나 수술을 통한 배농액 배양, 혹은 혈액 검사에서 원인균이 증명된 182명의 환자를 대상으로 후향적 임상고찰을 실시하였다. 결과: 화농성 농양을 가진 167명의 환자(남:94명, 여:73명)중 당뇨병을 가진 환자는 54명 (남:30명, 여:24명)으로 32.3%를 차지하였다. 환자의 평균나이는 당뇨군에서 62.1 ± 13.4세, 비당뇨군에서는 55.1 ±15.5세로 당뇨군에서 높았으며(p=0.0021), 남녀간의 평균나이 비교 시 두 군 모두 여성의 나이가 의미있게 높았다. 비당뇨군에 비하여 당뇨군의 경우 저알부민혈증의 빈도와 aspartate transaminase가 증가되어있었다. 원인균주는 K. pneumoniae (당뇨군54%, 비당뇨군 39.1%), E. coli(당뇨군 17.5%, 비당뇨군 18.8%), Pseudomonas aeruginosa, Streptococcus viridans, Enterococcus 순이었으며 두 군간의 의미는 차이는 없었으나 K. pneumoniae의 경우 다른 균종에 비해 단독감염의 빈도가 높았다(82.8%). 사망률은 당뇨군에서 16.7%, 비당뇨군에서 7.1%로 당뇨군에서 의미있게 높았으며(p=0.019), 당뇨군에서의 사망환자는 모두 패혈증이 동반되어 있었다. 당뇨환자의 사망군은 생존군에 비해 당뇨병 이환기간이 길고 혼합감염의 빈도가 높았다(p=0.046). 검사실 소견에서는 총 빌리루빈 및 알카리성 포스파타제의 증가가 관찰되어 있었다. 결론: E. coli가 간농양의 주된 원인균이라는 기존 보고와는 달리 당뇨군과 비당뇨군 모두 K. pneumoniae가 간농양의 주된 원인균이었다. 또한 당뇨 유병기간이 길고 당뇨조절이 불량한 환자에서 혼합감염이며 총 빌리루빈 및 알카리성 포스파타제의 증가등과 같은 위험인자가 존재할 경우 사망률이 증가될 수 있으므로 이들 군에서는 적극적인 치료가 필요할 것으로 사료된다. Background: Liver abscesses are commonly associated with underlying disease, particularly diabete mellitus. The number of the liver abscesses caused by Klebslella pneumoniae in diabetic patient has been increased in Korea nowadays. This study was conducted to clarify the clinical presentation and prognostic factors of pyogenic liver abscesses, especially in diabetic patients and to determine the proportion of K. pneunomiae as a pathogen in liver abscess in Korea. Methods: Medical records of 167 patients treated for pyogenic liver abscess from January, 1992 through June, 1999 were reviewed retrospectively in detail. Major Pathogenic organism, clinical manifestations, prognostic factors, the importance of diabetes mellilus as an underlying disease and its effect on clinical features and prognosis were analyzed. Results: Among 167 cases of pyogenic liver abscess, underlying diabetes mellitus was present in 32.3%. The mean age of patients was 62.7 13.4 years in diabetic liver abscess group and 55.1 15.5 years in non-diabetic liver abscess group. Most liver abscesses were cryptogenic in origin or secondary to the billary tree diseases. The clinical presentations among the two groups were not significantly different. When compared to patients without diabetes, patients with diabetes had significantly higher proportions of hypoalbuminemia and elevated aspartate transaminase level. The most common organism of the pyogenic abscess was K. pneumoniae in both groups. Percutaneous drainage of the abscess with appropriate antibiotics was the most commonly used therapeutic modality in liver abscess. The mortality rate of diabetic liver abscess is 16.1 % and 7.1% in nondiabetic liver abscess. Complications, especially septicemla, were found more frequently in patients with diabetes than in patients without diabetes (64.8% vs 28.3%, septicemia : 31.5% vs 9.7%). The factors influencing mortality in the diabetic liver abscess were disease duration, mixed infection, presence of septicemia, elevated bilirubin and alkaline phosphatase. Conclusions: In contrast to prior report that the E. coli was the most common pathogen in liver abscess, we found that K. pneumoniae was the most common organism cultured in liver abscess. Diabetic patients have more complications and higher mortality than patients without diabetes. Early detection and proper treatment are needed to improve the outcome for diabetic patient with liver abscess.

본 연구는 우리나라 장수노인들의 식품섭취상태를 분석 평가하고 건강한 장수를 위한 식생활 지침의 설정을 위한 기초자료를 얻고자 수행되었다. 연구 대상은 경상북도 성주군에 거주하는 85세 이상 고령노인 중에서 일상 생활에 문제가 없는 224명이었으며 이들을 대상으로 반복 24시간 회상법을 이용하여 1년간 4회의 계절별 식품 섭취 조사를 실시하였으며 결과를 요약하면 다음과 같다. 계절별 식품군별 섭취량에 있어서 남자노인은 감자류의 섭취량이 여름철에 유의적으로 높고, 버섯류의 섭취량이 가을철에 유의하게 높은 것을 제외하고는 모든 식품군의 섭취량에 있어 계절별 차이가 나타나지 않았다. 여자노인의 경우 곡류, 당류, 두류, 채소류, 과일류, 조미료류, 음료 및 주류, 육류, 우유류, 어패류의 섭취량이 겨울철에 유의하게 높았으며, 종실류, 해조류, 난류의 섭취량은 계절에 따른 차이가 없었다. 조사 대상자들의 연평균 1일 총 식품 섭취량은 594.4±186.1 g이었으며 이 중에서 513.5±161.2 g(86.4%)는 식물성 식품군으로부터 섭취하였고 79.3±56.7 g(13.3%)는 동물성 식품군으로부터 섭취하였다. 섭취량이 가장 높았던 식품군은 곡류였으며그 다음이 채소류, 과일류, 어패류, 음료 및 주류, 조미료류, 두류, 육류, 우유류, 감자류 등의 순이었다. 장수 노인대상자들의 음식별 섭취빈도는 밥류에 있어서는 쌀밥의 섭취비율이 80.1~89.2%로 가장 높았으며 국과 찌개류에서는 쇠고기국, 된장국, 된장찌개의 섭취 비율이 높았다. 어육류 식품의 경우 조기구이의 섭취비율(9.4~12.7%)이 높았으며, 해조류 식품의 경우 김구이의 섭취비율(7.4~20.5%)이 높았고, 각 계절마다 계절식품의 섭취비율이 높았다. 김치류의 경우 배추김치의 섭취비율이 4계절 모두 70% 이상으로 높게 나타났다. 일품요리류는 손쉽게 조리할 수 있는 라면과 국수의 섭취비율이 높았다.<br/> 본 연구의 결과에서 경북 성주지역 장수 노인들의 계절별 식품 섭취량에 있어 겨울철의 식품 섭취량이 다른계절에 비해 유의하게 높았으며, 남자노인보다는 여자노인이 식품섭취에 계절에 의한 영향을 더욱더 많이 받는 것으로 나타났다. 또한 장수 노인들은 가공식품보다는 계절마다 제철에 생산되는 자연 식품의 섭취비율이 높았다. 전반적으로 장수노인들은 소식의 경향을 보였으며 이와 같은 소식습관과 신선한 식물성 식품들의 일상 섭취가 건강한 장수에 영향을 미쳤을 가능성도 있을 것으로 사료된다. The purpose of this study was to estimate food intakes of the long-lived elderly and to obtain the data for establishing dietary guidelines that may be recommended for the general population for the sake of longevity. The subjects of the study were 224 elderly people of age over 85 years living in Kyungpook Sung-Ju area who have no problem in daily living. The food consumption survey was carried out seasonally by the repeated 24-hr recall method for one year. The subject group for this study was composed of 58 males and 166 females, the average age being 87 years old. Food intakes of the winter were more than any other seasons. The mean daily total food intake per capita was 594.4 g, 513.5 g (86.4%) from plant foods and 79.3 g (13.3%) from animal foods. The sequence of high intakes of food groups were cereals, vegetables, fruits, fishes, legumes and meats. Boiled white rice, beef soup, soybean paste soup, soybean paste stew, broiled yellow croaker, kimchi, ra myon and broiled noodles were consumed most frequently. In conclusion, the subjects consumed much more plants foods than animal foods and consumed more natural and seasonal foods than processed foods and also showed food habits of eating small meals. Dietary habits of eating small meals containing abundant amount of fresh plant foods might partially contribute to the longevity of the subjects.

The present study was performed to investigate the distribution, growth characteristics, phamacological components and utilization of 7 selected wild edible plants : Oenothera odorata Jacquin, Calystegia japonica Thunb., Sanguisorba officinalis L., Rumex coreanus, Agrimonia pilosa Ledeb., Portulacase oleracea L. and Commelina communis L. Based on the information collected from the cited references, their utilizations as edible plants were described in detail.

To elucidate the fatal blood concentration of paraquat(Gramoxone), a contact herbicide, we investigated blood concentration of paraquat in 91 cases of paraquat intoxication. The blood paraquat concentration of 91 cases due to ingestion of paraquat was 0.9 ㎍/ml -1023.5 ㎍/ml and has been subdivided according to survival time. 48 of 91 cases were identified survival time. In 34 of 48 cases died within a day after ingestion of paraquat, blood paraquat concentration was ranged from 2.3~ 636. 6 ㎍/ml, and in 12 of 48 cases died 1~4 days after ingestion of paraquat, blood paraquat concentration was ranged from 0.9~25.1㎍/ml.

TQM is a whole process of systemizing, documenting and measuring of service quality scientifically to achieve zero defects in product quality and to continue to improve the body of the product quality by itself. To have this process be improved continuously, first of all it needs to prepare an ideal environment to understand and to conform to its internal and external customers', that is every guests' and employees' expectations and demands. Then it has to be followed by the next step removing some negative aspects that could be derived from the first mentioned process The third step is to cut the cost by suitable compensation rather than an attitude trying to solve the problems in an easy way. The last essential factor for setting up TQM system is to make good use of the outstanding feature or process as it is reviewed against the superior benchmark. The purpose of this study is meet the customers' expectations and to maximize the hotel 's profits by searching TQM(total quality management) strategy in hotels. To attain this research's objective, it follows these three details. Firstly, it defines TQM and its contents. Secondly, it provides the standard of hotel TQM throughout some case studies. Thirdly, it suggests TQM strategic plan in tourism hotel as one of professional management strategies. Two useful methods are used together to achieve the goal of this study. To confirm its theoretical consideration, descriptive studies by the research documents published in arid out of the country is used. The other is case studies of TQM execution in real hospitality industry, which present the cases of the airline, Jefferson Memorial and the hotel. This research is targeting to demonstrate the successful TQM's exercise ill a hotel industry by completing the theoretical studies and case studies on service product quality with an understanding of its importance. As it is said earlier, to lead to the successful TQM's practice, it is necessary to maintain a constant training of the employees. Secondly, it also must identify where it has to focus on to deliver a professional quality management. Thirdly, effective quality management organization needs to be build up. The manager's strong will to accomplish and the employee' active participations are the last condition to be succeed. Once again this study places an emphasis on the fact that TQM is critical to maximize the customers' satisfaction and the hotel's profitability. It is also very worthwhile to have every people working on front line recognize why the TQM is important and further more how they can contribute to improve their service quality by a positive participation and a careful observation of TQM 's operating in their property.
