Prolonged Extreme Thrombocytosis in a Postsplenectomy Patient with Hereditary Spherocytosis

We report a case of prolonged extreme reactive thrombocytosis in a post-splenectomy patient with hereditary spherocytosis. A 29-year-old female patient presented with gall stones detected incidentally by abdominal ultrasonography. Her laboratory findings showed hemolytic anemia with spherocytosis on the peripheral blood smear and increased osmotic fragility. She was diagnosed with hereditary spherocytosis and underwent a laparoscopic cholecystectomy and splenectomy. After undergoing surgery, the hemolytic anemia was resolved but thrombocytosis was newly detected. Nineteen months after the splenectomy, the thrombocytosis was still persistent and extremely high. To our knowledge, this is the first report of a prolonged extreme reactive thrombocytosis after a splenectomy in Korea.

The present study was designed to clarify whether scuba diving at 5 meters of seawater influences cerebral hemodynamics, hematological and biochemical variables. Twenty healthy young men well trained scuba diving participated in this study. The blood flow velocity in the right and left middle cerebral arteries (L-MCAV and R-MCAV), blood pressure (BP), heart rate (HR), CBC and differential count, prothrombin time (PT), activated partial thromboplastin time (aPTT), biochemical variables, D-dimer and interleukin-8 (IL-8) levels were determined before, immediately after scuba diving for 30 min, and after 30 min of rest (Pre-scuba, Scuba and R-30m, respectively). L-MCAV and R-MCAV tended to increase, but the only significant increase was in L-MCAV in Scuba. SBP and HR significantly declined in R-30m compared with those of Pre-scuba and the Scuba. IL-8 levels were elevated in Scuba and R-30m compared with that of Pre-scuba. In Scuba and R-30m, hematological variables except PT and biochemical parameters excluding glucose and lactic acid did not significantly changed in comparison with those of Pre-scuba. PT level at Scuba and glucose level at R-30m significantly declined in Scuba, while lactate level at R-30m increased compared with each in Pre-scuba. However, PT level at Scuba was within a normal range. These results suggest that scuba diving at 5 m of seawater for 30 min has no adverse effects, is safe and useful for improving health. However, further study must be performed to clarify the mechanism of elevated IL-8 level following scuba diving.

Non-vitamin K antagonist oral anticoagulants (NOACs) are an alternative to vitamin K antagonists (VKAs) and have emerged as the treatment of choice in Korea. However, several questions remain regarding the optimal use of these agents in specific clinical situations. In this paper we discuss 1) patients with atrial fibrillation (AF) and coronary artery disease, 2) avoiding confusion with NOAC dosing across indications, 3) cardioversion in a patient treated with a NOAC, 4) AF patients who present with acute stroke while on NOACs, 5) NOACs in special situations, 6) anticoagulation in AF patients with a malignancy, and 7) optimizing VKA dose adjustments. Lowering low-density lipoprotein-cholesterol (LDL-C) is the primary target to prevent cardiovascular events in patients with dyslipidemia at high risk for cardiovascular disease. Many patients on statin therapy have initial or recurrent coronary heart disease events despite reductions in LDL-C. Indeed, 2/3 of patients on statin therapy suffer from residual risk. Low high-density lipoprotein- cholesterol (HDL-C) and high triglycerides levels are modifiable and important factors to resolve a residual risk. Especially, low serum levels of HDL-C (< 40 mg/dL for men, < 50 mg/dL for women) are highly prevalent and are recognized as an independent risk factor for cardiovascular morbidity (myocardial infarction, stroke, peripheral arterial disease, and restenosis after coronary stenting) and mortality. Thus, therapy focusing on raising HDL-C may be an important paradigm for treating and slowing progression of atherosclerosis, coronary heart disease, and co-morbid metabolic disorders. In this review, we discuss the importance of HDL-C based on experimental and large scaled clinical trials. (Korean J Med 2011;80:397-401)

Atrial fibrillation (AF) is the most common form of sustained arrhythmia in elderly patients. However, AF is often detected during health screening, or accidentally during testing for other diseases; some patients lack clinical symptoms. Nevertheless, AF increases the incidence of ischemic stroke and other thrombotic events, and compromises cardiovascular prognosis in terms of heart failure, dementia, and hospitalization. Therefore, initial AF management should be performed at the point of primary care, not only in specialized medical centers. We wish to propose a five-step management protocol for AF. We review the evidence supporting integrated management by primary care physicians new to AF, and by specialized physicians who often diagnose and manage AF. Further, we also outline a structured goal-based follow-up protocol; this is an important part of integrated management. (Korean J Med 2018;93:336-343)
