Polycythemia vera is one of the myeloproliferative diseases, and as such, is an example of clonal hematopoiesis. The progeny of a single, abnormal, hematopoietic stem cell gain a grwoth advantage over their normal counterparts resulting in overproduction of red cells generally accompanied by overproduction of granulocytes and platelets as well. There are a variety of nonspecific symptoms at onset related to the increased red cell mass and hematocrit accompanied by the more specific manifestations of pruhtis, erythromelalgia, and hepatic, portal, and mesenteric vein thrombosis. Splenomegaly and hypertension are common. Thrombosis and leukemia are frequent complication in polycythemia vera and are associated with severa organ damage and a high mortality. We report here the case of occurrence of myocardial and cerebral infarction in a patient with polycythemia vera.

We have studied the efficacy of rhG-CSF in patients with non-hodgkin's lymphoma, acute leukemia, and small cell lung cancer undergoing anticancer chemotherapy. These patients were below leukocyte count 3,000 cubic millimeter due to myelosuppression induced by the first cycle of intensive chemotherapy. Treatment with rhG-CSF (100㎍ per square meter of body surface area per day in a 30-minute intravenous infusion) was begun two days and for 14 consecutive days after the end of the second cycle of chemotherapy. The results were as follows. 1. The onset of myelosuppression was 6 days after chemotherapy, and the onset of recovery was 16.7 days after chemotherapy, and the duration of granulocytopenia was 10.7 days in patients with malignant tumor during contrast period. 2. The duration of granulocytopenia was shortened 5.2 days in patients administered rhG-CSF than without rhG-CSF, and we observed the shortest duration of granulocytopenia with increasing granulocyte in patients with small cell lung cancer. 3. Observing the differential count of leukocyte checked the highest level of leukocyte in contrast and rhG-CSF period, the persentage of neutrophil was increased in patients with acute leukemia and small cell lung cancer during rhG-CSF period, but not increased in patients with non-hodgkin's lymphoma. 4. rhG-CSF was not influenced on liver faction, renal fuction, uric acid, and glucose metabolism, also had no effect on recovery of platelet. In conclusion, rhG-CSF can be administered to patients with hematologic or nonhematologic malignant tumor that myelosuppression induced by anticancer chemotherapy result in shortening the duration of granulocytopenia and increasing the peripheral neutrophil, therefore full dose chemotherapy can be administered on time, and rhG-CSF may reduce the morbidity and mortality of patients with malignant tumor undergoing chemotherapy.

To develop a clotrimazole-loaded solid suppository with poloxamer and propylene glycol, the melting points of various formulations composed of poloxamer 188 (P 188) and propylene glycol were investigated. The dissolution study of clotrimazole delivered by the suppository composed of P 188 and propylene glycol was performed. The mixtures composed of P 188 and propylene glycol were homogeneous. Propylene glycol affected the melting points of poloxamer mixtures. In particular, the mixture [P 188/propylene glycol (70/30%)] with the melting point of about 32°C was a solid form at room temperature and instantly melted at physiological temperature. Furthermore, propylene glycol affected greatly the dissolution rates of clotrimazole from the suppository. Dissolution mechanism analysis showed the dissolution of clotrimazole was proportional to the time. Our results indicated that the solid suppository with P 188 and propylene glycol would be a candidate of rectal dosage form for clotrimazole.

<P>The signaling pathway of insulin/insulin-like growth factor/phosphatidylinositol-3 kinase/Akt/forkhead transcription factors is known to control life span and senescence in organisms ranging from yeast to mice. The FOXO family of forkhead transcription factors, FOXO1, FOXO3a, and FOXO4, play a critical role in this signal transduction pathway. However, the impact of FOXO3a activation on life span of primary cultured human dermal fibroblasts (HDFs) is unknown. To investigate the role of FOXO3a in the regulation of cellular senescence, we prepared FOXO3a-siRNA stable HDFs. We found that the down-regulation of FOXO3a RNA and protein in HDFs induced many senescent phenotypes, including changes in cell morphology, increases in population doubling times, senescence-associated beta-galactosidase staining and the cellular reactive oxygen species, and up-regulation of p53/p21 protein expression. Our data provide evidence of the key role of FOXO3a transcription factor as a mediator of cellular senescence in HDFs, and suggest that the mechanism of senescence is conserved in HDFs.</P>
Comparison of Bacterial Counts on the Hand Surface Based on Various Hand Washing Methods

The purpose of this study was to compare the effect of different methods of hand washing by counting the number of bacteria on the hand surface. Eighteen clinicians were chosen and divided into three groups, consisting of six clinicians each. Culturing of the right raw palms of all individuals was performed. Individuals in the control group washed hands for 5 seconds with antimicrobial soap. Group 1 washed their hands for 10 seconds with antimicrobial soap. Group 2 washed with an instant alcohol-based hand sanitizer. After the respective washes, re-culturing of the right raw palm was done for each member of all groups. The colony-forming units (CFU) were calculated at each time point, and the reduction rate of CFU among the three groups were statistically evaluated using student t-test. All groups showed a significant decrease in CFU, according to the time applied (P<0.01). In addition, the reduction rate of CFU between the groups were statistically evaluated with ANOVA (P<0.01). It showed statistically difference between the control group and group 1, control group and group 2. The present study confirmed that the hand washing method with antimicrobial soap for 10 seconds and hand sanitizer, including alcohol, were excellent for decreasing the number of bacteria on the hand surface.

<B>ABSTRACT</B><P>A mixed natural preservative, including grapefruit seed extract (GSE), cinnamaldehyde (CA), and nisin, was investigated for the reduction of Listeria monocytogenes growth on lettuce and raw pork loin. The MIC of each natural preservative was investigated for L. monocytogenes strains tested. Following central composite design, lettuce and pork loin were inoculated with a cocktail of three strains of L. monocytogenes (ATCC 15313, H7962, and NADC 2045 [Scott A]) and treated with the mixed natural preservative that included GSE (0.64 to 7.36 ppm), CA (1.6 to 18.4 ppm), and nisin (0.48 to 5.5 ppm). The MIC of GSE was 31.25 ppm in tested L. monocytogenes strains, and of CA was 500 and 1,000 ppm in L. monocytogenes ATCC 15313 and the other L. monocytogenes strains, respectively. The MIC of nisin was 250 ppm. The R2 value of this model was more than 0.9, and the lack of fit was not significant. The mixed natural preservative showed a synergistic antimicrobial effect and reduced the growth of L. monocytogenes by 4 to 5 log CFU/g on lettuce. In addition, the reduction of L. monocytogenes on pork loin was 3 log CFU/g. The mixed natural preservative, which consisted of GSE (6 to 8 ppm), CA (15 to 20 ppm), and nisin (5 to 6 ppm), increased the antibacterial effect against L. monocytogenes. These results suggest that the use of the mixed natural preservative could reduce the economic cost of food preparation, and response surface methodology is considered effective when measuring synergy among antimicrobials.</P>
An RTP-based dimming control system for visual comfort enhancement and energy optimization

<P><B>Abstract</B></P> <P>This study is to suggest an RTP based dimming control system designed to address visual unpleasantness among occupants due to drastic change of illuminance and to optimize power consumption and expenses. The suggested system adjusts the lighting output in the current space according to the target illuminance and the daylight quantity in that space. If the change of illuminance exceeds 20% of the previous illuminance, the control time interval and number of controls are adjusted through the polling period control algorithm so that the occupant cannot recognize the visual unpleasantness due to change of illuminance. In addition, the system classifies charges into three steps depending on the power load in reference to the RTP. The maximum target illuminance of office space is 600lx, the average 400lx, and the min. 300lx respectively. To evaluate the performance of the suggested system, two systems were applied to two comparison groups—comparison group A and comparison group B to which the polling period control algorithm was not applied: comparison group A was set to maintain the fixed target illuminance regardless of changes in electric charges while comparison group B was designed to change the target illuminance depending on the RTP.</P> <P><B>Highlights</B></P> <P> <UL> <LI> RTP-based lighting control system in the smart grid was developed. </LI> <LI> The target lighting level was differently applied according to the power load steps. </LI> <LI> Polling period was controlled according to changes in the amount of lighting levels. </LI> <LI> Energy consumption and energy cost and frequency of visual discomfort were evaluated. </LI> </UL> </P>
