Early repolarization is a common electrocardiographic (ECG) feature found in young adults, men and athletes, and has been considered to be a benign feature for the last several decades. But recent studies suggest that early repolarization may be related to idiopathic ventricular fibrillation and sudden cardiac death. We report a young man, 35 years old, who had life threatening ventricular fibrillation and sudden cardiac arrest. He was evaluated for cardiac causes of ventricular fibrillation. There was no explanation other than that his ECG showed an early repolarization pattern so we treated him with implantable cardioverter defibrillator. Thus, we suggest that early repolarization may be related with life threatening ventricular arrhythmia.
번데기동충하초(Cordyceps militaris CMO02)의 인공자실체 형성조건

This study was conducted to investigate cultural conditions for artificial fruiting body production of Cordyceps militaris CMO02. The artificial fruiting body showed redness in colour and the length was 89∼93mm, and the number was 150∼170 in a bottle. The length and number of fruiting body were longer and many in media of the hulled rice and naked barley than that of silkworm pupa. The culture medium of hulled rice and naked barley showed higher artificial fruiting body compared to that silkworm pupae. The formation period of an artificial fruiting body of Cordyceps militaris treated with hulled rice and naked barley were 45 and 46 days, respectively. The optimum illumination was 400 lux for the fruiting body formation. Pinheading and development of Cordyceps militaris was quickly decreased at passage of three subculture.

An increasing frequency of methicillin resistant S. aureus(MRSA), methicillin resistant coagulase negative staphylococci(MRCNS) and Enterococcal infection have been observed in recent years. Teicoplanin is a new glycopeptide antibiotic obstained from the Actinoplanes teicomycetius. The molecular structure and spectrum of antimicrobial activity of teicoplanin is simillar to those of vancomycin, and has been reported to have an excellent in vitro and in vivo effect against various gram-positive infections. Therefore, we evaluated the in vitor susceptibility of gram positive cocci, such as, S. aureus, coagulase negative Staphylococci(CNS), and Enterococci to teicoplanin and vancomycin. The total 253 strains consisted of MSSA(40), MRSA(53), MSCNS(47), MRCNS(48), and Enterococci(65). They were assayed by disc diffusion and agar dilution. During the study, 57% of S. aureus and 49% of CNS showed resistance to methicillin. The inhibitory diameter of teicoplanin was 15-20mm in MSSA, 12-19mm in MRSA, 13-24mm in MSCNS, 11-23mm in MRCNS, and 15-22mm in Enterococci respectively, and showed sensitivity in all but 8 strains(3.2%). The range of the minimum inhibitory concentration (MIC) of teicoplanin to MSSA, MRSA, MSCNS, MRCNS and Enterococci were 9.12-2.0㎍/ml, 0.25-2.0㎍/ml, & 0.25-32㎍/ml, 0.12-1.0㎍/ml respectively. One case of S. haemolyticus was resistant to teicoplanin (32㎍/ml) by the agar dilution method. Eight minor (3.2%) and one major(0.4%) error was observed when the MIC and disk diffusion data were correlated with teicoplanin. As for vancomycin the inhibitory diameter was 17-21mm in MSSA, 15-21mm in MRSA, 18-26mm in MSCNS, 18-25mm in MRCNS, and 16-22mm in Enterococci respectively. The range of the MIC of vancomycin to MSSA, MRSA, MSCNS, MRCNS, and Enterococci were 0.25-1.0㎍/ml, 0.25-4.0㎍/ml, 0.5-2.0㎍/ml and 0.5-2.0㎍/ml respectively. One minor error (0.4%) was seen with the vancomycin disk. The MIC90 of MSSA and MRSA exhibited the same results in teicoplanin (1.0㎍/ml, 1.0㎍/ml), and vancomycin(2.0㎍/ml, 2.0㎍/ml). MSCNS and MRCNS exhibited greater MIC90 with teicoplanin(4.0㎍/ml, 8.0㎍/ml) than vancomycin(2.0㎍/ml, 2.0㎍/ml). Incontrase Enterococci were more susceptible to teicoplanin(0.5㎍/ml) than to vancomucin (2.0㎍/ml). Results from this analysis indicated that both teicoplanin and vancomycin were very excellent for gram positive infections, especially those resistant to methicillin.

Brunner’s gland hyperplasia is a benign tumor of the duodenum and it is rarely associated with clinical symptoms. We report on a 64-yr-old man with Brunner’s gland hyperplasia who had undergone a duodenocephalo-pancreatectomy. The reason is that he presented upper gastrointestinal obstructive symptoms and the esophagogastroduodenoscopic finding revealed the lesion to be an infiltrating type mass on the second portion of the duodenum with luminal narrowing. An abdominal computed tomography showed a 2.5 cm-sized mass in the duodenal second portion with a suspicious pancreatic invasion and 7 mm-sized lymph node around the duodenum. Duodenocephalopancreatectomy was successfully performed. Histological examination revealed a Brunner’s gland hyperplasia. The final diagnosis was the coexistence of Brunner’s gland hyperplasia and pancreatic heterotopia with a pancreatic head invasion. The literature on Brunner’s gland hyperplasia is reviewed.

Recently, several clinical trials reported that intralymphatic immunotherapy (ILIT) for some allergens such as cat dander and pollen induced tolerance faster than conventional subcutaneous or sublingual immunotherapy with comparable duration of effect after only 3 injections, and did not provoke serious local or systemic reaction.?However, the efficacy and safety of ILIT for various allergens in allergic rhinitis still remains to be investigated. We evaluated the efficacy and adverse effect of ILIT for house dust mite, cat, and dog allergy in patients with allergic rhinitis.?A total of 11 subjects with allergic rhinitis sensitized to Dermatophagoides farinae, Dermatophagoides pteronyssinus, cat, and/or dog allergen received three intralymphatic inguinal injections of causal allergen extract (HollisterStier, New Orleans, USA). Clinical parameters were assessed before ILIT, 4 months, and 1 year after the day of 1st 14 injection of ILIT.?Rhinitis symptoms were alleviated and quality oflife was improved 4 months after ILIT, and these improvements lasted 1 year after ILIT.?However, we observed two cases of anaphylaxis, one case of other moderate-to-severe systemic hypersensitivity reaction and one case of severe local reaction at injection site after ILIT.?In conclusion, ILIT can rapidly improve allergy symptoms and quality of life, and this effect lasts for a year. However, ILIT can also provoke severe systemic or local hypersensitivity reaction, and we should reduce doses of ILIT in hypersensitized patients. <P><B>Abstract</B></P> <P>We prepared a high-concentration suspension of boron nitride (BN) nanoplates stabilized with benzyl alcohol (B-BN) in epoxy resin as a polymer composite adhesive for a highly efficient heat dissipation. At a BN concentration of 40 wt%, the polymer composite with B-BN exhibited a high thermal conductivity (<I>κ</I> = 1.51 W/m·K at 25 °C) comparable to that of a composite with bulk BN and significantly higher than that of a composite with a chemically modified BN (S-BN). Furthermore, the concentration of B-BN in the epoxy resin was increased to 46 wt% without a significant increase in viscosity, leading to a further improvement in the thermal conductivity to 2.11 W/m·K. The epoxy resin with B-BN exhibited a low coefficient of thermal expansion and high effective modulus owing to the strong affinity to the epoxy. Finally, we recycled benzyl alcohol more than 10 times for the preparation of B-BN, which may reduce the manufacturing cost and environmental pollution. Abdominal tuberculosis is not a rare disease, but obstructive jaundice caused by tuberculosis (tuberculous lymphadenitis, tuberculous enlargement of the head of pancreas, and/or tuberculous stricture of the biliary tree) is rare. We recently experienced a case of obstructive jaundice as a result of paradoxical reaction of periportal tuberculous lymphadenopathy that was treated successfully with corticosteroid and biliary drainage.
