HCV, Acute, LT : Cholestatic Hepatitis Related to Use of Fimasartan

Background: Fimasartan(KanarbR), originally developed by Boryung Pharmaceuticals, is new angiotensin ll receptor antagonist (ARB) used for the treatment of hypertension and heart failure. Adverse hepatic events, especially hepatotoxicity, have rarely been reported in ARB drugs. We report a case of toxin hepatocellular injury which is associated with fimasartan. Case: A 42-year-old woman developed jaundice a few months after the beginning of treatment with fimarsartan potassium 120 mg. Serological studies and computed tomography ruled out viral hepatitis, autoimmune hepatitis and extrahepatic ob- structive jaundice, respectively Bilirubin levels peaked at 19.0 mg/dL, and serum aspartate aminotransferase and alanine aminotransferase levels peaked at 531 and 743 U/L, respectively. A percutaneous liver biopsy showed a cholestatic hepatitis with predominant zone 3 hepatocyte necrosis, ballooning degeneration, congestion and mild portal inflammation. Serum levels of ALT and bilirubin rapidly decreased to normal range after discontinuation of Fimarsartan. Conclusions: The clinical-histologic data and the exclusion of known causes of liver disease led us to make a diagnosis of drug-induced cholestasis. To our knowledge, this is the first description of severe cholestatic hepatitis caused by an adverse reaction to fimarsartan potassium.
과시효처리된 7075 AI합금에 있어서 압연조건이 재결정조직과 강도에 미치는 영향

항공기용 고력 AI합금인 7075 AI합금의 물성개선을 위한 가공열처리 방안으로서 과시효처리후 온간 및 냉간 압연에 의한 소성가공이 재결정조직과 강도에 미치는 영향에 대하여 연구하였다. 소송가공후 재결정처리에 의해 결정립을 미세화하는 공정에서 소성가공 전 과시효를 함으로써 재결정립의 미세화 정도는 더욱 현저하였다. 이는 과시효에 의한 조대한 석출물이 재결정처리시 핵생성 site로서 작용하였음을 의미한다. 본 연구에서 항복강도와 재결정립의 크기와는 Hall-petch식을 만족하지 않았으며 이는 등축이 아닌 연신된 결정조직에 기인된 결과로 보이며, 항복강도가 재결정립의 종횡비에 직선적으로 비례하였다. 또한 결정립 미세화를 통한 강도, 인성향상을 목적으로 하는 TMT(Thermo mechanical Treatment)공정에서는 심한 냉간가공에 의해서보다는 온간에서의 소성가공이 바람직함을 알 수 있었다. We studied on the effects of hot-worm rolling on recrystalized structures and tensile strength in over-aged 7075 A1 alloy, to develop the process for improving properties. It showed more clear effect of the grain refinement with over-aging before plastic deformation. That means, the coarse precipitates from over-aging play a roll as nucleation sites in the course of recrystallization. And on this study, the relations between yield strength and grain size was not satisfied with Hall-Petch equation because of the elongated structure, but the yield strength is proportional to aspect ratio of grains. In TMT process for improving strength and toughness, the worm working is available for increase of those properties than cold working.

HVDC 시스템에서 AC 필터는 AC 계통으로 혼입되는 고조파를 억제하며 컨버터 운전에 필요한 무효전력을 공급하는 역할을 수행한다. 본 논문에서는 +/-80㎸ HVDC용 11/13차 Double-Tuned Filter와 24차 High-Pass Filter를 설계하여 시뮬레이션을 모의하였고, AC 필터 스위칭 시퀀스에 따른 무효전력 운영방안을 기술하였다.

Objective: The objectives of this study are to update the information on the characteristics of humidifier disinfectant (HD) usage with HD associated health problems and to discuss the role of HD exposure assessment in the national program to monitor health problems in patients with HD associated diseases. Method: A total of 201 HD associated patients who registered to undergo clinical examination at Kangbuk Samsung Hospital was interviewed to acquire their responses regarding several HD use characteristics, including type of HD brands used, HD use duration, average daily HD use hours, and the volume of the room in which HD was used. The responses of the HD associated patients to HD use-related questions were compared between responses in the lung injury investigation and this study. Results: The responses of HD associated patients in this study were found to be different from those in the lunginjury investigation. In particular, some of the patients who had not answered in the lung-injury investigation were able to answer thanks to assistance from the investigator in this study. For their responses regarding the name of the most commonly used HD brand, the number of patients who did not answer (N=11) was reduced to eight in this study. Significant changes in the responses of study subjects to questions related to HD use were achieved through the interview in this study. Conclusion: This study found significant changes in the responses regarding HD use characteristics. HD exposure assessment should be included in the program in order to allow surveillance of HD associated health problems.
