Fetal adenocarcinoma is a rare adenocarcinoma subtype of pulmonary blastoma. A 48-year-old male patient is being referred to our hospital due to progressive dyspnea. A chest X-ray showed a lung mass of unknown origin that was obstructing the right main bronchus. After relieving the airway obstruction with stent insertion via bronchoscopy, a diagnosis of fetal adenocarcinoma is being confirmed through thoracoscopic biopsy. Due to the locally advanced state of the lung cancer, it seemed to be inoperable, and concurrent chemo-radiation therapy was being administered with docetaxel. The stent was removed after improvements in the airway obstruction followed by a lung mass shrinkage. Comparing to other contexts which describe fetal adenocarcinoma as lower grade malignancy with low-associated mortality, herein, we describe a case of locally-advanced fetal adenocarcinoma (T4N3M0). This is the first documented case being treated with concurrent chemoradiation therapy. The followed-up image studies represent a partial response and the patient is currently under further observations.
Heterogeneous Trajectories of Cognitive Function in Older Adults with Diabetes

본 연구는 지역사회 거주 당뇨노인의 인지기능의 변화궤적 양상을 확인하고자 하였다. 한국복지패널 1차년도부터 6차년도 자료를 활용하여, 당뇨진단을 받은 65세 이상의 노인 672명의 자료를 분석하였다. 성장혼합모형 분석 결과, 당뇨노인의 인지기능의 변화양상은 ‘정상-유지형’ (41.5%), ‘정상-저하형’ (23.8%), ‘경도인지장애-유지형’ (17.1%), ‘경도인지장애-저하형’ (11.2%), ‘중증인지장애-저하형’ (6.4%)의 5가지 유형으로 확인되었고, 연령, 성별, 가계소득, 자기보고 건강상태가 영향 요인으로 확인되었다. 본 연구 결과를 통해 당뇨노인의 인지기능의 변화 양상은 이질적이며, 당뇨노인의 인지기능 저하를 예방하기 위해 대상자에 대한 개별적인 이해와 맞춤형 중재가 필요함을 확인할 수 있었다. Background: Given the rapidly aging population and increasing diabetes prevalence in Korea and worldwide, maintaining optimal cognitive function is a concern in older adults with diabetes. This study aimed to identify heterogeneous trajectories of cognitive function in older adults with diabetes based on the Mini-Mental State Examination and characterize these trajectories. Methods: Study participants consisted of 672 community-dwelling Korean older adults with diabetes, and data were obtained from the population-based Korean Longitudinal Study of Aging from 2006 through 2016. Growth mixture modeling (GMM) was employed to identify distinct trajectories of cognitive function in older adults with diabetes. Results: We found that older adults with diabetes followed five distinct cognitive function trajectories over 10 years: normal-maintained (41.5%), normal-declined (23.8%), mildly impaired-maintained (17.1%), mildly impaired-declined (11.2%), and severely impaired-declined (6.4%). Also, we found that age, gender, household income, and self-rated health status at baseline were associated with these trajectories. Conclusions: Our identification and characterization of heterogeneous cognitive function trajectories in older adults with diabetes have significant implications for developing tailored interventions to older adults with diabetes, specifically to whom at heightened risk for cognitive decline and consequent deterioration in essential diabetes self-management activities.
