      • 일 종합병원 입원환자의 병원 서비스 만족도와 관련 특성에 관한 연구

        국민의 전반적인 생활수준의 향상과 더불어 환자의 기대수준도 한층 더 높아짐에 따라 양적 충족보다는 질적인 측면으로 관심이 전환되었으며 모든 사람이 양질의 의료서비스를 받고자 하는 요구가 그 어느 때 보다도 높다고 할 수 있다. 병원서비스에 대한 환자의 만족도는 병원경영 관리상의 중요한 개념이며, 병원서비스의 질을 논의함에 있어서도 중요한 요소의 하 나가 된다. 본 연구는 입원환자가 느끼는 병원서비스 만족도와 관련 특성을 파악하기 위하여 시도되었다. 자료 수집은 강릉시 600병상 규모의 일 종합병원에 3일 이상 입원한 환자 350명을 대상으로 2004년 8월 2일부터 8월 31일까지 하였다. 연구도구로 병원서비스 만족도 도구는 한금옥(1995)의 연구에서 사용된 설문지를 본 연구 목적에 맞게 42문항으로 재구성하였다. 본 연구를 위한 자료 분석은 SPSS(10.0) 통계 프로그램을 이용한 전산통계처리로 다음과 같이 분석하였다. 1) 연구대상자의 일반적 특성과 병원서비스 관련 특성은 빈도와 백분율로 분석하였다. 2) 연구대상자의 병원서비스 만족도는 평균과 표준편차를 구하였다. 3) 연구대상자의 병원서비스 만족도의 요인을 규명하기 위하여 요인분석을 실시하였다. 4) 연구대상자의 일반적 특성과 병원서비스 관련 특성에 따른 병원서비스 만족도는 t-test와 ANOVA로 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1) 연구대상자의 일반적인 특성으로 성별은 여자 106명(41.2%)보다 남자가 137명(53.3%)으로 많았고, 연령은 40∼59세가 98명(38.1%)으로 가장 많았다. 학력은 고졸 93명(36.2%), 대졸 54명(21.0%)으로 연구대상자의 학력이 대체로 높게 나타났다. 2) 병원서비스 관련 특성으로 입원 후 치료경과에 대해 112명(43.5%)이 매우 만족하거나 만족해했으며, 221명(86.0%)이 본 병원을 재이용하겠다고 응답하였고, 병원선택으로는 최신의료장비와 시설 114명(44.4%), 해당과의 우수한 의료진 94명(36.6%), 좋은 치료결과 71명(27.6%)등을 들고 있었다. 3) 병원서비스 만족도의 정도는 최대 평점 5점에 대해 전체 평균평점 3.65±.49점으로 전반적인 병원서비스 만족도는 '만족스럽다'에 가까운 결과가 나타났으며, 병원서비스 만족도의 요인 중 간호서비스 요인이 평균평점 3.97±.55점으로 가장 높은 점수를 보였고, 검사 요인은 평균평점 3.83±.60점, 의사서비스 요인은 평균평점 3.81±.69점 순으로 나타났다. 4) 일반적 특성에 따른 병원서비스 만족도는 성별이 관리 요인에서 유의한 차이(t=3.77, p=.05)가 있었고, 종교는 종교 요인에서 유의한 차이(F=5.77, p=.05)가 있었다. 5) 병원서비스 관련 특성에 따른 병원서비스 만족도는 입원 후 치료경과(F=13.93, p=.00), 병원 재이용 유·무(t=10.70, p=.00), 병원 권유의사 유·무(t=21.86, p=.00)에서 유의한 차이가 있었고, 병원 선택요인에 따른 병원서비스 만족도에서는 타 의료기관의 권유가 유의한 차이(t=4.32, p=.04)를 나타냈다. 본 연구의 결과로 병원서비스 만족도에는 간호서비스의 영향이 크다는 것을 알 수 있었다. 또한 병원서비스에 만족하면 병원을 재이용하고, 다른 사람에게 병원이용을 권유한다는 것을 알 수 있었다. 병원서비스 만족도를 높이는 것이 병원의 경쟁력을 갖추는 데 중요한 영향을 미친다. 따라서 병원서비스의 만족도가 낮은 부분을 분석하여 입원환자들의 만족도를 높여야 하고, 특히 간호서비스가 병원서비스 만족도에 미치는 영향이 크므로 간호의 질을 높이는데 많은 관심을 가져야 할 것이다. As the level of expectation of patients increases according to the improvement of standard of living, desire for quality of hospital service has been stronger than ever, changing from quantitative satisfaction into qualitative one. The level of patient's satisfaction with hospital service provided by a hospital is thought to be one of the most important elements in discussion about the quality of hospital services and management. This study is to find service satisfaction and related characteristics by patients of general hospital. To collect data, questionnaires were completed by 350 patients hospitalized for more than three days in general hospitals categorized into secondary medical institution with 600 beds located in Kangneung from 2nd through 31st, August, 2004. For the purpose of studying hospital service satisfaction, the questionnaire initially used by Han Kum Ok(1995) was modified and compiled into 42 items to be fit for this study. The data obtained from the research through questionnaires were analyzed by computerized SPSS(10.0) statistics program in the manner explained in the following. 1) Frequency and percentage were employed for analyzing general characteristics of participants and hospital services provided by the hospitals. 2) Mean and standard deviation were used to analyze the satisfaction with the medical services of the participants. 3) In order to examine the factors influencing satisfaction with medical services of the participants, a factor analysis was used. 4) ANOVA and t-test was employed for analyzing satisfaction with medical services according to general characteristics of participants and medical services. The results from this study are summarized in the following. 1) In regard to sex among general characteristics of the participants, female patients, accounting for 106(41.2%), were more than their male counterparts of 137(53.3%), while the age of 40∼59 ranked top by 98(38.1%). Their educational background was high; high school graduates were 93(36.2%), and university graduates were 54(21.0%). 2) Concerning one of the general characteristics of hospital service, physical condition after hospitalization, 112(43.5%) of the participants responded that it was highly satisfactory. Regarding the item asking whether they would visit the hospital again, 221(86.0%) of them gave a positive answer, while decisive factors to choose a hospital included high-tech medical equipments and facilities(114 patients, which was 44.4%), excellent medical staff(94 patients, which accounted for 36.696), and good prognosis(71 patients, which was 27.6%). 3) Regarding 5-scale medical services item, overall average was 3.65?49, which meant that satisfaction with hospital services in general showed 'moderately satisfied'. Among the factors of satisfaction with hospital services, service by nurses was in the highest at average 3.97±.55, followed by test factor at average 3.83±.60 and service by doctors at 3.81±.69. 4) In relation with satisfaction with hospital services according to general characteristics, sex had a significant difference with controlling factors(t=3077, p=.05), while religion showing a significant difference with religion factors(t=5.77, p=.00). 5) In satisfaction with hospital services according to hospital services-related characteristics, physical condition after hospitalization showed a significant difference with all the factors(F=13.93, p=.00). Both willingness to recommend the hospital(t=21.86, p=.00) and possibility to visit the hospital again(t=10.70, p=.00) had a significant difference with all the factors. Regarding to satisfaction with hospital services according to decisive factors of hospital choice, recommendation by other medical institutions showed a significant difference with all the factors(t=4.32, p=.04). The findings from this study suggest that satisfaction with hospital services depend heavily on nursing service. Also, if satisfied with hospital services, patients tend to visit again and recommend to others. Therefore, it is essential to raise the satisfaction level with hospital services by analyzing the areas with low service satisfaction in order to increase the competitiveness of hospital. In particular, priority should be placed on improvement of services by nurses, because services by nurses proved to much influence the satisfaction level with hospital services.

      • 에스겔서와 요한복음의 메시아 이해 상호본문성 연구 : 에스겔 34-48장을 중심으로

        This dissertation argues that the Messianic idea of the Old Testament Ezekiel 34-48 provide John the author of the Gospel with a theological foundation to see Jesus as the Messiah. To accomplish this goal, I examine the contributions and limitations of scholars who have attempted to link the two texts, based on the lexical link between Ezekiel and John. And through textlinguistic methodology, I analyze the Messianic idea in Ezekiel 34: 1-3, 36: 16-38, 37: 1-14, 37: 15-28, 40-48 and Attempt a conceptual connection with the Gospel of John. In chapter 2 of this dissertation, I attempt to connect Ezekiel 34 with the Gospel of John. Ezekiel 34 examines three themes that not only provide the context for the vocabulary found in John 9-10, but also serve as a theological background for Jesus' identity, which appears throughout John. First, the shepherds of Ezekiel 34 and Jesus of John are kings. Second, the shepherd and Jesus are subordinate to God and are sent by God. Third, the shepherd and Jesus are those who do the work of God. In chapter 3, I attempts to connect Ezekiel 36:16-38 with the Gospel of John. Beyond the connection of the mention of water and spirit in Ezekiel 36:16-38 and John 3:5 no one can enter the kingdom of God unless he is born of water and the Holy Spirit.””, I examined four themes that appear throughout the Gospel of John. First, the Messiah in Ezekiel 36:16-38 is the one who purifies God's people with water, Jesus is baptized in John 1:29, baptized in John 3:22, In John 9 and 13:1-11, he appears to be washed with water to purify sin. Second, the Messiah in Ezekiel 36:16-38 is the giver of the Spirit of God, and in John 1:33 Jesus appears to be ‘the baptizer of the holy spirit’', In John 7: 37-39, 14:26 20:22, Jesus is presented as the Spirit-giver. Third, the Messiah in Ezekiel 36:16-38 appears as the demander of the law's works to God's people that renewed in water and spirit, and in John 13:34-35, Jesus also told his disciples that he had washed his feet. Give the new commandment, “Love one another as I have loved you.” Fourth, the Messiah in Ezekiel 36:16-38 appears to be involved in restoring God's honor, and John 17 makes us understand that all the work of Jesus is to give glory to God. In chapter 4, I attempts to connect Ezekiel 37: 1-14 with the Gospel of John. The main characteristic of Ezekiel 37: 1-14 is that it presents Israel's national restoration and the transition from death to life. In Gospel of John, Jesus is baptized with water and the Holy Spirit, present himself as the true vine and represent himself as Israel. Therefore, Jesus' death and resurrection is the death and resurrection of Israel in Ezekiel 37: 1-14. In chapter 5, I attempted to link Ezekiel 37:15-28 with the Gospel of John. The Davidic figure in Ezekiel 37: 15-28 fulfills the union of Judah and Joseph and guarantees the restoration of Israel's eternal covenant relationship with God. John 4:1-42 suggests that Jesus is David who brings the union of Judah and Joseph in Ezekiel 37: 15-28, and at the same time assures the eternal covenant of God and his people. In chapter 6, I examines character of           as the Messianic figure in Ezekiel 43:12-46:24. Clearly, Jesus of Gospel of John can be presented as a new temple in Ezekiel 47:1-2. Not only was it presented, but it was presented as          , the Messianic figure, as the only mediator to obey Jehovah's law, assuring Jehovah's presence, and connecting Jehovah and his people. Three results were obtained through the above study. First, John has authority in Ezekiel. John looked to Jesus on the Foundation of Ezekiel's Messianic idea and identified Jesus as the Messiah. In other words, when John wrote John's Gospel, he identified Jesus as the Messiah based on Ezekiel's Messianic idea, rather than giving it a new meaning in the context of Ezekiel's text. The fact that John looked to Jesus on the basis of the Messianic idea presented by Ezekiel and confirmed Jesus as the Messiah means that John was a "standard with authority" rather than "modifying" Ezekiel's message according to his own opinion Second, it is a new hermeneutic suggestion on how the New Testament uses the Old Testament. Clearly, when the New Testament authors use the Old Testament, they tend to attempt to link the two texts through clear lexical linkages and make them discoverable to readers. However, as shown in this study, when John uses Ezekiel's vocabulary or phrases, he uses not only words or phrases but also words and phrases within the conceptual connection between the two texts. And even though he does not use Ezekiel's vocabulary or phrases, he presents Jesus as the Messiah based on Ezekiel's Messianic idea. This can be seen as a hermeneutic suggestion for the earlier researchers to emphasize that the author of the New Testament uses lexical linkage when using the Old Testament. Third, this dissertation confirms that the main concept of John's Gospel is based on Ezekiel, and that Ezekiel's Messianic idea is fulfilled in John's Jesus. This continuity of the Old and New Testaments indicates that the readers of the Bible need a holistic view of the Old and New Testaments. [Key
