By adding Gyro sensor to support the steering wheel angle sensor, an improved functional DR solution is proposed in this paper. The proposed angle data algorism is developed based on the steering wheel with Gyro sensor for DR. The Gyro sensor support the error of steering wheel sensor to improve the angle data for the DR algorism.

편심성 심실중격비대를 동반한 다발성 관상동맥-좌심실간 미세누관의 1예를 관상동맥조영술 및 경흉부심장초음파를 통해 진단하였기에 이를 보고한다. Congenital coronary artery fistulas between a coronary artery and a cardiac chamber occur in 0.2% of diagnostic cardiac catheterizations and usually arise when the right coronary artery drains into a right heart structure, such as the right atrium, right ventricle, or pulmonary artery. Fistulas draining into the left ventricle are uncommon. We report a case of multiple coronary artery-left ventricular microfistulas with eccentric septal hypertrophy presented as squeezing chest pain and discomfort at rest. This was diagnosed by coronary angiography and transthoracic echocardiography. (Korean J Med 78:495-498, 2010)

We report the case of a patient with cardiac tamponade and pericarditis in type B intramural hematoma. A 75-year-old woman was admitted to the emergency department presenting with general weakness and dizziness for several hours and hemodynamic collapse. Thoracic echocardiography and computed tomography (CT) showed a large pericardial effusion and aortic intramural hematoma but no evidence of aortic dissection. Therefore, we concluded that the intramural hematoma did not involve the ascending aorta and thus immediately performed pericardiocentesis. Follow-up CT showed no pericardial effusion or specific changes in the range or depth of the intramural hematoma, and she was discharged continuing colchicines and ibuprofen therapy for acute pericarditis. Cardiac tamponade in type B intramural hematoma is extremely rare. Prompt diagnosis and initial treatment resulted in a substantial improvement in clinical status. (Korean J Med 2015;89:695-698)

An intramural hematoma is a rare, challenging cause of myocardial infarction generally seen in middle-aged females with no atherosclerotic risk factors. Intravascular ultrasound is useful in diagnosing and managing intramural hematomas. Here, we present anintramural hematoma presenting with ST-elevation myocardial infarction without definite intimal dissection in an elderly man who was diagnosed using intravascular ultrasound and managed accordingly. 고관절 골절 수술에서 수술 전 Enoxaparin 혈전 예방 요법 대 수술 후 Enoxaparin 혈전 예방 요법

목적: 고관절 골절 환자에서 정맥혈전색전증의 예방을 위해 수술 전 및 수술 후 시작한 enoxaparin 투여방법의 안전성과 유효성을 비교 분석하고자 한다. 대상 및 방법: 2009년 3월부터 2014년 3월까지 부산대학교병원에서 고관절 골절로 내원하였던 629명의 환자 중 연구 포함기준을 만족시키는 총 414명(65.8%)을 대상으로 하였다. 156명은 enoxaparin 40 ㎎을 수술 후 48시간부터 피하주사로 시작하였고(A군), 258명은 enoxaparin 40 ㎎을 수술 전부터 피하주사로 시작하였다(B군). 양 군에서 정맥혈전색전증의 발생 빈도와 입원 기간 중 출혈 위험성을 비교 분석하였다. 결과: 증상을 동반하는 정맥혈전색전증은 A군에서 7.1% (11명), B군에서 5.4% (14명)이었으며, 두 군 간의 유의한 차이는 없었다. 증상을 동반하는 심부정맥혈전증과 폐색전증의 발생 역시 두 군 간의 유의한 차이는 없었으나, A군 중 2예는 폐색전증으로 인하여 사망하였고, B군 중 1예는 폐출혈로 사망하였다. 주요 출혈 및 입원 기간 중 사망 빈도는 두 군 간에 유의한 차이가 없었다. 결론: 혈전색전증의 위험인자를 동반하는 고령의 고관절 골절 환자에서는 수술 전부터 시작하는 약물 예방 요법을 고려해 볼 수 있다. Purpose: The purpose of this study was to evaluate the safety and efficacy of pre- and postoperative initiation of enoxaparin in treatment of hip fracture. Materials and Methods: In this study, we enrolled 629 consecutive patients with hip fracture who Pusan National University Hospital between March 2009 and March 2014. Of these patients, 414 patients (65.8%) met the final inclusion criteria. Enoxaparin was administered subcutaneously at 40 mg once daily starting 48 hours after surgery in 156 patients (group A), and immediately after admission in 258 patients (group B). The incidence of symptomatic venous thromboembolism (VTE) and risk of bleeding during hospitalization period were compared between groups. Results: The incidence of symptomatic VTE during the hospitalization period was 7.1% (11 patients) in group A and 5.4% (14 patients) in group B. No significant difference in the rate of symptomatic VTE was found between the groups (unadjusted odds ratio [OR], 0.756; 95% confidence interval [CI], 0.334–1.710; adjusted OR, 0.554; 95% CI, 0.212–1.449). The incidence of symptomatic deep-vein thrombosis and pulmonary embolism (including fatal and non-fatal) did not significantly differ between groups. However, fatal pulmonary thromboembolism developed in two cases in group A and one patient expired postoperatively due to pulmonary hemorrhage in group B. Major bleeding and all-cause death did not differ between groups. Conclusion: Preoperative starting chemical thromboprophylaxis may be considered in the elderly patients with hip fracture who have risk factors for venous thromboembolism.

흉부대동맥류는 크기가 클수록 파열로 인한 사망의 가능성이 높아지는 위험한 질환이다. 이러한 경우 수술적 치료를 통한 대동맥류의 제거가 필요하나 위험인자가 많은 고위험군 환자에 있어서는 수술에 따른 부작용이 매우 높은 편이다. 따라서 최근에 이러한 환자에서는 흉부 대동맥 혈관 내스텐트이식편 삽입술이 수술을 대체하여 널리 시행되고 있다. 이에 저자들은 고령 및 다발성 골수종으로 수술 고위험군인 대동맥궁 대동맥류 환자에서 왼쪽 온목동맥에서 왼쪽 빗장밑동맥우회로술과 흉부 대동맥 혈관 내 스텐트이식편삽입술을 함께 시행하였으며 이상성 오른쪽 빗장밑동맥이 있음에도 불구하고 특별한 합병증 없이 양쪽 팔의 혈류가 원활한 좋은 결과를 경험하였기에 문헌고찰과 함께 1예를 보고하는 바이다. Aberrant right subclavian artery (ARSA) is a rare congenital anomaly but is the most common of the congenital vascular anomalies of the aortic arch. We report the case of a 68-year-old female undergoing chemotherapy for multiple myeloma who had a large thoracic aortic aneurysm (7.4 cm) with ARSA. She was treated with a hybrid procedure that combined a left common carotid-to-subclavian artery bypass with a "thoracic endovascular aortic repair (TEVAR)" because of the risk associated with a thoracotomy. A stent graft was deployed in the proximal part of the descending aorta to cover the thoracic aortic aneurysm after a left common carotid-to-subclavian bypass was made to restore blood flow in the left arm. There was no endoleak on digital subtraction angiography. Hybrid therapy can be performed successfully for the treatment of thoracic aortic aneurysm with ARSA. (Korean J Med 2014;87:193-199)

William Faulkner의 흑인관

The purpose of this study is to investigate the racial question in William Faulkner's Light in August, The Sound and the Fury, Go Down, Moses, and Intruder in the Dust. As a Southern writer, Faulkner dealt with uniquely Southern materials with much emphasis on Black and White relationship. I have carried out this study on the assumption that the racial theme is one of the main themes in his novels. In this study I have investigated the cause and effect of the racial question and Faulkner's attitudes toward the solution to this question.. The racial problem resulting from the peculiar history of the South, slavery, is one of the most complicated and subtlest social problems in America, and quite naturally many of intellectuals are seriously concerned for its ideal solution. By 1950, when the opposition to the civil rights program was threatening to become violent, Faulkner was compelled to express his views on desegregation, equality, and civil rights in serious nonfiction statements. But his nonfiction statements were made under the influence of the surrounding circumstances, some of which were ambivalent or contradictory. However, in his fictions where he could present his ideal without conflicting with reality, we can find a progression from his early Southern stereotype to personal vision of Negroes. These shifting attitudes were presented in his major Negro characters : Dilsey, Joe Christmas, and Lucas Beauchamp. Dilsey in The Sound and the Fury emerges from th traditional black image of a loyal old black servant and stands out as an embodiment of love and endurance. Yet, she accepts her subservient position as "nigger" and expresses no criticisms of social inequality or racial discrimination. Joe Christmas in Light in August is an image of a Negro in wander and resistance. Because of his dubious blood, he fails to identify himself and repudiates the society dichotomized to skin colors, and finally accepts death as the only way to peace. Evidently Faulkner deals with less sympathy mulattoes than pure Negroes, but he also suggests that miscegenation can be ultimate solution the problem of racial antagonism. Lucas Beauchamp in Intruder in the Dust is a self-proud black man who asserts his individuality and human dignity against white people, and finally makes white people recognize and accept him as an equal man. Faulkner suggests here that the only and the best way to the ideal black-white relationship is to transcend the racial barrier through pursuing and asserting humanity and dignity as individual human beings. Finally we should remember that Faulkner, as novelist, intends to portray various universal problems which a man can face through Negroes' concrete alienation and plight in the South America.
