        구순구개열 환자의 Le Fort I 골절단술 후 상악골의 위치적 안정성에 관한 연구 ; 예비보고

        It is well known that the postoperative skeletal instability after Le Fort I osteotomy for advancement of maxilla in the cleft patients is one of the major surgical problems. So we had tried to compare the amount of relapse after Le Fort I advancement surgery in the horizontal and vertical positional change, angular change of reference points between cleft patients and non-cleft patients. Longitudinal records of 10 consecutive cleft patients (test group) and 20 non-cleft patients (control group) were analyzed. Lateral cephalograms were taken preoperatively, immediately postoperatively, and 2, 6, 12 months postoperatively. We measured horizontal and vertical changes (ANS, PNS, AI) and angular change (SNA) of the reference points and lines. In the test group, horizontal relapse of ANS, PNS, AI point are 36.4%, 37.5%, 32.0% respectively at 12 months postoperatively. The vertical relapse of ANS, PNS, AI are 25.3%, 32.3%, 39.1% respectively at 12 months postoperatively. The angular change of SNA is 33.6% at 12 months postoperatively. In the control group, horizontal relapse of ANS, PNS, AI point are 23.8%, 30.2%, 21.7% respectively at 12 months postoperatively. The vertical relapse of ANS, PNS, AI are 22.7%, 27.3%, 25.1% respectively at 12 months postoperatively. The angular change of SNA is 22.2% at 12 months postoperatively. The cleft patients have a larger tendency of skeletal and dental relapse compared with non-cleft patients after Le Fort I surgery. So the oral and maxillofacial surgeons must keep in mind these facts in order to minimize the relapse phenomenon from the beginning of surgical planning to postoperative care.

        Sequential <sup>1</sup>H MR Spectroscopy(MRS) Studies of Kaolin-Induced Hydrocephalic Cat Brain

        목 적 : 실험적으로 유발된 고양이 수두증 모델을 통하여 양자 자기공명 분광상의 수두증에서의 임상적 적응과 경시적 뇌 대사산물의 변화를 규명하는데 목적이 있다. 대상 및 방법 : 30마리의 고양이에서 뇌대조내에 kaolin을 주입후 수두증을 유발한후 양자 자기공명 분광상을 이용하여 kaolin 주입전, 주입후 1, 3, 7, 14, 21, 28일째에 주요 대사산물인 N-acetyl aspartate(NAA), creatine(Cr), choline (Cho), lactate(Lac)의 분광상을 분석하였다. 결 과 : 30마리의 고양이에서 모두 수두증이 유발되었고, 급성기에 NAA/Cr비가 감소하는 소견보였으며, 1마리에서 lactate peak이 관찰되었다. 만성기(>14 days)까지 생존한 4마리의 고양이에서 NAA/Cr비의 증가가 관찰되었으며 이는 임상적 증상의 회복과 상응하는 결과를 나타내었다. 결 론 : Kaolin 유발 수두증모델에서 양자 자기공명 분광상은 급성기의 진단적 정보의 제공과 수두증에서의 뇌대사 및 생화학적 결과를 반영해 준다. 그리고, 급성기에서 lactate peak없이 NAA/Cr비의 감소와 만성기의 NAA/Cr비의 회복은 수두증으로인한 뇌실질의 손상이 신경세포 자체보다는 축색손상을 의미한다. 급성기말까지 초기 NAA/Cr비 감소가 회복없이 lactate peak이 동반되는 경우는 불량한 예후인자가 된다. 만성기의 NAA/Cr비의 정상화는 NAA/ Cr비의 감소를 보이는 뇌피질위축의 경우와의 감별진단에 실마리를 제공한다. Objectives : The aim of this study is to evaluate the sequential metabolic changes in experimental hydrocephalus and the clinical applicability to the diagnosis and prognosis of hydrocephalus using proton MR spectroscopy. Methods : Hydrocephalus was experimentally induced in 30 cats(2-3kg body weight) by injecting 1ml of sterile kaolin suspension(250mg/ml) into the cisterna magna. Proton MRS was performed with a 1.5 T MRI/MRS unit (Vision Plus, Siemens) at pre-treatment and at 1, 3, 7, 14, 21, and 28 days after the kaolin injection. PRESS(TR/TE=1500/270msec) technique was employed. The major metabolites which include N-acetyl aspartate (NAA), creatine(Cr), choline(Cho), and lactate(Lac) were quantitatively analyzed and the relative concentrations ratios were evaluated. Multislice $T_2$-weighted images were also obtained using fast spin echo sequence(TR/TE= 2500/96msec) to monitor the morphologic changes along with progression of hydrocephalus. Results : Hydrocephalus was successfully induced in all 30 cats. Twenty five cats died within 3 days and one at the end of the second week. In all animals, the NAA/Cr ratios initially decreased during the acute stage. In 4 surviving cats, the NAA/Cr ratios initially decreased during the acute stage(<14 days) and then gradually increased to the prekaolin level as follows : pre-kaolin($1.49{\pm}0.04$), day 1($1.11{\pm}0.07$), day 7($1.17{\pm}0.04$), day 14($1.40{\pm}0.03$), day 21 ($1.46{\pm}0.06$), day 28($1.43{\pm}0.03$). These levels were relatively well correlated with the symptomatologic improvement. Lactate peak, which reflects the evidence of ischemia, did not appear throughout the entire period except in one case which expired at the end of the second week. Conclusions : The NAA/Cr ratio of the sequential proton MRS in kaolin-induced hydrocephalic cats reflects a metabolic aspect of the hydrocephalus at each stage. A decreased NAA level at the early stage is from both neuronal and axonal damage which may provide diagnostic information in the acute stage of hydrocephalus. In addition, the initial fall of NAA/Cr ratio and recovery in the late stage, when no lactate peak emerges, may suggest that the main insult of the parenchyma is not to the neuron itself but to the axon, which may be related to a good prognosis. However, emergence of the lactate peak and unrecoverable NAA/Cr at the end of the acute phase may be a poor prognostic factor. In the chronic stage, recovery of NAA/Cr ratio may provide a diagnostic clue for the differentiation between hydrocephalus and cortical atrophy.

      • 모듈화된 구조에 기반한 H.263 비디오 코덱 VLSI의 설계

        본 논문에서는 H.263 비디오 코덱 구현을 위한 효율적인 하드웨어 구조를 제안하고, 이에 기반한 VLSI 구현을 기술한다. 제안 구조는 부호화 및 복호화에 사용되는 내부 하드웨어 블록들간의 연결 및 이 블록들과 내부 RISC 프로세서간의 연결에 단일화된 인터페이스 방법을 사용한다. 이러한 단일화된 인터페이스의 사용은 블록들의 모듈화된 설계를 가능하게 하며, 하드웨어/소프트웨어의 적절한 분할 및 효과적인 파이프라인 동작을 가능하게 한다. 개발된 VLSI는 H.263 버전 2의 프로파일 3 @ 레벨 10을 지원하고, 제어용 프로토콜인 H.245와 다중화 프로토콜인 H.223을 지원한다. 따라서 외부에 오디오 코덱 칩을 추가함으로써 완전한 ITU-T H.324 또는 3GPP 3G-324M 멀티미디어 터미널 기능 구현에 사용될 수 있으며, 하드웨어 블록들을 최대한 활용하면서 소프트웨어적으로 H.263++와 MPEG4 코덱을 구현 할 수 있는 구조이다. 구현된 H.263 코덱은 40MHz의 동작 주파수에서 초당 15 프레임 이상의 성능으로 QCIF 크기의 영상의 부호화 및 복호화를 동시에 수행할 수 있다. In this paper, we present an efficient hardware architecture for the H.263 video codec and its VLSI implementation. This architecture is based on the unified interface by which internal hardware engines and an internal RISC processor are connected one another. The unified interface enables the modular design of internal blocks, efficient hardware/software partitioning, and pipelined paralled operations. The developed VLSI supports the H.263 version 2 profile 3 @ level 10, and moreover, both the control protocol H.245 and the multiplexing protocol H.223. Therefore, it can be used for the complete ITU-T H.324 or 3GPP 3G 324M multimedia processor with the help of an external audio codec. Simultaneous encoding and decoding of QCIF format images at a rate greater than 15 frames per second is achieved at 40 MHz clock frequency.

        스마트시티 정책추진과 경기도 광역자치단체 전략

        본 연구에서는 중앙정부가 추진하는 스마트시티 정책에서 경기도 광역자치단체의 역할을 알아보고자 한다. 이를 위해 중앙정부 중심으로 추진되고 있는 스마트시티 정책 사업에 도내 기초지자체의 참여현황을 파악해보고 특성을 도출하여 경기도 광역자치단체가 마련해야 하는 세부과제를 제시한다. 현재 경기도 지역은 U-City 이후에 도내 기초지자체가 제도적 기반을 마련하고 중앙정부 스마트시티 정책사업에 참여하고 있다. 경기도 광역자치단체는 2020년에 스마트시티 관련 조례를 제정하고 스마트도시 지방자치단체 협의회에 참가하는 정도이다. 향후 경기도는 법 개정을 건의하여 스마트시티 정책추진의 동력을 확보하고 경기도만의 거버넌스를 확립하여 효율적인 정책추진을 위한 체계마련을 고민한다. 또한 중앙정부 스마트시티 정책사업에 "광역자치단체 모델의 통합플랫폼 구축사업" 참여를 검토하여 재난 안전상황에 효율적으로 대응할 수 있는 시스템을 마련하고, 정책의 사각지대를 중심으로 경기도 독자적인 스마트시티 정책추진을 검토해본다. The purpose of this study is to investigate the role of Gyeonggi provincial governments in smart city policy. In order to satisfy the purpose of the study, we closely identified how may local governments(si. gun) have been taking participated in smart city project of the central government, and its characteristics. Then we know the detailed strategies that Gyeonggi provincial governments should do. Gyeonggi-do provincial governments enacted smart city-related ordinances in 2020 and participated in the Smart City Local Council. In the future, Gyeonggi privincial government will propose amendments to the law, review participation in the central government smart city policy project, the "integrated platform construction project of the metropolitan government model", and propel Gyeonggi-do's own smart city policy based on the blind spots of the policy.

        FIBRIN SEALANTS IN MAXILLOFACIAL SURGERY : A INTRODUCTORY REPORT

        Fibrin Sealants 는 과거 간, 비장, 신장등의 일반외과 수술에서 단순한 외과적 봉합으로는 해결하지 못하는 넓은 면적의 조직의 유착을 위하여 1940 년경 부터 개발되기 시작한 것으로 악안면 영역에서는 1970 년대 중반부터 미세신경접합술과 피부이식을 위하여 사용되기 시작한 후, 현재, 골이식후의 골세편의 고정, 혈관봉합술, 연조직에서 조직들의 유착과 지혈, 그리고 혈관종의 치유등을 목적으로도 광범위하게 연구되고 사용되고 있다. 이것은 인체에서 채취한 혈액응고인자 XIII 을 포함하는 Fibrinogen 성분과, 소에서 추출한 Thrombin 의 두가지 주요 성분으로 구성되며, Fibrinogen 용해제인 Aprotinin 액과 Thrombin 용해제인 염화칼슘액과 함께 네부분으로 구성된다. 각제품에 따라 그리고 사용된 농도에 따라 차이는 있으나, 대개 수분후에 조직이 응고되어 달라붙기 시작하고, 수시간후에 최대접착효과에 도달하며, 응고된 접착효과는 12 일에서 15 일간 유지되고 그후 정상적인 섬유소분해작용과 식세포활동에 의하여 분해된다. 저자는최근 6 개월간 서울대학교병원 구강악안면외과에서 28 명의 각종 질환 및 기형 환자에서 미세신경봉합술, 피부이식, 악교정성형술과 구개파열 또는 하악골 재건을 요하는 환자의 골이식후의 골세편의 고정, 경부곽청술이나 종양제거술후 각종 피부판 또는 근피판을 이용한 연조직의 적합, 혈관종의 처치, 후이개접근 법에 의한 악관절수술후의 외이도의 접합등 다양한 목적을 위하여 적용된 Fibrin Sealants 를 사용하여 양호한 결과를 얻었기에 보고하는 바이다. The fibrin sealant was first designed as an alternative to surgical suture for the purpose of surface-to-surface union especially in parenchymal organs like the liver, spleen and kidney. The clinical application of currently used fibrin sealant was first introduced in 1972. The fibrin sealant consists of principal two components; lyophilized human fibrinogen and bovine thrombin. The fibrinogen component also contains coagulation factor XIII. A solution of aprotinin, an inhibitor of fibrinolysis is used to dissolve the fibrinogen and to provide the first component, and a solution of calcium chloride is also used to provide the second component. From July to December in 1990, during 6 months, we used fibrin sealant in the 28 patients of 33 various cases, in the following ways; supportive application of fibrin sealant after free autogenouse nerve graft for the repair of inferior alveolar nerve, facial nerve or accessory nerve, treament of hemangioma or lymphangioma to thrombosize and lead to the tumor shrinking, skin grafting to stimulate the adhesion and tissue repair, bone grafting in the patients of cleft alveolus, mandibular reconstruction or orthognathic surgery to facilitate the knitting of bone chips, tissue adhesion after tumor resection, radical neck dissection or flap reconstructions, and supportive adhesion of external auditory cannal after TMJ surgery via postauricular approach. No adverse effects were observed, none of the patients developed hepatitis or other blood transmitted disease, and the wound healing were acceptable.

        단일입자재현법(單一粒子再現法)을 활용한 고고유물의 진위판별 연구

        이 연구에서는 고고학 유물의 진위여부를 판단하기 위하여 미세시료채취법을 적용하여 시료를 채취한 후 화학적으로 분리된 세립 석영 입자의 고고선량을 산출하였다. 진위판별 연구에 앞서 미세시료채취법의 타당성은 백제무령왕릉 출토 전(塼)에서 산출된 고고선량 값들로부터 입증되었다. 이를 바탕으로 고구려 시대에 제작되었다고 주장되는 4점의 고고학 유물에 미세시료채취법을 적용하여 극소량의 시료를 채취한 후 단일입자재현법으로 고고선량을 산출하였다. 모든 유물에 대하여 고고선량 값은 0.2Gy 이하로 매우 낮았으며 단일시료재현법으로 산출된 값과 비교한 결과 매우 신뢰도가 높음을 알 수 있었다. 그러므로 유물의 고고학적 상황과 국내에서 고고선량은 평균적으로 1,000년 당 약 3.5Gy 내외의 값을 가진다는 점으로 미루어 판단하건데, 이 유물들은 고구려 시대에 제작된 유물이 아닐 가능성이 매우 높았다. This article aims to turn out the authentication of archaeological materials by using the paleodose measurement to fine sand-size quartz grains obtained by micro sampling technique. We firstly revealed the validity of micro sampling technique from the paleodoses of two bricks related to Muryong Royal Tomb of Baekje Kingdom. For the purpose of authentication test, four archaeological materials were selected, because they have been insisted that they were manufactured in Goguryeo Kingdom era. After obtaining very few quartz grains by micro sampling technique, each paleodose was evaluated by using SGR (single grain regenerative dose method). All values were very low below 0.2Gy and the reliability was found from those values by using SAR (single aliquot regenerative dose method). Considering the archaeological situation and the general paleodose, the burial time for 1,000 years generally corresponds to about 3.5Gy in Korea, it is concluded that these archaeological materials are all modern counterfeits.

        성심지황탕(醒心地黃湯)이 노화(老化) 백서(白鼠)의 혈액변화(血液變化) 및 혈청(血淸)과 뇌조직(腦組織)의 항산화물활성(抗酸化物活性)에 미치는 영향(影響)

        This study was to investigate the effects of SUNGSIMJIHWANGTANG(SSJHT) on the blood and brain tissues of aged rats. For the experiment, the aged rats were divided into three groups : Non treated group(NC), distilled water fed group(PC), SSJHT fed group(SSJHT). Each group was fed for ten days before administration of scopolamine. Then, we injected scopolamine intraperitoneally to PC and SSJHT group for 5 days. We observed the changes of their blood cell(WBC, RBC, platelet), blood serum(BUN, creatinine, glucose, uric acid), erythrocyte hemolysis, and the activities of cholinesterase and measured the amounts of malondialdehyde in the serum, catalase, and SOD in the brain tissue. The main results of this investigation are as follows. 1. In respect of the number of WBC, SSJHT group exhibited significant increase in comparison with PC. 2. In respect of the amount of creatinine and uric acid in the blood serum, SSJHT group exhibited significant decrease in comparison with PC. 3. In respect of erythrocyte hemolysis, SSJHT group exhibited significant suppression in comparison with PC. 4. In respect of the activity of cholinesterase in the serum, SSJHT group exhibited significant activation in comparison with PC. 5. In respect of the amounts of malondialdehyde in the serum, SSJHT group exhibited significant decrease in comparison with PC. 6. In respect of the activity of catalase in brain tissue, SSJHT group didn't exhibit significant change in comparison with PC. 7. In respect of the activity of SOD in brain tissue, SSJHT group exhibited significant activation in comparison with PC. As a result of this study, SSJHT is expected to have antiaging effect by suppressing the formation of free radicals, the accumulation of antioxidants and further study needs to be carried on about SSJHT.

        다양한 치주 골내낭과 이개부 병변의 처치를 위한 $BBP^{(R)}$ 이식재의 임상적 효과

        Purpose: Periodontal intrabony defects have great deal of importance since they contribute to the development of periodontal disease. Current treatment regimens for intrabony defects involve grafting of numerous bony materials, GTR using biocompatible barriers, and biomodification of root surface that will encourage the attachment of connective tissue. Xenograft using deproteinized bovine bone particles seems to be very convenient to adjust because it doesn't require any donor sites or imply the danger of cross infections. These particles are similar to human cancellous bone in structure and turned out to be effective in bone regeneration in vivo. We here represent the effectiveness of grafting deproteinized bovine bone particles in intrabony defect and furcation involvements that have various numbers of bony walls. Materials and methods: Open flap debridement was done to remove all root accretions and granulation tissue from the defects within persisting intrabony lesions demonstrating attachment loss of over 6mm even 3 months after nonsurgical periodontal therapy have been completed. Deproteinized bovine bone particles($BBP^{(R)}$, Oscotec, Seoul) was grafted in intrabony defects to encourage bone regeneration. Patients were instructed of mouthrinses with chlorohexidine-digluconate twice a day and to take antibiotics 2-3 times a day for 2 weeks. They were check-up regularly for oral hygiene performance and further development of disease. Probing depth, level of attachment and mobility were measured at baseline and 6 months after the surgery. The radiographic evidence of bone regenerations were also monitored at least for 6 months. Conclusion: In most cases, radio-opacities increased after 6 months. 2- and 3-wall defects showed greater improvements in pocket depth reduction when compared to 1-wall defects. Class I & II furcation involvements in mandibular molars demonstrated the similar results with acceptable pocket depth both horizontally and vertically comparable to other intrabony defects. Exact amount of bone gain could not be measured as the re-entry procedure has not been available. With in the limited data based on our clinical parameter to measure pocket depth reduction following $BBP^{(R)}$ grafts, it was comparable to the results observed following other regeneration techniques such as GTR.

        코로나19에 따른 유형별 소매유통시설의 아파트 가격 영향: 전역적·국지적 시계열 효과를 중심으로

        This study conducted an empirical analysis of how different types of living retail businesses affected housing prices during the COVID-19 pandemic, with a particular focus on both global and local time series effects. The main findings are three folds: First, from a global perspective, the study discovered that the presence of living retail businesses had a significant impact on prices of nearby apartment, varying according to their type. Secondly, the impact of COVID-19 on the retail industry varied depending on the type of business. Thirdly, when viewed from a local standpoint, the impact of the retail business sector on apartment prices due to COVID-19 pandemic was substantial, varying across regions and business types. This implies that external shocks like COVID-19 have the potential to alter the role and perception of living retail businesses. In light of this, the study has put forth policy implications aimed at mitigating the adverse effects of living retail businesses and enhancing residential quality.
