        암 환자의 한약-양약 상호작용에 대한 고찰

        Objectives : We present some opinions to reduce the risk of herb-drug interactions through scanning "About Herbs" of the Memorial Sloan Kettering Cancer Center Website. Methods : We searched the Memorial Sloan Kettering Cancer Center Website of About Herbs and investigated herb-drug interactions. Results : There are 237 herbs and 196 drugs on About Herbs. 81.1% of herbs have fewer than 2 interactions listed.: 86.3% of drugs fewer than 3 interactions. Especially, 13 herbs were reported to have interactions with inclusive chemoagents on About Herbs. Only L-theanine has positive interaction with inclusive chemoagents. The others have negative interactions with inclusive chemoagents. 12 single chemoagents were reported to have interactions with some herbs. Especially tamoxifen-black cohosh, methotrexate-glutamin and aldesleukin-Huang Chi have positive interactions to increase the effects of the chemoagent. Conclusions : We should urgently create a risk management system of herb-drug interactions and take note of the risk of herb-drug interactions. We should build up systemic, evidence-based informations on popular herbs used by Korean cancer patients and herb-drug interactions in oncology like About Herbs.

      • 관절염 환자의 민속요법이용 형태

        This sty was undertaken to explore the utilization patterns of medical treatment and herb medicine in patients with arthrits. The data were collected from 995 patients who were registered either in a center of rheumatology or resided in community with arthritis. The data were analyzed by percentile, t-test and chi-square test using SAS PC program. The results of this study were as follows; 1. According to characteristics of sample-population, mean age was 57.7 years, duration of arthritis 9.7 years and mean duration of pain 2.4 hours per day at present. 2. First diagnosis was identified at department of orthopedic surgery or local clinic. But 85.2% of patients have transferred to the center of reumatology currently. But ironically over 70% of patients experienced the herb treatment and herb treatment had hern medicine simultaneous with medical treatment. This indicated that they were shopping around of treatment during the first one year. 3. Mean cost of medical treatment was 67,400 won, while they payed to herb medicine as much as two or three times. The utilization rate of herb medicine was 36.5% of patients per one year. Costs of herb-treatment were 182,000 Won and herb medicine was 36.6% of patients per one year. Costs of herb -treatment were 182,000 Won and herb medicine 149,100 Won. 4. Eighty point six percent of patients were experienced herb medicine during life-time. One hundred and seventy four kinds herb medicine were found to be used by patients, most of which were nutrients, muscle relaxants, andtiphlogistics and analgesics. Thirty two point six percent of users recognized that herb medicine were effective but 51.7% did not. In conclusion, arthritis patients have been experienced various kinds of drug and remedies for treatment. And they wasted their limited medical and economical resources on these unresonable remedies. Therefore, they should be guided for wise decision about treatment. Futher studies in nursing will be required to explore the value system of patients about herb medicine, the determinant factors of shopping-around patterns of treatment, and the coping mechanism for chronic pain or aggravating symptom.

        The Screening of Fermented Medicinal Herbs to Identify Those with Anti-inflammatory Properties

        Objectives : Consumption of fermented foods has been known to alleviate some of the symptoms of atopy, may limit allergy development, and there are also many medicinal herbs proved to be effective for immunologically mediated diseases. In this study, we introduced modern zymology to ferment some herbs to see if fermentation has the possibility of increasing the anti-inflammatory effects of medicinal herbs. Interleukin-4 (IL-4) and interferon-gamma (INF-γ) have been demonstrated to be the main factors in the pathology of allergic diseases. Methods : We measured the levels of IL-4 and INF-γ on concanavalin A-induced BALB/c mice spleen cells, which were treated by fermented and unfermented herbs subsequently. We then compared the fermented groups with unfermented groups to see if the anti-inflammatory effects of the herbs were influenced by fermentation. Results and Conclusions : Our results showed that fermentation had the potential to increase the anti-inflammatory effects of some medicinal herbs, and Astragalus membranaceus and Salvia miltiorrhiza would be the most suitable medicinal herbs for fermentation among the herbs in this study.         기미(氣味)와 효능 용어를 활용한 해표약(解表藥)의 효능 고찰

        Objective : This study was conducted to infer the effect of superficies-resolveing herbs by analyzing effect terms which have extracted and refined, based on four temperatures and five tastes. Method : Firstly, temperatures, tastes and effect terms were extracted from the 27 kinds of superficies-resolving herbs written in Herbology. Then, each effect terms was divided into single meaning term and refined as typical term, using the inclusive effect terms I established. After that, herbs were grouped by tastes and found the effect terms which are mentioned most frequently. And it could be supposed to be classified into each herb’s new effects based on the relationship between properties and those effect terms. It is also inferred into new tastes from some herbs by analysing the representative effects group of each taste and finding which other tastes can be related to each herb’s effect. Result & Conclusion : All the superficies-resolving herbs can have wind-dispelling effect and superficies-resolving effect, except Bulpleuri Radix. This herb is able to has just the wind-dispelling effect. And it’s more appropriate to categorize Bulpleuri Radix to heat-clearing herb group than superficies-resolving herb group, considering its several, distinctly cold characteristics. Some effects are concentrated to wind-cold-dispersing herb group and others to wind-heat-dispersing herb group. Each tastes has its own representative effect group. And, according to its tastes’ representative effect, some of herbs are reasonable to get new effect term. With not so feasible reason, 4 kinds of herbs are supposed to contain all of hot, bitter, and sweet tastes. Also It’s needed to study much deeper whether Perillae Herba, Zingiberis Rhizoma Crudus and Elsholtziae Herba are belong to superficies-resolving herb group or to interior-warming or damp-dissolving aromatic herb group.

        "동무선사사상약성상험고가(東武先師四象藥性嘗驗古歌)"의 연원(淵源)과 의의(意義)에 대한 연구(硏究)

        I think that the study on the ${\ulcorner}$DongMuSunSaSaSangYakSungSangHumGoGa${\lrcorner}$ of ${\ulcorner}$DongMuYooGo${\lrcorner}$ is basic material explaining the character of sasang-herb and prescription. Through the study on the ${\ulcorner}$DongMuSunSaSaSangYakSungSangHumGoGa${\lrcorner}$'s origin and meaning, I can take the result as follows. The first, that is composed of total 237 herbs of 90 spleen-herb (Soeumin-herb), 61 kidney-herb (Soyangin-herb), 67 lung-herb (Taeeumin-herb) and 19 liver-herb (Taeyangin-herb). The second, the origin of that is ${\ulcorner}$JeJungSinPyun${\lrcorner}$, ${\ulcorner}$BangYakHapPyun${\lrcorner}$ and ${\ulcorner}$EuiHakIpMum${\lrcorner}$. And DongMu classified traditional YakSungGa as spleen-herb, kidney-herb, lung-herb and liver-herb which is suitable to Sasangin, not created that. So we can know that the first understanding for DongMu' s herb is focused on the traditional YakSungGa. The third, DongMu's unique terminology of spleen-herb, kidney-herb, lung-herb and liver herb show his sight of symptom, disease and pharmacology, that is focused on small JangBu. The forth, through herb-nature gathered from traditional YakSungGa, that purposed to classfying each constitutional herb. So we can know his idea that the choice of herb is not in general efficacy but in personal constitution. As mentioned abovet, that can refer to an herbal argument of DongMu and ${\ulcorner}$DongMuYooGoYakSungGa${\lrcorner}$ that make known to be common can refer to an prescriptional argument of DongMu.

        음식관광 세분화를 통한 지역관광 활성화 방안 -약선 음식을 중심으로-

        This study focused on the dishes prepared with medicinal herbs food tourism the local economy is to provide the basic plan. Drawn from previous research through the a variable plan, as well as the local economy and at the same time as the metrics presented dishes prepared with medicinal herbs food tourism for activation through regional tourism strategy presented. Data processing of statistical packages SPSS Window 19.0 use. Statistical processing technical analysis, frequency analysis, factor analysis, cluster analysis and logistic regression analysis was conducted and verification. Third, the dishes prepared with medicinal herbs food are likely to contribute to the globalization of Korean food tourism is granular as each appeared positively crowded star. Fourth, to review the operation of the dishes prepared with medicinal herbs food Hall. Fifth, The biggest problem of the tourism food hygiene is very high in dishes prepared with medicinal herbs food hygienic problem beforehand about the tourist dishes prepared with medicinal herbs food tourism is Korea's food and tourism resources should strive to be. The leading research into the academic implications drawn and measurement of variables through statistical indicators that measure through the academic validation. Expected results as food in order to enable the tourist areas of the region produce reflect the interest of men into the region with food ingredients, if men and women can all enjoy approximately dishes prepared with medicinal herbs is expected to be open for food. We expect that tourism for the dishes prepared with medicinal herbs will enjoy a steady increase by tourists because of the high interest in health based on the same origin theory between food and medicine.

        전통주에 이용되는 한약재의 특성 연구

        Objective : The aim of this study is to analyze medicinal herbs, the ingredients of traditional Korean liquor, focusing on efficacy in the traditional Korean medicine. Through this study, for vitalizing traditional Korean liquor market, the researchers attempt to explain the superiority of traditional Korean liquor and the advantages of using herbs to brew alcoholic beverages. Method : We conducted a research targeting traditional Korean liquors in domestic market from July through september 2012, and finally 70 kinds of liquor were selected and analyzed. Results : Among traditional liquors, fermented liquors are most common. When comparing regions of brewery, the central region of Korea, Gyeonggi-do and Chungcheongnam-do, is major brew area. 37 liquors contain just one medicinal herb. followed 10 liquors contain two medicinal herbs. The most frequently used medicinal herb is Ginseng Radix. And Lycii Fructus, Schisandrae Fructus, Angelicae Gigantis Radix and Chrysanthemi Indici Flos are well used medicinal herbs to brew traditional liquors. In the classification of efficacy of medicinal herbs, tonifying and replenishing medicinal herbs are most popular as ingredients in the traditional alcohols, astringent medicinal herbs, exterior releasing medicinal herbs and heat clearing medicinal herbs are also frequently used. Investigation of tastes and properties of the herbs, it reached a conclusion as below: sweet taste is most common, followed by sour taste and bitter taste. Warm property is account for 50% approximately, followed by cold property. Conclusion : Medicinal herbs have been widely used in the traditional Korean liquor. It came out into the open that the liquor with medicinal herbs is very good for our body in terms of longevity and invigoration. However, liquor consumers concern about not only the efficacy but also the taste, scent and color. Therefore, further study about these things would be required to demonstrate the superiority of the traditional Korean liquor.

        Systematic network analysis of herb formula in Traditional East Asian Medicine discloses synergistic operation of medicinal herb pairs with statistical significance

        Traditional East Asian Medicine (TEAM) prescriptions typically consist of several herbs based on the assumption that the herbs operate synergistically and/or cooperate on several related pathways simultaneously. This is a general concept that is widely accepted in TEAM, but it has not been tested systematically. To check this assumption statistically, we have text mined traditional Korean medicine text the Inje-ji(仁濟志, Collections of benevolent savings), a text that contains more than 5000 herb-cocktail prescriptions. We created herb–pairing network based on herb–herb pairing specificity and performed a systematic network analysis. Herbs were shown to be used selectively with other herbs and not randomly. Moreover, herb pairs were more specifically associated with symptoms than were single herbs. Single herbs and combinations of herbs specifically used for diabetes mellitus were successfully identified. As conclusion, herb-pairings in TEAM are not randomly constructed; instead, each herb was selectively used with other herbs. In terms of statistical significance, herb pairs were more specifically associated with symptoms than were single herbs alone. Collectively, these results suggest that it may be important to understand the interactions among multiple ingredients contained in herb pairs rather than trying to identify a single compound to resolve symptoms.

        첩약의 보험급여 적용을 위한 과제 및 접근방안에 대한 연구

        We met results like the followings through the literatures and questionnaires about the tasks and solutions about the insurance fee of packed medical herbs. 1) It's turned out that 74.8% of herb doctors agrees to the insurance fee of packed medical herbs. However, in comparison with the same survey of the herb doctor association the percentage of general approval went somewhat lower, and especially the percentage of 'positive approval' became notably lower$(43.7%{\rightarrow}26.5%)$ and the percentage of 'active objection' raised about 2 times$(6.8%{\rightarrow}12.9%)$. Inquiring into the approval reasons on the insurance fee application of packed medical herbs some heads such as 'development toward treatment medical science' and 'decrease of publics burden' were higher than the one of 'management income and expenditure.' 2) As a result of the research, 36.0% of the patients and 42.8% of the residents recognized that the pay range of Chinese herb health insurance is narrow. They recognized that less people have the experiences of Chinese medical hospital use and internal application of the packed medical herbs as they are older, men rather than women. 85.4% of the patients and 74.9% of the residents agreed on the insurance pay of packed medical herbs. It's shown that they agree on the Chinese medical hospital use more as the economic standard is lower, on the insurance pay as they have ever taken the packed medical herbs. In the aspect of increase of insurance fee, 66.7% of the patients and 44.3% of the residents agreed on the insurance pay of packed medical herbs, and 18.1% and 36.1% disagreed on the insurance pay of packed medical herbs. The main objective reason why they disagree on the insurance pay of packed medical herbs was 'because the insurance fee goes up higher,' which answered 95.2% of the patients and 78.8% of the residents. 7.22% of the patients and 1.80% of the residents answered that they can pay more insurance fee in case of the insurance pay of packed medical herbs. However, in the priority order of the insurance pay, it hold the 5th position between 2 target research groups which was less than medical examination, charges for hospital accommodation and taking MRI. 3) According to the result of analysis about the cost of packed medical herbs, current practice price is 115,000 won and the average prime cost of a packed medical herb is 73,000 to 106,000 won. It's examined that the herb doctors regard that 95,000 won will be reasonable when the packed medical herb is payed in insurance. However, it was found out that the public generally thinks that the price would be appropriate on the level of 30,000 to 40,000 won and the percentage of the answers of 20,000 won to 30,000 was fairly high. 4) the central system of a prescription should be change into the central system of demonstration and the sick and wounded. 5) To solve this problem, the government should regulate it to pass by the circulation gradation of [importer, $peasantry{\rightarrow}manufacturer{\rightarrow}wholesaler{\rightarrow}distributor$(Chinese medical hospital, pharmacy dispensary of Chinese medicine)]And it should intervene into the quality and the circulation steps of Chinese medicine through 'the office or organization which is in charge of certification of Chinese medicine' and 'the office or organization which is in charge of the circulation of Chinese medicine.' And some actions such as simple severance, lavation, drying should be included into the conception of manufacture and the boundary between food and medical supplies should be made at a manufactory. And the regulation of standardized goods at one's own house should be improved so that, the peasantry can sell the materials of Chinese medicine only to the manufacturer. 6) In company with the insurance pay of packed medical herbs, the study about the separation of dispensary from medical practice in the Chinese medicine should be accomplished.

        唐宋 시기 해외·남방 本草 지식의 집적과 전승

        이 논문은 해외에서 유입된 또는 남방 등 지역의 본초를 전문적으로 다룬 李珣(907-965)의 海藥本草가 중국 본초학사 상 가지는 특징과 의의를 논하였다. 먼저 해약본초는 ‘지역’ 본초서로서의 특징이 두드러지는데, 먼저 서술 방식의 측면에서 이전 소위 전통적 본초서에서 보이는 특징과는 달리 본초의 원산지를 가장 먼저 언급하며 본초의 원산지를 명확히 밝히는 데 주안점을 두었다. 그리고 관련 정보를 소개하면서 다양한 참고문헌을 인용하였는데, 그 인용된 문헌이 대부분 남방의 지리지, 박물지 등이었다. 이러한 점들은 모두 해약본초의 지역 본초서로서의 특징을 보여준다. 또한 해약본초에 수록된 약 10종의 본초는 그 이전 본초서에는 보이지 않는 것이며, 이외 6종의 본초 역시 8세기 진장기의 『본초습유』에도 언급된 것이지만 이후 증류본초에 수록될 때 해약본초를 원출처로 하여 수록된다. 아울러 이를 포함한 총 19종이 본초강목에 수록될 때 역시 해약본초를 원출처로 하여 수록된다. 분명 해약본초는 지역의 새로운 본초에 대한 소개와 그것이 이후 전국 범위의 권위 있는 본초서에 수록되어 전승되는 데에 중요한 역할을 하였다. 이 외에 기존의 널리 알려진 해약에 대해 산지와 성질 및 효능 부분에서 해약본초는 다양한 참고문헌을 인용하여 관련 지식을 보충하였고, 이러한 보충된 지식에는 본초와 관련된 다양한 역사적 사실과 일화도 포함된다. 또 이렇게 보충된 지식은 거의 빠짐없이 증류본초나 본초강목으로 수렴되는 것을 확인할 수 있다. 해외 또는 남방의 본초 지식의 축적과 전승에서 해약본초가 가지는 의미를 확인할 수 있는 부분이다. 결론적으로, 해약본초는 ‘지역’의 지식을 다루기에 기존 전통 본초서와는 다른 특징을 나타내며, 산발적으로 전해지는 다양한 지방 문헌 기록을 수집하여 본초학 지식으로 체계화했다. 또한 그 결과물은 이후 북송 시기 전국 규모의 본초서인 증류본초에 수록되고, 또 중국 본초학의 집대성이라 할 수 있는 명 이시진 본초강목에 수록된다. 이를 통해 오대 시기 남방 출신 저자의 지역 본초서가 이후 ‘제국’ 본초학의 형성에 미친 영향 볼 수 있다. This study is about Haiyaobencao(海藥本草) by Li Xun (907-965), a unique book on medicinal herbs in the history of Chinese herbal medicine and explore how the knowledge of Haiyao(海藥) brought in from abroad or from the southern region around the 10th century was accumulated and disseminated in later periods. When the contents of Haiyaobencao examined closely, its characteristics as a ‘regional’ medicinal herb book stand out. First, in terms of the description method, unlike the characteristics seen in the previous so-called traditional medicinal herb books, the origin of the herb is mentioned first, and an emphasis was placed on clarifying the herb’s origin. Moreover, various references were cited while introducing related information. Most of the references cited were geography(地志) and natural history(博物志) of the south. All of these points show the characteristics of Haiyaobencao as a regional medicinal herb book. Haiyaobencao is significant because the knowledge of herbal medicine from scattered regions is gathered together in it. The significance of Haiyaobencao is very clear in terms of accumulation and expansion of Chinese herbal knowledge. First, about 10 kinds of herbs listed in Haiyaobencao are those that are not observed in previous medicinal herb books. In addition, 6 kinds of herbs were already mentioned in Bencaoshiyi(本草拾遺) in the 8th century, but later the author of Zhengleibencao(證類本草) specified Haiyaobencao as the original source. Additionally, when a total of 19 types of herbs including these are included in Bencaogangmu(本草綱目), Haiyaobencao is specified as the original source. Clearly, Haiyaobencao played an important role in introducing the new herbs of the region and disseminating it after being included in the authoritative medical herb book on a national scale. Haiyaobencao also supplemented related knowledge by citing various references in the area of production and efficacy of Haiyao which was already widely known. This supplementary knowledge also includes various historical facts and anecdotes related to herbs. Almost all of the supplemented knowledge is converged into Zhengleibencao and Bencaogangmu. This is the part where we can confirm the significance of Haiyaobencao in the accumulation and dissemination of knowledge on exotic medical herbs abroad or in the south. In conclusion, Haiyaobencao, which collected overseas and southern herbal knowledge during the pre-Five Dynasties period, has characteristics which are different from existing traditional medicinal herb books, as it addresses 'regional' knowledge. It holds various local literature records that have been handed down sporadically and systematized as herbal knowledge. Moreover, the results were later included in Zhengleibencao, a nationwide medicinal herb book during the Song Period. In addition, it is included in Bencaogangmu, which can be called a compilation of herbal medicine in China. Through this, the influence of the regional medicinal herb book of the southern author during the Five Dynasties period on the formation of the ‘imperial’ herbology thereafter can be explored.
