        간호대학생의 영적 간호 수행능력이 영적 간호 수행정도에 미치는 영향

        Background: This study was to survey the effect of the degree of spiritual nursing care performance on the spiritual nursing care ability of the nursing studentsMethods: The researcher sampled 130 nursing students for a questionnaire survey conducted from September 17 until September 27, 2018. The data of analysis used SPSS 23.0 program. Results: The spiritual nursing care ability was 4.4±0.8(total score 6) and the degree of spiritual nursing care performance was 2.9±1.8(total score 4). The spiritual nursing care ability differed significantly depending on religion(F=7.570, p<.001), the level of spiritual nursing knowledge(F=19.873, p<.001), education type(F=14.626, p<.001), necessity of hospice(t=2.280, p=.024). The degree of spiritual nursing care performance differed significantly depending on spiritual nursing education time(F=2.932, p=.036). The correlation of two variable was statistically significant difference(r=.206, p=.019). The influencing factors on the spiritual nursing care ability was religion, the level of spiritual nursing knowledge, education type, dying experience(R2=0.378, Adj R2=0.353), the degree of spiritual nursing care performance was spiritual nursing education time(R2=0.065, Adj R2=0.043). Conclusion: These results show that nursing students are not able to perform spiritual nursing care properly to subjects who need spiritual nursing care. Therefore, it is necessary to develope programs to meet the spiritual nursing care needs of nursing students and to provide practical education in accordance with the program by cooperating with the nursing education staff, clergy and clinical nurse etc. And spiritual nursing care should be taught as a required subjects in the curriculum.

      • The Effect of a Scenario-Based Spiritual Care Course on Spiritual Care Competence among Clinical Nurses

        Aim(s): Clinical nurses are facing patients’ biopsychosocial and spiritual problems at diverse clinical settings, but they are lack of knowledge, skills, and confidence on providing spiritual care. However, no studies adopted simulated educational program and objective structure clinical examination (OSCE) to educate nurses and to assess the outcomes. Therefore, this study was to validate the effect of a scenario-based spiritual care course on spiritual care competence in clinical nurses. Methods: This non-equivalent quasi-experimental study with a pre-test and two post-tests (end of the intervention and three months later) was conducted between August 2019 and February 2021 (11 sessions). The recruitment posters with a QR code were distributed to different units at three branches of a large medical institute for recruiting clinical nurses, who provided direct patient care. The attendees of one-day scenario-based spiritual care course are the experimental group (n=53) and the control group (n=85) was matched with the experimental group (1:1-3 ratio) by similar units, ages, working experience, and clinical ladder. Instruments include basic information, self-evaluated and head nurses evaluated spiritual care competence scale (SCCS), spiritual perspective scale (SPS), and spiritual care perspective scale-revised (SCPSR) for both groups and reflective log, course satisfaction scale, OSCE checklist, and standardized patient feedback scale for the experimental group. The data were analyzed using descriptive statistics, repeated measures ANOVA, linear regression, and paired t test. Results: After controlling for pre-test scores and interest in attending spirituality/spiritual care, the experimental group showed significant higher SPS (p=0.012) and self-evaluated SCCS (p=0.002) and lower SCPSR (p=0.015) than the control group at the 2nd post-test. Repeated measure ANOVA also showed significant within-subject effects across three time points of the experimental group on SPS (p<0.001), SCPSR (p<0.001), and self-evaluated SCCS (p<0.001), but paired t test showed non-significant differences on SPS, SCPSR, and self-evaluated SCCS between pre-test and post-test with an exception of head nurse evaluated SCCS (p=0.035). The mean overall course satisfaction of the experimental group was 4.34±0.62. The mean global performance of OSCE was 3.40±0.91 and the majority of experimental group was pass (43.4%) and good (35.8%). 64.5% and 26.1% of standardized patients were partial and strongly agreed with examinees’ performance in spiritual care assessment respectively. Conclusions: The scenario-based spiritual care course is effectively to enhance clinical nurses’ spiritual care competence. It may cultivate clinical preceptors with better spiritual care pedagogy through simulation and OSCE for bedside teaching of nurse post-graduate year.

        임상 간호사의 영적간호수행 영향요인

        The purpose of this study was to identify the factor factors influencing spiritual care practices among clinical nurses. The subjects were 121 nurses of C hospital located in Gyunggi-do. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson’s correlation and multiple regression. Spiritual well-being was significantly positive relationship between spiritual nursing competency (r=.406, p<.001) and spiritual care practice (r=.497, p<.001). Spiritual nursing competency was significantly positive relationship between spiritual care practice (r=.621, p<.001). Factors that significantly influenced nurses' spiritual care practice was spiritual nursing competency (β=.481, p<.001), needs of spiritual nursing education (β=.229, p=.001) and spiritual well-being(β=.218, p=.005). These three variables explained 47.6% (F=28.298, p<.001) of spiritual care practice. The findings suggest that it is needed to develop effective programs to improve spiritual nursing competency of clinical nurses. More studies are required to identify other predictors of spiritual care practices. 본 연구는 임상 간호사의 영적간호 수행에 영향을 미치는 요인을 파악하기 위한 서술적 상관관계연구이다. 연구대상자는 경기도 소재 C병원에 재직하고 있는 간호사 121명을 대상으로 조사하였다. 수집된 자료는 SPSS 23.0 프로그램을 사용하여 기술통계, t-test, one-way ANOVA, 사후검정은 Scheffe´s test, Pearson´s correlation 및 multiple regression analysis하였다. 대상자의 일반적 특성 및 근무 특성은 빈도와 백분율, 평균과 표준편차 등을 사용하여 분석하였다. 영적안녕은 영적간호역량(r=.406, p<.001), 영적간호수행(r=.497, p<.001)과 유의한 양의 상관관계를 보였고, 영적간호역량은 영적간호수행(r=.621, p<.001)과 유의한 양의 상관관계를 보였다. 임상 간호사의 영적간호수행에 통계적으로 유의하게 영향을 미치는 요인은 영적간호역량(β=.481, p<.001), 영적간호 교육요구도(β=.229, p=.001), 영적안녕(β=.218, p=.005) 순으로 확인되었으며 영적간호수행에 대한 설명력은 47.6%(F=28.298, p<.001)로 나타났다. 본 연구결과를 토대로 임상 간호사의 영적간호수행을 강화하기 위해서는 영적간호역량의 향상이 선행되어야 하고, 이를 위한 교육 프로그램의 개발이 필요하겠다. 아울러, 임상간호사의 교육요구 및 수준을 고려한 맞춤형 영적간호 교육 및 임상간호사의 영적안녕을 증진할 수 있는 자가 관리 내용을 포함한 체계적인 영적간호 교육 프로그램의 개발이 필요하다.

        This thesis is to study on the spirituaI nursing care as the, core concept in caring the patient with the best. Especially, it, helps to grasp the meaning of the spiritual well-being of the, nurse and to analyze how correlations the spirituaI nursing care, and spirituaI well-being has. The results of this study were as follow:, 1. The average value of the spirituaI well-being of nurses, was 55,89 among the possible ranges of 38-78. In cases, of the low domains of the spirituaI well-being, the average, value for the existential well-being was 29.35 and, 26.04 for the religious well-being. Therefore, these results, indicate relatively higher value of the spiritual lwell-being,, and the average vaIue for the exitential well-being was, higher than that of the religious well-being. 2. The average vaIue for the spiritual nursing care was, 43.68 among the possible ranges of 29~66. In case of the, lower domains of spirituaI nursing care, the average value, of the need for love and relatedness was 19.54 and 19.87, to the need for meaning and purpose, and 4.25 to the need, for being forgiven. these results show that the nurses, practice lower spiritual nursing care and the average values, are in order: need for love and relatedness, need for, being forgiven, and need for neaning and purpose. 3. The analysis of the spiritual well-being according to general, characteristics of nurses showed statistically significant , difference on believe in God(F=12.515, p=0.001),, religion(F=21.143, p=0.000), the influence of the religion to, daily life(F=33.61, p=0.000), duration of religion to believe(F=12.07, p=0.000). 4. The analysis of the spiritual nursing care according to, generaI characteristics of surses showed a statistically, significant difference on marital state(t=-2.620, p=0.010),, education(F=3.095 p=0.048), experience(F=2.849, 0.039),, working place(F=3.99. 0.002). 5. There was a positive correlation(r=0.303, p=0.000) between, the spiritual well-being and the spiritual nursing care.

      • 임상 빅데이터를 이용한 영적간호진단 적용 현황 분석

        Purpose: The aim of this retrospective study was to analyze spiritual nursing diagnosis records using clinical big data to learn spiritual nursing activities and to help improve spiritual nursing practice in the future. Methods: Eleven types of spiritual nursing diagnoses were applied to 422,733 patients who were admitted to a certified tertiary hospital between April 2011 and September 2018. Descriptive statistics and x2 tests were used to analyze the characteristics of the patients and nursing records. Results: There were 51,423 (12.2%) patients who were diagnosed through spiritual nursing. The patients received a total of 59,925 spiritual nursing diagnoses, including duplicate diagnoses. The most common spiritual nursing diagnoses was anxiety (58.4%), followed by dying (16.5%) and helplessness (7.6%) over the past eight years. Spiritual nursing data have a tense and anxious appearance, decreased activity, and decreased speech. Spiritual care plans and interventions have been used to encourage, listen, and express interest in expressing emotions. Conclusion: Through an analysis of big data, this study found the frequency of various spiritual nursing diagnoses, care plans, and interventions. Improving nurses’ perceptions of the spiritual nursing process, spiritual care education, and encouraging the performance of spiritual care may be an effective pathway to enhance the spiritual care competence of nurses.

      • 병원 중간관리 간호사의 영적성장 체험

        본 연구는 Max van Manen의 해석학적 현상학 방법을 이용하여 병원 중간 관리 간호사가 체험한 영적 성장의 본질을 탐구하고자 한다. 연구 참여자는 c대학병원에 근무 중인 중간관리 간호사로서 8주간의 영성 기반의 스트레스 완화 프로그램 (Spirituality based stress reduction Program)에 참여한 대상자 중 적절한 과제수행과 꾸준한 개인적 성장을 보여 온 13명의 참여자를 대상으로 하였다. 연구 기간은 2009년 6월부터 2011년 3월까지로 연구 자료는 참여자가 작성한 개인 일지와 체험 수기 및 참여자와의 심층 면담내용을 이용하였다. 연구 결과 병원 중간관리 간호사들이 경험한 영적 성장의 의미는 "삶을 변화시키고 싶은 갈망(Aspi rati on for changing life)", "영적 훈련(Spiritual training)", "영적 깨어남(Spiritual awakening)", "본질적 삶을 위한 고뇌(Struggling for essential life)", "영적 성숙으로 나아감(Spiritual maturing)", "평화롭게 살아감(Peaceful living)" 의 6가지로 나타났다. 결론적으로, 영적 성장에 대한 체험은 일련의 단계적 과정을 반복적으로 순환하며 나아가는 나선형적 순환 과정으로 이해할 수 있었으며 성찰과 실행을 반복하며 점진적으로 나아가게 된다 이상의 결과를 종합하면 간호사의 영적 성장은 영적 돌봄과 간호의 돌봉 예술을 실현할 수 있는 하나의 역량으로 이해될 수 있었다. 본 연구결과를 토대로 임상에서 간호사의 영적 성장을 촉진 시킬 수 있는 다양한 중재가 적용되어 간호의 돌봄 역량을 보다 강화시킬 수 있게 되기를 바란다. The purpose of this study was to explore the experience of spiritual growth in hospital middle manager nurses and research was conducted based on hermeneutic phenomenology by applying van Mana n's research method. Thirteen participants were enrolled this study and have been worked at C university hospital for an average of 147.9 months. They all completed spirituality based stress reduction program for eight-weeks and have constantly practiced holy name repetition. The study was conducted from June 2009 to March 2011, and data were collected from participant 's daily record, autobiography and semi- structured interviews. The lived experience themes of spiritual growth in middle manager nurses were extracted as "Aspiration for changing life", "Spiritual training", "Spiritual awakening", "Struggling for essential life", "Spiritual maturing", "Peaceful living". In conclusion, the experience of spiritual growth occurs as a gradually ascending spiral through repeated reflection and execution. Spiritual growth in middle manager nurses was integrated with their nursing care and have on effect competency to actualize spiritual care and "Art" of nursing . As a result of this research, it is expected that various intervention should be adapted in clinical setting to increase spiritual growth in nurses.

        말기 환자간호 실습교육이 간호대학생의 죽음에 대한 태도, 임종간호 태도, 영적간호역량에 미치는 효과

        . Purpose: This study aimed to assess the effect of palliative care practical training for nursing students. Methods: This quasi-experimental study included 38 third-grade nursing students form one university. Practical training, develooed by experts, was provided for 2 weeks (90 h) in a palliative care hospital. Participants received education on palliative care but no clinical practical experience. Collected data were analyzed using independent t-test, χ2 test and paired t-test using the WIN SPSS 23.0 program. Results: Students showed significant pretest-posttest differences in attitude toward death (t=-2.43, p=.021), end-of-life nursing attitude (t=3.90, p=<.001) and spiritual nursing competency (t=3.82, p=.001). Conclusion: The study results revealed that palliative care practical training was an effective learning method to improve nursing attitude, toward death, end-of-life nursing attitude and spiritual nursing competency. Further studied are needed to assess the effects of various education programs of palliative care.

        말기환자를 돌보는 기독간호사의 삶의 의미와 영적간호수행

        말기환자를 위한 영적간호수행은 환자와 간호제공자의 삶의 의미와 영적 안녕을 높일 수 있는 중재로 삶의 마지막 순간까지 안위를 도모하며 전인간호를 수행할 수 있는 기틀을 마련하게 될 것이다. 본 연구의 목적은 말기환자를 돌보는 기독간호사의 삶의 의미와 영적간호수행 정도를 알아보고 관계를 파악하여 간호사의 삶의 의미추구와 영적간호수행 증진 프로그램의 기초자료를 제공하기 위함이다. 연구 설계는 서술적 상관관계연구이며, 연구대상은 J시에 소재한 1개 기독교종합병원에서 최근 1년 이상 말기환자를 돌본 경험이 있는 간호사 209명이었다. 연구도구로 삶의 의미는 크롬바흐와 머홀릭(Crumbaugh and Maholik, 1969)이 개발하고 남궁(Namkung, 1980)이 번역한 도구를 사용하였으며, 영적간호수행 도구는 테일러 등(Taylor et al., 1995)이 개발하고 오복자(1996)가 번안한 것을 수정·보완하여 사용하였다. 자료 수집은 2008년 9월 17일부터 10월 2일까지였으며, 자료 분석은 SPSS/PC 14.0 프로그램을 이용하였다. 연구결과를 요약하면 다음과 같다. 첫째, 대상자의 삶의 의미 정도는 보통정도보다 높게 나타났다. 둘째, 대상자의 영적간호수행 정도는 보통정도보다 낮게 나타났으며, 하위영역에서 용서에 대한 영적간호수행, 사랑과 관심에 대한 영적간호수행, 의미와 목적에 대한 영적간호수행의 순으로 나타났다. 셋째, 대상자의 삶의 의미와 영적간호수행과의 관계는 유의한 순상관관계가 있었고(r=.363, p=.000), 삶의 의미와 영적 간호수행의 하위영역인 의미에 대한 영적간호수행(r=.352, p=.000), 사랑에 대한 영적간호수행(r=.308, p=.000). 용서에 대한 영적간호수행(r=.305, p= .000) 모두에서 순 상관관계를 나타냈다. 이런 결과를 바탕으로, 기독간호사의 삶의 의미와 영적간호수행의 향상을 위한 표준화된 교육프로그램이 개발되고 활용되어야 할 것이다. 이런 접근들은 영적 간호 수행에 영향을 미치는 다양한 요인들을 탐색하고, 진정한 삶의 의미를 부여하는 내용을 반영할 수 있어야 하겠다. The purpose of this study was to identify the correlation between meaning of life and spiritual nursing care in christian nurses for terminal patients. There were 209 nurses involved in the study who care for terminal patients, and they were from Christian General Hospital in J city. Data were collected and organized from September to October 2008 using structured questionnaires. The collected data were analyzed by using the SPSS/WIN 12.0 program with mean, standard deviation, t-test, ANOVA, Duncan`s test, and Pearson`s correlation coefficients. The mean score of meaning of life was 96.21±14.00(range 20~140) and that of spiritual nursing care was 26.96±7.05(range 15~60). According to general characteristics of the nurses, there were significant differences in meaning of life and spiritual nursing care in terms of age, marital status, education, role, career, period of faith, worship participation, religion influence to life and degree of faith. The meaning of life and spiritual nursing care have a positive correlation. Based on the findings, it is needed that for enhancement of christian nurses` meaning of life and spiritual nursing care, a standardized education program and instructions be developed and used. These approaches should be able to search for a variety of variables which has an influence on spiritual nursing care, and reflect the contents in which a genuine meaning of life is found.

        기독교대학 간호대학생의 영적 돌봄에 대한 주관적 인식

        This study was attempted to confirm the structure of the perception of spiritual care among nursing students at a Christian college and to explain the characteristics of each type from a Christian point of view, and applied the Q methodology. Although 38 P samples classified 38 Q samples, the final 34 P samples were analyzed by invalidating the data of 4 students with invalid loading values. The first is the 'therapeutic relationship-oriented type', which recognizes therapeutic communication and therapeutic relationships as important elements of spiritual care. The second is the 'religious approach-accepting type', in which religious beliefs are important in spiritual care. Contributing to the improvement of well-being, religion was recognized as beneficial for nurses to perform spiritual care. The third type is the 'selective acceptance type', which emphasizes the identification of different spiritual needs for each subject and recognizes it as a type of nursing that can be performed as needed. The 4th type is the ‘non-religious care-oriented type’, that the characteristics of excluding the influence of spiritual care on religion were confirmed. This study is significant in that it provided basic data for follow-up studies, by presenting a Christian approach strategy for each type of nursing students at a Christian college. 본 연구는 기독교대학 간호대학생의 영적 돌봄에 대한 인식의 구조를 확인하고 그 유형별 특성을 명하기 위해 Q방법론을 적용하였다. 기독교대학 간호대학생 P표본 38명을 대상으로 34개의 Q표본을 분류하게 하였으나, 적재치가 유효하지 않은 4명의 데이터를 무효처리하여 최종 34명을 분석하였다. 그 결과, 기독교대학 간호대학생의 영적 돌봄에 대한 4개의 인식유형이 도출되었다. 제1유형은 ‘치료적 관계 중시형’으로 치료적 의사소통 및 치료적 관계를 영적 돌봄의 중요 요소로 인식하였고, 제2유형은 ‘종교적 접근 수용형’으로 영적 돌봄에서 종교적 신념과 믿음이 영적 안녕향상에 기여하고 종교는 간호사가 영적 돌봄을 수행하는 데 유익하다고 인식하였다. 제3유형은 ‘선택적 수용형’으로 대상자마다 다른 영적 요구 확인을 중시하며, 필요에 따라 수행될 수 있는 간호로 종류로 인식하였고, 제4유형은 ‘비종교적 돌봄 중시형’으로 영적 돌봄과 종교의 관련성을 부정하고, 간호사의 종교 유무가 영적 돌봄에 영향하는 바를 배제하고자 하는 특성이 확인되었다. 본 연구는 기독교대학 간호대학생의 유형별 기독교적 접근 전략을 제시함으로써 추후 기독교적 관점에서의 영적 돌봄역량 중재 프로그램 개발 및 효과 확인 등의 후속 연구를 위한 기초자료를 제공하였다는 데 그 의의가 있다.

        간호대학생의 영적안녕과 죽음에 대한 태도가 임종간호태도에 미치는 영향

        Purpose: This study aimed to explore the influences of spiritual wellbeing and attitude toward death on terminal care attitude among Korean nursing students. Methods: The participants were 146 nursing undergraduates from two nursing schools in Korea. The students responded to a self-report questionnaire that included demographics, spiritual wellbeing, attitude toward death and terminal care attitude. Results: In total, 71.9% of the participants were religious, 95.2%, and 67.8% had hospice education. The mean score of spiritual wellbeing of the students who were religious was significantly higher than among those who were not. Regression analysis indicated that existential spiritual wellbeing, attitude toward death, and Christian schooling were the most significant predictors of terminal care attitude. These explained 39.6% of variation in terminal care attitude. Conclusion: Death-related education is needed throughout the nursing curriculum including to develop a positive terminal care attitude. In this way, nursing undergraduates will be better prepared to cope positively and constructively with the suffering and death they will encounter and thus may minimize the distress they experience with patient death. may also create a significant positive increase in nurses’ terminal care attitude.
