        표준화 환자를 활용한 정신간호실습교육의 학습 성과 효과

        본 연구는 표준화 환자를 활용한 시뮬레이션 정신간호실습교육을 위한 시나리오를 개발하여 정신간호교육의 학습 성과 효과와시뮬레이션 교육 경험의 내용을 분석하고자 양적연구와 질적연구를 혼합한 트라이앵귤레이션 연구이다. 연구대상자는 K대학교 간호학과 3학년 학생 54명이었으며, 표준화 환자를 활용한 시뮬레이션 정신간호실습교육을 위한 시나리오를 개발하고, 시뮬레이션정신간호실습교육 전ㆍ후의 학습 성과를 비교하였다. 또한 비구조화된 설문을 통해 시뮬레이션 정신간호실습교육 후 간호대학생의 경험을 파악하였으며, 질적 연구방법으로 내용을 분석하였다. 본 연구 결과, 표준화 환자를 활용한 시뮬레이션 정신간호실습교육 후 간호대학생들의 학습 성과는 모든 항목에서 유의하게 증가하였다. 표준화 환자를 활용한 시뮬레이션 정신간호실습교육의경험내용은 21개의 주제와 4개의 영역으로 도출되었는데, 각 영역은 ‘자아성찰’, ‘학습동기부여’, ‘시뮬레이션 수업의 긍정적 효과’, ‘시뮬레이션 수업의 어려움’이었다. 본 연구를 통해 표준화 환자를 활용한 시뮬레이션 정신간호실습교육이 간호대학생의 학습 성과를 향상시키는데 효과적이며, 긍정적인 실습경험을 제공했음을 확인할 수 있었다. This is triangulation study in order to analyze the learning effect of psychiatric nursing education and the content of experience of simulation education through developing scenario for simulation psychiatric nursing practical education using standardized patients. Objects are 54 students who have studied nursing at K university located in Gangwon-do. Researchers developed scenario for simulation psychiatric nursing practical education using standardized patients and used questionnaire composed by researcher in order to compare before and after learning outcomes of simulation psychiatric nursing practical education. Gathered data was analyzed by using SPSS/WIN 18.0 program. Also, through non-structured questionnaire, understood experience of nursing students after simulation psychiatric nursing practical education and analyzed the content using qualitative research method. Program outcomes of nursing students after simulation psychiatric nursing practical education using standardized patients are increased in all category. Experience contents of nursing students are appeared as 21 subjects and 4 categories and each categories were 'self-examination', 'learning motivation', 'positive effect of simulation class' and 'difficulty if simulation class'. Through this research, it is confirmed that psychiatric nursing simulation education using standardized patients is effective to improve nursing students' learning outcome and it provided positive practical experience to nursing students.

        정신장애인의 인권존중수준에 대한 인식이 고등교육지원 욕구에 미치는 영향 -청년기와 타 연령집단 간 비교를 중심으로

        This study aims (1) to examine the effect of perception of the level of respect for human rights on the higher education needs among individuals with psychiatric disabilities, and (2) to examine whether young adults differs compared to other age groups in the relationship between perception of the level of respect for human rights and the higher education needs. For this purpose, multiple regression analysis was performed using survey data involving 305 individuals with psychiatric disabilities who are using a psychiatric rehabilitation facility. Key findings are as follows. (1) The higher level of perceived human right friendly environment, the higher needs of the higher education. (2) Young adults with psychiatric disabilities presented higher levels of needs for higher education support services than their counterparts. (3) The effects of perception of the level of respect for human rights on needs for higher education support services differ by age groups. Based on these results, implications for policy and practice were discussed to establish a higher education support system that meets human rights and needs for higher education among young adults with psychiatric disabilities 본 연구의 목적은 정신장애인의 인권존중수준에 대한 인식이 고등교육지원 욕구에미치는 영향을 살펴보고, 생애전환기 청년기가 중장년기 및 기타 연령집단과 비교하여인권존중수준에 대한 인식과 고등교육지원 욕구 간의 관계에서 차이를 보이는지를 확인하는 것이다. 이를 위해 정신재활시설 이용자 305명을 대상으로 조사한 자료를 활용하여 다중회귀분석을 실시하였다. 주요 결과는 다음과 같다. 첫째, 정신장애인의 인권존중수준에 대한 인식이 높을수록 고등교육지원 욕구가 높은 것으로 나타났다. 둘째, 청년기 정신장애인은 다른 연령집단에 비해 고등교육지원 욕구가 높았으며, 셋째, 인권존중수준에 대한 인식과 고등교육지원 욕구 간의 관계에서 청년기 여부의 조절 효과가 유의미하게 나타났다. 연구결과를 토대로, 정신장애인의 고등교육지원 욕구에 부응하는 인권을 존중하는 환경과 청년기 욕구에 부합하는 고등교육 지원체계 구축을 위한 실천적⋅정책적 함의와 발전방안을 논하였다

        정신병원의 간호사가 인식하는 환자안전 실태조사

        Purpose: The purpose of this study is to investigate the patient safety status and patient safety education contents and methods perceived by nurses working in psychiatric hospitals. Methods: Data were collected using a semi-structured questionnaire for psychiatric hospital nurses who had worked for more than one year. A total of 157 questionnaires were filled and were used for analysis using descriptive statistics. Open-ended questions about the contents and methods of patient safety education were classified by each researcher and a reclassification was conducted by consensus through discussions. Results: Overall, 142 nurses (90.4%) experienced patient safety accident during the previous 1 month. Among those who experienced a patient safety accident, profanity, violence, and aggression-related accident counted for the highest percentage (82.8%). The demand for education on workplace stress management and education on psychiatric medication were the highest. Required education methods included case-by-case education on patient safety accidents, regular and continuous education, and small-scale face-to-face education. Conclusion: In the future, it is necessary to actively utilize the results of this study as basic data for in-depth research on the causes of patient safety accidents, accident prevention, and the workload of nursing personnel.

        한국의 정신분열병 환자 재활을 위한 웹기반 약물 및 증상 자가 관리 교육 프로그램 개발

        This study was purposed to develop web-based education program for schizophrenic patients in order to identify symptom, facilitate medication and prevent recurrence. Research methodology was two phases as following: 1) Web-based education program was developed based on the learning objectives. 2) The effectiveness of the program was studied on quasi experimental research with experimental group and control group. The experimental group received web-based education and the control group received face-to-face education. After receiving the education each with different methods, the experimental group attainted the higher learning achievement in the knowledges of medication and symptom management. And there was a significant difference between the experimental group and the control group. This result proves that the effect of the web-based education better than that of the face-to-face education in the increase of knowledges. 본 연구는 정신분열병환자를 대상으로 약물 및 증상관리 교육을 위한 웹기반 교육프로그램을 개발하고 그 효과를 검증하고자 시도되었다. 본 연구는 2 단계로 구성되었다. 1 단계에서 정인성(1999)의 교수-학습 체제 설계 모형에 근거하여 정신분열병환자의 인지적 특성이 고려된 개별화 학습을 위해 약물 및 증상관리 내용을 주제별로 나누어 웹기반 교육 프로그램을 개발하였다. 연구의 2단계에서는 개발된 웹 기반 교육 프로그램의 효과를 구체적으로 규명하기 위해 비동등성 대조군 유사 실험 연구를 시행하여 웹 기반 교육 프로그램을 적용한 실험군과 면대면 약물 및 증상 관리 교육 프로그램 적용한 대조군 간에 약물 및 증상에 관한 지식, 약물 복용도 및 질병 재발경고증상 변화의 차이를 파악하고자 하였다. 연구결과 실험군과 대조군의 비교에서 약물 및 증상에 관한 지식에서 두 집단 간에 유의한 차이가 있는 것으로 나타났다.

        정신간호교육에서 활용되는 시뮬레이션의 통합적 고찰 : 정신간호학 학습목표와 간호사 핵심역량을 중심으로

        The purpose of this study was to the literature review the types and methods of simulation used in psychiatric nursing education. Also, the simulation results were classified according to the learning goal of psychiatric nursing, and this was analyzed according to the core competency of nurse. Key words in Koran and English were used to search 8 electronic databases: RISS, KISS, NDSL, DBpia, MEDLINE, PsycINFO, Cochrane library and CINAHL. This study was performed according to Whittemore and Knafl integrative review strategy(problem identification, literature search, data evaluation, data analysis, presntaion). The simulations used in mental health care have been largely categorized into standardized patient, audio utilization, computer-based, simulator (audio function), and mannequin simulation. Each simulation result was confirmed to reach nursing learning objectives; such as application of knowledge based nursing skills, selection and implementation of core basic nursing skills according to the nursing situation, application of therapeutic communications and application of nursing process based on critical thinking. It was found that the core competency of nurses could be reached as a result of the simulation according to the learning objectives(competency of integrated application of knowledge and nursing skills to provide holistic care, competency of critical thinking to solve nursing problems, competency to communicate and cooperate between specialties to improve the client). Based on these results, it is suggested that various simulations could be applied to psychiatric nursing practice in the future. 본 연구는 정신간호 교육에서 활용되는 시뮬레이션 기법의 종류와 적용방법을 통합적으로 고찰하기 위해 수행되었다. 시뮬레이션의 효과를 정신간호 학습목표에 따라 분류하고, 이를 간호사 핵심역량에 따라 분석하였다. 한글과 영문 주제어를 이용하여 국내 문헌 검색은 RISS, KISS, NDSL, DBpia를, 국외는 MEDLINE, PsycINFO, Cochrane Library, CINAHL을 이용하였다. 이 연구는 Whittemore와 Knafl의 통합적 고찰방법(문제규명, 문헌검색, 자료평가, 자료분석, 의미해석)에 따라 총 30개 문헌을 선정하여 분석하였다. 정신간호에서 활용된 시뮬레이션 기법은 크게 표준화 환자 활용, 오디오 활용, 컴퓨터 기반, 시뮬레이터(음성기능), 비디오, 마네킹 등으로 분류할 수 있다. 각 시뮬레이션 기법은 간호학의 학습목표인 ‘전공지식에 근거한 간호술 적용’, ‘간호상황에 따른 핵심기본간호술 선택 및 실행’, ‘치료적 의사소통술 적용’, ‘비판적 사고에 근거한 간호과정 적용’에 도달하는 것으로 나타났다. 또한, 정신간호에 적용된 다양한 방법의 시뮬레이션 수업은 ‘전인간호제공을 위한 전공지식과 간호술의 통합적용 능력’, ‘간호문제 해결을 위한 비판적 사고능력’과 같은 간호사 핵심역량에 도달하는 것으로 조사되었다. 이러한 결과를 바탕으로 추후 정신간호 실습에서 다양한 방법의 시뮬레이션을 혼합하여 적용해 볼 것을 제언한다.

        선추적검사 A, B, B-A, B/A점수와 연령, 학력 그리고 뇌손상환자와 정신과환자의 수행비교

        This study was performed to two purpose. First, performance time evaluation of Trail Making Test(TMT) was a component of the neuropsychological test battery to be examined age and education were considered. Second, A difference(B-A) and a ration(B/A) score to be examined to offered useful information for severity of head injury. Test implement used to be TMT A, TMT B, wechsler intelligence subscale. The subject of normative control group consisted of number 94, subjects of patient included four group consisted of head injury number 53, postconcussional disorder number 26, schizophrenia number 24, neurotic patient number 53. The result are that TMT A, B, B-A were positive correlated with age and negative correlated with education in normative control group. Four patient group performance are significant difference appeared decending in order from neurotic group, schizophrenia group, postconcussional group, head injury group. The inclusion implicated that neuropsychological test result interpretation were considered with age and education. Also, a difference score B-A be useful to head injury evaluation.

      • Psychiatric-Mental Health Nursing Education and Cultural Competence

        Purpose : The objectives of the presentation are to (1) review the key concepts related to culture and mental health; and (2) to discuss the process for developing and implementing a graduate course which was designed to foster nurses’ understanding of the influence of various cultures (e. g., racial, ethnic, socio-economic, educational, and geographic) on mental health and nursing care. This abstract mainly focuses on the qualitative research study process that has been used to develop the graduate course. Methods : First, qualitative descriptive study design was used to (1) explore Korean nursing students’ nursing care experiences with patients from diverse cultural backgrounds; and (2) explore Korean nursing students’ needs for cultural competence education integrated into the nursing curriculum. Undergraduate and graduate students attending one of the nursing schools in Korea participated in the focus group. Next, new graduate course was developed based on the findings from the focus group interviews. Results : A total of 18 undergraduate and 20 graduate students (n = 38) participated in one of six focus groups. The findings from the qualitative data demonstrated that participants have encountered their boundaries and prejudices when caring for ethnically-diverse patients. The participants described their experience of caring for patients from diverse cultural backgrounds as “encountering the boundaries of my taken-for-granted world” and “being aware of the core nature of human communication and caring.” The findings confirmed that the interaction between two cultures is reciprocal and that health care providers and patients were mutually influenced by each other. The findings of this study were incorporated into the nursing curriculum and I developed and offered a new graduate course for nursing students, called “Sociocultural diversity and health.” A total of 22 students completed the course and reported that they experienced self-reflection and became more sensitive to the power imbalance between health care providers from dominant cultures and patients from different cultural backgrounds. Conclusions : More courses addressing cultural diversity and cultural competence should be developed for nursing student who are working for patients from diverse cultural backgrounds.

        정신장애인 공공후견제도와 사회복지사의 역할

        This study aims to examine the changing roles of social workers after launching public guardianship(PG) for individuals with psychiatric disabilities(IPD) in 2017 and its implications for social work education. In order to address these purposes, focus group interviews were done with specific focuses on the experience of 16 social workers participated in PG services for IPD. Major results are summarized as follows. Participants reported three role categories involving primary care-takers, case managers, and advocators with 15 sub-themes. The role of primary care-taker turned out to be a new role generated by the newly launched public guardianship services. Although the roles of case managers and advocators are similar to the traditional social workers’roles, the spectrums of detailed roles are much more expanded and diverse, increasing burnout risks of public guardian social workers. Regardless of the burnout risk due to the dramatic expansions of burden, there are very few opportunities for guardianship education. Based on these results, implications for future directions of social work education to develop a sustainable public guardianship system for IPD. 본 연구의 목적은 정신장애인 공공후견사업 후견인으로 활동하고 있는 사회복지사들의 역할과 경험을 바탕으로 기존 사회복지사 역할과의 비교를 통해 사회복지교육에 주는 함의를 논의하는 것이다. 이러한 목적을 달성하기 위해서 4개의 후견기관 사회복지사 성년후견인 16명을 대상으로 초점집단 인터뷰를 시행하고 이를 주제 분석하였다. 분석 결과 성년후견 사회복지사의 역할은 보호자, 사례관리자, 옹호자의 세 가지 범주와 15개 주제가 도출되었다. 보호자 역할은 기존 사회복지사 역할과 차별화되는 새로운 역할이고, 사례관리자와 옹호자 역할은 전통적 사회복지사와 유사한 역할로 나타났다. 하지만 광범위한 보호자 역할로 인해 사례관리 및 옹호의 영역도 피후견인의 신상과 금전 관리에 이르기까지 다양하고 새로운 역할들이 수행을 요구받고 있었다. 특히, 관련 교육이나 지원의 부재로 사회복지사 후견인들의 과도한 업무 부담과 소진의 위험성이 보고되었다. 이러한 연구결과를 바탕으로 지속가능한 공공후견 사회복지영역을 구축하기 위해서 필요한 사회복지교육 및 제도의 발전방향에 대한 함의를 논의하였다.

        Effects of a Competency-Based Education Program for Inpatient Psychiatric Nurses: A Pre-Post Intervention Study

        Purpose: This study aimed to develop a competency-based educational program for strengthening the competency of psychiatric nurses and evaluate its effects. Methods: The study used a quasi-experimental design. Participants included 64 psychiatric nurses in South Korea: 33 nurses (32 females, 1 male) in the experimental group and 31 nurses (26 females, 5 males) in the control group. A competency-based education program for psychiatric nurses was developed through a literature review and discussion with 12 experts. The program addressed eight topics and provided 24 hours of education over 4 days, for 6 hours each day. The collected data via questionnaires were analyzed statistically. Results: The education program enhanced overall clinical nursing competence and strengthened professional self-growth competence. The management and teaching competencies of both the experimental and the control groups also increased significantly. Conclusion: The findings of this study presented an educational program to strengthen the clinical competency of psychiatric nurses and it was proven that the education had a capacity-building effect.

        비정신과 간호사의 정신간호역량 강화 교육 요구도

        Purpose: The purpose of this study is to identify and prioritize educational needs for psychiatric nursing competencies among 142 non-psychiatric nurses from general hospitals. Methods: Data were collected from November 13 to 19 2022 through self-administered questionnaires consisting of measures of psychiatric nursing competencies. Data were analyzed using descriptive statistics, paired t-test, Borich's needs assessment model, and the Locus for Focus model to determine the priority of educational needs. Results: Non-psychiatric nurses reported that enhancing skills in psychiatric nursing were more significant than improving knowledge and attitude using Borich’s Needs Assessment Model and the Locus for Focus Model. The highest priority educational needs were ‘Skill to distinguish whether symptoms in psychiatric patients are symptoms of mental illness or side effects of psychiatric drugs’, ‘Skill to calm and supportive communication in psychiatric emergency when psychiatric patients are at risk of harm’, ‘Attitude to remain calm about the psychotic behaviors of psychiatric patients’, and ‘Knowledge of non-therapeutic communication that interferes with effective communication with psychiatric patients’. These items should be considered as priorities among the 39 items to improve psychiatric nursing competencies. Conclusion: Results of this study suggest that developing intervention programs to enhance psychiatric nursing competencies are needed among non-psychiatric Nurses.
