        기능성 발보조기 착용 전후의 시상면의 편평족 발목각도 변화에 관한 연구

        The foot performs an important function in supporting the body and keeping body balance. An abnormal walking habit breaks the balance of the human body as well as the normal function of the foot. The influence of a flatfoot(pes planus) occupies a considerable portion of the various causes resulting in the wrong walking habit. But, little studies has been done by the functional foot orthotics for the flat foot. The object of this study, therefore, is to propose a new approach method to reveal the effects of the improvement of the foot function by using orthotics. The essential point of this study is to measure and analyze the change of ankle angle in the sagittal plane for flat foot subjects wearing the orthotics. Before and after wearing the functional orthotics, the gait analysis of flat foot subjects was conducted in three experimental aspects : the change of ankle angle, the change of the total Ankle ROM and the difference of left & right ankle angle in the sagittal plane. 1. The average ankle angle differences of before-and-after wearing the orthotics have declined like this; left : $2.71^{\circ}$, right: $1.91^{\circ}$ (p<0.05). 2. Total ankle ROM also showed decrease in both sides while the left side's is rather slight; left : $0.57^{\circ}$, right : $2.07^{\circ}$ (p<0.05). 3. The difference of left and right ankle angle in the sagittal plane decreased by $0.71^{\circ}$ (p<0.05). In result, it is confirmed that the functional foot orthotics have a significant effect on mechanical movement of ankle joint for flat foot. it is expected that this paper will be further studied and improved as a practical estimation method in the research on the effect of foot-orthotics.

        행군 시 발보장구 착용이 지면반력에 미치는 영향

        The purpose of this study was to examine the effects of customized foot orthotics during march in combat boots. Four volunteers with normal foot and six volunteers with excessive pronation foot among soldiers on service were fitted with foot orthotics. The Ground reaction forces signal from activity of Pre-After military marching. The marching was performed with a speed of 4.5 km/h. The experiment design for research wes composed two experimentation. The first experiment was to examine the ground reaction forces between normal foot and excessive pronator foot during march. The second experiment was to examine the ground reaction forces between wearing orthotics and no wearing orthotics. These data were analyzed by One-way ANOVA method and paired T-test method of SPSS(windows version 15.0). The result of the study were the maximum vertical impact peak and the maximum vertical loading rate on pronator foot tend to statistically significant increase after march. But the maximum vertical impact peak and the maximum vertical loading rate decrease the customized foot orthotics in volunteers with excessive pronation foot compared to volunteers with normal foot. Clinically, the application of orthotics for the soldiers with excessive pronation foot appears to keep nomal foot function and to decrease loading rate.

        편평족 중학생의 고관절 교정 운동 프로그램과 발교정구 착용 유무가 안정시 종골기립 각도, 발목의 가동범위, 코어 근력에 미치는 영향

        Objective : The purpose of this study is to evaluate the effects of hip joint exercises and orthotics on RCSP, ankle``s range of motion, and core muscle strength of middle school students with pes planus. Method : Out of the original pool of 200 students, 60 students with pes planus (RCSP < -2) were selected for the study. The selected 60 students were then divided into four groups. The first group was a combined orthotics and exercise group (12 students), the second was the orthotics-only group (9 students), the third was the exercise-only group (8 students), and the last was the control group (10 students). Exercise groups worked out twice a week for 60 minutes per session over 8 weeks. The independent variables were corrective hip joint exercises and orthotics. The dependant variables consisted of kinematic and kinetic variables. The kinematic variables were RCSP, and ankle``s range of motion (dorsiflexion and plantarflexion). The kinetic variables were muscles forces that consist in core muscle strength, which are hip joint adduction, abduction, and flexion muscles forces. Statistical analysis was performed via SPSS 18.0 with multivariate analysis of covariance (MANCOVA) and a paired t-test was used. Results : The left foot was more responsive to the treatments, both exercise and orthotics, than the right foot. RCSP improved significantly in the left foot for the first and third groups. Only the first group significantly improved hip joint adduction, abduction, and flexion muscles`` strengths. As for the ankle``s range of motion of the left foot, plantarflexion showed improvement when treated with exercise, orthotics, or both. Conclusion : This study found that exercise is more effective in correcting RCSP and foot orthotics is more effective in reinforcing core muscle strength. Future studies should expand on these results to examine the relationship between the ankle, hip, and pelvis.

        The Effectiveness of the Use of Custom-Made Foot Orthotics on Temporal-Spatial Gait Parameters in Children with Spastic Cerebral Palsy

        This study examined the effects of custom-made foot orthotics on the temporal-spatial gait parameters in children with cerebral palsy. Twenty spastic bilateral cerebral palsy (spastic CP) children (11 boys and 9 girls) participated in this study. GAITRite was used to examine the velocity, cadence, step length differential, step length, stride length, stance time, single support time, double support time, base of support, and toe angle while walking with and without foot orthotics. The differences in temporal-spatial parameters were analyzed using paired t-test. The significance level was set at .05. The velocity, cadence, both step lengths, both stride lengths, both bases of support and right toe angle significantly increased when the children with spastic CP with foot orthotics compared to without foot orthotics (p<.05). The step length differential between the two extremities, left stance time and left single support time, significantly decreased with foot orthotics (p<.05). Right stance time, right single support time, both double support times and left toe angle showed little change (p>.05). This study demonstrated that foot orthotics were beneficial for children with spastic CP as a gait assistance tool.

        발 교정구 착용과 신체 발육발달

        The purpose of this review provides an overview of the current knowledge on the effects of wearing foot orthotics and physical growth development in human body. In this paper, the foot plays an important role in supporting the body and keeping body balance. An abnormal gait habit breaks the balance of the human body as well as the function of the foot. The foot orthotics which is designed to consider biomechanics effectively distributes the load of the human body on the insole of the foot. As a literature analysis of gait, posture, planter pressure, balance and pain was improved and foot orthotics might engage more muscle for the lower and upper body, resulting not only in decreased foot pressure, but also improved balance and posture. In conclusion, foot orthotics is to insert a special treatment so that weight is applied and helps to facilitate the comfort of the user, stability, gait. In addition, foot orthotics is to shock absorption, ankle and foot joint distributed, reducing weight by gaiting on fatigue, effective for physical development, in that minimizes the development of the muscles not.

        후향적 연구를 통한 맞춤형 족부보조기와 한방치료를 병행한 편평족 환자의 인체중심선 변화에 대한 연구

        Objectives : The purpose of this study is to find out the effect of Korean Medical treatments combined with Orthotics on patients's center of gravity with flat foot. Methods : We reviewed the medical records of 24 patients diagnosed with flat foot. They had received Korean medical treatments combined with Foot Orthotics. To estimate the efficacy in rebalancing of body, we analyzed the changes in center of gravity. Results : Among the 24 patients, 13 were male and 11 were female. There was no statistically significant change in frontal angle at the time of treatment. But side angle was changed statistically significant(P=0.009). There was no difference according to sex of male and female. Conclusion : In this study, We found that Korean Medical treatments with Foot Orthotics was effective for correction of patients's center of gravity with flat foot.

        신발의 종류와 발교정구의 착용 유·무가 중학생의 보행에 미치는 영향

        본 연구의 목적은 신발의 종류와 발교정구 착용 유·무에 따른 중학생의 보행에 미치는 영향을 규명하는 것이다. 이를 위해 8명의 편평족 중학생을 대상으로 신발 3종(슬리퍼, 크록스, 운동화)과 발교정구 착용 유·무에따른 보행패턴의 즉각적인 효과를 살펴보았다. 이후 6주 간 발교정구를 적용한 후 보행패턴의 지속적인 효과를 검증하였다. 보행패턴을 분석하기 위해 지면반력기(Kissler, Germery)와 E-med(Novel, Germery)를 이용하여지면반력과 족저압력을 측정하였다. 보행 시 신발의 종류와 발교정구의 효과를 비교하기 위해 반복측정 이원분산 분석을 실시하였다. 분석결과, 첫째, 편평족 중학생의 보행에 있어 지면반력, 무게중심, 좌·우 역적 값은 발교정구 착용의 영향을 받는 것으로 나타났으며 신발의 종류에는 영향을 받지 않았다. 둘째, 신발 종류는 밑창의 경도에 따라 족저압력에 영향을 미치는 것으로 나타났다. 구체적으로 낮은 경도의 밑창을 가진 신발은 족저압력이 중경도의 밑창을 가진 신발보다 더 높게 나타났다. 따라서 발교정구의 효율성을 높이기 위해서는 중경도의 밑창을 가진 신발이 적합하다는 결론을 얻을 수 있었다. 이러한 연구결과를 통해 향후 기능성 발교정구를 장착한 신발의 조건을 정형화시키고 현재 나오는 신발 모델들의 발교정구 적합성의 등급을부여할 수 있는 기초자료로 사용될 수 있을 것이다. The purpose of this study was to analyze the effects of shoe type and foot orthotics(FO) experience on gait for adolescent students with Pes Planus from Seoul City. In pretest, we examined the immediate effect of gait pattern on 8 adolescent student with pes planus with 3 types of shoes(slipper, crocs, sneaker) and with/without foot orthotics. In post-test, we verified effects of foot orthotics after applying the orthodontic foot for 6 weeks. 8 adolescent student with pes planus were evaluated foot type test(RCSP <4°). During Gait, Ground Reaction Force(GRF; F(x), F(y)), Center of Pressure(COP), F(x)Integral, gait time, Plantar Pressure(PP) were measured. Statistical analysis was performed with Repeated two-way ANOVA via IBM SPSS 22.0 version. As the result, First, in the Gait of adolscent student, GRF(x), GRF(y), COP, F(x)integral were significantly affected by Foot Orthotics, not Shoe Type. Second, Shoe Type was affected by PP according to the stiffness of outsole. PP was higher for the shoe with low stiffness outsoles than for shoes with over medium stiffness. Therefore, it was concluded that shoes with over medium stiffness outsole are suitable to improve the efficiency of FO. It is expected that This study can be used as a basic data to standardize the conditions of Footwear to make synergy effect with FO for adolscent student with Pes Planus.

        달리기시 맞춤형 발보조기 사용이 피로물질변화에 미치는 영향

        The purpose of this study is to examine the changes in blood fatigue induction factors and the comfort when the orthotics was put on during running. There were ten subjects, who participated in absolute intensity exercise(6mile/hr, 5%). They ran on the treadmill with and without the custom foot orthotics. Lactate, ammonia, and phosphorous were analyzed after blood sampling was taken three times(before exercise, after finishing exercise performance, and 30min after recovery). The data concerning the overall comfort was collected by a questionairre. Experimental data were analysed using SPSS program. The level of blood lactate, ammonia, and phosphorous with custom foot orthotics were the lower than that of without custom foot orthotics. During running the overall comfort was higher for the foot orthotic condition than the nonorthotic conditions.

        평지와 오르막경사의 트레드밀 걷기와 달리기 동안 발보장구가 발바닥근막염과 관련된 발아치 스트레인에 미치는 영향

There is some evidence that one of major factors to produce plantar fasciitis depends on the magnitude of the foot arch strain. The orthotics that can reduce the foot arch strain during locomotion may be effective to prevent or treat plantar fasciitis. Therefore, the purpose of this study was to investigate the effect of control condition and three types of foot orthotics on 3-dimensional foot arch strain that can produce plantar fasciitis during treadmill level and uphill walking and running. Sixteen male subjects are recruited and the arch length and height strain according to three types of foot orthotics with respect to control condition were measured by using two digital video cameras. The first hypothesis which the comfort of foot orthotics would be increased from arch pad, half length orthotics to full length orthotics was mostly accepted. It suggested that the types of the foot orthotics could be properly prescribed according foot regions that is pain or abnormal. The second hypothesis which the foot arch strain can be reduced by foot orthotics during level heel-toe walking and running and the third hypothesis which the foot arch strain can be reduced by foot orthotics during uphill heel-toe walking and running were rejected. The foot arch length and height strain during walking and running showed small and subject-specific characteristics and could not be optimal biomechanical variable to prove the overall comfort. The forth hypothesis which the foot arch strain cannot be reduced by foot orthotics during uphill toe walking and running was accepted. With the foot arch length and height strain during uphill toe walking and running the windlass mechanism suggested by Hicks can be explained successfully and excessive uphill toe walking and running can be one of cause of plantar fasciitis. The dynamic investigation on the foot arch such as walking and running should be carefully observed with integrated insights considering ligaments and foot bones as well as plantar fascia, extrinsic muscles and tendons, and intrinsic muscles and tendons.
