A retrospective study was made of the previous medical records of 47 infants with congenital syphilis born from mothers with untreated syphilis at Severance Hospital, Yonsei University College of Medicine during the years 1972 through 1983 The VDRL titer of these infants was higher than 1: 8 with reactive FT.A-ABS test. They were subjectd to hematologic examinations and the results were as follow s, 1. Of 46 infants with congenital syphilis, anemia was observed in thirty-one (68%) Reticulocytosis occurred in seventeen(89%) of nineteen infants with congenital syphilis having the mean value of 6. 0%. 3, Of 4.7 infants with congenital syphilis, leucocytosis was noted in seveenteen 4, Thrombocytopenia was observed in twenty (64%) of thirty-one infants with congenital syphilis.

본 증례는 극소미숙아에서 선천매독을 성공적으로 치료한 것에 대한 내용이다. 재태주수 29주 3일, 1,395 g의 여아가 응급 제왕절개 분만으로 태어났으며, 당시 1분 아프가 점수는 2점, 5분 아프가 점수는 4점이었다. 산모는 산전진찰을 전혀 받지 않았던 불법 체류자로, 분만 직전에 매독으로 진단받았다. 환아는 심각한 호흡곤란 증후군, 신생아 폐 고혈압 지속증, 파종성 혈관 내 응고증, 피부의 낙설, 긴 뼈 끝 부위에 골용해성 변화 등을 보였었다. 2주간 Penicillin G 치료를 받았으며, 퇴원 시에는 다른 합병증 없이 건강한 상태로 퇴원할 수 있었다. 최근 한국뿐만 아니라 전 세계적으로 선천 매독의 발생률이 늘어나고 있다. 신생아 진료의들은 선천 매독의 임상 양상을 충분히 알고 미리 대처하여 장기적 합병증 및 사회적, 경제적 부담을 줄일 수 있도록 해야 할 것이다. We report a case of successful treatment of congenital syphilis in an extremely preterm baby. A 1,395 g female infant was born by emergency Caesarean section due to preterm labor and breech presentation at gestational age at 29 weeks and 3 days with an Apgar score of 2 and 4 at 1 minute and 5 minutes, respectively. The mother of the newborn, an illegal immigrant who did not receive any antenatal care, was diagnosed as active syphilis infection by reactive rapid plasma regain (RPR) (titer 1:128) just before the delivery. Upon birth, the newborn presented with various clinical manifestations, including severe respiratory distress syndrome, persistent pulmonary hypertension of the newborn, disseminated intravascular coagulopathy, desquamation and scaling of the whole body, and osteolytic changes of long bone ends. Results of laboratory tests showed signs of early congenital syphilis, including positive syphilis reagin test (12.7 R.U.), reactive with RPR titer of 1:64, and positive for immunoglobulin (Ig) M and IgG fluorescent treponemal antibody absorption test. However, after completion of penicillin G treatment for two weeks, laboratory results dramatically improved, showing a negative syphilis reagin test (0.5 R.U.) and non-reactive in RPR. In conclusion, the incidence of congenital syphilis is prone to be resurgent in South Korea, neonatologists should be fully aware of the clinical features of congenital syphilis because early diagnosis and prompt treatment are essential in order to reduce the social and economic burden due to congenital syphilis.

선천성 매독은 산전진찰로 예방할 수 있음에도 불구하고, 여전히 문제가 되고 있다. 최근에는 선천성 매독과 관련된 혈뇨나 폐렴이 보고된 증례가 없었으나 저자들은 적절한 산전치료를 받지 못한 산모에게서 태어난 22일된 남아에게서 발생한 선천성 매독에 동반된 신증후군과 폐렴의 증례를 보고한다. 선천 매독의 진단은 혈청학적 검사로 확진하였고, 환아는 페니실린 G 치료를 받고 회복되었다. 신생아에게서 임상증상은 매우 불확실할 수 있어, 질환에 대한 인식이 지연 될 수 있다. 따라서 선천성 매독의 예방에는 철저한 산전관리가 필수적이다. 만약 산모가 적절한 산전진찰을 받지 않았을 경우에는 신속한 혈청학적 진단이 반드시 시행되어야 할 것이다. Although congenital syphilis can be prevented with prenatal screening, the disease remains problematic. Currently, there are no cases that describe hematuria and pneumonia related to congenital syphilis. We report a case of congenital syphilis that involved nephrotic syndrome and pneumonia alba in a 22-day-old male infant whose mother did not receive adequate prenatal care. The congenital syphilis diagnosis was confirmed with a serologic test and the patient recovered with penicillin treatment. Clinical findings may be subtle in neonates and delayed recognition occurs frequently, thus complete prenatal screening is critical for congenital syphilis prevention. Immediate serologic testing should be performed to obtain a differential diagnosis if an infant is delivered by a mother that has not received appropriate prenatal examinations.

A preterm newborn affected by congenital syphilis, born to mother not treated during pregnancy is described. The clinical picture was characterized by respiratory distress, cutaneous manifestations, massive hepatosplenomegaly, severe anemia, thrombocytopenia, disseminated intravascular coagulation syndrome and hypoalbuminemia. The patient was treated with daily injections of 190,500 units of crystalline penicillin G for 14 days. Premature infants with these symptoms and signs should be evaluated for congenital syphilis.

Purpose: Syphilis infections are becoming more prevalent in the Republic of Korea, and inadequately treated syphilis can lead to congenital syphilis (CS) in newborns. This study aimed to analyze the clinical manifestations of syphilis in mothers and newborns and to make suggestions to improve disease prognosis. Methods: This single-center study was performed between August 2009 and August 2019 and included 29 newborns with CS. We retrospectively evaluated the clinical features, rapid plasma reagin (RPR) card test, fluorescent treponemal antibody absorption test (FTA-ABS), morbidity, and treatment regimen of all the syphilis-affected mothers and their newborns. Results: At the time of delivery, mean maternal age was 29.0±6.1 years old, and newborn gestational age was 38.0 weeks. In cases when syphilis was confirmed during the second and third trimesters of pregnancy, the newborn with CS had morbidity (p=0.004). The mean RPR titer was related to morbidity (p= 0.036). Positive results of FTA-ABS IgM (p<0.001) and pleocytosis in the cerebrospinal fluid (CSF) (p= 0.020) also increase morbidity. The most common symptoms were desquamation and skin rash, followed by hepatomegaly, neurodevelopmental disability, and bone abnormalities. The highest number of CS cases per 1,000 live births in this hospital was in 2014. Conclusion: CS is a preventable and treatable disease if physicians detect symptoms and provide appropriate treatment through RPR examinations during every trimester. General practitioners should be widely trained on various aspects including early detection, formal treatment, and regular follow-up. Additionally, medical services should be provided for the entire childbearing population regardless of the socioeconomic status.

We describe the case of a 4-month-old male infant diagnosed with early congenital syphilis during evaluation of a left distal humerus fracture. This report emphasizes the importance of screening for syphilis among pregnant women and newborns, and is a reminder of the continued existence of congenital syphilis.

Congenital syphilis is a rare disease nowadays because of prenatal screening test and early treatment. In the absence of proper maternal therapy, infection can result in stillbirth, prematurity, or a wide spectrum of clinical manifestations involving multiple organ systems. We took care of a preterm infant with apnea, desquamation on the palms of hands and thrombocytopenia at birth, and the serologic tests revealed congenital syphilis. We present a fatal case of early congenital syphilis of preterm newborn delivered by an untreated mother, with a brief review of literature.
Early congenital syphilis presenting with skin eruption alone: a case report

Congenital syphilis is one of the most well-known congenital infections,yet it remains a worldwide public health problem. Congenital syphilis can involve any organ system and present with various symptoms. However, early diagnosis of congenital syphilis is difficult because more than half of the affected infants are asymptomatic, and the signs in symptomatic infants may be subtle and nonspecific. Here, we report a case of congenital syphilis with only a skin rash, which led to a delay in diagnosis. This case indicates that congenital syphilis should be considered throughout early childhood.

Congenital syphilis occurs when Treponema pallidum crosses the placenta during pregnancy or from contact with an infectious genital lesion during delivery. Cutaneous manifestations of congenital syphilis are relatively common, occurring in approximately 30% to 70% of patients. Maculopapular lesions, vesiculobullous lesions, condylomata lata lesions, annular lesions, and erythema multiforme-like targetoid lesions have been reported.
