        중등도 족관절 외측 인대 염좌 후 재활치료

        Objective: To evaluate the effect of comprehensive rehabilitation treatment in patients with moderate lateral ankle ligament sprain. Method: We reviewed the medical records of 22 patients who had been in rehabilitation program after moderate lateral ankle ligament sprain. We measured the range of motion of ankle joint, pain intensity by visual analogue scale (VAS) score, the difference of ankle circumference between normal and affected side, and strength of dorsiflexor, plantarflexor, invertor and evertor of ankle before and after rehabilitation. Results: The range of motion, VAS score, ankle circumference, strength of dorsiflexor, invertor and evertor of the affected ankle significantly improved after rehabilitation exercise (p<0.05). Eighteen out of 22 patients returned to the sports activities successfully after systemic rehabilitation. Conclusion: Systemic and appropriate rehabilitation is inevitably necessary after acute ankle sprain for the successful return to the sports activities.         발목 손상 환자의 재활 적용 방법 및 효과에 관한 문헌고찰

        본 연구는 발목관절 손상 빈도가 높은 태권도 선수들에게 최적의 재활 운동 방법에 대한 정보를 제공하고자 온라인 데이터베이스를활용한 문헌고찰을 실시하였다. 문헌의 범위는 2018년 1월부터 2022년 12월까지 국내외에서 발행된 학술지 논문을 중심으로, 발목관절, 손상, 재활, 운동, Ankle joint, injury, rehabilitation, exercise 등의 키워드로 검색하였다. 검색된 문헌은 총 8,236편으로, 문헌 선정기준에 따라 발목관절 손상 환자를 대상으로 재활 운동의 적용 효과를 분석한 국내 10편, 국외 14편의 문헌이 최종적으로 선정되었다. 발목관절 손상 환자를 대상으로 균형 운동, 저항성 운동, PNF 운동, 소도구 운동, 복합 운동, 진동자극 운동, 반중력 트레드밀 운동, 혈류 제한 운동 등을 주 2~7회 빈도로 10~90분간 적용한 결과 근력, 정적 및 동적 균형 능력, 발목 기능, 고유수용성감각, 진동감각역치, 근육단면적, 낙상위험도, 족부압력, AOFAS, CAIT, FAAM, FAOS, LEFS, ROM, VAS 등이 통계적으로 유의하게 향상된 결과를 확인할 수 있었다. 따라서, 태권도 선수들이 발목관절의 즉각적인 기능 회복을 통한 빠른 현장 복귀 및 재손상을 방지하기 위해서는 전문치료사와 함께 손상 유형 및 재활 운동의 적응 능력을 고려한 체계적인 재활 과정이 필요할 것으로 사료된다. This study conducted a literature review using an online database to provide information on optimal rehabilitation exercise methods for Taekwondo players with high frequency of ankle joint injuries. The scope of the literature was searched with keywords such as ankle joint, injury, rehabilitation, exercise, etc., focusing on academic journal papers published at home and abroad from January 2018 to December 2022. A total of 8,236 literatures were searched, and 10 domestic and 14 international papers that analyzed the effect of applying rehabilitation exercises to patients with ankle joint injuries were finally selected according to the literature selection criteria. Balance exercise, resistance exercise, PNF exercise, exercise using props, compound exercise, vibration stimulation exercise, anti-gravity treadmill exercise, and blood flow restriction exercise were applied to patients with ankle joint damage for 10 to 90 minutes, 2 to 7 times a week. Balance exercises, resistance exercises, PNF exercises, exercise using props, complex exercises, vibration stimulation exercises, anti-gravity treadmill exercises, and blood flow restriction exercises were applied to patients with ankle joint injuries for 10 to 90 minutes, 2 to 7 times a week. As a result, muscle strength, static and dynamic balance ability, ankle function, proprioception, vibration sensation threshold, muscle cross-sectional area, fall risk, foot pressure, AOFAS, CAIT, FAAM, FAOS, LEFS, ROM, VAS, etc. were statistically significantly improved. Therefore, in order for Taekwondo players to quickly return to the field through immediate functional recovery of the ankle joint and prevent re-injury, it is believed that a systematic rehabilitation process considering the type of injury and the ability to adapt to rehabilitation exercises.

        만성 발목관절 불안정성의 재활운동 접근에 관한 고찰

        The purpose of this study is to testify the effectiveness of the approaching with exercise for ankle joint instability caused by repetitive ankle sprain, and to provide the essential data on rehabilitation exercise. Chronic ankle joint instability can cause the change of structure of the soft tissue around the ankle by repetitive ankle sprain, and it also brings out a lot of issue such as dysfunction on the safety of neuromuscular control and musculoskeletal system on the ankle joint. Therefore we have studied about what is the function of the ankle, and definition, classify, feature, treatment and rehabilitation exercise approach of injury mechanism and ankle joint instability. The outcome of this study tells us when programing the rehabilitation exercise program, not only the ankle joint, it also need to consider about the balance with the other part of the body, power of the muscular and neuromuscular control, and it is required to have effective guideline of rehabilitation exercise program for preventing from becoming chronic.

        신경근병증 환자의 추간판절제술 후 발목재활운동을 통한 시·공간 및 압력분포 변인 중심의 보행 특성 분석

        [INTRODUCTION] The purpose of this study was to provide the changes ankle rehabilitation exercise how altered gait analysis of temporo-spatial parameters in herniated intervertebral disc patients after discectomy surgery. [METHOD] The subject was 40-year-male patient who had operated due to herniated intervertebral disc. after surgery, he showed low back pain & leg pain greater 90%, but showed radiculopathy caused by an abnormal gait characteristics, 12th December 2011 for ankle rehabilitation exercise program, the patient was sent to the Y hospital spine&joint exercise center. performed temporo-saptial parameters test 3 time; pre-rehabilitation, 4 weeks after rehabilitation, and 8weeks after rehabilitation. [RESULT] Rehabilitation exercise program of 8weeks improve the involved ankle range of motion, deficit rate was significantly reduced. In addition, temporo-spatial parameters and variable distribution of foot pressure was improved. [CONCLUSION] After Discectomy with radiculopathy in patients with enhancement of dorsiflexion, plantarflexion ankle rehabilitation exercise programs that applies for the case reported. increased participation in daily life and social life have to return to the postoperative rehabilitation program participation is considered very beneficial. [서론] 본 연구의 목적은 신경근병증으로 인한 추간판절제술을 받은 환자에게 발목 재활운동 프로그램을 적용하여 시·공간 및 압력분포 변인 중심으로 보행의 특성을 살펴보는데 있다. [방법] 본 연구의 대상자는 40세 남성으로 L4-5 레벨 왼쪽의 추간판탈출증을 진단받아 추간판절제술을 시행하였다. 수술 후 허리와 다리통증은 90%이상 좋아졌으나, 신경근병증으로 인한 비정상적인 보행특성을 보여 2011년 12월 12일부터 발목재활운동을 Y병원 척추관절운동센터에서 8주간 시행하였다. 재활운동 시작, 재활운동 4주후, 재활운동 8주후에 발목관절의 운동범위와 시·공간 및 압력분포 변인을 측정하였다. [결과] 8주간의 재활 운동프로그램을 적용한 결과 환측의 발목 관절운동범위가 향상되었으며, 건측과의 결손률도 현저하게 감소되었다. 또한 시·공간적 변인 및 발의 압력분포 변인이 개선되는 것으로 나타났다. [결론] 추간판 절제술 후 신경근병증 환자에게 발등굽힘근, 발바닥굽힘근의 기능 개선을 위한 발목 재활운동프로그램을 적용시킨 증례보고로서, 일상생활 참여증대 및 사회생활의 복귀시점을 앞당기는데 있어 수술 후 재활운동 프로그램 참여는 매우 유익한 것으로 생각된다.

        The purpose of this study is to report the treatment effects of Korean medicine for patients with chronic ankle instability. A-56-year old female patient who had pain on both ankles due to chronic ankle instability was treated for eleven times. We used Chuna therapy, acupuncture and rehabilitation exercise. After eleven times treatment both Numeral rating scale(NRS) and Pain Disability Index(PDI) was decreased. Chuna therapy, acupuncture and rehabilitation exercise are thought to be one of the effective treatment for patients with chronic ankle instability.

        Trampoline Exercise Improves Functional Performance in Young Adults with Ankle Instability

        PURPOSE The purpose of this study was to examine the effect of trampoline exercise on functional performance in persons with chronic ankle instability. METHODS A total of 24 college students were assigned to one of three groups: control (CON), ankle sprained without chronic ankle instability (NAI), and ankle sprained with chronic ankle instability (CAI). Both NAI and CAI exercised on the trampoline for 40-70 min, three times a week during six weeks. To determine ankle’s functional performance, Romberg test, Y-balance test, Side hop test, Figure-of-8 hop test, and Square hop test were conducted both injured and uninjured sides of foot before and after exercise. The data were analyzed utilizing two-way repeated ANOVA. RESULTS The result showed significant differences among groups and time in Romberg test(NI), Y-balance test posteromedial(NI), Y-balance test posteromedial(I), Y-balance test posterolateral(I), Figure-of-8 hop test(NI), Square hop test(NI), and Square hop test(I). The result showed a significant interaction in Romberg test(I), and there was a significant differences between time in Romberg test(I). There were significant differences according to time in Y-balance test front(NI), Y-balance test front(I), Y-balance test posterolateral(NI), Side hop test(NI), Side hop test(I), and Figure-of-8 hop test(I). Trampoline exercise significantly improved functional performance measures including static and dynamic balance, postural control, muscle strength, and proprioception although the positive effectiveness is differential depending on the degree of ankle instability. CONCLUSIONS Our data demonstrate positive effects of rehabilitative trampoline exercise against impaired functional performance of ankle with instability.

        발목 염좌 및 만성 발목 불안정성 환자들에 대한 4주간의 고유수용감각 운동 프로그램의 효과

        Background : Ankle inversion sprains are one of the most common injuries in sports and activities of daily living that mostly concern physically active individuals. In most researches, proprioceptive deficit, muscle weakness and/or absent coordination have been regarded as a contributing factors. Despite the high incidence of ankle sprain and instability, therapeutic approaches to properly manage the symptoms have rarely been investigated. This study aimed to identify the effect of proprioceptive exercise program that is easy to integrate in normal training program. Methods : Subjects were randomly allocated to control group and experimental group consisting of 11 and 10 patients, respectively. The conservative treatment for the control group consisted of hot packs, ultrasound and TENS. In addition, the experimental group performed 7 exercises to enhance proprioceptive function of ankle joint. The therapeutic intervention of the controland experimental groups was performed a total of 20 exercise sessions, averaging 50 hour each, 5 times per week for 4 weeks. To compare the two groups, the level of ankle disability was assessed by using the ankle injury score scale in pre- and post-treatment. Results : On assessment of post-treatment, there were statistically significant differences in the scores of all sub-items, except for ankle laxity and range of motion, and the total score of ankle injury score scale between the two groups(p<0.05). In comparison between pre- and post-treatments, the significant difference in the scores of all sub-items and total score didn't appear for the control group, while the scores of most sub-items and total score of the experimental group were shown the statistically significant difference(p<0.05). Conclusion : The findings suggest that the proprioceptive exercise program is more effective for relieving ankle disability than conservative treatment therefore, the program to improve proprioceptive function should be recommended for prevention and rehabilitation of recurrent ankle inversion injuries.

        Changes of Functional Recovery of Ankle Joint with Application of Rehabilitation Exercise Program after Arthroscopic Modified Brostrom Operation: A Pilot Study

        [OBJECTIVES] The purpose of this study was to demonstrate its effectiveness by applying a systematic rehabilitation exercise program to patients after arthroscopic modified Brostrom operation. [METHODS] The research subjects were 13 patients who had arthroscopic modified Brostrom operation, the 60-minute rehabilitation exercise program was applied twice a week for six weeks, and then, subjective evaluation of ankle joint, and its dynamic stability and isokinetic muscle strength were measured. [RESULTS] After application of the rehabilitation exercise program, subjective evaluation of the ankle joint(p<.001), its dynamic stability(p<.001) and strengths of the involved side for isokinetic muscle strength peak torque inversion(p<.001), eversion(p<.01), total work inversion(p<.001), eversion(p<.01) uninvolved side peak torque inversion(p<.001), eversion(p<.01), total work(p<.01), muscle endurance of the involved side peak torque inversion(p<.001), eversion(p<.01), total work inversion(p<.001), eversion(p<.01), uninvolved side peak torque inversion(p<.01), total work inversion(p<.01) were significantly increased. [CONCLUSIONS] It was confirmed that application of rehabilitation exercise program after arthroscopic modified Brostrom operation was effective for positive possibilities of its settlement as regular rehabilitation exercise program.

        발목재활 운동이 태권도 선수의 손상 받은 발목에 미치는 영향

        Objective: In Taekwondo, strengthening, muscular endurance, agility, and flexibility are all important muscle functions that determine an athlete's ability. This research aims to explore the effects of ankle rehabilitation exercise on ankle muscle strength, muscular endurance, lower balance ability and other functional abilities. Method: 30 taekwondo athletes suffering from ankle injuries were divided into 3 groups and put under rehabilitation exercises from Jun 1st. 2010 until August 31st, 2010. Examinations were conducted before and after, including strengthening test, muscular endurance test, lower balance test, and functional test. Results: Strengthening showed a significant difference(p<.05) among groups in dorsi-flexion, eversion at 60°/sec; a significant difference(p<.05) in eversion at 180°/sec. Lower balance showed a significant difference(p<.05) among groups in overall balance index, anterior/posterior index and medial/lateral index. Finally, for the functional test, there was a significant difference(p<.05) among groups in hopping and side hopping. Conclusion: Strengthening, muscular endurance, lower balance ability and functional ability showed greatest improvement in the proprioception exercise group. Hence, it can be concluded that the proprioception exercise group has positive effects when used as ankle rehabilitation exercise for injured athletes.

        만성족부불안정성 환자에서 변형된 Brostrom 수술 후 조기 재활운동이 발목근육 회복에 미치는 영향

        The purpose of this study was to examine the effect of early rehabilitation exercise after modified Brostrom operation (MBO) on ankle muscle recovery in Chronic ankle instability. A intervention study design was used to assess 22 patient aged 34 to 37 years. Following written, informed consent, 11 MBO patient group (mean age = 37.00 ± 14.37 years; mean wt = 58.73 ± 8.21 kg; mean ht = 166.55 ± 6.50 cm) and 11 Con (non operation) patient group (mean age = 34.55 ± 13.03 years; mean wt = 68.73 ± 9.43 kg; mean ht = 168.73 ± 6.81 cm) completed a battery of test to determine muscle power (ankle eversion and inversion tests were performed concentrically at 60°·80° / sec by use of the Cybex 770; peak torque body weight (%); PT/BW). The subjects were tested in body composition by use of the Inbody 12.0. Results from the early rehabilitation exercise have shown that were significant differences between two groups in 60º/sec (Inversion, p < .01; Eversion, p < .05), 180º/sec (Inversion, p < .01; Eversion, p < .05). In conclusion, the early rehabilitation exercise found in modified Brostrom operation patients suggests that characteristic of modified Brostrom operation or instability atients may be caused by lower muscle power. These data suggest that these muscle power increase, which may positive affect the early recovery and ormal activity.
