Background/Aims: The serum of cancer patients often harbors increased free DNA levels, which can potentially be used for cancer detection. Recently, several genes (including NPTX2) have been discovered to be aberrantly methylated in primary pancreatic cancer tissue. The aim of study was to investigate possibility of differential diagnostic method in high risk group of pancreatic cancer such as chronic pancreatitis based on epigenetic characteristics. Methods: We enrolled 33 pancreatic cancer and 22 benign pancreaticobiliary disease patients. ERCP guided pancreatic duct brush cytology samples were obtained. After that, we enrolled 104 pancreatic cancer, 60 chronic pancreatitis and 5 benign biliary tract stone disease patients. We collected blood samples before surgery or any kind of treatment. DNA was extracted and NPTX2 CpG island hypermethylation was examined quantitatively by real-time PCR amplification after chemical modification. Results: There were 104 pancreatic cancers and their AJCC stages were as followed; 3 in stage I, 21 in II, 43 in III, 37 in IV. We could see statistically significant higher levels of NPTX2 hypermethylation compared to chronic pancreatitis (p=0.016). From ROC curve, we could get 79% sensitivity and 75% specificity (cut off=0.015%). NPTX2 methylation appeared in all stages of pancreatic cancer, but it got significantly higher as AJCC stages progress. This was in contrast to the fact that there were statistically significant higher levels of NPTX2 hypermethylation compared to chronic pancreatitis but there were no significant difference among the stages in cytology samples of pancreatic cancer. Conclusions: The quantitative analysis revealed that aberrantly hypermethylated NPTX2 gene could be detected from the plasma and cytology samples of pancreatic cancer and it was significantly higher compared to chronic pancreatitis. Therefore, aberrantly methylated NPTX2 gene may help differentiate from chronic pancreatitis with conventional diagnostic tools.

We investigate the energetics involved in the ( 101̄2)[1̄011] tension and ( 101̄1)[1̄012] compression twinning deformationprocesses in magnesium via fi rst-principles studies. Through identifi cation of structural changes associated with each deformationprocess, we study the energetics of each deformation process and the local instability in the twin boundary region. We observe that the energy barrier in the (10̄11)[̄1012] compression twinning deformation pathway is higher than that inthe ( 101̄2)[1̄011] tension twinning deformation pathway, even though the ( 101̄1) compression twin boundary is more stablethan the ( 101̄2) tension twin boundary. We extend our study to examine the Effects of Y and Li as alloying elements on eachtwinning deformation process. Our calculations predict that the addition of Y causes a reduction in the probability of fractureby an order of magnitude when the twinning deformation occurs and weakening of the resistivity to twinning deformation. However, the Effect of Li addition on the twinning deformations is weaker than that of Y addition.

The volume of hip arthroplasty is stiffly increasing because of excellent clinical outcomes, however it has not been shown to decrease the incidence of transfusions due to bleeding related to this surgery. This is an important consideration since there are concerns about the side effects and social costs of transfusions. First, anemia should be assessed at least 30 days before elective hip arthroplasty, and if the subject is diagnosed as having anemia, an additional examination of the cause of the anemia should be carried and steps taken to address the anemia. Available iron treatments for anemia take 7 to 10 days to facilitate erythropoiesis, and preoperative iron supplementation, either oral or intravenous, is recommended. When using oral supplements for iron storage, administer elemental iron 100 mg daily for 2 to 6 weeks before surgery, and calculate the dose using intravenous supplement. Tranexamic acid (TXA) is a synthetic derivative of the lysine component, which reduces blood loss by inhibiting fibrinolysis and clot degradation. TXA is known to be an effective agent for reducing postoperative bleeding and reducing the need for transfusions in primary and revision total hip arthroplasties. Patient blood management has improved the clinical outcome after hip arthroplasty through the introduction and research of various agents, thereby reducing the need for allogeneic blood transfusions and reducing the risk of transfusion-related infections and the duration of hospitalizations.

Aims: Magnetic resonance elastrography (MRE) is a noninvasive method to assess liver fibrosis in chronic liver disease. This study aimed to investigate the risk factors for liver fibrosis, assessed by MRE, in patients with nonalcoholic fatty liver disease (NAFLD). Methods: A cross-sectional study was conducted in 641 patients with NAFLD based on a cohort from Kangbuk Samsung Hospital health screening programs 2015-2018. Liver stiffness measurement (LSM) was evaluated using 2-dimensional real-time MRE. Significant liver fibrosis was defined as LSM ≥2.97 kPa. We investigated significant liver fibrosis and its risk factors in patients with NAFLD. Results: The mean age was 50.8 years and male was 546 patients (85%). The proportions of hypertension, diabetes mellitus, and metabolic syndrome were 30%, 17%, and 34%, respectively. The mean value of LSM in MRE was 2.44 ± 0.40 kPa. Of 641 patients, significant liver fibrosis (LSM ≥2.97 kPa) was observed in 41 patients (6.4%). The multivariable analysis showed that significant liver fibrosis was associated with body mass index ≥27.5 kg/m2 (odds ratio [OR], 3.18; 95% confidence interval [CI], 1.56-6.46; P=0.001), Fibrosis-4 (FIB-4) index ≥1.3 (OR, 2.35; 95% CI, 1.15-4.80; P=0.020), metabolic syndrome (OR, 2.69; 95% CI, 1.26-5.75; P=0.010), and diabetes mellitus (OR, 4.55; 95% CI, 2.27-9.15; P<0.001). Conclusions: High levels of BMI and FIB-4 index, metabolic syndrome, and diabetes mellitus were the risk factors for significant liver fibrosis in patients with NAFLD.

We report a rare case of granulocytic sarcoma infiltrating the bile duct in a patient with acute myeloid leukemia. A 23-year-old man presented with jaundice and weight loss. A peripheral blood smear revealed blast cells, and the results of an examination of bone marrow aspirate were consistent with acute myeloid leukemia. The bilirubin level increased gradually after induction chemotherapy with cytarabine. Magnetic resonance cholangiopancreatography (MRCP) revealed dilatation of the intrahepatic bile ducts and smooth tapering off at the level of the common hepatic bile duct. Endoscopic retrograde cholangiopancreatography (ERCP) also revealed diffuse narrowing of the proximal common hepatic bile duct. Obstructive jaundice resolved after endoscopic nasobiliary drainage. Remission induction chemotherapy with cytarabine and idarubicin was administered, and the patient remained complete hematological remission with normal liver function tests. [P338] A rare dermoscopic pattern of nodular basal cell carcinoma with amyloid deposition

An 80-year-old woman was referred for a skin colored dome shaped papule on the nose of 2 years duration. In polarized dermoscopy, the lesion was composed of large whitish globules. In the periphery, there were short telangiectatic vessels that were not arborizing. Histopathologic finding revealed variable-sized nodules of basaloid cells with peripheral palisading pattern and amorphous eosinophilic material in the center, and amyloid deposits were present in the stroma between clumps of tumor cells. Congo red stain showed these deposits as a reddish substance, and these depositions showed apple-green birefringence under polarized light, confirming the presence of amyloid. These findings suggested this tumor was nodular basal cell carcinoma (BCC) with amyloid deposition. The most common dermoscopic features of nodular BCC are irregular vascularity and translucency. Dermoscopic characteristics of macular amyloidosis are known as whitish or brownish central hub surrounded by various configurations of pigmentation. The central hub was replaced by a scarlike morphology in lichen amyloidosis, which can be similar to the characteristics of sebaceous gland origin tumor, molluscum contagiosum, and dermatofibroma does. In our case, it can be assumed that massive deposition of amyloid bodies presented as whitish globules on dermoscopy. Dermatologists should consider amyloid deposition can be manifested as whitish globules dermoscopically.

Silicon nitride thin lms were deposited by the atomic layer deposition (ALD) technique in a batch-type reactor by alternating exposures of a Si precursor and NH3. SiCl4 and SiH2Cl2 were examined as the Si precursor, and the properties of the deposited lms were comparatively characterized. The ALD by using SiH2Cl2 exhibited larger deposition rates at lower precursor exposures as compared with the ALD by using SiCl4, and the deposited lms from SiH2Cl2 had lower wet etch rates in a diluted HF solution. Silicon nitride lms with a Si : N ratio of approximately 1 : 1 were prepared by using either Si precursor at 500 C; however, the lms deposited by using SiH2Cl2 had higher H concentrations. Silicon nitride lms deposited by ALD showed physical properties comparable with those of the silicon nitride lms deposited by low-pressure chemical vapor deposition, lowering the deposition temperature by more than 200 C.
