        Single Shot Epidural Injection for Cervical and Lumbosaccral Radiculopathies: A Preliminary Study

        Background: Epidural steroid injection is an established treatment modality for intervertebral disc prolapse leading to radiculopathy. In cases where two levels of radiculopathy are present, two separate injections are warranted. Herein, we present our experience of management of such cases with a single epidural injection of local anaesthetic, tramadol and methylprednisolone, and table tilt for management of both radiculopathies. Methods: 50 patients of either sex aged between 35-65 years presenting with features of cervical and lumbar radiculopathic pain were included and were subjected to single lumbar epidural injection of local anaesthetic, tramadol and methylprednisolone, in the lateral position. The table was then tilted in the trendelberg position with a tilt of 25 degrees, and patients were maintained for 10 minutes before being turned supine. All patients were administered 3 such injections with an interval of 2 weeks between subsequent injections, and pain relief was assessed with a visual analogue scale. Immediate complications after the block were assessed. Results: Immediate and post procedural complications observed were nausea and vomiting (20%), painful injection site (4%), hypotension (10%) and high block (4%). Pain relief was assessed after the three injections by three grades: 37 (74%) had complete resolution of symptoms; 18% had partial relief and 8 % did not benefit from the procedure. Conclusions: This technique may be used as an alternative technique for pain relief in patients with unilateral cervical and lumbar radiculopathies. (Korean J Pain 2010; 23: 254-257)

        Thermokinetics behavior of epoxy adhesive reinforced with low viscous aliphatic reactive diluent and nano-fillers

        The current study reports the effect of low viscous aliphatic reactive diluent (RD), MWCNT and VGCF on the curing kinetics of amine cured epoxy adhesive system employing non-isothermal differential scanning calorimetric (DSC) technique. Non-isothermal DSC thermograms of epoxy adhesive were studied at various heating rates: 5, 10 and 15 oC/min. The decrease in the exothermic peak height with the introduction of MWCNTs and VGCFs was taken as proof of the acceleration effect of nano-fillers on the epoxy-amine curing reaction. Also, increased Tonset, TP and ΔHcurs values were observed for epoxy/RD adhesive system at all heating rates. The value of activation energy (Ea) was determined using Kissinger and Flynn-Wall-Ozawa methods. Experimental results showed that the addition of 10 wt% RD increased the Ea from 60 to 63 kJ/mol on account of the reduced viscosity, allowing better contact of resin with the curing agent. Furthermore, MWCNTs have an accelerating effect on the cure kinetics that does not change the autocatalytic cure reaction mechanism of epoxy resin. It was also found that the addition of MWCNT and VGCF decreases the overall degree of conversion, as evident with lower ΔHcure and Ea of the cured adhesive when compared with epoxy/RD system. The dependency of Ea on degree of conversion ranging from α =0.1 to 0.9was also investigated. The two normalized functions y(α) and z(α) were also considered in order to study the complex curing mechanism. The kinetic parameters m, n and lnA were obtained by using two parameter autocatalytic Sestak-Berggren model. The curves revealed good agreement between experimentally determined and theoretically obtained MWCNT/VGCF reinforced epoxy adhesive systems.

        Natural convection in Bingham plastic fluids from an isothermal spheroid: Effects of fluid yield stress, viscous dissipation and temperature-dependent viscosity

        In this work, the buoyancy-induced convection from an isothermal spheroid is studied in a Bingham plastic fluid. Extensive results on the morphology of approximate yield surfaces, temperature profiles, and the local and average Nusselt numbers are reported to elucidate the effects of the pertinent dimensionless parameters: Rayleigh number, 102 ≤ Ra ≤ 10 6; Prandtl number, 20 ≤ Pr ≤ 100; Bingham number, 0 ≤ Bn ≤ 10 3, and aspect ratio, 0.2 ≤ e ≤ 5. Due to the fluid yield stress, fluid-like (yielded) and solid-like (unyielded) regions coexist in the flow domain depending upon the prevailing stress levels vis-a-vis the value of the fluid yield stress. The yielded parts progressively grow in size with the rising Rayleigh number while this tendency is countered by the increasing Bingham and Prandtl numbers. Due to these two competing effects, a limiting value of the Bingham number (Bnmax) is observed beyond which heat transfer occurs solely by conduction due to the solid-like behaviour of the fluid everywhere in the domain. Such limiting values bear a positive dependence on the Rayleigh number (Ra) and aspect ratio (e). In addition to this, oblate shapes (e < 1) foster heat transfer with respect to spheres (e = 1) while prolate shapes (e > 1) impede it. Finally, simple predictive expressions for the maximum Bingham number and the average Nusselt number are developed which can be used to predict a priori the overall heat transfer coefficient in a new application. Also, a criterion is developed in terms of the composite parameter which predicts the onset of convection in such fluids. Similarly, another criterion is developed which delineates the conditions for the onset of settling due to buoyancy effects. The paper is concluded by presenting limited results to delineate the effects of viscous dissipation and the temperature-dependent viscosity on the Nusselt number. Both these effects are seen to be rather small in Bingham plastic fluids.

        Awareness regarding Safe Abortion among Adolescent Girls in Rural area of Mahottari district of Nepal

        목적 : 이 연구는 네팔의 마호타리 지방에 거주하고 있는 사춘기 소녀들의 낙태에 대한 인식 수준을 평가하기 위해 수행하였다. 방법 : 이 연구는 지역사회 기반 단면연구로서 2019년 1월부터 3월까지 네팔의 마호타리 지역의 농촌 마을 사춘기 소녀 412명 대상으로 실시하였다. 대상자는 10세에서 19세까지의 사춘기 소녀로서 다단계집락추출 방법으로 선정하였고, 사춘기 소녀의 낙태에 대한 인식 수준에 미치는 요인을 알기위해다변량로지스틱회귀을 시행하였다. 결과 : 조사 결과 사춘기 소녀의 45.6%가 안전한 낙태에 대해 높은 인식도를 가지고 있었다. 결혼한 청소년의 인식도는 미혼의 청소년보다 높았고(OR = 2.16, 95 % CI : 1.01-4.87), 중등교육 이상을 받은 청소년의 인식도가 높았다(OR = 2.21, 95 % CI : 1.13-3.04). 또한 월 소득이 10,000 ~ 20,000 루피(NRs)와 그 이상이 되는 경우 10,000 루피에 비해 각각 2.33 배(OR=2.33, 95 % CI : 1.07-3.55), 3.17 배(OR=3.17, 95 % CI : 2.19-8.94)더 높았다. 결론 : 이 연구를 통해 낙태에 대한 전반적인 지식 수준은 낮았다. 결혼상태, 교육수준, 가족수입과 같은 사회 경제적 요인이 낙태에 대한 인식수준과 관련이 있어, 이를 통해 원치 않는 임신, 낙태 및 그로 인한 합병증을 줄이기 위해 낙태에 대한 인식 수준을 높일 수 있도록 노력하여야 한다. This study aimed to assess the level of awareness on safe abortion among adolescent girls in rural area of Mahottari district of Nepal. A community based cross-sectional study was conducted in rural areas of Mahottari district of Southern Nepal between January and March 2019.A sample of 412 adolescent girls was selected using multi-stage cluster sampling. Multivariable logistic regression model was adapted to explore level of awareness among adolescent girls. This study found that 45.6% of adolescent girls had high awareness regarding safe abortion. The odds of awareness among the married adolescents was higher (AOR=2.16; 95% CI: 1.01-4.87) than unmarried adolescent whereas the odds of awareness among the adolescents who had education of secondary level and more had higher (AOR=2.21; 95%CI: 1.13-3.04) than those who had primary or lower secondary level of education. Similarly, the adolescents who had monthly family income of Nepalese Rupees (NRs.), 10,000-20,000 and more than NRs. 20,000 were respectively, 2.33 times (AOR = 2.33; 95% CI: 1.07–3.55) and 3.17 times (AOR = 3.17; 95% CI: 2.19–8.94) more likelihood to have high awareness regarding safe abortion than those their counterparts. The study showed that overall level of knowledge towards safe abortion was found low. Socio-demographic factors like marital status, level of education, and family income were the factors independently associated with level of awareness on safe abortion. Therefore, efforts should be exerted towards arising and improving the awareness of abortion care which may reduce unwanted pregnancy, abortion and other complications related to abortion.

        Effect of Supersonic Shock Wave Loading on Thin Metallic Sheets: Experimental and Numerical Studies

        Defence structures are under high threat of blast loading in recent years due to rising terrorist activities. Protection of defence structures against blast conditions has received a huge interest in the past few years. Blast mitigation requires to study the behavior of various metals under supersonic shock loading. An experimental study was carried out using in-house developed shock tube apparatus to understand effect of shock loading on mild steel and aluminum sheets. A limitation of current study is 1-D loading occurring in shock tube which is unlike 3-D loading occurring in a blast event. Experiments reveal that the properties of thin sheets in through-thickness direction plays an essential role in the shock transfer, which affects considerably the failure and final deformed shape of the sheet. The study showed that metallic thin sheets absorbed shock energy via plastic deformation and failed via a tensile failure mode. Results from this study indicate that aluminum sheets and mild steel sheets can offer shock-resistant properties and mitigate more energy via absorption and plastic deformation. Aluminum and mild steel sheets can therefore be used as a face and back sheet for the development of any sandwich composites.

        First report on CRISPR/Cas9 mediated editing of the eye colour gene, Tryptophan 2, 3-dioxygenase in egg plant shoot and fruit borer Leucinodes orbonalis Guenée (Lepidoptera: Crambidae)

        Overuse of synthetic chemicals over a long period of time has not only resulted in control failures but also enormous ecological damage. This necessitates developing eco-friendly, effective alternatives for sustainable pest management. In this regard, potential of CRISPR/Cas9 mediated genome editing to introduce site specific mu tations that mostly resulting in loss of function has been successfully demonstrated in wide varieties of organisms including insects. This has opened a new avenue to design and implement futuristic pest management strategies like precision guided sterile insect technique (pgSIT) for an area wide suppression. This requires validation of target genes employing Ribonucleoprotein (RNP) complex containing sgRNA and Cas9 protein before under taking transgenesis for achieving pgSIT. In the present study, we have for the first time, supporting CRISPR/Cas9 mediated editing of the eye colour gene, Tryptophan 2, 3-dioxygenase of the eggplant shoot and fruit borer, Leucinodes orbonalis using RNP complex. The mutant moths of both sexes exhibited reddish brown eyed phenotype. Having established an RNP mediated editing system in L. orbonalis, the key genes involved in sex determination and spermatogenesis will be validated for developing a pgSIT system for L. orbonalis.

        Frequency of group A rotavirus with mixed G and P genotypes in bovines: predominance of G3 genotype and its emergence in combination with G8/G10 types

        The present study describes the genotypic distribution of rotaviruses (RVs) in an Indian bovine population with unexpectedly higher proportions of G3 alone or in combination of G8/G10. PCR-genotyping confirmed that 39.4% (13/33) of the prevalent RVs were the G3 type while 60.6% (20/33) were dual G3G10 or G3G8 types. P typing revealed that 93.9% (31/33) of the samples were P[11] while 6.1% (2/33) possessed a dual P[1]P[11] type. Sequence analysis of the VP7 gene from G3 strains viz. B-46, 0970, and BR-133 showed that these strains had sequence identities of 90.5% to 100% with other bovine G3 strains. The highest identity (98.9% to 100%) was observed with RUBV3 bovine G3 strains from eastern India. The G3 strains (B-46, 0970, and BR-133) showed 97.5% to 98.8% sequence homologies with the Indian equine RV strain Erv-80. Phylogenetic analysis demonstrated that G3 strains clustered with bovine RUBV3 and J-63, and equine Erv-80 G3. Overall, these results confirmed that the incidence of infection by RVs with the G3 genotype and mixed genotypes in the bovine population was higher than previously predicted. This finding reinforces the importance of constantly monitoring circulating viral strains with the G3 genotype in future surveillance studies.
