        저혈량성 쇼크를 동반한 자발성 복직근초 혈종

        Background: Intermediate myasthenia syndrome(IMS) is thought to have clinical importance because it may cause sudden respiratory failure during the recovery phase of a cholinergic crisis of organophosphate poisoning. We designed this study to identify the prevalence, the inducing agent, clinical predictor, and the proposed treatment of IMS. Methods: Patients who had admitted with the diagnosis of acute organophosphate poisoning from 1992 to 1998 at two teaching hospitals were enrolled in this study. We selected the cases of IMS based on a review of medical records using modified He' s criteria. Results: Twelve(12) out of 110 patients with acute organophosphate poisoning were diagnosed for a prevalence at 10.9%. The drug inducing IMS were identified as dichlorvos, fenthion, EPN, methidathion, and phosphamidon. The occurrence of IMS was not related to either the initial treatment with atropine and pralidoxime, or the level of serum cholinesterase. Complications were pneumonia, sepsis, pancreatitis, and pseudomembranous colitis, etc. Eleven(11) patients were discharged without sequelae, and one patient was discharged as a hopeless case. Conclusion: This study suggests that IMS is not rare, so close observation is required to detect IMS in organophosphate-poisoning patients. Also, more studies are required to find predictors and treatments.

        급성 신우신염 환자에서 혈액배양의 유용성

        Background: Routine cultures of blood in febrile patients being hospitalized with uncomplicated pyelonephritis are parts of accepted management and are advocated in several textbooks of medicine and in several review articles, For a better understanding of the importance of blood cultures for patients admitted with pyelonephritis, we did a retrospective chart review of patients admitted to a medical center over a 2-year period. Methods: To evaluate the usefulness of blood cultures in patients admitted with pyelonephritis, we conducted a retrospective chart review of patients who were admitted to hospital from 1998 through 1999 with a primary discharge diagnosis of uncomplicated pyelonephritis. Results: Among the 329 patients admitted, 177 patients were included in the study. Sixteen(9.0%) were men, and 161(91%) were women. The mean age was 52.24±18.08. The most common precipitating factor was diabetes mellitus (19.2%). Urinalysis results showed E.coli as the causative agent in 155 patients(87.6%). Positive blood culture results were be seen in 67 patients with a 37.9% sensitivity. The usefulness of blood cultures in acute pyelonephritis was statistically significant only in the case of fever(P=0.003). Conclusion: In the setting of uncomplicated pyelonephritis in the adults, if urine cultures can be obtained, blood cultures are not usually necessary. Settings where blood cultures might be appropriate include immunosuppressed patients, patients in whom the diagnosis is not clear, patients in whom bacteremic seeding of the kidney is suspected, and patients who have a prior history of urinary tract infection,

        허혈성 장질환의 조기진단을 위한 혈청 락테이트(lactate)의 유용성

        Background: Intestinal ischemia remains a devastating event despite improvements in clinical recognition and in diagnostic and therapeutic modalities. The ischemic bowel diseases encompass a wide clinical spectrum from mild, reversible disease to severe, irreversible injury. The clinical picture is characterized initially by poorly localized visceral pain. Thus, it is difficult to diagnose this ischemia at an early stage. This study investigated whether an increased serum lactate level is a recognized danger signal marker for intestinal ischemia in patients who present at the emergency department because of abdominal complaints. Methods: Patients who came to our emergency department with abdominal pain and the risk factors of intestinal ischemia between Apr. 1999 and Nov. 1999 were included in this study. The data analysis included age, sex, final diagnosis, pathogenesis of bowel ischemia, and serum lactate level. Results: The serum lactate level in the intestinal ischemia group was 28.54±22.51 mg/dl; in non-ischemia group, it was 15.49±22.52 mg/dl. This difference between the two groups was significant(p<0.05). An increased serum lactate level had a sensitivity of 88.2% and specificity of 59.2%, a positive likelihood ratio of 4.92, and a negative likelihood ratio of 0.47 as a marker of bowel ischemia. These results do not represent a very meaningful revision of bowel ischemlc provability, but may make a small contribution to management of the disease, depending upon their magnitude and the clinical context in which they are applied. Conclusion: In patients with abdominal complaints, an increased serum lactate level is usually a useful aid as a diagnostic marker of bowel ischemia.

        A Comparison of Methods for Estimating the Productivity of Zostera marina

        Because seagrass production significantly contributes to the biodiversity and production of coastal and estuarine ecosystems, accurate estimation of seagrass productivity is a critical step toward understanding the ecological roles of seagrass in these ecosystems. To develop an accurate and effective method of measuring seagrass productivity, we estimated leaf productivity of eelgrass (Zostera marina) on the southern coast of Korea using three methods, the conventional leaf marking method, the elongation-mass method (Short '87 method), and the plastochrone method. In each season, shoots were pierced through the bundle sheath using a hypodermic needle and were collected after 2-4 weeks had elapsed to estimate their productivity. The leaf elongation and the leaf plastochrone intervals varied significantly among seasons. On an annual basis, the conventional leaf marking method showed the lowest leaf productivity estimates compared to the elongation-mass method and the plastochrone method, suggesting that the conventional leaf marking method underestimated leaf productivity as it ignored leaf maturation processes and new leaf growth within the sheath. Since the elongation-mass method considered leaf maturation processes, this method produced higher leaf productivity estimates than the conventional leaf marking method. On an annual basis, the plastochrone method produced the highest leaf productivity estimates. Below-ground productivity, which can be easily estimated using the plastochrone method, ranged between 3.29 and 5.73 (mg dry weight $shoot^{-1}\;day^{-1}$) and accounted for about 17.8% to 30.3% of total productivity. Because of the high contributions of below-ground productivity to total seagrass production, we suggest that the plastochrone method is an effective and simple technique for assessing both above- and below-ground productivities.

        한국산 갈조식물 지충이의 지리적 형태변이

        이번 연구는 한국산 지충이 [Sargassum thunbergii(Mertens ex Roth) Kuntze]의 지리적 형태변이를 확인하고, 형태변이에 따른 품종의 구분에 대하여 분류학적 재검토를 시도하였다. 동해, 서해와 남해안에 분포하는 지충이개체군의 지리적 형태변이를 조사하기 위해서 개체가 가장 성숙한 시기인 봄철에 채집하였다. 지충이의 관찰대상형질은 부착기, 줄기의 길이와 직경, 체장, 중심가지의 직경, 마디간격, 측지의 길이와 직경, 잎의 길이와 폭 및 두께, 기낭의 길이와 직경, 기낭 자루와 기낭 돌기의 길이,생식기탁의 길이와 직경, 질감이었다. 서해안의 지충이는 엽체가 짧고 가늘며, 마디간격이 넓어서 성긴 느낌이고 질감이 부드러워 다른 두 해역의 지충이와는 다른 형태적 특징을 보였다. 동해안의 지충이는 체장이 서해안의것과 비슷하지만 중심가지와 측지가 굵어서 질감이 거칠고, 측지가 없거나 매우 짧은 것도 있으며, 잎과 기낭이다른 해역에 비해 컸다. 남해안의 지충이는 체장과 측지가 가장 길고 잎과 기낭이 짧은 것이 특징이었다. 동해안북부에서 채집된 개체들은 f. latifolium과 형질이 유사하였다. 그러나 다른 해역에서는 네 품종의 원기재와 정확히 일치하지 않아 품종을 구분하는 식별형질은 한국산지충이의 형태 변이에 적용할 수 없었다. 따라서 생육지에 따라 나타나는 지충이의 다양한 형태변이는 종 이하의 분류군으로 나누는 것보다는 생육지 환경조건에 따른변이체로 보는 것이 타당할 것으로 사료된다. In this study, we investigated the effect of geographical variations on Sargassum thunbergii morphology to make a taxonomic reconsideration about infraspecific taxa in this species. In order to examine the morphological characteristics of S. thunbergii, total 27 matured and morphologically intact thalli were collected from the east, west and south coast in spring 2011. Interestingly, it was observed that the species populations on the west coast were characterized by short, thin and coarse thallus, and soft texture. However, the populations on the east coast showed thicker thallus, larger leaf and vesicle, and tougher texture. Thallus height of S. thunbergii was found to be similar at both east and west coast. Further, the height of the thallus and lateral branch of the species populations residing south coast were highest whereas the size of leaf and vesicle are shortest,in comparison with east and west coast species. Although morphological characteristics of the north east coast populations corresponded to the original description of S. thunbergii f. latifolium, we could not find exact morphological features and diagnostic characters to distinguish form in S. thunbergii. These results indicated that it is not the optimal characteristics to identify infraspecific form in this species. In contrast, morphological variations may signify the adaptation of this species to local environmental factors. Thus, we recommend that intraspecific morphological variation of S. thunbergii should be carefully used to identify infraspecific taxa.
