      • 糖尿白鼠의 大腿動脈 微細血管吻合 後 內皮細胞의 再生에 관한 硏究

        Recently, Diabetic patients are increasing in the field of microvascular surgery. Diabetes mellitus is known to be related to arterial damage, platelet malfunction and thrombus formation. After microvascular anastomosis, delayed repair and vascular occlusion occurred more frequently in diabetic state. This study was performed to investigate the patency rate and process of endothelial healing after microvascular anastomosis of femoral artery in diabeitc rat by scanning electron microscopy. The animals were divided into two groups, 20 diabetic-induced and 20 non-diabetic groups. Diabetes was induced with a injection of Streptozotocin(50㎎/㎏ b.w., Sigma Chemical Co.) to tail vein. The results were obtained : 1. Macroscopically, anastomotic site was intact except a few cases showed minimal inflammatory sign around the wound site. But the inflammatory change was frequently occurred in diabetic-induced group. 2. The patency rate was 95% (19/20) in non-diabetic group and 65% (13/20) in diabetic-induced group. 3. In the non-diabetic group, anstomotic region was mostly endothelized by the alignment along the long axis of vessel but stitchs were not covered with endothelial cells. The thickening of vessel wall was not observed. 4. In the diabetic-induced group, anastomotic region was not endothelized but covered with blood cellular components and connective tissue instead of endothelial cells. The thickening of the vessed wall was prominent in some diabetic-induced rats. These results suggest that diabetes was related to delayed regeneration of endothelium of vessels after microsurgical anastomosis.

      • 한국인 제2형 당뇨병 환자에 있어 지속적 피하 인슐린 주입법 치료시 Lispro insulin와 Regular insulin의 효과 비교

        Objective: The Lispro insulin has faster onset of effects and shorter duration than the human regular insulin, an analog of human insulin. The differences in the pharmacodynamics between the two types of insulin in the continuous subcutaneous insulin infusion(CSII) with insulin pump for treating the Korean Type 2 DM patients were analyzed. The effects on the regulation of blood sugar, the amount of insulin injected, and the frequency of hypoglycemia were compared between the lispro insulin and the regular human insulin. Method and Material: One hundred and three Korean Type 2 DM patients were randomly divided into two groups, 49 Lispro insulin and 54 Regular insulin.(Table 1) Both groups were treated for 14 days. Also at a set time before meal, insulin was injected as bolus, using a portable insulin pump, in both groups, 15 minutes before for lispro insulin and 30 minutes before for human insulin The HbA1C, C-peptide, AC/PC, height and weight were measured for each patients at the time of admission. The peripheral blood sugar level and insulin injection profile were recorded 7 times per day for each patients. Results: After treating for 7 days and 14 days, the blood sugar level normalized in both groups. There weren't any significant differences in blood sugar levels in both groups at point of the study, and also there weren't any differences in the frequency of hypoglycemia. In order to maintain the normalized blood sugar level, the lispro insulin group had higher total daily insulin requirement than the regular insulin group after 14 days of treatment (0.90±0.35 vs 0.73±0.39u/kg/day, p<0.05) This difference is probably due to the difference in basement insulin requirements rather than prandial insulin requirements (0.32±0.1 vs 0.43± 0.09u/kg/day, p<0.001). Discussion: In the hospitalized Korean Type 2 DM patients with CSII treatment, continuous subcutaneous insulin infusion therapy using lispro insulin has convenience in usage compared to the regular insulin. However, there were not any differences in the blood sugar levels. Also lispro insulin requires more insulin in order to maintain normal blood sugar level, which is probably due to the increase in the basal requirement. Also there were not any difference in the frequency of hypoglycemia.

      • 항우울제의 타액분비 감소효과에 대한 세포내 칼슘증가제의 영향

        Tri-and tetra-cyclic antidepressants are known to cause dry mouth among other several major complications. The present study was designed to compare the degree of reduced salivation due to antidepressants and to explore whether intracellular calcium-increasing agents ameliorate the salivation. Effects of antidepressants and agents increasing intracellular calcium on the cholinergic submandibular secretion and blood flow induced by the chorda stimulation or intra-arterial acetylcholine were observed in anesthetized cats. Effects of antidepressants and calcium-mobilizing agents on K% efflux were also observed in excised gland slices. The results obtained were as follows. 1. Salivary secretion in response to the chorda stimulation(3 V, 20 Hz, 1 msec) was significantly attenuated by antidepressants in a dose-dependent manner, whereas the blood flow was not affacted. 2. Salivary secretion and increased blood flow evoked by intra-arterial acetylcholine(20 ㎍/㎏) were markedly diminished by antidepressants, the magnitude of which was amitryptyline>imipramine>mianserin in order. 3. Cholinergic salivation was significantly decreased by cyclopiazonic acid, a calcium pump inhibitor of the endoplasmic reticulum, or by BAPTA/AM, a specific intracellular calcium chelator. 4. Caffeine and ryanodine potentiated the cholinergic salivation and ameliorated the depressed salivary secretion due to antidepressants. 5. Calcium ionophore A23187 ameliorated the depressed salivation due to antidepressants. 6. Antidepressants inhibited the K^+ efflux, which were restored by caffeine or A23187. These results suggest that the depressed salivary secretion due to antidepressants is ameliorated by increasing intracellular calcium levels.

        To make a progress of the seventh elementary art curriculum is that we reorganize every chapter and approach the new teaching-learning model which leads a better educational background. At the point of theory background for seventh educational curriculum, we can see the fact that both understanding art education and teaching-learning are focusing on the composition. We introduce how to reorganize the chapter for theoretical background of understanding art. For the seventh art educational curriculum, learners and teachers lead the activity of teaching-learning. Teachers should do their own role as a supporter, helper for their learners to let them do their best. So this can lead more active experiences of art education which satisfies the admitting of composition. To complete the goal of seventh art educational curriculum is that we find out their creativity for information of art, and give them a model of very right teaching-learning. This is generally based on the fact that we choose the focus of experiencing art, finding out art materials, theory, thoughts, imagination, and examination. It is very important for students interests, desire, their own choices, and free in the model of teaching-learning for seeking theory. When this model gives us theory of learning with teachers & students communication, we can choose and learn more specific theme, method of expressing. Generally, this model performs the procedures of coming up with thinking, drawing, experiencing, expressing, criticizing, and self-examination. The model of these two teaching-learning as above is not definite model but teachers do need to reorganize and perform flexibly, considering of the characteristics and situation of school.

        수술칼,CO₂및 Nd:YAG레이저에 의한 절개 창상의 치유에 관한 비교 연구

        The purpose of this study was to compare the wound healing process after skin incision using scalpel, CO₂laser and pulsed Nd:YAG laser in rats. After skin on the back was incised 3 cm long, rats were sacrificed at 1, 3, 7, 14, 21 and 28 days. Macroscopic, histologic and immunohistochemical examinations using the collagen type IV and the CD34 antibodies which are necessary to the forming process of new capillary were performed. Results obtained were as follows : Macroscopically the initial wound healing of the laser group was about 1~2 weeks slower than that of the scalpel group. There weren't however any remarkable differences in all groups in 4 weeks after incision. By histologic finding, acute inflammatory cells were more prominent during the initial wound healing in the scalpel group than in the other groups. Epithelialization started in the order of scalpel, CO₂and Nd:YAG laser group after skin incision. By the Masson's trichrome stain, collagen synthesis in the Nd:YAG laser group was more slowly initiated than in the other groups. But it was completed at the 3~4 weeks in all groups. Immunohistochemically, collagen type IV and CD34 expression were markedly increased at 2 weeks in the scalpel and CO₂laser group. Meanwhile, in the Nd:YAG laser group, these reactions were observed later tan the other groups. Collagen type IV and CD34 expression were decreased in all groups after 4 weeks. These results suggest that CO₂and Nd:YAG laser showed similar healing process compared with scalpel and a potential substitute for scalpel in skin incision.

        국내 Human Immunodeficiency Virus(HIV) 감염자와 정상인의 면역학적 표지인자 비교연구

        HIV감염자는 질병의 진전에 무관하게 감염 후의 경과시기에 따라서 CD4 T림프세포등 각종 면역상태를 나타내는 표지가 변한다. 따라서 HIV감염자의 질병진전을 예보하기 위하여서는 정기적으로 CD4등 각종표지를 측정하여 감염자의 질병상태를 monitoring하게 된다. 그러나 이러한 수치를 감염자관리에 적용하기 위하여서는 우리나라 일반인의 정상치를 파악하여 이를 지표로 해야하므로 국내정상인의 각종면역치에 대한 조사가 요구된다. 현재의 기준으로는 500이하로 떨어질 때에는 예방차원에서 AZT를 복용하게 되며 200이하로 떨어지면 질병의 유무에 관계없이 환자로 관리하게 된다. 본연구에서는 한국인 185명의 감염자와 140명의 비감염자에 대하여 정기적으로 CD4 및 CD8T 림프세포와 CD4/CD8비를 측정하였다. 시험은 Flow cytometer(Facstar)를 이용하여 각각의 CD 분자에 대한 모노크로날 항체를 이용하여 2중혈광색소염색방법으로 측정하였다. HIV감염자의 CD4-T림프세로 절대수 및 백분율은 각각 462 및 18.2%이었는 반면, CD8의 수치는 1,170 및 47.0%이었다. 또한 CD4/CD8비는 0.43이었다. 이와는 대조적으로 비감염자의 경우, 한국인의 CD4의 평균세포수는 886, 백분율은 32.9%이었으며, CD8 세포수는 730, 백분율은 26.8 그리고 CD4/CD8비는 1.31이었다. 외국인과 한국인과의 면역지표수치를 비교하였을 때에 CD4세포수와 백분율, CD8의 백분율에서는 현저한 차이가 없었으나 외국인 비감염자의 경우 CD4백분율이 43.6%, CD8 T림프세포의 절대수가 560으로 한국인과 약간의 차이가 있었다. 따라서 HIV감염자관리를 위한 면역지표측정시험에서의 각종수치의 정확한 해석을 위하여서는 한국인 비감염자수치를 고려해야할 것으로 판단된다. Several studies showed that the immunological factors such as CD4+ cell number, CD4%, CD8+ cell number and CD4/CD8 ratio and the serological factors such as β²-microglobulin(β²-MG), neopterin, soluble CD4, and soluble CD8 are related to the risk of development of AIDS. Especially, the CD4+ cell counts have been used to monitor progresson of HIV disease, to stratify, and to follow patients in clinical trials. Recently, the Centers for Disease Control and Prevention(CDCP) in USA has made the CD4+ cell count as a part of the classification of HIV disease. It is composed of 3 categories such as 1, 2, and 3 which asr ≥500/㎣, 200/㎣ ≥and <500/㎣, and < 200/㎣, respectively. In this study, to estimate the differences of immunological factors between HIV-infected and normal human groups in Korea, CD4+ T and CD8+ T cells, and the CD4/CD8 ratio were measured in 185 HIV-infected subjects and 140 healthy adult subjects. The lymphocyte subsets such as CD4+ T and CD8+ T were analysed by flow cytometer(FACStar) with two-color immunofluorescent stain using monoclonal antibodies such as anti-CD4 and anti-CD8 antibodies. The absolute numbers and percentages of CD4+ T and CD8+ T and the CD4/CD8 ratio of HIV infected persons were 462±277/㎣, 18.2±7.7%, 1,170±534/㎣, 47.0±10.6% and 0.43±0.26 whereas those of uninfected persons were 886±299/㎣, 32.9±7.0%, 730± 259/㎣, 26.8±6.4% and l.31±0.46(P<0.01). In addition, estimating the reference values of peripheral blood lymphocyte subsets of Korean, the absolute numbers and percentages of CD4+ T and CD8+ T and the CD4/CD8 ratio of 140 healthy adults persons were measured and compared with those of foreigners. The reference ranges of CD4+ T cells, CD8+ T cells, CD4%, CD8%, and the CD4/CD8 ratio and 1.31t0.46, respectively. The significant differences were not observed when compared with those of foreigners. However a little difference was observed in the percentages of CD4+ T and the absolute numbers of CD8+ T between the normal values of Korean and those of foreigners were 43.6±8.9%, 560±230/㎣. This result can also be useful as a basic data for the treatment and surveillance of HIV-infected patients in Korea.

      • 소엽의 Nitric Oxide Synthase 저해활성 및 Peroxynitrite소거활성

        Nitric Oxide (NO), derived from L-arginine, is produced by two types of nitric oxide synthase (constitutive NOS: cNOS, inducible NOS: iNOS). The NO produced in large amounts by the iNOS is known to be responsible for the vasodilation and hypotension observed in septic shock and inflammation. The inhibitors of iNOS, thus, may be useful candidates for the treatment 'of inflammatory diseases accompanied by the overproduction of NO. In order to find new inhibitors of iNOS, we have performed intensive screening of medicinal herbs for the inhibitory activity of NO production in lipopolysaccaride (LPS)-activated macrophages. We found that the ethanol extract of Perillae herba showed strong inhibition of NO production in activated macrophages. The solvent fractions of Perillae herba not only inhibited the expression of iNOS at 20 ㎍/㎖, but also showed scavenging activity of peroxynitrite anion. From the activity guided column chromatography of the ethanol extract, luteolin was identified as an active compound that showed dose-dependent inhibition of NO synthesis in LPS-activated macrophages (IC50 value; 6.9 μM). Luteolin may be an useful candidate for the development of new drug to treat endotoxemia and inflammation accompanied by the overproduction of NO.
