      • Anisotropic Terahertz Emission from Bi <sub>2</sub> Se <sub>3</sub> Thin Films with Inclined Crystal Planes

        <P>We investigate the surface states of topological insulator (TI) Bi<SUB>2</SUB>Se<SUB>3</SUB> thin films grown on Si nanocrystals and Al<SUB>2</SUB>O<SUB>3</SUB> substrates by using terahertz (THz) emission spectroscopy. Compared to bulk crystalline Bi<SUB>2</SUB>Te<SUB>2</SUB>Se, film TIs exhibit distinct behaviors in the phase and amplitude of emitted THz radiation. In particular, Bi<SUB>2</SUB>Se<SUB>3</SUB> grown on Al<SUB>2</SUB>O<SUB>3</SUB> shows an anisotropic response with a strong modulation of the THz signal in its phase. From x-ray diffraction, we find that the crystal plane of the Bi<SUB>2</SUB>Se<SUB>3</SUB> films is inclined with respect to the plane of the Al<SUB>2</SUB>O<SUB>3</SUB> substrate by about 0.27°. This structural anisotropy affects the dynamics of photocarriers and hence leads to the observed anisotropic response in the THz emission. Such relevance demonstrates that THz emission spectroscopy can be a sensitive tool to investigate the fine details of the surface crystallography and electrostatics of thin film TIs.</P>

        인공호흡기 적용환자에서 폐쇄형 기도흡인 전·후 과환기와 과산소 적용의 효과

        Purpose: The purpose of this study was to identify the effects of hyperventilation and hyperoxygenation before and after endotracheal suctioning using closed-suction system on mechanically ventilated patients. Method: We selected 20 patients who were mechanically ventilated and randomly assigned into 4 groups according to the type of treatments using repeated measure. We applied 3 types of treatment, hyperventilation, hyperoxygenation, hyperventilation+hyperoxygenation, and no treatment and measured physiologic effects-SpO_(2), HR and, mena BP. Result: There were no significant differences between the group with no treatment and the group with hyperventilation, before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperventilation hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(P=.970) and mean BP(P=.638)). Conclusion: As the results, all of the methods, hyperventilation, hyperoxygenation, and hyperventilation hyperoxygenation can be used when nurses apply endotracheal suctioning using closed-suction systems. In case of hypoxemic patients, hyperoxygenation is recommended for prevention of deoxygenation due to endotracheal suctioning using closed-suction systems.

      • Analysis of the genome of a Korean isolate of the Pieris rapae granulovirus enabled by its separation from total host genomic DNA by pulse-field gel electrophoresis

        Most traditional genome sequencing projects involving infectious viruses include culturing and purification of the virus. This can present difficulties as an analysis of multiple populations from multiple locations may be required to acquire sufficient amount of high-quality DNA for sequence analysis. The electrophoretic method provides a strategy whereby the genomic DNA sequences of the Korean isolate of Pieris rapae granulovirus (PiraGV-K) were analyzed by purifying it from host DNA by pulsed-field gel electrophoresis, thus simplifying sampling and labor time. The genomic DNA of infected P. rapae was embedded in agarose plugs, digested with a restriction nuclease and methylase, and pulsed-field gel electrophoresis (PFGE) was used to separate PiraGV-K DNA from the DNA of P. rapae, followed by mapping of fosmid clones of the separated viral DNA. The double-stranded circular genome of PiraGV-K encodes 120 open reading frames (ORFs), covering 92% of the sequenced genome. BLAST and ORF arrangement showed the presence of 78 homologs to other genes in the database. The mean overall amino acid identity of PiraGV-K ORFs was highest with the Chinese isolate of PiraGV (~99%), followed up with Choristoneura occidentalis ORFs at 58%. PiraGV-K ORFs were grouped, according to function, into 10 genes involved in transcription, 11 involved in replication, 25 structural protein genes, and 15 auxiliary genes. Genes for Chitinase (ORF 10) and cathepsin (ORF11), involved in the liquefaction of the host, were found in the genome. The recovery of PiraGV-K DNA genome by pulse-field electrophoretic separation from host genomic DNA had several advantages, compared with its isolation from particles harvested as virions or inclusions from the P. rapae host. We have sequenced and analyzed the 108,658 bp PiraGV-K genome purified by the pulsed field electrophoretic method. The method appears to be applicable to the analysis of genomes of large viruses. The chitinase, identified by PiraGV-K genome sequence, was functionally characterized by quantitative PCR, Western blot analysis, immunohistochemistry and transmission electron microscopy.

      • Antenna Mask Design for SAR Performance Optimization

        <P>In this letter, an effective technique for synthetic aperture radar (SAR) antenna mask design is presented for optimizing the system performance of an active phased array SAR. The SAR antenna radiation pattern has an important effect on the system performance. Therefore, the authors derived the quantitative equations for the SAR antenna mainlobe and sidelobe mask design on the basis of the system performance measures such as the range-to-ambiguity ratio (RAR), the noise-equivalent sigma zero (NESZ), and the radiometric accuracy. The antenna mask template should be designed to minimize the ambiguous signals reflected from the antenna sidelobes and maximize the system sensitivity, i.e., the NESZ, determined by the antenna mainlobe. The simple iterative method such as random-mutation hill climbing was utilized to successfully assign the sidelobe level at each ambiguous area using the derived equations. Finally, the antenna patterns were synthesized with reference to the optimized antenna mask templates using the particle swarm optimization, and the swath width, RAR, and NESZ performances were evaluated in order to confirm the effectiveness of the proposed technique.</P>

        Endometrial polyp surveillance in premenopausal breast cancer patients using tamoxifen

        Objective To describe the endometrial pathologic lesions in premenopausal breast cancer patients with a history of tamoxifen (TMX) use. Methods We retrospectively reviewed the medical records of 120 premenopausal breast cancer patients with a history of TMX use that had undergone a gynecological examination. Results Among 120 patients, 44.2% (n=53) were asymptomatic with an endometrial thickness ≥5 mm, as assessed by transvaginal ultrasonography. Of the patients that reported abnormal uterine bleeding, 5% (n=6) had an endometrial thickness <5 mm and 20% (n=24) had an endometrial thickness ≥5 mm by transvaginal ultrasonography. The final group of patients were asymptomatic, but showed an abnormal endometrial lesion, such as an endometrial polyp, by transvaginal ultrasonography (30.8%, n=37). Of the 56 benign lesions that were histologically reviewed, 50 (41.7%) were endometrial polyps, 3 (2.5%) were submucosal myomas, 2 (1.7%) were endometrial hyperplasias, and 1 (0.8%) was chronic endometritis. There were 64 (53.3%) other non-pathologic conditions, including secreting, proliferative, and atrophic endometrium, or in some cases, there was insufficient material for diagnosis. In our data, only one case was reported as a complex hyperplasia without atypia arising from an endometrial polyp, and one patient was diagnosed with endometrioid adenocarcinoma. Conclusion For premenopausal breast cancer patients with a history of TMX use, the majority of the patients were asymptomatic, and endometrial polyps were the most common endometrial pathology observed. Therefore, we believe that endometrial assessment before starting TMX treatment, and regular endometrial screening throughout TMX treatment, are reasonable suggestions for premenopausal breast cancer patients.

        Ansanella granifera gen. et sp. nov. (Dinophyceae), a new dinoflagellate from the coastal waters of Korea

        A small dinoflagellate, Ansanella granifera gen. et sp. nov., was isolated from estuarine and marine waters, and examined by light microscopy, scanning electron microscopy, and transmission electron microscopy. In addition, the identity of the sequences (3,663-bp product) of the small subunit (SSU), internal transcribed spacer (ITS) region (ITS1, 5.8S, ITS2), and D1-D3 large subunit (LSU) rDNA were determined. This newly isolated, thin-walled dinoflagellate has a type E eyespot and a single elongated apical vesicle, and it is closely related to species belonging to the family Suessiaceae. A. granifera has 10-14 horizontal rows of amphiesmal vesicles, comparable to Biecheleria spp. and Biecheleriopsis adriatica, but greater in number than in other species of the family Suessiaceae. Unlike Biecheleria spp. and B. adriatica, A. granifera has grana-like thylakoids. Further, A. granifera lacks a nuclear fibrous connective, which is present in B. adriatica. B. adriatica and A. granifera also show a morphological difference in the shape of the margin of the cingulum. In A. granifera, the cingular margin formed a zigzag line, and in B. adriatica a straight line, especially on the dorsal side of the cell. The episome is conical with a round apex, whereas the hyposome is trapezoidal. Cells growing photosynthetically are $10.0-15.0{\mu}m$ long and $8.5-12.4{\mu}m$ wide. The cingulum is descending, the two ends displaced about its own width. Cells of A. granifera contain 5-8 peripheral chloroplasts, stalked pyrenoids, and a pusule system, but lack nuclear envelope chambers, a nuclear fibrous connective, lamellar body, rhizocysts, and a peduncle. The main accessory pigment is peridinin. The SSU, ITS regions, and D1-D3 LSU rDNA sequences differ by 1.2-7.4%, >8.8%, and >2.5%, respectively, from those of the other known genera in the order Suessiales. Moreover, the SSU rDNA sequence differed by 1-2% from that of the three most closely related species, Polarella glacialis, Pelagodinium bei, and Protodinium simplex. In addition, the ITS1-5.8S-ITS2 rDNA sequence differed by 16-19% from that of the three most closely related species, Gymnodinium corii, Pr. simplex, and Pel. bei, and the LSU rDNA sequence differed by 3-4% from that of the three most closely related species, Protodinium sp. CCMP419, B. adriatica, and Gymnodinium sp. CCMP425. A. granifera had a 51-base pair fragment in domain D2 of the large subunit of ribosomal DNA, which is absent in the genus Biecheleria. In the phylogenetic tree based on the SSU and LSU sequences, A. granifera is located in the large clade of the family Suessiaceae, but it forms an independent clade.

        Supramolecular Cl⋅⋅⋅H and O⋅⋅⋅H Interactions in Self‐Assembled 1,5‐Dichloroanthraquinone Layers on Au(111)

        <P><B>Abstract</B></P><P>The role of halogen bonds in self‐assembled networks for systems with Br and I ligands has recently been studied with scanning tunneling microscopy (STM), which provides physical insight at the atomic scale. Here, we study the supramolecular interactions of 1,5‐dichloroanthraquinone molecules on Au(111), including Cl ligands, by using STM. Two different molecular structures of chevron and square networks are observed, and their molecular models are proposed. Both molecular structures are stabilized by intermolecular Cl⋅⋅⋅H and O⋅⋅⋅H hydrogen bonds with marginal contributions from Cl‐related halogen bonds, as revealed by density functional theory calculations. Our study shows that, in contrast to Br‐ and I‐related halogen bonds, Cl‐related halogen bonds weakly contribute to the molecular structure due to a modest positive potential (σ hole) of the Cl ligands.</P>

        Extensive Neuromusculoskeletal Complications—Including Multi-Level Vertebral Osteomyelitis, Myelopathy, Polyradiculopathy, and Multiple Muscle Abscesses—After Lumbar Transforaminal Epidural Block: A Case Report

        Vertebral osteomyelitis (VO) is a rare, slowly progressing disease that often causes neuromuscular complications. Epidural block is a non-surgical treatment used in patients with radicular pain to deliver drugs to the epidural space; it is generally known to be a safe method, but it can occasionally cause infection. Herein, we present a rare case of VO with severe neuromuscular complications that developed in a patient who underwent lumbar transforaminal epidural block for back pain. Imaging studies showed VO, multiple pyogenic abscesses, and compressive cervical myelopathy. Electrodiagnostic studies showed clear evidence of cervical myelopathy and polyradiculopathy. With early treatment using a multidisciplinary approach, including medical treatment, surgery, and comprehensive rehabilitation, recovery of motor weakness and functional improvement were achieved after 2 months of treatment. Electrodiagnostic studies are advantageous for localizing and determining the degree of neuromuscular damage following VO. A multidisciplinary approach to the diagnosis and treatment of VO could improve patients’ prognosis, functional ability, and quality of life.

        Prospective phase II trial of regional hyperthermia and whole liver irradiation for numerous chemorefractory liver metastases from colorectal cancer

        Purpose: A prospective phase II trial was conducted to evaluate the effectiveness and toxicity of regional hyperthermia and whole liver irradiation (WLI) for numerous chemorefractory liver metastases from colorectal cancer. Materials and Methods: Enrolled patients had numerous chemorefractory hepatic metastases from colorectal cancer. Five sessions of hyperthermia and seven fractions of 3-gray WLI were planned. Health-related quality of life (HRQoL) was determined using the Korean version of the European Organization for Research and Treatment of Cancer quality of life questionnaire C-30 and the Functional Assessment of Cancer Therapy-Hepatobiliary version 4.0. Objective and pain response was evaluated. Results: A total of 12 patients consented to the study and the 10 who received WLI and hyperthermia were analyzed. WLI was completed as planned in nine patients and hyperthermia in eight. Pain response was partial in four patients and stable in four. Partial objective response was achieved in three patients (30.0%) and stable disease was seen in four patients at the 1-month follow-up. One patient died 1 month after treatment because of respiratory failure related to pleural metastasis progression. Other grade III or higher toxicities were detected in three patients; however, all severe toxicities were related to disease progression rather than treatment. No significant difference in HRQoL was noted at the time of assessment for patients who were available for questionnaires. Conclusion: Combined WLI and hyperthermia were well tolerated without severe treatment-related toxicity with a promising response from numerous chemorefractory hepatic metastases from colorectal cancer.
