Management of Traumatic Cervical Spondyloptosis with an Unsealable Dura Tear: A Case Report

Cervical spondyloptosis is defined as dislocation of the spinal column, most often caused by trauma. Due to transection of the spinal cord, severe neurological deficits are common. Here, we reviewed the case of a young man who presented with mental stupor and complete tetraplegia below the level of C5. The patient's left shoulder was sucked into a machine and subjected to strong lateral bending and distraction. Computed tomography (CT) scan and magnetic resonance imaging revealed fracture dislocation and complete transection of the spinal cord at the C5–6 level. Three-dimensional CT scan showed coronal and sagittal spondyloptosis. He underwent open reduction with two surgeries performed via the anterior and posterior approaches: C5–6 anterior cervical discectomy and fusion and lateral mass screw fixation with allograft from C3 to C7. In addition, both ends of the huge dura defect were sutured. We report the clinical history, imaging findings, and surgical management of spondyloptosis with a complete transected spinal cord containing a considerable dura tear.

Objective The study aims to show both the short- and long-term treatment outcome of occipital nerve block (ONB) patients with occipital neuralgia (ON). Methods Patients who visited our hospital between 2013 and 2020 were reviewed retrospectively. Patients were excluded if the medical records were incomplete, if they had received a cervical operation, if they had a traumatic event prior to the symptom onset, or if the follow-up period was less than 3 months. ONB targeted the greater occipital nerve, the lesser occipital nerve or both. Short term follow-up period was defined as 3 months and long term was defined as 12 months. Injection consisted of a mixture of triamcinolone acetonide, lidocaine, and normal saline. Visual analogue scale (VAS) was recorded and compared before and during the follow-up to period to assess treatment outcome. Results Clinical charts of 309 candidates were reviewed and 72 patients were excluded, making it 237 patients. VAS scores significantly decreased in both 3-month follow-up (from 7.20±0.94 to 3.48 ±1.66, p<0.05) and after 1 year follow-up (from 7.20±0.94 to 2.71±1.07, p<0.05). Only 8 patients (3.3%) were refractory to ONB and the procedure was found to be relatively safe since only 2 patients (0.8%) showed transient side effects. Conclusion There are many treatment options for ON. However, from the results of our study, conservative treatment via ONB may have sufficient effect in controlling symptoms of ON in both short and long term.
Finite Element Numerical Analysis on Tidal Characteristic Changes due to Seadike Construction

Abstract [] The prediction of changes in the tidal regime due to the sea dike closure in the Saemankum area was performed using the nonlinear finite element model, TIDE. Based upon an overall comparison of calibrated model results with available field data, the TIDE model behaves well and is good representation of the hydrodynamic of the Saemankum tidal project area. It is shown that the TIDE model does an excellent job of computing the changes of tidal characteristics resulting in sea dike closure in an estuary area.

Purpose: Overcrowding of emergency departments remains an unsolved international issue and has led to not a few unfavorable consequences. For a solution of this, appropriate patient triage and selection of appropriate hospitals to which to be transported will be essential. Methods: The authors performed a prospective analysis of 690 patients transported via emergency transport services to the Emergency Department of Pusan National University Hospital between May 1,2000 and June 30,2000. Results: Five hundred five cases (73.2%) were considered as appropriate transport, but 185 (26.8%) were not. In 300 cases (43.5%), the hospitals to which the patients were transported had been decided by the patient's family members, in 283 cases (41.0%) by medical professionals, in 70 cases (10.1%) by EMS personnels, and in 37 cases (5.4%) by the patients themselves. In the selection of the hospital to be transported to, patients and family members took the patient's medical condition into consideration less seriously than the other groups, but they had the greatest influence on the decision. Conclusion: The patients and their family members turned out to be unsuitable for the selection of the hospital to be transported to, but they actually had the greatest influence on the selection. The EMS personnels had limited influence on the selection, and they were lacking in ability to select appropriate hospital with the consideration of the patient's condition. For now there's no active regional ambulance destination policy or decision scheme. For the optimization of emergency transport activity, every effort should be made to enhance the role and ability of EMS personnels in patient triage and selection of transported hospital and to develope well-functioning ambulance destination policies as soon as possible.

The probiotic properties of ten lactic acid bacteria and antioxidant and α-glucosidase inhibitory activities of the exopolysaccharide (EPS) of the selected strain were investigated in this study. Levilactobacillus brevis L010 was one of the most active strains across all the in vitro tests. The cellfree supernatant (50 g/l) of L. brevis L010 showed high levels of both α-glucosidase inhibitory activity (98.73 ± 1.32%) and 2-diphenyl-1-picrylhydrazyl (DPPH) radical-scavenging activity (32.29 ± 3.86%). The EPS isolated from cell-free supernatant of L. brevis L010 showed 2,2'-azino-bis (3- ethylbenzothiazoline-6-sulfonic acid) radical-scavenging activity (80.27 ± 2.51%) at 80 g/l, DPPH radical-scavenging activity (38.19 ± 9.61%) at 40 g/l, and ferric reducing antioxidant power (17.35 ± 0.20 mg/l) at 80 g/l. Further, EPS exhibited inhibitory activities against α-glucosidase at different substrate concentrations. Kinetic analysis suggests that the mode of inhibition was competitive, with a kinetic constant of Km = 2.87 ± 0.88 mM and Vmax = 0.39 ± 0.06 μmole/min. It was concluded that the EPS might be one of the plausible candidates for possible antioxidant and α-glucosidase activities of the L. brevis L010 strain.

<P>Si<SUB>1−<I>x</I></SUB>C<SUB><I>x</I></SUB> nanowires (NWs) were synthesized by varying the ratio of SiH<SUB>4</SUB> and CH<SUB>3</SUB>SiH<SUB>3</SUB> gases using a vapor–liquid–solid (VLS) procedure using Au as a catalyst. The growth rate of the Si<SUB>1−<I>x</I></SUB>C<SUB><I>x</I></SUB> NWs and the change in the wire shape from straight to helical near the Au tip were found to be closely related to the ratio of the CH<SUB>3</SUB>SiH<SUB>3</SUB> content. The large concentration of C in the Si<SUB>1−<I>x</I></SUB>C<SUB><I>x</I></SUB> NWs was proportional to the CH<SUB>3</SUB>SiH<SUB>3</SUB> content, overcoming the extremely low solubility of C in Si, resulting in an interstitial incorporation of C atoms in the wire. This incorporation can be attributed to the cleavage of Si–C bonds in the CH<SUB>3</SUB>SiH<SUB>3</SUB> compound through the Au catalyst (an Au–Si liquid-state cluster of about 70–100 nm) during wire growth by the VLS method. Simultaneously supplying CH<SUB>3</SUB>SiH<SUB>3</SUB> and SiH<SUB>4</SUB> gases enhanced the diffusion of Au atoms from the tip to the sidewall of the wire, while also deforming the shape of the Au tip. When the CH<SUB>3</SUB>SiH<SUB>3</SUB> gas was increased to 1.5 sccm, the number of Au nanoparticles (2–3 nm in size) at the lateral surface induced a surface plasmon resonance (SPR) and improved the optical conductivity (<I>σ</I>) of the Si<SUB>1−<I>x</I></SUB>C<SUB><I>x</I></SUB> NWs. Through the hydraulic model test, a more convenient and accurate method of deter- mining discharge coefficients in the sluice type of tide gates can be derived by the use of submergence ratio as a parameter. The results obtained are summarized as follows; 1. Discharge coefficients under submerged flow conditions can be obtained by the application of submergence ratio (S) to the free flow equation of the broad-crested wer. 2. The critical submergence ratios (S<sub>cr</sub>) for the flat basin and the broad-crested types of sill have the same value of 0.8. 3. Under free flow conditions, the discharge coefficient (μ) are 0.37 and 0.35 for the flat basin and the broad-crested types of sill respectively. However, when submerged flow condition exists, the discharge coefficients for both types of sill is given by a regression equation of discharge coefficients (μ) on submergence ratios (S) expressed as; μ=1.3-1.17S. 4. The relationships between S and Froude number (F<sub>r</sub>), for the flat basin and the broad-crested types of sill are F<sub>r</sub>=2.79-2.49S and F<sub>r</sub><sup>2.5</sup>=5.7-6.16S respectively. From the above relationships, it can be concluded that m can also be expressed in terms of the Froude number which is a very relevant hydraulic parameter of the open channel hydraulics.
